class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Fabry disease (FD) is an X chromosome-linked disease caused by a deficiency in the activity of the lysosomal enzyme alpha-galactosidase A (α-GAL-A), which results in a pathological deposition of glycosphingolipids, mainly globotriaosylceramide, in the vascular endothelium and other cell types (podocytes, cardiomyocytes, etc.), as well as an increase in the plasma concentration of globotriaosylsphingosine.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The most commonly affected organs are the heart, the vascular endothelium of the kidney, the nervous system, the eyes and the skin. The classical or severe form is due to a α-GAL-A deficiency below 1% of its normal value, while partial defects (1%–30%) produce incomplete forms with predominantly cardiac and/or renal involvement,<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> with left ventricular hypertrophy (LVH) being the most common cardiac manifestation.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4</span></a> Simultaneous cardiac and renal involvement (cardiorenal syndrome) is common and significantly complicates the prognosis of these patients.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Because of its inheritance pattern, it has classically been considered a male disease, with females being asymptomatic carriers. However, women may be affected at a later age and manifest atypical forms with normal enzyme activity.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">FD is considered a rare disease with an incidence of between 1/40,000 and 1/240,000 males.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–9</span></a> However, the actual incidence is unknown. Neonatal screening programmes suggest a higher incidence that could range from 1/3100 to 1/4100 for late-onset forms and 1/37,000 births for the classical phenotype.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10–12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Previous screening studies have studied the prevalence of FD in different clinical scenarios with structural heart disease, ranging from 0.5% to 12%.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13–15</span></a> Furthermore, a prevalence of FD in patients with kidney disease has been reported from 0.2% in moderate grades to 0.5% in patients on dialysis.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–20</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">To date, no screening studies have been conducted in patients with cardiac involvement and concomitant renal disease.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Therefore, the aim of the present study was to determine the prevalence of FD in patients with LVH and any degree of chronic kidney disease (CKD).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0030" class="elsevierStylePara elsevierViewall">This is a cross-sectional observational study. Patients of both sexes diagnosed with LVH according to the European Association of Echocardiography consensus document<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> (interventricular septum and/or posterior wall thickness in end-diastole ≥ 13 mm) and CKD according to the Kidney Disease: Improving Global Outcomes guidelines<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> (impaired renal structure or function for more than 3 months, with G1–G5 glomerular filtration rate categories and/or albuminuria A1–A3), who were able to give written informed consent, were included.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Patients who could not meet the requirements set out in the protocol (due to limitations in understanding written language) were excluded.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Sociodemographic variables (age, sex, family history), as well as data on other comorbidities causing LVH (arterial hypertension, aortic stenosis, amyloidosis, hypertrophic cardiomyopathy) and FD target organ involvement were collected in all patients included.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The study was approved by the relevant Ethics Committee, and patients had to give informed consent to participate in the registry.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Fabry disease study</span><p id="par0050" class="elsevierStylePara elsevierViewall">A blood sample was obtained at the baseline visit, using the dried blood spot technique.</p><p id="par0055" class="elsevierStylePara elsevierViewall">All FD-specific laboratory determinations were carried out at Centogene GmbH (Rostock, Germany) following the established workflow: in male probands, the activity of α-GAL-A was measured by fluorimetry and, if below the laboratory reference limit (Normal: ≥15.3 μmol/l/h), the coding region of the gene <span class="elsevierStyleItalic">GLA</span> (NM_000169.2) using Next Generation Sequencing (Illumina); in female probands, the coding region of the gene <span class="elsevierStyleItalic">GLA</span> was sequenced using Next Generation Sequencing (Illumina) and gene rearrangements were studied using the multiplex ligation-dependent probe amplification (MLPA) technique.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The diagnosis of FD was established on the basis of the following criteria<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a>:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0065" class="elsevierStylePara elsevierViewall">Presence of a pathogenic variant in the gene GLA.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0070" class="elsevierStylePara elsevierViewall">AND at least one of the following:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">o</span><p id="par0075" class="elsevierStylePara elsevierViewall">Presence of one or more FD characteristic signs (cornea verticillata, angiokeratomas or acroparaesthesias).