Ensayo clínico aleatorizado" ] ] "resumenGrafico" => array:2 [ "original" => 0 "multimedia" => array:7 [ "identificador" => "fig0005" "etiqueta" => "Fig. 1" "tipo" => "MULTIMEDIAFIGURA" "mostrarFloat" => true "mostrarDisplay" => false "figura" => array:1 [ 0 => array:4 [ "imagen" => "gr1.jpeg" "Alto" => 1463 "Ancho" => 2337 "Tamanyo" => 136917 ] ] "descripcion" => array:1 [ "en" => "<p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">Structure of the study.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">In recent decades, mortality due to cardiovascular disease (CVD) has decreased<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">1</span></a> and this fact has been related to the improvement of various factors, including treatment, prevention and management of risk factors.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">2</span></a> However, this decrease from the 1980s to the present has been slightly higher in men than in women,<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">1,3</span></a> for example in Europe this reduction has been of 32 and 30%, respectively.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">4</span></a> Other studies indicate that the rate of CVD in women has almost doubled from 30% in 1997 to 54% in 2009.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">5</span></a> In 2012 the mortality rates attributable to this cause in Spain maintained a similar dynamic, that is they were 282.2 women and 239.4 men per 100,000 inhabitants.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Several studies have shown that the cessation of menstruation causes increased cardiovascular risk in women, due to oestrogen deficiency, a cardioprotective hormone.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">7,8</span></a> In addition, metabolic changes occur in this phase, such as elevations of total cholesterol (total-C), low density lipoproteins (LDL-C) and triglycerides, and decreases in high-density lipoproteins (HDL-C).<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">9–11</span></a> On the other hand, menopause plays a prominent role in the increase of blood pressure, since the reduction of both oestradiol and oestrogen causes endothelial dysfunction and arterial stiffness and, consequently, an increase in sympathetic activation that subsequently favours an increase in renin and angiotensin II.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Modifying the lifestyle is the most effective method to prevent this serious illness.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">13,14</span></a> Some authors have shown that positively modifying certain lifestyle habits can make a person healthier.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">15–17</span></a> However, here in Spain, there have been few intervention studies on lifestyle specifically in perimenopausal women with cardiovascular risk factors.</p><p id="par0020" class="elsevierStylePara elsevierViewall">For these reasons, the objective of this study was to evaluate whether an educational intervention in women of perimenopausal age with hypertension, diabetes mellitus and/or dyslipidaemia could achieve significant changes in the reduction of biochemical and haemodynamic risk parameters.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0025" class="elsevierStylePara elsevierViewall">This is a randomised, open-label clinical trial in primary care setting, conducted in women with a cardiovascular risk factor, diagnosed with dyslipidaemia, hypertension and/or diabetes. The women were randomly assigned to two groups: intervention group and control group, with a one-year follow-up period.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study subjects</span><p id="par0030" class="elsevierStylePara elsevierViewall">The study was conducted in 2 of the 5 health centres in the city of Ourense (Spain). A population of 3541 women aged between 45 and 60 years attend these centres. To calculate the sample size, and based on previous studies,<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">10</span></a> a decrease in mean systolic blood pressure of 5.7<span class="elsevierStyleHsp" style=""></span>mmHg between the intervention and control groups was assumed. Assuming a common standard deviation of 15.2<span class="elsevierStyleHsp" style=""></span>mmHg for a 95% confidence level, with 160 subjects in each group, a statistical power of 80% was available to obtain a statistically significant result, with a bilateral alpha error of 5%, allowing for a 30% loss rate.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Women between 45 and 60 years of age with a medical diagnosis of diabetes mellitus, dyslipidaemia and/or hypertension were selected. The exclusion criteria were: women who did not give their consent or who presented a psychological situation or disease that made their participation inadvisable, women whose telephone data had not been correctly recorded in the database, or women whose habitual residence was not in the area of influence of the study.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Ethical considerations</span><p id="par0040" class="elsevierStylePara elsevierViewall">The study objective was explained to the participants, and they all provided their written informed consent so as to be included in the same, while understanding that their anonymity was guaranteed. The study was carried out in accordance with the stipulations of the Declaration of Helsinki,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">18</span></a> and was approved by the Clinical Research Ethics Committee of Galicia (registration number 2013/114).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Procedure</span><p id="par0045" class="elsevierStylePara elsevierViewall">The participants were recruited from the doctors’ and nursing consultancies. The doctor or nurse verified that they met the inclusion criteria and none of the exclusion criteria, informed them regarding the study and if they showed interest in participating, the principal investigator, who is a physiotherapy and nursing graduate, contacted them by telephone to arrange an appointment at the health centre itself. During this appointment the objective of the study was explained, the informed consent form was signed, and the necessary measurements were taken. If no recent blood test was available (within the last 6 months, as established in the cardiovascular prevention protocol of the Prevention Activities and Health Promotion Program),<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">19</span></a> it was requested by the corresponding physician. The study period was from June 2013 to January 2015, with the initial data collection performed between the months of June and September 2013, enrolling the first 320 women who agreed to participate. Between October and December of that same year, the interactive workshops were held and one year after their completion, between January and February 2015, the second data collection was performed by the same professional who carried out the first one.