A) Before chemotherapy, B) after the first cycle of antibiotic therapy, C) after the second cycle of chemotherapy and D) in follow-up after chemotherapy.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Mesenteric panniculitis refers to a continuum of fibroinflammatory diseases, including sclerosing mesenteritis and mesenteric lipodystrophy, affecting mesenteric adipose tissue.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> It is usually a subclinical or oligosymptomatic entity, diagnosed by imaging tests during the work-up of undiagnosed gastrointestinal involvement or other underlying disease. Neoplasms stand out among the diseases with which it is associated, accounting for 50–75% of cases of mesenteric panniculitis, as well as a 2–5 times greater likelihood of neoplasm in the presence of mesenteric panniculitis.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The most widespread theory is a possible paraneoplastic mechanism<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> and, although this hypothesis has not been tested and the chronological association between the two entities has not been established, in the specific case of non-Hodgkin's lymphoma half of the cases associated with mesenteric panniculitis could not be explained by direct invasion or lymphatic obstruction.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> To illustrate the difficult correlation of the two entities, we report a clinical case of worsening mesenteric panniculitis after completion of treatment for the underlying lymphoma and resolution of this disease, showing subsequent spontaneous improvement on successive imaging studies.</p><p id="par0010" class="elsevierStylePara elsevierViewall">A 46-year-old male, with no other history of interest except for follow-up in the outpatient urology department for the study of recurrent renal colic. A CT scan of the abdomen during the work-up revealed findings suggestive of mesenteric panniculitis. With this result, the Urology Department referred the patient to Systemic Diseases for further investigation.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The patient denies abdominal symptoms during history taking or evidence of systemic autoimmune disease, and the physical examination revealed no findings of interest. In previous laboratory studies and in subsequent extended tests, there was an absence of significant data, with acute phase reactants, general biochemistry, blood count, immunoglobulins, autoimmune profile (ANA, ENA, ANCA), ACE, calciuria and subpopulations with parameters in range; IGRA was negative. A PET-CT scan was performed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), showing multiple retroperitoneal and left inguinal hypermetabolic lymph nodes with no other findings compatible with tumour activity, suggestive of granulomatous mesenteric panniculitis without being able to rule out slow-growing lymphoma. In view of these findings, a lymph node biopsy of the inguinal lymphadenopathy was performed and reported as grade 1–2 follicular B lymphoma.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">With the diagnosis established, the patient was referred to haematology and chemotherapy was started with R-CHOP for 3 cycles. At the end of these, a control PET-CT scan was requested, which showed hypermetabolic areas in the abdomen corresponding to possible mesenteric panniculitis, with lymph node lesions disappearing. On suspicion of non-response, three new cycles of R-CHOP were administered. At the end of these, the control PET-CT scan showed an increase in the number, size and metabolic activity rate of the lesions in the abdomen compatible with recurrence of follicular lymphoma or panniculitis, suggesting functional progression of the disease/no response (Deauville 5).</p><p id="par0025" class="elsevierStylePara elsevierViewall">Haematology considers the lymphoma to be in remission with these findings, leaving rituximab maintenance every 2 months for 2 years. Given the lack of response to the established treatment, the inflammatory aetiology of the persistent symptoms is considered unlikely. A biopsy was considered, but the uptake area was inaccessible for CT-guided biopsy and the only option was laparoscopy or laparotomy. It was agreed with the patient, given that he persisted without general symptoms and only reported very unspecific digestive discomfort, to continue monitoring by means of PET-CT. The six-month follow-up (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) showed a decrease in the number, size and rate of metabolic activity of the lesions described in the abdomen, indicating a partial metabolic response (Deauville 4/5). Control laboratory tests persist without significant findings.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The association between lymphoma and mesenteric panniculitis is well established in the literature. The incidence of panniculitis in non-Hodgkin's lymphoma is approximately 1.6%, similar to the general population.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> However, in patients with abdominal lymphoma, this proportion rises to 28–33%. Of these patients, approximately half appear at the time of diagnosis, and the other half throughout the course of the disease.
