In the different types of the disease, the heart can be affected in different ways<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>, as the amyloid can end up deposited in different cardiac structures, with the most common being the myocardium and the conduction system, which is why it often leads to heart failure, but also in a high percentage of cases it manifests as arrhythmias (especially atrial fibrillation) and conduction disturbances. Due to the deposition of amyloid in the myocardium, electrocardiographic abnormalities more or less specific to the disease, such as low voltage QRS complexes or a pseudoinfarction pattern in different leads, can also be observed in many cases<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–5</span></a>. This is why the electrocardiogram is such a useful tool in establishing the initial diagnostic suspicion of the disease. Moreover, even some electrocardiographic findings have been reported as event predictors in the follow-up of these patients<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a>.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The main objective of the study is to describe the most common baseline electrocardiographic abnormalities present in patients diagnosed with cardiac amyloidosis (CA) in our centre, and to analyse whether there are differential electrocardiographic characteristics between light chain cardiac amyloidosis (AL-CA) and transthyretin cardiac amyloidosis (TTR-CA). In addition, its mid-term impact on the need for pacemakers was studied.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients. Diagnosis of cardiac amyloidosis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Retrospective study that includes patients with an established diagnosis of cardiac amyloidosis (AL-CA and TTR-CA) and who had a baseline electrocardiogram available at the time of diagnosis, between January 2013 and March 2021.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The diagnosis of AL amyloidosis was made through a biopsy of the affected organ, by demonstration of amyloid deposits characterised as AL type by immunohistochemistry, light/immunoelectron microscopy. Cardiac involvement in AL was established according to current guidelines<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The diagnosis of TTR-CA was made in patients admitted for heart failure with ventricular hypertrophy (interventricular septal thickness greater than 12 mm on echocardiography), with no alternative cause to explain the hypertrophy, either by performing an endomyocardial biopsy, or in those patients who had cardiac uptake on bone scintigraphy with <span class="elsevierStyleSup">99m</span>Tc-DPD, with a grade 2 or 3 Perugini et al. classification<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>, and who had no evidence of a monoclonal peak on serum or urine immunofixation, nor presence of free light chains in serum. All these patients underwent a genetic study to rule out the mutant variant of TTR-CA<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a>.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Electrocardiographic study. Definition of baseline electrocardiographic abnormalities</span><p id="par0070" class="elsevierStylePara elsevierViewall">All patients analysed had a baseline electrocardiogram, performed at the time of CA diagnosis. The surface electrocardiogram was performed with the usual technical parameters: correctly filtered, with a paper recording speed of 25 mm/s, and a voltage of 10 mm/mV.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In the baseline rhythm analysis, the patient was considered to be in sinus rhythm (SR) when he/she had a positive P wave in inferior leads, negative in leads aVR and V1. The absence of P waves, with irregular QRS complexes, allowed the patient to be diagnosed with atrial fibrillation (AF). New-onset AF was also considered during follow-up in patients who developed this arrhythmia and were in SR at the time of diagnosis.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Low voltage was considered present when QRS complexes had an amplitude ≤0.5 mV in all limb leads or ≤1 mV in all precordial leads<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A and D). Electrical signs of left ventricular hypertrophy were considered to exist when the Cornell criteria were met (RaVL + SV3 >28 mm in males >20 mm in females).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">A pseudoinfarction pattern was considered when pathological Q waves (1/4 of the R amplitude) or QS waves were present in two consecutive leads, in the absence of previous ischaemic heart disease or CLBBB<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A, C and E).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Conduction abnormalities were considered when any degree of atrioventricular block (first, second or third degree) was present, or when intraventricular conduction disturbances, such as bundle branch block (right or left), or left bundle branch hemiblock (anterior or posterior) were present (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B and C). The presence of these conduction abnormalities was defined according to the diagnostic criteria based on the American Heart Association guidelines for the standardization and interpretation of electrocardiograms<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Impact on the need for a permanent pacemaker</span><p id="par0095" class="elsevierStylePara elsevierViewall">The presence of these baseline electrocardiographic abnormalities, and their impact on the need for pacemakers during follow-up were studied. The impact of permanent cardiac pacing on mortality and hospital readmissions for heart failure was analysed.