Whether a computed tomography (CT) scan should be carried out or not in these patients has been traditionally based on previously validated rules such as the <span class="elsevierStyleItalic">Canadian CT Head Rule</span> (CCHR) and the <span class="elsevierStyleItalic">New Orleans Criteria</span> (NOC),<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">2,3</span></a> although these guidelines do not take into account medications that increase the risk of posttraumatic haemorrhagic injury.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The use of anticoagulant drugs, in the context of MHT, exponentially increases the risk of haemorrhagic injury.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> Furthermore, the use of platelet antiaggregants is increasing because they are recommended for both primary and secondary prevention of cardiovascular disease. However, unlike that described for anticoagulants, there is disagreement about whether taking platelet antiaggregants can increase the risk of intracranial bleeding in the context of a MHT.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">5,6</span></a> Despite this, the Scandinavian guide includes antiplatelet agents as a criterion when requesting cranial CT in people aged over 65.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Based on the above, the objective of this study was to analyse the factors associated with posttraumatic intracranial bleeding in patients consulting for MHT.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A retrospective, observational study was designed at the Virgen del Rocío University Hospital Emergency Department for the 1 January to 31 December, 2016 period. All patients treated for MHT during this period were included in the study, provided they fulfilled the following criteria: (a) age >16; (b) recent MHT (<2<span class="elsevierStyleHsp" style=""></span>h); (c) a Glasgow Comma Scale (GCS) score upon arrival to the emergency room of >14 points; and (d) having received a CT scan because of the presence of clinical symptoms, according to the CCHR or NOC. Patients undergoing anticoagulant therapy were excluded, since their treatment for MHT follows a different protocol. Patients were classified into two groups according to the outcome of the CT scan: (a) normal CT, if no posttraumatic intracranial haemorrhagic injury was observed; and (b) pathological CT, when a posttraumatic haemorrhagic injury was detected.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0025" class="elsevierStylePara elsevierViewall">Continuous variables are expressed as mean (IQR), while categorical variables are expressed as number of cases (%). A comparison analysis of means was designed to detect differences in various baseline variables between patients with normal CT and those with pathological CT, Student's <span class="elsevierStyleItalic">t</span>-test was used for continuous variables and the Chi square was used for categorical variables. Finally, a logistic regression analysis was carried out to analyse the variables associated with the presence of posttraumatic haemorrhagic injury in the CT scans; those variables with a <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.1 value in the univariate analysis were included in the multivariate, in order to identify factors independently associated with posttraumatic haemorrhagic injury. Statistical analysis was performed using version 17.0 of the Statistical Package for Social Sciences (SPSS 17.0).</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0030" class="elsevierStylePara elsevierViewall">A total of 566 patients were included in the study. Patients’ baseline characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. 18% of the sample were undergoing antiplatelet therapy and the vast majority of patients presented a GCS score of 15 points (91.5%).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">CT was pathological in 91 patients (16.1%). The most frequent posttraumatic intracranial injury was traumatic subarachnoid haemorrhage (52%), followed by subdural haematoma (45%), haemorrhagic contusion (30%) and epidural haematoma (6%); 29 patients had two or more associated haematomas. The vast majority of patients experienced a favourable evolution with symptomatic treatment, without requiring surgical intervention. However, 11 patients had a poor outcome (11/566, 1.94%), experiencing a clinical and radiologic deterioration within 24<span class="elsevierStyleHsp" style=""></span>h and required admission to the intensive care unit and/or surgical evacuation of intracranial haematoma. No patient died as a result of their MHT.</p><p id="par0040" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the comparison between those patients with normal CT versus those who had a pathological CT, observing that advanced age, a GCS score of <15, taking antiplatelet agents, diabetes, hypertension and high-energy trauma were more represented in the subgroup of patients with pathological CT. Finally, the logistic regression analysis showed that advanced age, antiaggregants intake, high-energy trauma and a GCS score of <15 points were factors independently associated with the presence of posttraumatic haemorrhagic injury on CT (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0045" class="elsevierStylePara elsevierViewall">The results of this study found that 16% of patients who visit an emergency department for MHT show a haemorrhagic injury on their CT scan; in many cases of low clinical significance, the factors associated with intracranial bleeding were identified to be, advanced age, taking antiplatelet medication, high-energy trauma and a GCS score of <15 points.</p><p id="par0050" class="elsevierStylePara elsevierViewall">As shown by this study and also recently described by other authors,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> in addition to advanced age and a GCS score of <15 points, high-energy trauma was also associated with the presence of post-traumatic haemorrhagic injury. Regarding antiplatelet medication, our results suggest that those patients undergoing this therapy have a higher risk of developing haemorrhagic injury after MHT, in line with results already put forward by other authors.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a> However, there is currently no protocol that includes taking antiplatelet agents to be an independent criterion when requesting a CT scan after MHT, except for the Scandinavian guide when associated with advanced age.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> Additionally, we did not observe differences according to the type of antiplatelet therapy used (data not shown), although the percentage of patients undergoing antiplatelet therapy other than acetylsalicylic acid (ASA) [aspirin] was very low. This needs to be further explored in future studies with different designs. Finally, other comorbidities such as hypertension or diabetes did not increase the risk of haemorrhagic intracranial injury.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Our study has some limitations, such as its retrospective design and the absence of a control group. Additionally, we did not carry out an aggregate analysis among antiplatelet patients to determine the degree of platelet dysfunction in each of them so as to assess whether there is an association between the level of antiplatelet and the development of haemorrhagic injury. As it is a retrospective study, we were not able to carry out a specific subgroup analysis of patients who are at high risk of bleeding after MHT, such as patients who have suffered a minor stroke,<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> which should be analysed in future studies with a different design. Finally, most of the patients took ASA in monotherapy as an antiplatelet agent, which recently made these results extractable to drugs such as clopidogrel, prasugrel or ticagrelor. Future studies are necessary to explore these issues.</p><p id="par0060" class="elsevierStylePara elsevierViewall">In conclusion, MHT is a very prevalent condition in the emergency services, mainly in elderly patients who have significant comorbidities and who are undergoing treatment that alters platelet aggregation. The proportion of patients with haemorrhagic injury after MHT is approximately 16%, although most cases show injuries of little clinical relevance.