</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">o</span><p id="par0080" class="elsevierStylePara elsevierViewall">Presence of a family member with a definite diagnosis of FD and presence of the same pathogenic variant in the gene <span class="elsevierStyleItalic">GLA</span>.</p></li></ul></p></li></ul></p><p id="par0085" class="elsevierStylePara elsevierViewall">In case of a pathogenic variant in the gene <span class="elsevierStyleItalic">GLA</span> and not fulfilling the other criteria, the diagnosis of FD was considered uncertain and a determination of globotriaosylsphingosine levels (lyso-Gb3) or histological study (of the affected organ or skin) was performed at the discretion of the responsible physician.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0090" class="elsevierStylePara elsevierViewall">The SPSS statistical software® (version 21.0; SPSS, USA) was used for data analysis.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Statistical analysis was carried out using a multivariate approach to examine key associations related to FD. Demographic variables, such as age, gender, weight, height and originating research centre, were included as covariates to control for potential confounding factors. The distribution of continuous variables was assessed by normal distribution tests and, where necessary, appropriate transformations were applied. Chi-square tests for categorical variables and Student's <span class="elsevierStyleItalic">t</span>-test for continuous variables were used to compare clinical characteristics between groups. The prevalence of FD in the sample and the 95% confidence interval (CI) were calculated. Statistical significance was established at a level of <span class="elsevierStyleItalic">P</span> < .05.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0100" class="elsevierStylePara elsevierViewall">From October 2018 to July 2022, 898 patients from 29 Spanish hospitals were included. The socio-demographic and clinical characteristics of the patients are detailed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and stratified by gender. The majority of patients were male (605 [68.4%]), with a mean age of 69 ± 17 years.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Cardiovascular manifestations were common, 433 patients (48.2%) had cardiovascular disease and 414 (46.1%) had heart failure, with the majority of these patients (350 [80.8%]) being in functional class II or III. 51.2% of patients were in sinus rhythm, with some degree of atrioventricular block in 162 (18%) and repolarisation abnormalities in 188 patients (20.9%). The presence of ventricular arrhythmias was uncommon (1.6%).</p><p id="par0110" class="elsevierStylePara elsevierViewall">For renal involvement, the mean glomerular filtration rate was 44.5 ± 26.6 ml/min/1.73 m<span class="elsevierStyleSup">2</span>. Two hundred and three patients (22.6%) had some degree of albuminuria and 100 patients (11.1%) received renal replacement therapy, most commonly haemodialysis (51%) and renal transplantation (35%).</p><p id="par0115" class="elsevierStylePara elsevierViewall">The concomitant presence of heart failure and renal involvement (chronic cardiorenal syndrome) occurred in 366 patients (40.8%), affecting 39.7% of men and 43% of women.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Other manifestations were common, with peripheral nervous system involvement in the form of acroparaesthesia (45.7%), gastrointestinal manifestations (altered bowel habits or abdominal pain, 13.8%) and cerebrovascular disease (12.3%), mostly ischaemic strokes (89%).</p><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Fabry disease study</span><p id="par0125" class="elsevierStylePara elsevierViewall">The laboratory analysis of the FD was performed according to the described protocol.</p><p id="par0130" class="elsevierStylePara elsevierViewall">α-GAL-A enzyme activity was determined in male patients, with a mean of 22.68 ± x5_"14.52 μmol/l/h. Of these, 113 were found to have enzyme activity below the established normal limit, and therefore proceeded to genetic testing.</p><p id="par0135" class="elsevierStylePara elsevierViewall">A total of 406 patients (113 men and 293 women) were tested for the GLA gene, of which 13 (3.2%) could not be tested because of insufficient sample. The results of the genetic study are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">In 6 patients (0.66%, 95% CI 0.24%–1.45%) (3 males and 3 females, mean age 74 ± 7 years), the p.Asp313Tyr variant was found, which is currently considered of uncertain significance.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> The mean enzyme activity of males with this variant was 9.3 ± 1.9 μmol/l/h. With respect to the general study population, the high rate of stroke (2 patients, 33.3%) and vestibulocochlear involvement (4 patients, 66.6%) is noteworthy. None of these patients had peripheral nervous system involvement or classical symptoms of FD (angiokeratomas, cornea verticillata).</p><p id="par0145" class="elsevierStylePara elsevierViewall">The diagnosis of FD was confirmed in 3 patients, a prevalence of 0.33% (95% CI 0.06%–1%). Their characteristics are set out in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The late presentation in these patients is noteworthy. All patients had electrocardiographic abnormalities. Other organ involvement was common, especially dermatological and peripheral nervous system involvement, with no cerebrovascular involvement found in these patients. All patients showed classic signs of FD, fulfilling the criteria described for diagnosis, and therefore no additional tests (lyso-Gb3 determination or histological study) were performed.