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0100" class="elsevierStylePara elsevierViewall">All quantitative data are presented as mean ± standard deviation. Qualitative variables are expressed as absolute frequencies and percentages. The normal distribution of all variables was analysed using the Shapiro–Wilk test. Comparison between groups of independent quantitative variables was performed using Student’s <span class="elsevierStyleItalic">t</span>-test as a parametric test or the Mann-Whitney U-test as a non-parametric test. Where necessary, the Chi-square test or Fisher's exact test was used for the comparison of qualitative variables. Survival and event-free probability were studied using the Kaplan–Meier method and the <span class="elsevierStyleItalic">logrank</span> test. For the analysis of the impact of permanent cardiac pacing on mortality and hospital readmissions for heart failure, only the time from pacemaker implantation until the event occurred was taken into account. Cox regression was used for the univariate analysis. Statistically significant values were accepted as those with a p < 0.05. The data was collected, processed and analysed using the IBM SPSS ® v.25 software.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0105" class="elsevierStylePara elsevierViewall">For the analysis of baseline electrocardiographic characteristics and event predictor variables, a total of 58 registered cases of CA were included (20 patients with AL-CA and 38 patients with TTR-CA, and of these, 7 were hereditary —6 due to the Val50Met mutation, and one by deletion in the amino acid Val122—). The mean age at diagnosis was 73 ± 12 years, with a male prevalence (69%). More than half of the series started with symptoms of heart failure (52%), and more than half had preserved ejection fraction with left ventricular hypertrophy on echocardiography (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Patients underwent outpatient follow-up, with a mean follow-up of 39 ± 40 months from symptom onset. A total of 26 patients (44.8%) died during follow-up, and 10 (17%) required permanent pacemaker implantation.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Rhythm and impact on prognosis</span><p id="par0110" class="elsevierStylePara elsevierViewall">At the time of diagnosis, 37 patients (62%) were in SR, 21 (36%) had AF, and 11 developed AF during follow-up (18%). Atrial fibrillation at diagnosis was more prevalent in the TTR-CA type than in the AL-CA (47% vs. 15%; p = 0.01) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The presence of AF at the time of diagnosis of amyloid heart disease had no prognostic impact on the need for definitive pacing at follow-up (HR: 2.00; 95% CI: 0.51−7.93; p = 0.32).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Electrocardiographic pattern of pseudoinfarction and impact on the need for pacemakers</span><p id="par0115" class="elsevierStylePara elsevierViewall">At the time of CA diagnosis, 35 patients (60%) had a pseudoinfarction pattern on the baseline electrocardiogram. There was a higher prevalence of this electrocardiographic pattern in patients with AL-CA than in TTR-CA, but without reaching statistical significance (75 vs. 53%; p = 0.09) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). There was no difference in terms of need for pacing for patients with this electrocardiographic pattern (HR: 0.60; 95% CI: 0.15–2.36; p = 0.46) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Low electrical voltage and impact on the need for pacemakers</span><p id="par0120" class="elsevierStylePara elsevierViewall">At the time of CA diagnosis, 20 patients (35%) had low-voltage QRS complexes. In contrast, up to 13 patients (22%) met the Cornell criteria for ventricular hypertrophy. There was no difference in the frequency of presentation of this electrocardiographic pattern in both types of amyloidosis (29 vs. 45%; p = 0.22) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The low electrical voltage pattern of QRS complexes had no prognostic impact on the need for pacing (HR: 0.42; 95% CI: 0.08−2.18; p = 0.30) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conduction abnormalities and impact on prognosis</span><p id="par0125" class="elsevierStylePara elsevierViewall">At the time of diagnosis, up to 38 patients (66%) had some conduction abnormality on their baseline electrocardiogram (first-degree atrioventricular block, 18 patients [31%]; CRBBB, 12 patients [21%]; CLBBB, 3 patients [5%]; some type of hemiblock, 25 patients [46%]). There were no statistically significant differences in the distribution of these types of conduction abnormality in both types of CA. What was a differential feature was the QRS complex duration (ms), somewhat wider in patients with TTR-CA than in AL-CA (94 ± 23 vs. 89 ± 17; p = 0.03). On the other hand, up to 10 patients (17%) required permanent cardiac pacing for different reasons in long-term follow-up (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). This circumstance was a differential characteristic between both types of CA, more common in TTR-CA than in AL-CA, although statistical significance was not reached in our series (24 vs. 5%; p = 0.07) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Among the patients with TTRv-CA, only the patient with the Val122 deletion required a permanent pacing due to bifascicular block and nocturnal bradycardia on Holter.