HR: hazard ratio; CI: confidence interval.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The role of beta-blockers in secondary prevention, after ST-segment elevation myocardial infarction (STEMI), continues to be a controversial therapeutic alternative in the age of reperfusion, especially in patients with preserved left ventricular function.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a> Recent studies demonstrate a benefit of this therapy in patients who have undergone a high-risk percutaneous coronary intervention.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6</span></a> However, some questions remain in certain aspects of treatment, such as its duration in patients without heart failure or with a left ventricular ejection fraction (LVEF) ≥ 40%. The observational studies available for STEMI, lasting between one and 5 years, show inconsistent results regarding impact on morbidity and mortality.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6–11</span></a> This lack of conclusive evidence causes discrepancies in the minimum duration of treatment recommended by the main clinical practice reference guidelines.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12–14</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Among the main factors that limit the effectiveness of a treatment is the correct taking of the medication in the long term, defined as secondary adherence. An alarming problem today is that approximately 50% of patients do not adhere to the prescribed treatment,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> with a clear impact on prognosis.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> In this sense, there are few studies dedicated to long-term adherence to beta-blockers and their influence on the progression of patients with coronary heart disease,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–21</span></a> especially in low-risk patients.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The main objective of this study was to determine the medium and long term impact of prescription and secondary adherence to beta-blockers on cardiovascular morbidity and mortality in a cohort of patients diagnosed with a first episode of type-1 STEMI without heart failure or with LVEF ≥ 40%.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study population</span><p id="par0020" class="elsevierStylePara elsevierViewall">The observational and retrospective study was carried out at the Hospital Clinico Universitario de Valencia on a population of patients over 18 years of age who, between January 2008 and December 2013, had experienced a first episode of type-1 STEMI associated with the presence of atherosclerosis as the main diagnosis of hospital admission (codes of the International Classification of Diseases 9th edition [ICD-9] 410.0-410.9), without heart failure or with LVEF ≥ 40% and who were discharged. Patients terminally ill at hospital discharge, those who did not reach 30 days of follow-up, those not belonging to that department, residents in other autonomous communities or countries, and those with a pharmaceutical benefit not covered by the National Health System or without health coverage were excluded. The study was approved by the Clinical Research Ethics Committee of the Hospital Clinico Universitario de Valencia.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Patients underwent follow-up until death or until the end of the study, 1 st January 2015, so that all of them were evaluated for at least one-year post-STEMI.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Variables analysed</span><p id="par0030" class="elsevierStylePara elsevierViewall">In order to identify the patients to be included in the study and obtain data related to hospitalization episodes, the information available in the hospital discharge reports managed by the Documentation Unit was used. The coronary revascularization technique and certain clinical findings that could condition the prescription of beta-blockers were recorded: bradycardia, atrioventricular block, and atrial fibrillation.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Through the Population Information System of the Ministry of Health of the Generalitat Valenciana, the individual demographic data were identified: sex, date of birth, country of birth and accreditation modality of the pharmaceutical assistance.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Cardiovascular risk factors, defined by the diagnoses: diabetes mellitus (ICD-9 250 code), dyslipidaemia (ICD-9 272 code), high blood pressure (ICD-9 codes 401–405), obesity (ICD-9 code 278) and smoking (ICD-9 codes 989.84, 305.1, V15.82); cardiovascular history, defined by the diagnoses: other acute coronary syndromes (ICD-9 codes 410–412), stable angina (ICD-9 codes 413–414), stroke (ICD-9 codes 430–434, 436–438), transient ischemic accident (ICD-9 code 435) and peripheral vascular disease (ICD-9 code 443–445), and the presence of other relevant chronic diseases, defined by the diagnoses: liver failure (ICD-9 code 571), kidney failure (ICD-9 codes 583, 585), neoplastic history (ICD-9 codes 140–239), neuropsychiatric history (ICD-9 codes 290–319, 331, 332) and respiratory history (ICD-9 codes 490–496) were identified from episodes recorded before admission in the electronic outpatient medical records.</p><p id="par0045" class="elsevierStylePara elsevierViewall">During the follow-up period, prescriptions (daily dosage, start date and end date) and outpatient dispensing services (pharmaceutical presentation dispensed with the number of units per box and dispensing dates) of group C07 of the ATC classification of the World Health Organization, available from the Pharmaceutical Service Manager of the Department of Health of the Generalitat Valenciana. Secondary adherence was estimated by the proportion of days covered, which is equivalent to the percentage of days that the patient has a medication in relation to the entire study period,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> through the following formula:<elsevierMultimedia ident="eq0005"></elsevierMultimedia></p><p id="par0050" class="elsevierStylePara elsevierViewall">A proportion of covered days equal to or greater than 80% was established as a cut-off point to consider a patient as compliant.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">For the evaluation of health outcomes after hospital discharge, the following variables were collected:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">1</span><p id="par0060" class="elsevierStylePara elsevierViewall">Occurrence of a new cardiovascular event (which includes the occurrence of myocardial infarction or unstable angina, stroke, peripheral artery disease).