id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Thrombotic microangiopathies (TMA) constitute a heterogeneous group of diseases of high diagnostic complexity whose common denominator is the endothelial lesion with formation of thrombi in microcirculation that leads to tissue ischaemia with systemic clinical consequences dependent on the organs involved. They can be potentially very serious and all of them have thrombocytopenia and microangiopathic haemolytic anaemia (MHA) in common. It is characterized by the detection of schistocytes (fragmented erythrocytes) in blood smear and a negative direct Coombs (DC) test.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The classification of these processes is complex, since the diverse entities that present with TMA can be inherited or acquired, can affect children or adults, can begin acutely or subacutely and have different prognosis and treatment. Also, this classification has often been based on clinical and pathogenic criteria that may require specialized studies in reference laboratories with results that are not available until days or months after the initial suspicion. However, the overlapping of clinical and laboratory findings between the different entities that cause TMA, especially in the first hours of its development, makes it necessary to address these diseases jointly,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">4</span></a> allowing a fast-therapeutic action that will, in most cases, determine the vital prognosis of the patient.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1,2,5,6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">According to the aetiology of the process, two groups have been identified, whose limits are not well defined:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">•</span><p id="par0020" class="elsevierStylePara elsevierViewall">Primary TMA: of well-known pathophysiology on which the treatment is based and which include the different varieties of thrombotic thrombocytopenic purpura (TTP) and haemolytic uraemic syndrome (HUS).</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">•</span><p id="par0025" class="elsevierStylePara elsevierViewall">Secondary TMA: associated with an identified etiological agent (drugs, infections, neoplasms, transplantation, nephropathies, autoimmune diseases, etc.) whose treatment consists mainly of controlling the underlying causes.</p></li></ul></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Guidelines rationale and objectives</span><p id="par0030" class="elsevierStylePara elsevierViewall">TMA in general, but especially the primary one, is a vital emergency that requires rapid diagnosis and adequate treatment which determine the progression of patients. Given the difficulty of obtaining an established diagnosis in the first hours, to begin with, they must be globally addressed as a <span class="elsevierStyleItalic">microangiopathic haemolytic anaemia and thrombocytopenia</span> (MHAT) syndrome until an established diagnosis is available<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">3</span></a> (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). One of the most common problems is the delay between the moment the patient starts receiving medical attention until diagnosis of TMA, which entails the delay of the treatment and sometimes irreversible microangiopathy complications. Given the clinical-biological suspicion of a severe MHAT syndrome, it is indicated to start plasma exchange (PE) as soon as possible, and it is advisable to carry it out in the first hours after the arrival of the patient in the emergency department, which improves the response and, therefore, the prognosis of this disease.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1–7</span></a></p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">There are guidelines focused on the study of diagnosis and treatment of TMA,<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1,2,8</span></a> but there are no specific guidelines for emergency treatment during the first hours after being suspected. In this review we provide a series of recommendations aimed specifically at optimizing the initial approach to these conditions that serve as a basis for specialists regarding the diagnosis and urgent treatment of patients with TMA. <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows the steps to follow in the diagnosis and treatment of TMA that will serve as guidelines in this protocol.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Objectives</span><p id="par0040" class="elsevierStylePara elsevierViewall"><ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">•</span><p id="par0045" class="elsevierStylePara elsevierViewall">Rapid and correct TMA study completion during the emergency period.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">•</span><p id="par0050" class="elsevierStylePara elsevierViewall">Provide recommendation guidelines for the early identification and urgent treatment of patients with suspected TMA.</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">•</span><p id="par0055" class="elsevierStylePara elsevierViewall">Organize the extraction of samples that speed up the final etiological diagnosis.