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0150" class="elsevierStylePara elsevierViewall">This cross-sectional study explores for the first time the prevalence of FD in patients presenting with both LVH and CKD. A prevalence of 0.33% (95% CI: 0.06%–1%) was found in this group of patients.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The prevalence found is significantly higher than previously reported in general population newborn screening programmes.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10–12</span></a> Therefore, 2 simple characteristics such as LVH and CKD could increase the diagnostic yield in this group of patients. In this study, the presence of other comorbidities causing LVH was common, both in the general population and in patients diagnosed with FD. Of particular relevance is the diagnosis of diabetes mellitus, as a possible cause of LVH and CKD with a course similar to CKD. However, there was no significant difference in the prevalence of this disease between the general population and FD patients (22.5 vs. 33.3%; <span class="elsevierStyleItalic">P</span> = .652). While the exclusion of these patients may have increased the diagnostic efficacy, the pathologies described are not mutually exclusive, and this may affect the sensitivity of the diagnostic model.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Previous studies have reported highly variable prevalences in patients with unexplained ventricular hypertrophy. Studies in patients with hypertrophic cardiomyopathy have shown a prevalence of FD wave between 1% and 12%.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14,25</span></a> In a European cohort with unexplained LVH, 0.5% of patients had variants in the <span class="elsevierStyleItalic">GLA</span> gene associated with FD.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Differences with the present study could be explained by the inclusion criteria used. These studies included patients with ventricular thickness greater than 15 mm. Since FD can manifest with smaller or even normal ventricular thicknesses,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> this study used the European Association of Echocardiography definition of ventricular hypertrophy,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> including patients with a thickness greater than 13 mm, which may have influenced the lower prevalence. All patients with FD in this study had a ventricular thickness of less than 15  mm and would have been excluded from the previously mentioned studies.</p><p id="par0165" class="elsevierStylePara elsevierViewall">A prevalence of 0.1%−0.5% of FD has been reported in CKD patients, especially those on renal replacement therapy,<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–20</span></a> similar to that found in this study. The patients with FD in this study had moderate degrees of renal dysfunction (according to glomerular filtration rate in 2 patients and albuminuria in one patient), none of them requiring renal replacement therapy.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Another difference with most previous studies is the systematic use of genetic testing in women. The aforementioned studies used measurement of enzyme activity for diagnosis, which may introduce gender bias by excluding women who may have normal enzyme activity and be equally affected.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">Regarding the presence of variants of uncertain significance, in particular p.Asp313Tyr, a prevalence of 0.66% was found in this study. This variant is associated with a Fabry-like phenotype, more common in women, and especially with neurological involvement, where a prevalence of 0.6% has been described, and less common in patients with cardiac (0.4%) or renal (0.3%) involvement.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> In this population, patients with this variant had a high rate of central nervous system (33.3%) and vestibulocochlear (66.6%) involvement.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">As previously mentioned, this is the first study to describe the prevalence of FD in patients with cardiac involvement in the form of LVH and renal involvement. The relevance of the coexistence of involvement of these 2 organs lies in the prognostic overshadowing produced by the appearance of cardiorenal syndrome in patients with FD, increasing the risk of cardiorenal events up to 4 times.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> This means that this group of patients particularly benefits from early diagnosis and treatment.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">26,27</span></a> In this study, only one patient with FD had heart failure and kidney disease, the rest were asymptomatic from a cardiovascular standpoint.</p><p id="par0185" class="elsevierStylePara elsevierViewall">This study is limited by its observational nature. In addition, due to the exploratory nature of the study, the sample size was not pre-determined, although it was anticipated that a larger sample would be recruited. The SARS-CoV-2 pandemic led to a loss of patient follow-up and reduced availability of the laboratory test used. This, coupled with the percentage (3.2%) of samples that were impossible to analyse due to insufficient blood sample, led to a lower final sample size than expected, which may limit the power of the study. On the other hand, other complementary FD tests (determination of lyso-Gb3 or histological examination) were not performed according to protocol, which could have provided valuable information, especially in carriers of variants of uncertain significance.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusions</span><p id="par0190" class="elsevierStylePara elsevierViewall">FD is an important cause of LVH and CKD, with a prevalence of 0.33% and a significant delay in diagnosis. Genetic diagnosis is crucial to avoid bias and ensure accurate identification of FD, especially in women.
