class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Atrial fibrillation (AF) is the most common cardiac arrhythmia and is associated with an increased risk of stroke, heart failure, dementia, and mortality.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Its prevalence in the general population is 1.5–2%,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> increasing with age, and a substantial increase in incidence and prevalence is expected in the coming decades of the 21<span class="elsevierStyleHsp" style=""></span>st century.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">While current treatments are effective in controlling symptoms and stroke risk,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> clinical studies indicate a positive effect of physical activity in AF patients on arrhythmia progression and functional capacity.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">To achieve the benefits associated with physical activity, it is necessary to identify the optimal exercise doses for AF patients. Schnohr et al.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> suggested a U-shaped relationship between aerobic exercise dose and risk of all-cause mortality. This leads to the hypothesis that different mechanisms could explain the reduction in AF risk with moderate physical activity and the increase in AF risk with intense exercise (>8<span class="elsevierStyleHsp" style=""></span>METs) performed at high volumes (>5<span class="elsevierStyleHsp" style=""></span>h/week) over the long term.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The international recommendations for physical exercise in accordance with the guidelines for the prevention of cardiovascular diseases for the general population indicate a minimum of 150<span class="elsevierStyleHsp" style=""></span>min per week of moderate-intensity aerobic exercise (64–76% HR<span class="elsevierStyleInf">max</span>) or 75<span class="elsevierStyleHsp" style=""></span>min per week of vigorous intensity (77–93% HR<span class="elsevierStyleInf">max</span>).<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> However, sarcopenia is present in many patients due to physical inactivity, and it is necessary to complement aerobic exercise with strength training.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Even so, an evaluation of the patient's health status is necessary before applying these physical exercise recommendations.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Two meta-analyses<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,11</span></a> have evaluated the effects of various forms of physical activity in patients with AF, showing a positive effect on exercise capacity and quality of life. But in order to achieve a critical analysis, it is necessary to take into account the modality, duration and intensity of the exercise, as well as the clinical profile of the patient. Therefore, in this meta-analysis we focus on physical exercise programmes, following the guidelines of the American College of Sports Medicine.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Therefore, the aim of this meta-analysis was to assess the effects of physical exercise on functional capacity, quality of life, symptoms, and adverse events in patients with AF.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study design</span><p id="par0030" class="elsevierStylePara elsevierViewall">This meta-analysis followed the recommendations of the <span class="elsevierStyleItalic">Preferred Reporting Items for Systematic reviews and Meta-Analysis</span> (PRISMA).<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Two of the authors (AOM, VHL) conducted the search and selected the articles to be included. In case of disagreement, the third investigator, GBE, was consulted. All authors were responsible for data extraction and participated in the drafting of the manuscript.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Search strategy</span><p id="par0035" class="elsevierStylePara elsevierViewall">A systematic literature search was conducted in the electronic databases Pubmed, Web of Science, Science Direct and CENTRAL (<span class="elsevierStyleItalic">The Cochrane Central Register of Controlled Trials</span>), with a publication date up to 10 January 2021. The chosen search terms were the following: ("Atrial Fibrillation") AND ("Exercise” OR "Cardiac Rehabilitation” OR "Physical Activity”) AND ("Functional Capacity” OR "Quality of Life” OR "Symptoms” OR "Adverse Events”), with a combination of filters depending on the database (see Appendix). Articles included in reviews and meta-analyses on the topic of interest were also reviewed.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Inclusion and exclusion criteria</span><p id="par0040" class="elsevierStylePara elsevierViewall">The inclusion criteria were: a) randomised controlled trials; b) studies in English or Spanish published in journals with an impact factor; c) participants aged 18 years or older; d) patients with AF regardless of AF-associated comorbidities and drug therapies; e) studies that used an aerobic exercise intervention or the combination of aerobic exercise and strength, lasting at least one month; f) only direct comparisons with control interventions were considered: regular treatment (e.g. drug therapy); g) studies that provided results on functional capacity, quality of life, symptoms, and adverse events in patients with AF.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Studies based on yoga, respiratory muscle exercises or other physical activity modalities were excluded. Studies where the control group exercised at a different intensity to the intervention group were also excluded.