elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The incidence of osteomyelitis has been reduced by more than half in recent decades.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a> It is more common in children, especially in children under the age of 5.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Though methicillin-sensitive <span class="elsevierStyleItalic">Staphylococcus aureus</span><span class="elsevierStyleItalic">(S. aureus)</span> (MSSA) is responsible for 70–90% of confirmed cases,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">2</span></a> methicillin resistant <span class="elsevierStyleItalic">S. aureus</span> (MRSA) is increasing its frequency in recent years.<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The mortality rate is very low compared to the pre-antibiotic era.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> Certain factors, such as MRSA infection, determine a worse prognosis.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">There are gaps in the knowledge of the epidemiology of osteomyelitis in Spain and, to our knowledge, there is no study based on large series, such as that of admissions in all Spanish public hospitals for an extended period. With the hypothesis that bone infection continues to be a significant cause of morbidity and mortality, the main objective of the work has been to know the incidence and mortality of hospital admissions due to osteomyelitis in Spain.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">Information regarding patients hospitalized for osteomyelitis was collected from the Spanish minimum basic data set (MBDS). Spanish hospitals, especially the public ones, register the MBDS upon discharge of each patient treated.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a> The National Health System covers 99.5% of the Spanish population, and the MBDS receives reports from around 98% of Spanish public hospitals. The Spanish Ministry of Health, Social Services and Equality sets the standards for record keeping and conducts regular audits.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The diagnoses and procedures collected were coded until 2015 following the International Classification of Diseases, in its clinical modification (ICD 9-MC), allowing the grouping of the different care episodes attended by a hospital into diagnosis related groups (DRG),<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a> based on which costs are calculated. In Spain, the Ministry of Health prepares the costs for the National Health System based on a process for estimating DRG's hospital costs, obtained by analytical accounting systems in a sample of hospitals.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The study's design was subsequent to the generation of the data. The data for the study were extracted from the Spanish MBDS (Institute of Health Information, Ministry of Health, Consumption and Social Welfare (MSCBS), including, among others, the anonymized identification of the patient, autonomous community, date of admission, age, sex, length of hospital stay, hospitalization department, primary diagnosis and 13 secondary diagnoses, characteristics of hospital discharge (destination of patient or death) and cost. The data, in addition to being anonymized, were treated with absolute confidentiality according to the Spanish legislation and regulations of the MSCBS.</p><p id="par0040" class="elsevierStylePara elsevierViewall">An observational study of hospitalizations was carried out during the period 1997–2014, whose main diagnosis at hospital discharge in the MBDS was osteomyelitis. Patients whose primary diagnosis had been coded as 730 (osteomyelitis, periostitis and other bone-related infections), according to ICD 9-MC in the MBDS, were considered a “case”. Records of duplicate diagnoses due to readmissions or when entered as a secondary diagnosis were excluded. Codes 730.3 (periostitis without mention of osteomyelitis) and 730.7 (osteopathy resulting from poliomyelitis) were also excluded. 29,290 cases met the inclusion and exclusion criteria.</p><p id="par0045" class="elsevierStylePara elsevierViewall">As it is a passive population monitoring (search in a pre-set registry information system), the eligibility criteria were applied when the data was extracted from the ministry's MBDS.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The first osteomyelitis admission was taken into account to calculate the incidence. The incidence rate for osteomyelitis admissions in Spain was calculated based on the population register of the National Statistics Institute in 2006, half of the study period.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> The ages were distributed by groups, children (0–14), adults (15–64) and older (>64 years old).</p><p id="par0055" class="elsevierStylePara elsevierViewall">The main outcome variable was the patient's death during hospital admission. Only the last admission for osteomyelitis was considered for the calculation of patient mortality rates.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The presence of inconsistent or lost data in each variable was analyzed to exclude them from the analysis. The statistical analysis was performed using SPSS version 23 and Excel. Comparison between the values of 2 qualitative variables was made using the Chi square test. A significance level of less than 0.01 was accepted for all bivariate contrasts. For the multivariate analysis, the independent variables significantly associated with hospital death (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.1) were introduced in the multiple logistic regression model and analyzed with the “include” method. The adjusted <span class="elsevierStyleItalic">odds ratio</span> (adjusted OR) and 99% confidence interval were estimated.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">29,290 patients were detected with the main diagnosis of osteomyelitis; the type of progression, acute or chronic, was recorded in 17,423 cases (59.48%), and the location in 14,966 patients (51%). Patients admitted with osteomyelitis, but who had been coded as a secondary diagnosis, were not included.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Acute osteomyelitis prevailed among children (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Bone infection was more common in men (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The incidence of patients admitted for osteomyelitis was 3.85<span class="elsevierStyleHsp" style=""></span>cases/100,000 inhabitants and year (99% CI: 3.79–3.91). The incidence per 100,000 inhabitants and year was 5.02 cases in the paediatric age (0–14), 2.9 cases between 15 and 64 and 6.79 cases in those over 64. The incidence was higher in men in all age groups, 66.4% of cases (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), with a relative risk (RR) for women of 2.02 (99% CI: 1.96–2.09).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The incidence by autonomous communities was heterogeneous, as observed in the incidence map (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Chronic disease admissions, 52.4%, exceeded those of acute osteomyelitis, 47.6%. 60.1% of hospitalizations required surgical intervention. The average cost of admission was €5752.82, with an interquartile range of €3497.48; the evolution of the costs per admissions during the period 1997–2010 is shown in <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">60% of patients were admitted through the emergency department. 63.3% of emergency cases were acute, while elective admissions were, above all, patients with chronic osteomyelitis (75.25%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The 3 departments with the highest number of admissions were, in order of frequency, traumatology, paediatrics and internal medicine.