</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">2</span><p id="par0065" class="elsevierStylePara elsevierViewall">Death from a new cardiovascular event (including death from myocardial infarction, stroke, or peripheral artery disease). The selection of potential factors influencing secondary adherence to beta-blockers during the first year of follow-up was performed using the χ<span class="elsevierStyleSup">2</span> test. In the variables with statistical significance, the corresponding <span class="elsevierStyleItalic">odds ratio</span> (OR) and its 95% confidence interval (95% CI) was calculated using a logistic regression analysis.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The relationship between beta-blockers and cardiovascular morbidity and mortality was studied using the cumulative incidence analysis. Mortality due to causes other than cardiovascular was considered a competing event.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> The cumulative incidence was expressed as the probability (%) of the cardiovascular event occurring; comparisons were made using Gray's univariate test,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> calculating the <span class="elsevierStyleItalic">hazard ratio</span> (HR) and its 95% CI. Subsequently, a multivariate Fine-Gray<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> analysis was performed in case of a significant HR.</p><p id="par0090" class="elsevierStylePara elsevierViewall">To assess the impact on morbidity and mortality, 3 different populations were compared during the first year and throughout the follow-up period. In the first of these populations, patients without a prescription for beta-blockers are included. The comparison group consists of patients with prescription of beta-blockers. The second population of analysis consists of those patients who were prescribed beta-blockers but were not compliant with the treatment. The comparison group consists of patients with beta-blocker prescription and adequate adherence to treatment. The third population group is made up of the group of patients who did not receive an adequate beta-blocker treatment, either due to the lack of prescription or due to lack of adherence. The comparison group is made up of the remaining patients in the total selected population who did not meet the corresponding requirements.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Descriptive statistical analysis and logistic regression analysis were performed using the statistical software SPSS®, version 17, for Windows®. Cumulative incidence and Fine-Gray analysis were performed using the R programming language and environment version 3.4.0.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">Results were considered statistically significant when the p value was less than 0.05.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0105" class="elsevierStylePara elsevierViewall">A total of 487 patients discharged after a first episode of type-1 STEMI, without heart failure or with a LVEF ≥ 40% were retrospectively reviewed. 27 (5.5%) patients were excluded: Nine because of belonging to other autonomous communities, 6 due to being registered in another health department of the Valencian Community, 5 because their pharmaceutical assistance was not covered by the National Health System, 4 due to living abroad and 3 due to being in a terminal condition at discharge. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the demographic and clinical characteristics of the 460 patients finally included in the study. The mean age of the selected patients was 63.8 ± 13.8 years, with 47.2% of patients over 65. 75.7% were men. 98% of the patients had some cardiovascular risk factor, with smoking and high blood pressure being the most common. 65.8% had associated comorbidity. Neuropsychiatric (33.7%) along with cardiovascular (29.1%) were the most common types of medical history. Regarding the clinical findings that could condition the prescription of beta-blockers, atrioventricular block was more common (5%). Percutaneous coronary intervention was performed in 93.5% of the episodes, mainly with <span class="elsevierStyleItalic">stent</span> placement. The median follow-up was 46.7 months (interquartile range; 30.8–65.4).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Pharmacological prescription and conditioning factors</span><p id="par0110" class="elsevierStylePara elsevierViewall">Beta blockers were prescribed in 417 patients (90.7%) during the first year after discharge (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>); the proportion of prescriptions was significantly lower in patients over 65 years of age (OR = 0.491; 95% CI, 0.260-0.948; p = 0.034), in patients with a respiratory history (OR = 0.254, 95% CI, 0.127−0.509; p < 0.001) and in patients with bradycardia (OR = 0.055, 95% CI = 0.013−0.239; p < 0.001).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Secondary pharmacological adherence and conditioning factors</span><p id="par0115" class="elsevierStylePara elsevierViewall">During the first year of follow-up, of the total number of patients prescribed with beta-blockers, 356 (85.4%) were compliant. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> expands this information, based on the different variables analysed. Thus, adherence was significantly lower in foreign patients (OR = 0.312; 95% CI, 0.127−0.764; p = 0.011), in patients with a history of smoking (OR = 0.477; 95% CI, 0.249−0.913 ; p = 0.025), with a cardiovascular history (OR = 0.539; 95% CI, 0.308−0.945; p = 0.031) or with atrioventricular block (OR = 0.347; 95% CI, 0.127−0.952); on the contrary, it was higher in pensioners (OR = 1,920; 95% CI, 1,110-3,322; p = 0.020), in patients with hypertension (OR = 2,058; 95% CI, 1,190-3,560; p = 0.010 ) and in those who underwent percutaneous coronary intervention (OR = 3,167, 95% CI, 1,223-8,201; p = 0.018).</p><p id="par0120" class="elsevierStylePara elsevierViewall">Overall, a total of 321 patients (69.8%) were identified as compliant throughout the follow-up period; 139 patients (30.2%) did not adequately follow the treatment recommended by clinical guidelines, an apparently high proportion.