</p></li></ul></p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methodology</span><p id="par0060" class="elsevierStylePara elsevierViewall">In order to tackle the TMA more agile and efficiently in our centre, the Thrombotic Microangiopathy Group of the La Fe University and Polytechnic Hospital was created. The working group initially involved in the preparation of the guidelines was composed of specialists in haematology, paediatrics, intensive medicine, internal medicine, nephrology, hospital emergency service physicians and microbiology. To develop and evaluate the methodological quality of this consensus document, the authors followed the criteria established in the AGREE<span class="elsevierStyleHsp" style=""></span>II tool (<a class="elsevierStyleCrossRef" href="#sec0100">Annex 1</a>).<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">9</span></a> A literature search was carried out aimed at the emergency treatment of TMA in Pubmed using the key terms “thrombotic microangiopathy”, selecting the most relevant literature published until September 2017. The authors analyzed all the information collected and established a series of recommendations based on the evidence obtained. To classify the quality of the literature reviewed, the <span class="elsevierStyleItalic">Grading of Recommendations Assessment, Development and Evaluation</span> (GRADE) system was used (<a class="elsevierStyleCrossRef" href="#tbl0040">Table 1</a>).<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">10,11</span></a> Of the areas in which no scientific evidence was found, consensus recommendations were established based on the clinical experience of the different authors.</p><elsevierMultimedia ident="tbl0040"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Step 1. Diagnostic suspicion</span><p id="par0065" class="elsevierStylePara elsevierViewall">The non-specificity and heterogeneity in the clinical presentation of patients with TMA determine the initial diagnosis, including paucisymptomatic patients and cases of extreme severity. History-taking and physical examination should include the evaluation of symptoms or warning signs (neurological, gastrointestinal, renal, cardiac, ocular, fever, asthenia, haemorrhagic diathesis, etc.), the time elapsed since onset of symptomatology and the consideration of possible diseases or causal agents that could direct towards a secondary TMA.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The results observed in the first laboratory tests in the complete blood count are: anaemia and thrombocytopenia of variable intensity, with normal or discreetly elevated leucocyte count. In serum biochemistry, an increase in lactate dehydrogenase (LDH) enzyme and bilirubin (at the expense of the indirect one) and, to a greater or lesser extent, alterations in renal function parameters (increased creatinine and urea). Haemostasis tests do not usually show relevant changes.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The detection of anaemia and thrombocytopenia of uncertain cause implies completing the laboratory tests to rule out TMA.<elsevierMultimedia ident="tb0005"></elsevierMultimedia></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Step 2. Syndromic confirmation</span><p id="par0110" class="elsevierStylePara elsevierViewall">Given the initial suspicion of TMA, the initial laboratory tests should be expanded, quickly processed and completed. In biochemistry it is necessary to determine LDH and bilirubin (including the indirect fraction), which will probably be elevated and should be included in all subsequent lab tests monitoring. In order to determine the origin (central or peripheral) of anaemia, a reticulocyte count is recommended, as these are usually increased in these cases. The haemostasis study should include D-dimers, which may also be elevated. The DC must be negative, although a weak positive result does not rule out the diagnosis of TMA. Peripheral blood smear is essential to rule out other possible causes of cytopenia and allows microangiopathy to be confirmed by the specific evaluation of the red series with detection and quantification of schistocyte percentage and morphological confirmation of thrombocytopenia<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">12</span></a> (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). A proportion higher than 1% of schistocytes is highly suggestive of TMA.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">13,14</span></a> An electrocardiogram should be performed, and ultrasensitive troponins and pro-BNP should be determined in serum biochemistry for the assessment of cardiac involvement. In the urinalysis (sediment and abnormalities), haematuria and proteinuria can be observed. If there is a recent history of diarrhoea, a stool sample should be processed for the determination of Shiga toxin. Finally, if the patient is of childbearing age, a pregnancy test should be performed to guide a TMA associated with gestation and for the evaluation of subsequent therapeutic processes.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">The integrated evaluation of the targeted history-taking, the patient's symptoms and the laboratory results allow the confirmation of suspected diagnosis of TMA and ruling out secondary causes. The lack of available results that could guide an etiological diagnosis, as well as the overlap in the clinical presentation of these entities make a joint MHAT syndrome approach necessary.