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The lower limbs were affected in 78.3% of hospitalizations due to chronic infection, while the location in the upper limbs was mainly due to acute infection. The location was multiple in 1.2% of cases.</p><p id="par0095" class="elsevierStylePara elsevierViewall">27.2% of cases reported no comorbidities. The most common associated diagnoses were MSSA infection (10.5% of cases), followed by diabetes mellitus (8.13% of cases).</p><p id="par0100" class="elsevierStylePara elsevierViewall">504 cases died during hospitalization, 1.72% (99% CI: 1.5–1.9) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The death rate was higher in women (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), with a death RR of 1.7 compared to men (99% CI: 1.4–2.2); this higher mortality was modulated by age and determined by those over 64 years. Emergency admission mortality was higher than that of those with elective admission (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The mortality of patients over 64 years of age reached 4.65% and exceeded that of adults and children (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The rate of deaths with acute osteomyelitis significantly exceeded those admitted with a chronic course (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). Deaths were also significantly associated with the co-diagnosis of sepsis (27.12%) or MRSA infection (5.5% vs. 1.72% of MSSA infection) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The relative risk of in-hospital death was 18.03 when associated with a secondary diagnosis of sepsis and 3.23 with an MRSA infection.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">In the multivariate analysis (logistic regression), the ORs were adjusted for sex, age<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>64 years, comorbidity, diabetes, acute osteomyelitis, sepsis, staphylococcal infection and MRSA infection. The largest contribution to the risk of death, significantly (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01) to the multivariate analysis was the presence of septicaemia (adjusted OR 17.47), age over 64 (adjusted OR 8.04), presence of comorbidity (adjusted OR 3.73), MRSA infection (adjusted OR 2.32) and, with smaller effect, acute infection and female gender (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">There were no significant differences between the incidence of osteomyelitis and its mortality in the periods 1997–2005 and 2006–2014.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">This study offers estimates of the epidemiological characteristics of osteomyelitis admissions based on the Spanish population and shows that the incidence of osteomyelitis admissions in Spain is not negligible and that sepsis, age over 64 and MRSA infection are important risk factors for death. The pre-established hypothesis of the considerable morbidity and mortality of osteomyelitis is confirmed, and the analysis of mortality data suggests that it shares risk factors for death with other infectious diseases.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The main limitation in this study is the retrospective nature of the MBDS and the variability that may exist in the diagnostic criteria and in their coding in hospitals. The MBDS may also contain coding errors. On the other hand, some bone infections are not included in code 730 (osteomyelitis). The database does not include adequate information on the aetiology, pathophysiological mechanisms, nor does it allow us to know the progression after discharge. However, the analysis of a large number of hospitalizations for osteomyelitis, corresponding to most of the cases registered for 18 years, allows to obtain very adjusted data on Spanish hospital morbidity and mortality.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Bone infection requires hospitalization<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">9,10</span></a> in about 4 cases per 100,000 inhabitants and year in Spain, a figure similar to that found in previous Spanish studies of 2 different healthcare areas.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">11,12</span></a> The different incidence among the autonomous communities can be justified by diagnostic coding habits, although it could also be associated to differences between the risk factors of their population. However, the incidence rate of hospital admissions for osteomyelitis underestimates the frequency of the disease. Thus Kremers et al. found incidents of 21.8 cases per 100,000 inhabitants and year in the American state of Minnesota,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> and Riise found an annual incidence in children of 13 cases per 100,000 inhabitants in Norwegian population.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">The risk of osteomyelitis in the Spanish male population doubled the female population; male predominance has been previously documented.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">13–15</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">A bimodal distribution was observed with a peak at one year of age, and a progressive increase in cases, especially around 60 years of age, due to the existence of risk factors such as diabetes, surgery or trauma.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">16</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Children are mainly admitted due to acute osteomyelitis; Ceroni et al. think that this predominance may be favoured by activity-induced injuries.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">17</span></a> In adults, osteomyelitis is triggered by open fractures<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">18</span></a> and other factors such as the use of osteosynthesis material, joint prostheses or ischaemic ulcers due to diabetes.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">The 1.72% of in-hospital mortality due to osteomyelitis in Spain is lower than 4.5% of the total hospital admissions due to infectious diseases.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">12</span></a> Antimicrobials reduced mortality and surgical interventions.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> However, surgery remains essential for the treatment of certain types of osteomyelitis, such as chronic infection.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">Mortality rates remained high, around 33%, due to sepsis, until the introduction of penicillin and other antibiotics that drastically changed the prognosis of osteomyelitis.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> However, currently the association of sepsis raises the relative risk of in-hospital death by 17 times in Spain. In addition to sepsis, the multivariate analysis showed other risk factors for independent mortality such as age over 64, the presence of comorbidity, MRSA infection, acute infection and female gender.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Staphylococci are the predominant etiologic agents in bone infection.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">4,13</span></a> MSSA was responsible for most of these infections, with a low mortality rate; on the other hand, MRSA was the cause of fewer bone infections in Spain, but with a higher mortality. These infections have been increasing in recent years, finding geographical differences in different countries.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">This study, from a perspective of the Spanish population, represents an estimate of the characteristics of hospitalizations for osteomyelitis, which could be similar to those of other European countries with a population comparable to ours. Relative risk for males was 2.02.</p><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">In-hospital mortality was 1.72% (99% CI: 1.4–1.93).