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Randomisation and blinding</span><p id="par0050" class="elsevierStylePara elsevierViewall">The assignment of the patients to one of the two groups was carried out using a simple randomisation list, generated automatically by a technician not related to the study. The sequence was concealed from the researchers until the intervention was assigned. The doctors and nurses who participated in the recruitment were blinded as to the group in which their patients participated. To guarantee the quality of the data, the records were monitored by external and internal auditors.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Intervention</span><p id="par0055" class="elsevierStylePara elsevierViewall">After randomisation, the sample was divided into an intervention group (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>160) and a control group (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>160). The women of the intervention group were included in three interactive workshops over the space of a week, in groups of 15 women. Each lasted approximately one and a half hours (30<span class="elsevierStyleHsp" style=""></span>min presentation and 1<span class="elsevierStyleHsp" style=""></span>h for questions and answers) and was aided by teaching materials, aimed at achieving better control of the cardiovascular risk. The sessions were carried out in both health centres and were always presented by 3 members of the research team (2 physicians who were both department heads, one being a family and community medicine specialist and the other specialising in internal medicine, and a nurse/physiotherapist who was a university teacher with the academic degree of doctor), who had previous experience in health education activities. An identical script was used in each group. The purpose of the first session was to improve the knowledge of CVD and to change attitudes towards this serious health problem. The second session was directed to the interest in following a healthy-heart diet, with the most recommended one being the Mediterranean diet. The third workshop focused on the importance of physical exercise and stopping or reducing harmful habits such as tobacco, coffee or alcohol. The women in the control group only received written information about CVD and how to prevent it, by post at their homes. One year after completing the interactive sessions, the study's principal investigator took care of contacting both groups of women and carrying out the second data collection (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Study variables</span><p id="par0060" class="elsevierStylePara elsevierViewall">The following data were collected at baseline and one year after the educational workshops: age, body mass index (BMI), presence of hypertension, dyslipidaemia and/or diabetes mellitus, antihypertensive, lipid-lowering and/or oral antidiabetic drugs or insulin, lab test values such as glucose, glycosylated haemoglobin (HbA1c), triglycerides, Total-C, HDL-C, LDL-C, cholesterol bound to very low density lipoproteins (VLDL-C) and the alanine aminotransferase enzymes (ALAT/GPT) and gamma-glutamyltransferase (GGT). An Omron (M-2) tensiometer was used to measure the systolic and diastolic blood pressure levels and three heart rate measurements were determined. The mean of these was calculated according to the recommendations of international protocol.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">20</span></a></p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Statistic analysis</span><p id="par0065" class="elsevierStylePara elsevierViewall">The continuous variables were presented in the form of mean and standard deviation, and the dichotomous variables in the form of counts and percentages. The normal distribution of the continuous variables was evaluated using the Kolmogorov–Smirnov test (Lilliefors correction), and for its study, the Student's <span class="elsevierStyleItalic">t</span>-test was used for independent samples and paired data; non-parametric tests (Wilcoxon test for paired data and the Mann–Whitney <span class="elsevierStyleItalic">U</span> test for independent data) were used to compare mean values. A <span class="elsevierStyleItalic">p</span>-value<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant. The analyses were carried out with free R-software.</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Results</span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Baseline characteristics</span><p id="par0070" class="elsevierStylePara elsevierViewall">230 women completed the study, with 28.1% total loss rate, 47 were lost in the control group (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>113) and 43 were lost in the intervention group (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>117). No significant baseline differences were observed between women lost to follow-up and those who completed the study (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). An analysis of the characteristics of the lost cases showed 65.5% (59 women) withdrew due to the heavy work or family responsibilities, 20% (18) were unable to be located again by phone, and 14.4% (13) did not show interest in continuing the study. No common pattern was found to explain this high drop-out rate. The women's average age was 53.17<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.31 years, the BMI was 27.45<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.40 and the distribution of risk factors was described in other previously published studies where different aspects of the same sample were analysed.