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">Regarding the analysis of predictors, the presence of any CBBB was associated with an increased need for pacing at follow-up (HR: 23.43; 95% CI: 4.09−134.09; p = 0.01), both the presence of a CLBBB (HR: 42.61; 95% CI: 4.53−401.63; p = 0.01), as well as the presence of a CRBBB (HR: 5.87; 95% CI: 1.34−25.63; p = 0.01). Likewise, the patients who required a permanent pacemaker during follow-up had a wider baseline QRS complex than those who did not (116 ± 21 vs. 88 ± 18; p = 0.03) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> and <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). In our series, the need for permanent cardiac pacing had no prognostic impact on mortality or hospital readmissions for heart failure (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0135" class="elsevierStylePara elsevierViewall">This study describes the most common electrocardiographic findings in patients with an established diagnosis of CA, either type AL or TTR. Although some authors<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> conclude that current electrocardiographic criteria are not sufficiently sensitive to identify CA, it is true that the electrocardiogram can be of great help in establishing an initial clinical suspicion, for subsequent established diagnosis with other diagnostic techniques.</p><p id="par0140" class="elsevierStylePara elsevierViewall">A diagnosis of CA has traditionally been considered when the surface electrocardiogram showed a low QRS voltage accompanied by evident signs of ventricular hypertrophy on transthoracic echocardiogram. This was mainly due to the higher proportion of diagnosed cases of AL-CA until a few years ago, a condition in which this electrical pattern is frequently described<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,12,15,16</span></a>. Since the advent of non-invasive nuclear medicine techniques for the diagnosis of TTR-CA without the need for endomyocardial biopsy<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–11</span></a>, the number of diagnosed cases of this disease has increased in recent years, which has led to a renewed focus on the electrocardiogram as a tool to make a possible differential diagnosis between the two types of amyloidosis. In our series, the pseudoinfarction pattern was more common than the low voltage pattern, with a similar distribution in both types of CA, AL-CA and TTR-CA, with the prevalence of these electrocardiographic patterns being highly variable in other similar published works (22–60% for the pseudoinfarction pattern and 27–84% for the low voltage pattern)<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–5</span></a>. On the other hand, the only differential electrical parameter between the two types of amyloidosis was the presence of atrial fibrillation, an arrhythmia more common in TTR-CA than in AL-CA. This higher prevalence of atrial fibrillation in patients with TTR-CA has also been described by other authors<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,14,15,17</span></a>, and it is probably due to the older age of patients with TTRwt-CA and to a slower and more insidious course of the disease, which would eventually lead to a remodelling with dilatation of the atrial cavities, with a higher incidence of this arrhythmia.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Another electrocardiographic finding of interest in the diagnosis of CA is that of conduction disturbances of any type, whether atrioventricular or intraventricular (bundle branch block and hemiblock). In our case, up to two thirds of the patients had some type of disturbance in any point of the conduction system, a figure somewhat higher than those reported in other series<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,12</span></a>, probably because in our case we have also considered the presence of hemiblocks, both anterior and posterior, as well as first-degree atrioventricular blocks; no differences were found between both types of CA. What is a distinguishing feature is the need for cardiac pacing, up to 9 times more common in our series in TTR-CA than in AL-CA, as in other published studies<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,12,18</span></a>. Thus, in patients who have heart failure with ventricular hypertrophy, and whose electrocardiogram shows conduction disturbances or the need for cardiac pacing, the suspicion of CA, especially TTR-CA, should be ruled out and the study should be continued. Therefore, the electrocardiogram, although not very sensitive for the diagnosis of CA, is an evidently useful tool for suspecting this infiltrative cardiomyopathy. On the other hand, in our series no patient with diagnosed CA had a strictly normal electrocardiogram (all patients had at least one electrocardiographic abnormality as mentioned above), although there are cases in CA series where they do have a normal electrocardiogram, although with low probability (0–10% in the series of Rapezzi et al.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>).</p><p id="par0150" class="elsevierStylePara elsevierViewall">Another potential utility of the electrocardiogram in the assessment of CA is its prognostic value, such that certain patterns have already been described as predictors of both mortality and the need for admission for heart failure. Specifically, low voltage measured by Sokolow criteria <15 mm<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> or QRS complex angle >102<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> have been described as prognostic variables for mortality. In our case, neither the presence of a low-voltage pattern nor the presence of pseudoinfarction was associated with the need for pacemaker implantation at follow-up.