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Study selection</span><p id="par0050" class="elsevierStylePara elsevierViewall">To select studies, duplicate publications were eliminated and, with the results obtained, they were independently analysed on the basis of title and abstract. After obtaining the full text, the eligibility criteria were applied, generating a final list of articles included by consensus. The results were imported into a PRISMA flowchart (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Data extraction and quality assessment</span><p id="par0055" class="elsevierStylePara elsevierViewall">For data collection, relevant data from the studies that were finally included were extracted into an Excel spreadsheet and checked for accuracy (see Appendix). The methodological quality of the studies was assessed using the PEDro scale (see Appendix, Table A1). The risk of bias for each study was assessed by the three investigators using the Cochrane Collaboration tools, using the rating high, low, or unclear.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">All analyses were performed using the R metafor software package (<span class="elsevierStyleItalic">R Foundation for Statistical Computing, Vienna, Austria</span>). The measure of effect was the difference in the standardized mean change (<span class="elsevierStyleItalic">SMC</span>) by <span class="elsevierStyleItalic">raw score standardization (SMCR</span>) between the intervention and control groups, and with their 95% confidence interval (CI95%).<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> For the estimation of the combined SMCR, a random effects model with restricted maximum likelihood (REML) estimation was used.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Once the random effects model was adjusted, the type of atrial fibrillation was included as a moderator. The inverse variance method was selected to determine the weight of each study through the random effects model, accounting for sampling error or intra-study variability and inter-study variability.</p><p id="par0065" class="elsevierStylePara elsevierViewall">To estimate and quantify heterogeneity between studies, the <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> statistic was used, where values of 25, 50 and 75% would correspond to low, medium and high degrees of heterogeneity, respectively. The Leave-one-out method was used for sensitivity analysis, determining the influence of a study on the final result. The possible existence of publication bias in each outcome variable was assessed visually using a funnel plot by applying the Begg method, which compares the effect of the studies (SMCR) with their standard errors, and a p value ≤ 0.05 was considered statistically significant.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">A total of 1978 articles were identified using the search criteria. After removal of duplicates, 1223 references were analysed, of which 1194 were excluded because their title or abstract had no relation to the results of this systematic review. The full texts of 29 studies were obtained. After a detailed evaluation, 10 studies were included in this meta-analysis, 5 of them in paroxysmal and persistent AF,<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–20</span></a> and 5 in permanent AF.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21–25</span></a> encompassing a total of 782 randomised patients.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The main characteristics of the studies and of the training interventions are described in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, respectively.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The assessment of the risk of bias is shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>. There was an uncertain assessment regarding allocation concealment and, in almost all studies, a lack of blinding of participants and study staff. However, this could not be omitted due to the intervention applied (exercise vs. no exercise) and had to be considered in perspective.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Regarding the sensitivity analysis through the Leave-one-out method, no problems were observed, except in the 6-min walk test variable (6<span class="elsevierStyleHsp" style=""></span>MWT) and resting heart rate (<span class="elsevierStyleInf">resting</span>HR). We found that the exclusion of the studies by Osbak et al.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a> reduce the estimated effect and increase heterogeneity in the 6<span class="elsevierStyleHsp" style=""></span>MWT variable. In contrast, in the <span class="elsevierStyleInf">resting</span>HR variable, the exclusion of Malmo et al.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> is shown to reduce heterogeneity and increase SMCR. The funnel plots seem to indicate the absence of bias in the different variables analysed (see Appendix).</p><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Peak oxygen consumption</span><p id="par0090" class="elsevierStylePara elsevierViewall">The effects of exercise in patients with paroxysmal and persistent AF were significantly favourable for peak oxygen consumption (peak VO<span class="elsevierStyleInf">2</span>) (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.387; CI95%, 0.214−0.561; p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). Although the <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> value was 0% (95% CI, 0–84.2), we cannot conclude that there is no heterogeneity since the confidence interval reached very high values (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>A).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">6-min walk test</span><p id="par0095" class="elsevierStylePara elsevierViewall">A significant improvement in the 6<span class="elsevierStyleHsp" style=""></span>MWT was observed in patients with permanent AF (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.74; CI95%, 0.31–1.17; p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). In contrast, in patients with paroxysmal and persistent AF, improvement was poor in the exercise group versus control (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04; CI95%, −0.13 to 0.21; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.68). This is due to the clinical instability of these patients compared to patients with permanent AF who have a better control of the disease. Statistical heterogeneity between the different studies was medium-high (<span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>62.7%) (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>B).