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">16,17</span></a> Before starting the comparative analyses between both groups, the homogeneity was verified showing no statistically significant differences between the women in the control group and those in the intervention group (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). In the control group, 63.7% (72) of the women took at least one of these medicinal products: antihypertensive, lipid-lowering and oral antidiabetic drugs and/or insulin. In the intervention group, 65% (76) of the women said they followed at least one of these treatments to control their risk factors (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The mean and standard deviation of the biochemical variables (glucose, HbA1c, triglycerides, Total-C, HDL-C, LDL-C, VLDL-C, enzyme ALAT/GPT and GGT) of the women in both study groups are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding the intragroup differences in the biochemical variables, we observed that one year after the educational intervention, the women in the control group had increased values of: glucose (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001); HbA1c (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006); GGT (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002); and triglycerides (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.032). And in that same period of time, the women of the intervention group had undergone positive changes in some parameters of the lipid profile, specifically they had decreased LDL-C (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.044) and increased HDL-C (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.008). In this same group, slight reductions were also observed in the Total-C and VLDL-C figures, and although these were not significant, they may be clinically relevant (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding systolic blood pressure, no significant differences were found in any of the groups. However, the diastolic blood pressure decreased significantly in the experimental group after the educational action (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001), but it remained similar in women in the control group. The increase in the heart rate in the control group was also statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003), however, in the intervention group it decreased slightly, although this figure was not statistically significant, but it was clinically relevant (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Regarding the taking of drugs overall, there were no significant differences found in the control group, as figures prior to were 72 women (63.7%) and after were 75 women (66.4%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.581), nor in the intervention group, figures prior to were 76 women (65%) and after were 82 (70.1%) women (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.109). Also, a specific study of each group of drugs (antihypertensive, lipid-lowering and oral antidiabetic/insulin) showed no differences in either of the two groups before and after the intervention (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0085" class="elsevierStylePara elsevierViewall">One year after the training activities, the difference between both groups was significant in the biochemical variables: glucose (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.041) ([GI: −13.07<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>25.2; 95% CI: −13.1/25.3] [GL: 4.84<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>15.5; 95% CI: −0.75/31.3]), LDL-C (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.034) ([GI: −5.89<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>29.8; 95% CI: −13.1/0.27] [GL: 1.39<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>28.6; 95% CI: −16.7/12.13]) and GGT (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.031) ([GI: 0.55<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>24.1; 95% CI: −3.85/4.9] [GL: 3.61<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.7; 95% CI: 0.87/6.36]) showed a more pronounced increase in the control group, whereas the HDL-C variable was higher (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.013) in the intervention group ([GI: 2.71<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.6; 95% CI: −1.36/6.20] [GL: −0.98<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.2; 95% CI: −5.46/3.24]) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). On the other hand, the difference between both groups was significant in the haemodynamic variables of systolic blood pressure (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.016) ([GI: −2.16<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.8; 95% CI: −4.4/0.01] [GL: 1.29<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.9; 95% CI: −1.31/3.9]) and heart rate (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) ([GI: −1.46<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.3; 95% CI: −3.34/0.42] [GL: 1.97<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.46; 95% CI: 0.39/3.55]), which showed more pronounced increases in the control group (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">In view of the results obtained in this study, we can consider that, with a simple educational intervention in perimenopausal women with a cardiovascular risk factor, the biochemical and haemodynamic parameters related to cardiovascular risk can be improved.</p><p id="par0095" class="elsevierStylePara elsevierViewall">It has been found that the results obtained coincide with other studies that show the effectiveness of health education for women's cardiovascular health. Specifically, educational interventions aimed at healthy perimenopausal women have been carried out, resulting in improved figures regarding the lipid and haemodynamic profile.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">21–23</span></a> However, in the present study, the participating women presented, a priori, a cardiovascular risk factor (diabetes, hypertension and/or dyslipidaemia), which was not present in the women in the aforementioned studies.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In relation to the lipid profile of the women studied, one year after the educational actions, we have verified significant changes in both groups. The women in the control group have increased their LDL-C levels and maintained the HDL-C levels, however, the women in the experimental group have decreased the mean LDL-C and simultaneously increased the mean HDL-C. In the study conducted by Kuller et al.