elsevierViewall">There is growing interest in the role of cholesterol metabolism in the pathogenesis of tuberculosis.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">1</span></a> Although <span class="elsevierStyleItalic">Mycobacterium tuberculosis</span> cannot produce steroids, it uses cholesterol from infected hosts.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">2</span></a> Before the effective combination therapy became widespread, low concentrations of total plasma cholesterol (TPC) have been described in patients with active pulmonary tuberculosis (TB), a finding that has also been reported recently.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">3,4</span></a> However, there are factors that have not been sufficiently considered and that can influence the levels of TPC in patients with TB. Age, sex, body weight, comorbidities and certain biochemical data are TPC determinants and should be considered. It should be noted that body weight is a confounding factor to be considered when assessing cholesterol in patients with tuberculosis.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">5</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The main objective of this study was to analyze the levels of TPC, and the factors influencing them in patients with tuberculosis infection – TB or latent tuberculosis infection (LTBI) – paying special attention to the role of active disease and body weight.</p><p id="par0015" class="elsevierStylePara elsevierViewall">We conducted an observational and retrospective study in a 950-bed public university hospital. All patients over 14 years of age consecutively diagnosed with TB and LTBI in our center (including outpatients and inpatients) for a period of 3 years were eligible as subjects of the study. Those patients who continued treatment with lipid-lowering drugs or were infected with HIV were excluded.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The clinical variables collected included age, sex, type of infection (TB or LTBI), previous history of diabetes mellitus, smoking, alcohol abuse, use of any lipid-lowering drugs, weight, height and body mass index (BMI). Laboratory data prior to initiation of treatment included white blood cell count (WBC), platelet count, TPC and total protein in plasma. The study protocol was approved by the local ethics committee, not requiring informed consent.</p><p id="par0025" class="elsevierStylePara elsevierViewall">A univariate analysis was performed to identify factors associated with TPC, as well as any significant differences between patients with TB and those with LTBI. The following tests were carried out: <span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span>, Fisher's exact, Student's <span class="elsevierStyleItalic">t</span>, Mann–Whitney's <span class="elsevierStyleItalic">U</span>, analysis of variance and correlation tests (Pearson's <span class="elsevierStyleItalic">r</span> or Spearman's rho) to study the factors associated with TPC. A stratified TPC level analysis was also conducted in patients with TB and LTBI according to BMI. Finally, a multiple linear regression model was built aimed to identify TPC level predictors; variables statistically associated with TPC levels in the univariate tests were introduced into the model; potential confounders and interactions between covariates were also considered. SPSS<span class="elsevierStyleSup">®</span> v.15 software was used.</p><p id="par0030" class="elsevierStylePara elsevierViewall">366 eligible patients were identified during the study period: 171 diagnosed with TB and 195 diagnosed with LTBI; 93 met exclusion criteria (52 in the TB group and 41 in the LTBI group), with 273 patients being finally included (119 and 154, respectively). Among patients with TB, 103 (87%) only had respiratory compromise. Most patients with LTBI had been detected while conducting a contact investigation after a recent exposure to a patient with active TB, but 45 patients in this group were diagnosed during screening of LTBI prior to the start of immunosuppressive therapy with biological agents for rheumatic diseases, inflammatory bowel disease or psoriasis.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Baseline conditions are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Several statistically significant differences were detected between patients with TB and those with LTBI; in particular, patients with TB had a lower BMI. As for laboratory data, TB patients had higher platelet, neutrophil and monocyte counts but lower lymphocyte counts. The average concentration of TPC was 153<span class="elsevierStyleHsp" style=""></span>mg/dl for patients with TB and 191<span class="elsevierStyleHsp" style=""></span>mg/dl for patients with LTBI.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The variables that showed a statistically significant association with TPC levels were: status of infection (TB or LTBI), age, sex, alcohol abuse, BMI and WBC (neutrophils and lymphocytes count). Similar results were obtained when the analysis was performed excluding patients with LTBI that were to undergo biological immunotherapy. All variables with a statistical association with TPC were entered as covariates in a linear regression model in the univariate analysis (i.e., sex, age, infection status, BMI, smoking, alcohol abuse and neutrophil and lymphocyte counts), and the interaction between BMI and the status of the infection. Infection status, BMI, age and sex showed a statistical association (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) with TPC levels. An interaction between the status of infection and BMI was detected. A stratified analysis of the cholesterol levels according to BMI was also conducted: differences persisted in patient subgroups with low weight (136 vs 184<span class="elsevierStyleHsp" style=""></span>mg/dl; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01), normal weight (150 vs 189<span class="elsevierStyleHsp" style=""></span>mg/dl; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01) and overweight (188 vs 218<span class="elsevierStyleHsp" style=""></span>mg/dl; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04).</p><p id="par0045" class="elsevierStylePara elsevierViewall">Compared with previous studies, our research provides data on cholesterol levels based on at least three influence factors – that means, weight (expressed as BMI), age and sex – and also takes into account other clinical and laboratory variables. As expected, after the completion of the multivariate analysis, we found that BMI, sex and age were factors independently associated to TPC levels. Most interesting, however, was to observe that the status of infection showed an association with TPC levels independently from these factors. Because of the association between the infection status and BMI, we considered the interaction factor (BMI*infection status) in the multivariate model, and performed a stratified analysis to assess the effect of infection on the TPC, according to the BMI group: cholesterol levels were lower in patients with TB in the underweight, normal weight and overweight subgroups. An interaction between the status of infection and BMI was detected, which means that the reducing effect of the infection status on TPC was progressively lower as BMI increased.</p><p id="par0050" class="elsevierStylePara elsevierViewall">In conclusion, we found that concentrations of TPC were lower in TB patients compared to patients with LTBI (after controlling for potential confounders and interactions).