</p><p id="par0155" class="elsevierStylePara elsevierViewall">As far as conduction disorders are concerned, we have found some interesting findings. In our series, patients with CBBB, both those with CLBBB and those with CRBBB, have a higher incidence of needing a pacemaker during follow-up. Of the 10 patients (17%) who required a pacemaker during follow-up, almost all had some degree of BBB (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), as well as a wider baseline QRS complex than those who did not require permanent cardiac pacing, with a mean QRS value of 116 ms. Moreover, the devices were almost always implanted at least one year after diagnosis, demonstrating the insidious and gradual nature of amyloid deposition on the conduction tissue in the bundle of His. These data are in line with those published by authors who have studied electrophysiological findings in patients with CA, such that the analysis by Reisinger et al. and Barbhaiya et al. demonstrate that in patients with CA there is a prolonged HV interval (>55 ms), despite having a QRS complex that is not excessively wide, due to diffuse amyloid deposition and infiltration throughout the conduction system, creating an equal delay in right and left bundle branches of the bundle of His<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,21</span></a>. In our series only one patient was indicated for pacemaker implantation without conduction disturbances in his baseline electrocardiogram. Although the entire conduction system is affected in CA, atrioventricular conduction and the presence of blocks is more common than sinus node disease<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a>.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Regarding the indication for device implantation, the most recent published recommendation to implant a pacemaker in CA is given by the ACC/AHA guidelines<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a>, which recommends implanting it in the presence of advanced conduction abnormalities (class IIa recommendation, defined as Mobitz II second degree AVB, high-degree AVB, or third-degree AVB), without reference to other conduction disorders. Therefore, in the case of patients with CA who may have a CBBB, this eventuality should be taken into account when making decisions.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Although AV block has been linked to death in CA, in general pacing does not change the prognosis of the disease and is done primarily for symptom relief. In fact, there are reported series<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23,25</span></a>, in which the presence of a pacemaker was associated with worse survival in TTR-CA, probably related to the advanced stage of the disease. In our series, permanent pacemaker implantation had no impact on mortality or hospital readmissions for heart failure.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusions</span><p id="par0170" class="elsevierStylePara elsevierViewall">Electrocardiographic abnormalities in patients diagnosed with CA are heterogeneous, with the most common being the presence of conduction disturbances, pseudoinfarction pattern, and low voltage of the QRS complex. There is little baseline electrocardiographic differentiation between both types of CA, except for the presence of AF and the need for a pacemaker, which is more common in patients with TTR-CA. The objective of this study was to describe the most common basal electrocardiographic alterations in patients diagnosed with cardiac amyloidosis (CA) and to determine if these abnormalities have an impact on the need of pacemaker.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">This retrospective study included patients who had an established diagnosis of CA [light-chain cardiac amyloidosis (LA-CA) or transthyretin cardiac amyloidosis (TTR-CA)] between January 2013 and March 2021. The baseline heart rate, the percentage of patients with a pseudo-infarct pattern, low-voltage pattern or cardiac conduction disturbances, and the impact of these factors on the need of pacemaker were analysed.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">Fifty-eight patients with CA (20 with LA-CA and 38 with TTR-CA) were included, and the majority were male (69.0%). Twenty-one patients had atrial fibrillation (AF) at diagnosis. Thirty-five patients had a pseudo-infarct pattern, 35% had a low-voltage pattern, and 22% had criteria for ventricular hypertrophy. Two hirds had a conduction disorder: 18 patients with first degree atrioventricular block, 12 right bundle branch block, 3 left bundle branch block and 25 with a branch hemiblock. There were no differences between LA-CA and TTR-CA. Patients with TTR-CA had a greater need for pacemakers in the follow-up (39 ± 40 meses). Bundle branch block was a predictor of the need for a permanent pacemaker (HR 23.43, 4.09–134.09, IC 95%, p = 0.01).</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Electrocardiographic abnormalities in patients diagnosed wich CA are heterogeneus. Most frecuent is the presence of conduction disorders, the pseudoinfarction pattern, followed by the low voltage pattern. El objetivo fue describir qué alteraciones electrocardiográficas son más frecuentes en pacientes con AC, analizando su impacto en la necesidad de marcapasos.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Metodos</span><p id="spar0085" class="elsevierStyleSimplePara elsevierViewall">Estudio retrospectivo que incluye pacientes diagnosticados de AC por cadenas ligeras (AC-AL) y AC por transtirretina (AC-TTR), entre Enero-2013 y Marzo-2021. Se analizó el ritmo basal, el porcentaje con patrón de pseudoinfarto, bajo voltaje o alteraciones de la conducción; se analizó el impacto en la necesidad de marcapasos definitivo.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0090" class="elsevierStyleSimplePara elsevierViewall">Se incluyeron 58 pacientes con AC (20 AC-AL, 38 AC-TTR). Varones (69%), 21 (36%) tenían FA al diagnóstico. El 60% tenía patrón de pseudoinfarto, 35% bajo voltaje y un 22% tenían criterios de hipertrofia ventricular. Dos tercios tenían algún trastorno de conducción: Bloqueo Auriculoventricular de primer grado, 18 pacientes (31%); 12 bloqueo completo de rama derecha (BCRD), 3 bloqueo completo de rama izquierda (BCRI) y 25 con un hemibloqueo de rama. No hubo diferencias entre AC-AL y AC-TTR. Los pacientes con AC-TTR tuvieron mayor necesidad de marcapasos en el seguimiento (39 ± 40 meses).