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">21</span></a> an educational intervention was carried out with healthy pre-menopausal women (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>535) for 6 months. Five years later, both groups of women had increased LDL-C levels, however, the increase in women in the control group was higher (GI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.5<span class="elsevierStyleHsp" style=""></span>mg/dl, GL<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>8.9<span class="elsevierStyleHsp" style=""></span>mg/dl). Likewise, Deibert et al.,<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">22</span></a> who combined an exercise program with educational intervention in pre- and post-menopausal healthy women (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>72), showed an improvement of the levels of Total-C and HDL-C one year after the intervention. However, it is interesting to highlight the profile of the study subjects–healthy women without previous cardiovascular risk factors. In another study, conducted by Khare et al. with pre- and post-menopausal women (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>67) who underwent an educational intervention for three months, at one year after the intervention no significant improvements were found in the lipemic profile.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">14</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">In relation to the haemodynamic data of the women studied, positive changes have also been confirmed for women in the experimental group, given that their heart rate and systolic blood pressure figures have decreased, while the latter showed increase in the women in the control group. Similar results were observed in the work of Soca et al.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">24</span></a> where the efficacy of a combined program of physical exercise and nutritional advice during 12 months was evaluated, aimed at women with a body mass index equal to or greater than 30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>122). However, in the aforementioned work of Khare et al., similar to the lipemic profile, one year after the intervention no significant improvements in blood pressure were found.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">14</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">In a study similar to ours, conducted by Taylor and Wu,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">25</span></a> but with a smaller number of participants (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>98) and lasting 6 months, the impact of an educational intervention on women with at least one cardiovascular risk factor was investigated. In this study, slight improvements in blood pressure and heart rate levels were observed, however, these changes were not statistically significant, which differs from our study.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Other research carried out in several Spanish municipalities<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">26</span></a> also demonstrated beneficial effects on cardiovascular risk with the improvement of certain healthy habits, by means of a simple educational intervention in patients with at least one cardiovascular risk factor. However, the average age of this study was much lower than ours and the study sample (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>546) included patients of both sexes.</p><p id="par0120" class="elsevierStylePara elsevierViewall">These studies show that cardiovascular prevention is an important activity to be performed by health professionals, and that the female population is receptive to these initiatives.<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">27–29</span></a> In the specific case of perimenopausal women who already present at least one major risk factor, the need to carry out preventive actions becomes much more evident, given the greater possibility of developing a CVD.</p><p id="par0125" class="elsevierStylePara elsevierViewall">To date no study has been conducted in Spain to assess the effect of an educational intervention on perimenopausal women with at least one cardiovascular risk factor. For this reason, the strength of this study is our focus on a sample with very specific characteristics. On the other hand, among the limitations we must point out that the sample, despite being representative, is a local population, which could limit the generalisation of results and therefore further studies taking this aspect into account are required. A further limitation was a possible risk of attrition bias, given that the abandonment rate was very high, almost reaching the expected percentage of 30%; and although there were no differences found between those lost in both groups, this fact cannot guarantee that those lost in the control group corresponded to women who were improving whereas those lost in the intervention group were those who were least progressive. However, it should be noted that studies similar to this have also had very high drop-out rates.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">5,14,15</span></a> Another limitation to take into account is the possible awareness bias in the subjects of the control group included in the follow-up, due to the mere fact of knowing that they were being observed, although we cannot rule out a possible information flow either. Another aspect to take into account is that as it is an educational intervention, a lack of blinding is not possible, although to minimise its effects, blinding was ensured in the transfer and analysis of the data, the physicians and nurses participating in the recruitment were not aware of the random distribution of women and, lastly, the records were monitored by external and internal auditors.</p><p id="par0130" class="elsevierStylePara elsevierViewall">This study has proved that for perimenopausal women with a cardiovascular risk factor in an urban environment, a simple educational intervention can be an effective strategy to decrease compromising biochemical parameters, such as LDL-C and to increase HDL-C, as well as to improve diastolic blood pressure.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Conclusion</span><p id="par0135" class="elsevierStylePara elsevierViewall">In summary, taking into account the objectives and hypotheses proposed for this research, an educational intervention carried out with women of perimenopausal age from 2 urban health centres with at least one major cardiovascular risk factor (diabetes mellitus, dyslipidaemia or hypertension) may have managed to reduce or maintain the parameters associated with an increased likelihood of developing CVD, such as blood pressure, heart rate and lipid profile, by possibly improving certain behavioural habits in their diet and lifestyle.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">16,17</span></a> It would be interesting to follow up on this cohort of women to see their progress from a cardiovascular point of view in the future.</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Conflict of interests</span><p id="par0140"