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">3</span></a> If a MHAT syndrome is suspected (<a class="elsevierStyleCrossRef" href="#tbl0045">Table 2</a>), on-call haematology and/or nephrology specialists should be contacted for a joint assessment of the patient and urgently initiate PE and/or associated dialysis in case of acute renal failure.<elsevierMultimedia ident="tb0010"></elsevierMultimedia></p><elsevierMultimedia ident="tbl0045"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Step 3. Emergency treatment</span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Clinical symptoms of presentation (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>)</span><p id="par0135" class="elsevierStylePara elsevierViewall">Most cases of MHAT start with severe symptoms and/or cytopenia. In these patients, PE should be performed as soon as possible in hospitals equipped with apheresis units (AU), contacting the nursing staff on duty who will schedule and organize the first PE session. Simultaneously, the blood product needs to carry out the procedure will be estimated in the corresponding blood bank and/or transfusion centre. In the case of not having personnel to carry out PE, the reference hospital should be contacted for the urgent transfer of the patient.</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">There is a small group of patients who initially show mild MHAT with little clinical consequences, absence of serious cytopenia, slight increase in haemolysis parameters and scarce presence of schistocytes in peripheral blood smear. These cases should be closely monitored with a clinical-biological evaluation every 4–6<span class="elsevierStyleHsp" style=""></span>h that allows, in case of deterioration, to carry out PE immediately.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The initial treatment with PE should be performed in the intensive care unit, which allows a close monitoring of the patient during the first hours and days of TMA progression.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">15,16</span></a></p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Indication of urgent plasma exchange</span><p id="par0150" class="elsevierStylePara elsevierViewall">Syndromic diagnosis of MHAT entails carrying out urgent PE<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1,2,8,17</span></a> in symptomatic cases or with severe cytopenia (platelets <30<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/l and/or haemoglobin <10<span class="elsevierStyleHsp" style=""></span>g/dl or decrease of 2<span class="elsevierStyleHsp" style=""></span>g/dl),<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">18</span></a> even when it comes to pregnant patients. The early detection of these syndromes and the rapid onset of PE are essential in the progression and prognosis of the patient. It is recommended to perform PE in the first 4–8<span class="elsevierStyleHsp" style=""></span>h after the patient's arrival in the emergency department.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">1</span></a> In cases where there is a high suspicion of atypical HUS due to a significant renal involvement, the initial treatment during the emergency should be PE, which will be maintained until the etiological diagnosis is reached, which will allow the initiation of anticomplement therapy with eculizumab.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">19–22</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">Although plasma infusion may provide a temporary benefit until the completion of PE,<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">23</span></a> it should be a priority to send the patient as quickly as possible to a hospital centre that has an AU.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">16</span></a> The patient will only be infused with plasma at a dose of 25–30<span class="elsevierStyleHsp" style=""></span>ml/kg/day<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1,8,24,25</span></a> if the delay in the start of PE is inevitable. In this situation, the extraction of peripheral blood samples for the detection of ADAMTS13 (<span class="elsevierStyleItalic">A Disintegrin And Metalloproteinase with ThromboSpondin-1 motifs, 13rd member of the family</span>) and the study of the complement system should be performed before plasma administration. The samples, correctly identified, must be sent to the reference hospital together with the patient.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The subtypes of TMA in which PE is not indicated are: congenital TTP (Upshaw-Schulman syndrome), which is indistinguishable from that acquired in the initial stages of diagnosis until the activity levels of ADAMTS13 and inhibitory antibody titres are available; HUS secondary to STEC (<span class="elsevierStyleItalic">Shiga-like toxin-producing E. coli</span>) infection and HUS associated with pneumococcal infection; atypical HUS due to genetic variants of MPC (transmembrane protein), and TMA associated with disseminated neoplasms, with chemotherapy or hematopoietic stem cell transplantation, whose treatment, in general, aims to control the triggering cause.<a class="elsevierStyleCrossRefs" href="#bib0320"><span class="elsevierStyleSup">26,27</span></a> Some TMA conditions secondary to vasculitis with antineutrophil cytoplasmic antibodies (ANCA) and systemic lupus erythematosus (SLE) with pulmonary involvement may occasionally require urgent treatment with PE<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">28</span></a> due to poor and rapid progression, requiring individualized assessment of each case.