On examination there was a smooth swelling in the base of tongue in the midline (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). She was euthyroid and the ultrasound of the neck revealed absent thyroid gland in the neck. A provisional diagnosis of lingual thyroid was made, which was confirmed on [99mTc] sodium pertechnetate scan which showed increased 99mTc uptake in the base of tongue and no uptake in the neck. The mass was excised transorally and she was started Levothyroxine post operatively. She is asymptomatic at 6 months of follow up. Hickmann recorded the first case of lingual thyroid in 1869. It is caused due to aberrant embryogenesis during the descent of thyroid gland to the neck. Lingual thyroid is the most frequent ectopic location of thyroid gland and prevalence rates vary from 1 in 100,000 to 1 in 300,000. Majority of these patients are asymptomatic, however large lesions can cause symptoms like dysphagia, dyspnoea, foreign body sensation and sleep apnoea. Although most lesions require no treatment, larger symptomatic lesions should be surgically removed and patients started on thyroid replacement.