</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Maximal exercise capacity</span><p id="par0100" class="elsevierStylePara elsevierViewall">The exercise group compared to the control group managed to produce significantly more watts regardless of AF type (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.35; 95%CI, 0.18−0.51; p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). Specifically, patients with permanent AF managed to perform more watts than patients with paroxysmal and persistent AF, without significant differences between groups (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.43; CI95%, 0.02−0.84; SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.33; CI95%, 0.15−0.51; respectively). However, patients with paroxysmal and persistent AF show stability in maximal exercise capacity, without much variability. Although the <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> value was 0% (95% CI, 0–95), the presence of heterogeneity should be indicated by the high values of the confidence interval (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>C).</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Resting heart rate</span><p id="par0105" class="elsevierStylePara elsevierViewall">Compared to drug therapy, physical exercise significantly improves <span class="elsevierStyleInf">resting</span> HR control in patients with permanent AF (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>- 0.51; 95% CI [−0.93, −0.10], p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.015). However, patients with paroxysmal and persistent AF have a less favourable result regarding <span class="elsevierStyleInf">resting</span>HR control (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.07; CI95%, −0.45 to 0.32; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.735), with the patients being in a phase of arrhythmia control. However, the statistical heterogeneity between the different studies was low-medium (<span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>36.1%) (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>D).</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Systolic blood pressure</span><p id="par0110" class="elsevierStylePara elsevierViewall">Compared with physical exercise, pharmacological treatment shows a tendency to regulate systolic blood pressure (SBP) in patients with AF (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.13; CI95%, −0.03 to 0.3; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.11). Statistical heterogeneity was <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0% (95% CI, 0–67), but due to the confidence interval, it should indicate the presence of medium-high heterogeneity (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>E).</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Quality of life</span><p id="par0115" class="elsevierStylePara elsevierViewall">In the summary of the physical component in the SF-36, physical exercise tends to improve the score on physical level items in patients with paroxysmal and persistent AF (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.13; 95% CI, −0.05 to 0.31; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.167). However, patients with permanent AF have a less favourable result, showing greater variability (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.29; CI95%, −4.06 to 3.48; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.881). However, statistical heterogeneity measured <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0% (95%CI, 0–60.4), although it should show the presence of medium-high heterogeneity (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>A).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">Physical exercise tends to improve scores on mental level items in the SF-36 mental component summary in patients with permanent AF (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>4.39; 95% CI, -0.15−8.92; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.058). In contrast, in patients with paroxysmal and persistent AF, improvement was poor (SMCR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.08; CI95%, −0.10 to 0.26; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.39). However, statistical heterogeneity was low (<span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.3%; 95%CI, 0–99), although it should show the presence of medium-high heterogeneity (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>B).</p><p id="par0125" class="elsevierStylePara elsevierViewall">Three studies<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23–25</span></a> looked at the impact of exercise on quality of life in AF patients but did not provide data in relation to physical and mental component summaries and could have provided more consistent findings.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Symptoms and adverse events</span><p id="par0130" class="elsevierStylePara elsevierViewall">The studies did not provide sufficient data on the occurrence of symptoms and adverse events, which would provide information on the safety and efficacy of the interventions. We can only highlight the study by Malmo et al.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> which emphasised that exercise reduces the frequency of AF symptoms. In contrast, Hegbom et al.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> found no statistical difference in symptom frequency but did observe a significant difference in symptom severity. Lastly, Risom et al.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> only reported the number of patients with exercise-independent and exercise-related AF symptoms, without specifying what type of episodes they were. All the studies indicated that there were no adverse events in the exercise group, applying a moderate intensity in the physical exercise programmes (70% <span class="elsevierStyleInf">max</span>HR, 70% MEC or 15 points on the Borg scale).</p></span></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Discussion</span><p id="par0135" class="elsevierStylePara elsevierViewall">The results of this meta-analysis provide insight into the efficacy of an aerobic physical exercise intervention or the combination of aerobic and strength exercise on different physiological parameters, functional capacity and quality of life depending on the type of AF.