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Before starting urgent plasma exchange</span><p id="par0165" class="elsevierStylePara elsevierViewall">As previously mentioned, once the decision to perform PE or close monitoring of patients in mild cases has been made, it is essential to extract adequate samples of peripheral blood before transfusing blood products or administering immunosuppressive drugs.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Samples for etiological confirmation:<ul class="elsevierStyleList" id="lis0025"><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">•</span><p id="par0175" class="elsevierStylePara elsevierViewall">Peripheral blood extraction in two citrate tubes of a minimum of 2.7<span class="elsevierStyleHsp" style=""></span>ml for the study of the activity and the titre of ADAMTS13 inhibitors, which will be kept refrigerated at 4<span class="elsevierStyleHsp" style=""></span>°C until they can be submitted to the reference laboratory. In the case of paediatric patients, the volume of extraction can be reduced to two 1.8<span class="elsevierStyleHsp" style=""></span>ml citrate tubes. When the study of the sample may take more than 48<span class="elsevierStyleHsp" style=""></span>h, the blood will be centrifuged, and the plasma will be separated and frozen at −80<span class="elsevierStyleHsp" style=""></span>°C.</p></li><li class="elsevierStyleListItem" id="lsti0040"><span class="elsevierStyleLabel">•</span><p id="par0180" class="elsevierStylePara elsevierViewall">Extraction of 5–10<span class="elsevierStyleHsp" style=""></span>ml of blood in a tube with EDTA and 10<span class="elsevierStyleHsp" style=""></span>ml in a dry tube without gel for genetic and autoimmune study of the complement system. The sample will be immediately centrifuged for 10<span class="elsevierStyleHsp" style=""></span>min to 3000<span class="elsevierStyleHsp" style=""></span>G at a temperature of −8<span class="elsevierStyleHsp" style=""></span>°C, the plasma will be separated from the EDTA tube and the serum from the dry tube and stored frozen at −80<span class="elsevierStyleHsp" style=""></span>°C together with the sedimented material (<span class="elsevierStyleItalic">pellet</span>) and the clot of the original tubes until delivery, if applicable.</p></li><li class="elsevierStyleListItem" id="lsti0045"><span class="elsevierStyleLabel">•</span><p id="par0185" class="elsevierStylePara elsevierViewall">Request viral serologies (HAV, HBV, HCV, HIV, EBV, CMV, parvovirus, and syphilis) before transfusing blood products to rule out specific types of secondary TMA.</p></li><li class="elsevierStyleListItem" id="lsti0050"><span class="elsevierStyleLabel">•</span><p id="par0190" class="elsevierStylePara elsevierViewall">If there is gastrointestinal involvement with diarrhoea, request an urgent Shiga toxin test, the result of which will be available in 1–2<span class="elsevierStyleHsp" style=""></span>h from the reception of the sample in the reference laboratory. If it is positive, it can support the diagnosis of HUS associated with STEC infection and initially contraindicate the urgent PE. The test should preferably be performed before administering antibiotics in a fresh stool sample (not in a swab).</p></li><li class="elsevierStyleListItem" id="lsti0055"><span class="elsevierStyleLabel">•</span><p id="par0195" class="elsevierStylePara elsevierViewall">If it has not been previously requested, a pregnancy test will be carried out on patients of childbearing age.</p></li></ul></p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Plasma exchange</span><p id="par0200" class="elsevierStylePara elsevierViewall">PE should be performed by AU staff, using fresh frozen plasma as a replacement solution and exchanging 1–1.5 plasma volumes in each session. It is recommended to avoid the inactivated plasma with methylene blue due to its lower effectiveness.<a class="elsevierStyleCrossRefs" href="#bib0335"><span class="elsevierStyleSup">29,30</span></a> A daily session will be carried out until the normalization of the haemolysis parameters and until reaching a platelet count >150<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/l for 2 consecutive days.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Other therapeutic measures</span><p id="par0205" class="elsevierStylePara elsevierViewall">Most patients with MHAT syndrome who require urgent PE will be subsequently diagnosed with acquired TTP. Therefore, it is recommended to start corticosteroids at high doses of 1<span class="elsevierStyleHsp" style=""></span>mg/kg/day of methylprednisolone intravenously (IV), divided into three times a day.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1,8,31</span></a> In cases with severe clinical presentation (e.g., neurological), some authors recommend an initial megadose of 1<span class="elsevierStyleHsp" style=""></span>g of methylprednisolone IV during the first 3 days.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">1,8,31,32</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">The transfusion of packed red blood cells should follow the usual criteria of indication according to the symptoms and laboratory results of the patient. In case of platelet transfusion, although in general it should be avoided, it is not contraindicated.