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Applied analyses suggest a beneficial effect on maximal exercise capacity as a result of physical exercise. Furthermore, patients with paroxysmal and persistent AF significantly improved peakVO<span class="elsevierStyleInf">2</span>. Functional capacity has been shown to be a predictor of heart failure, mortality, and other cardiovascular events in patients with cardiovascular disease, including AF.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Previous meta-analyses also reported significant results for peakVO<span class="elsevierStyleInf">2</span> and in the exercise capacity analysing different modalities of physical activity.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,11</span></a> The underlying mechanisms responsible for the increased exercise capacity are likely related to improvements in cardiac output, increased muscle mass and strength, and factors that enhance oxygen extraction in peripheral skeletal muscle.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">The 6<span class="elsevierStyleHsp" style=""></span>MWT is primarily used to assess the efficacy of treatments in patients with moderate and severe cardiopulmonary disease. Our analysis points to significant results only in patients with permanent AF. Previous meta-analyses showed clinically important results in 6<span class="elsevierStyleHsp" style=""></span>MWT.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,11</span></a> This is due to the inclusion of different forms of physical activity, which leads to a bias in the results as each type of physical activity has a different organisation and focus. In addition, it is important to take into account the patient's disease burden and functional capacity. Therefore, patients with paroxysmal and persistent AF achieved better results in maximal exercise capacity in cycle ergometer tests compared to 6<span class="elsevierStyleHsp" style=""></span>MWT.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Literature points to a reduction in <span class="elsevierStyleInf">resting</span>HR in patients with AF thanks to physical activity, improving arrhythmia control.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,11</span></a> However, our results confirm this positive effect only in patients with permanent AF. One study showed that moderate and high levels of physical activity were significantly associated with a lower risk of atrial tachyarrhythmia recurrence in patients undergoing AF ablation.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> In addition, the reserve heart rate was not reported in any previous study, which, if impaired, contributes to the increased risk of hospitalisation in patients with heart failure.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> On the other hand, drug therapy tends to control SBP, although Malmo et al.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> managed to better control SBP thanks to exercise. This implies the need for a more exhaustive study of the doses of exercise applied to patients with atrial fibrillation.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Quality of life assessment is an important outcome parameter after physical exercise intervention in patients with AF.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Smart et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> demonstrated significant improvements in the SF-36 physical and mental component summaries, whereas our results show a trend of improvement in the same variables after exercise. That is why it is important to assess the type of exercise performed and its duration. Shi et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> demonstrated that aerobic exercise and yoga provided significant improvements in quality of life compared to the combination of aerobic and strength exercise.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The main symptoms experienced by patients with AF are dyspnoea, palpitations, and reduced exercise tolerance.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Due to lack of data, it was not possible to assess the impact of physical exercise on symptoms and adverse events. Literature suggests that moderate-intensity physical exercise in patients with AF can reduce AF-related symptoms, as well as adverse events, mortality, and hospitalization.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,29,30</span></a> In addition, the studies analysed highlight results on the benefits and safety of physical exercise at moderate intensity. However, it is difficult to develop training programmes for AF patients due to the lack of data on the type of exercise, intensity, frequency and duration, as well as the technical and professional means for its correct implementation and operation.</p></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0155">Limitations</span><p id="par0165" class="elsevierStylePara elsevierViewall">The main limitations of this study are the following: a) small number of randomised controlled trials; b) relatively small studies with short-term follow-up; c) limited number of reported episodes; d) some studies did not report complete results; e) heterogeneous exercise protocols; f) lack of assessment of optimal aerobic and strength exercise dose for AF patients.</p></span><span id="sec0095" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0160">Conclusions</span><p id="par0170" class="elsevierStylePara elsevierViewall">The results of this study indicate that physical exercise is an effective method to improve maximal exercise capacity. In patients with paroxysmal and persistent AF, peakVO<span class="elsevierStyleInf">2</span> is significantly improved, in contrast, patients with permanent AF showed significant gains in 6<span class="elsevierStyleHsp" style=""></span>MWT and in <span class="elsevierStyleInf">resting</span>HR. However, quality of life results tend to improve with exercise, but there is insufficient data. Studies using an endurance and/or strength exercise of at least one-month duration were included. The meta-analysis was conducted using the random-effects method.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">10 randomised controlled trials were selected.