<a class="elsevierStyleCrossRefs" href="#bib0355"><span class="elsevierStyleSup">33–38</span></a> In case of clinical situations of serious haemorrhagic risk, such as active bleeding or the performance of an invasive procedure, the administration will be considered individually, assessing the risks and benefits of the patient.<elsevierMultimedia ident="tb0015"></elsevierMultimedia></p></span></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Step 4. Complementary studies</span><p id="par0270" class="elsevierStylePara elsevierViewall">During the emergency, it is not a priority to identify the specific TMA entity to initiate PE, but complementary tests should be requested to establish the etiological diagnosis as soon as possible.</p><p id="par0275" class="elsevierStylePara elsevierViewall">The ordinary complementary tests must include all the laboratory determinations necessary to complete the study and TMA monitoring: complete blood count with reticulocytes and peripheral blood smears with assessment of morphology; complete immunohematological study; levels of haptoglobin in serum and free haemoglobin in plasma; serum total protein test; viral serologies; influenza A and B in nasal discharge (according to time of year and suspected etiological diagnosis); study of autoimmunity with antinuclear antibodies (ANA), antibodies against extractable nuclear antigens (ENA), antineutrophil cytoplasmic antibodies (ANCA), rheumatoid factor (RF) and cryoglobulins; laboratory parameters of antiphospholipid syndrome, including lupus anticoagulant (LA), anticardiolipin and anti-β<span class="elsevierStyleInf">2</span>-glucoprotein antibodies; study of C3 and C4 fraction of the complement system; vitamin B<span class="elsevierStyleInf">12</span>, folic acid and homocysteine; study of methylmalonic acidaemia (if applicable under clinical suspicion), and other additional studies according to the diagnostic approach.</p><p id="par0280" class="elsevierStylePara elsevierViewall">Based on the results of the ADAMTS13, processing the sample prior to PE for the immunological and genetic study of the complement can be considered.<elsevierMultimedia ident="tb0020"></elsevierMultimedia></p></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Conclusions</span><p id="par0350" class="elsevierStylePara elsevierViewall">TMAs are a medical emergency with serious clinical consequences that require the coordination of the different specialists involved in the care of these patients in order to guide the diagnosis and initial treatment as soon as possible. The main objective in the emergency is not to establish the etiological diagnosis or to differentiate between TTP and atypical HUS, but to arrive at the diagnosis of an MHAT syndrome that allows rapid initiation of PE. These guidelines are intended to provide a series of recommendations to guide the physician in the treatment of this type of patient during the first hours in the emergency services, summarized together with the quality of the evidence and the degree of recommendation in <a class="elsevierStyleCrossRef" href="#tbl0050">Table 3</a>. The different steps included in it are aimed at helping in the identification and urgent treatment of patients with severe cases of TMA, favouring the adequate extraction of samples that speed up the final etiological diagnosis of these entities.</p><elsevierMultimedia ident="tbl0050"></elsevierMultimedia><p id="par0355" class="elsevierStylePara elsevierViewall">An additional objective is to disseminate the importance of the multidisciplinary approach of these diseases supported by specific work groups, such as the one established in our hospital, which has made possible to optimize the treatment of these patients.</p></span><span id="sec0095" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0155">Conflict of interests</span><p id="par0360" class="elsevierStylePara elsevierViewall">Drs. Its management during its initial hours is essential to improving the prognostic of these patients. The aim of this review is to give recommendations about the optimization of TMA initial treatment and to accelerate the aetiological diagnosis.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Patients and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">We provide a practice guideline based on four steps for the initial management of TMA: diagnosis of suspicion, syndromic confirmation, emergent treatment and complementary tests.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The detection of microangiopathic haemolytic anaemia (characterized by elevated reticulocytes, LDH and indirect bilirubin, negative direct Coombs test and schistocytes in peripheral blood), and thrombocytopaenia not explained by other secondary aetiologies confirm the syndromic diagnosis of microangiopathic haemolytic anaemia and thrombocytopaenia (MAHAT). These patients require admission to an Intensive Care Unit to initiate plasma exchange therapy as soon as possible, ideally within the first 4–8<span class="elsevierStyleHsp" style=""></span>h. Prior to this, samples for ADAMTS13 and complement study should be obtained. Estos pacientes requieren ingreso en la unidad de cuidados intensivos para iniciar lo antes posible el recambio plasmático, preferiblemente en las primeras 4–8<span class="elsevierStyleHsp" style=""></span>h. Antes de realizar el recambio plasmático deben extraerse las muestras para el estudio de ADAMTS13 y de complemento.