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Health outcomes during the first year of follow-up</span><p id="par0125" class="elsevierStylePara elsevierViewall">A total of 32 of the patients (7%) had a new cardiovascular event during the first 12 months, 7  (1.5%) of whom died (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), one of them (0.2%) for unknown cause. Three patients (0.6%) died from a non-cardiovascular cause and one (0.2%) was lost to follow-up.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">The overall population analysis showed that the prescription of beta-blockers (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) had no impact on the incidence of a new cardiovascular event without death, 5.5% compared to 4.7% (HR = 1,190; CI of 95%, 0.281–5.070; p = 0.810), nor in mortality during the first year post-STEMI, 1.4% compared to 2.3% (HR = 0.624; 95% CI, 0.076–5.060; p = 0.660).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">However (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), when the subpopulation with beta-blocker prescription was analysed, compliant patients had a significantly lower incidence of cardiovascular mortality, 0.6% compared to 6.6% (HR = 0.083, 95% CI, 0.015−0.488; p = 0.003). On the other hand, this circumstance did not significantly condition the occurrence of a new cardiovascular event without death, 5.1% compared to 8.2% (HR = 0.570; 95% CI, 0.211–1.540; p = 0.270).</p><p id="par0140" class="elsevierStylePara elsevierViewall">The Fine-Gray multivariate analysis showed that adherence to treatment together with the patient's age were significant factors in the incidence of cardiovascular mortality (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0145" class="elsevierStylePara elsevierViewall">Patients adhering to beta-blocker treatment showed a better prognosis compared to those who did not receive treatment due to lack of prescription or lack of adherence (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>): the incidence of mortality was 0.6% compared to 4.8% ( HR = 0.115; 95% CI, 0.022-0.587; p = 0.009). On the contrary, there was no difference in the occurrence of a new vascular event without death, 5.3% vs. 5.8% (HR = 0.716, 95% CI, 0.298-1.720; p = 0.456).</p><p id="par0150" class="elsevierStylePara elsevierViewall">In the Fine-Gray multivariate analysis (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>), secondary adherence and age independently determined cardiovascular mortality.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Health outcomes throughout the follow-up period</span><p id="par0155" class="elsevierStylePara elsevierViewall">During the entire follow-up period (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), 85 (18.5%) new cardiovascular events occurred, of those, 29 (6.3%) died, 5 (1.1%) of unknown cause; 21 patients (4.6%) died of non-cardiovascular causes and 7  (1.5%) were lost to follow-up.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0160" class="elsevierStylePara elsevierViewall">When the impact of prescription and beta-blocker adherence on clinical course during this period was analysed (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), no statistical significance was found in all the items analysed.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0165" class="elsevierStylePara elsevierViewall">In the present study, the prescription of beta-blockers does not condition the results in morbidity and mortality during the first year after STEMI with preserved left ventricular systolic function. In contrast, the incidence of cardiovascular mortality in patients adhering to beta-blockers was significantly lower compared to non-compliants or to the total number of patients who did not receive beta-blocker treatment. However, these results were not shown when the observation period was longer.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Undoubtedly, current pharmacological treatment and the implementation of percutaneous coronary intervention have improved the prognosis of STEMI. The scientific evidence on which the prescription of beta-blockers is based is supported in clinical trials prior to the era of thrombolysis and reperfusion, with an increased risk of myocardial damage, which is why its prescription is currently questioned in certain patients.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3,28</span></a> Studies in the population without left ventricular dysfunction after STEMI and one year of follow-up found no relationship between the prescription of beta-blockers and the reduction in total mortality.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a> Longer-term studies, from 3  years of follow-up onwards, do not show a reduction in cardiovascular mortality,<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,9</span></a> with the exception of Konishi et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Cardiovascular morbidity and total mortality, in line with our results, also did not decrease significantly in any of the studies consulted.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,9–11</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">Regarding adherence, Puymirat et al.,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> in a wide series of patients, did not show a better progression at 5-year follow-up in those patients with myocardial infarction who were compliant with beta-blocker treatment during the first year of follow-up. However, this study<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> does not allow to know if the patient has taken the medication correctly during the entire study period, only if the treatment was permanently discontinued.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Lack of secondary adherence to prescribed treatment is a serious problem in cardiovascular disease.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> In this sense, it is important to remember the close relationship between therapeutic non-adherence and increased morbidity and mortality after myocardial infarction.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Despite this, no studies have been found evaluating the impact of secondary adherence to beta-blockers after STEMI in low-risk patients and there is little information available after myocardial infarction without differentiating the risk.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–21</span></a> Two of the studies, with a follow-up of 2.8 and 4.1 years, found a significantly lower incidence of global mortality, cardiovascular mortality, or coronary revascularization in patients compliant with beta-blocker therapy.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,21</span></a> In contrast, other 2  studies with a duration of 15 months and 4.5 years, respectively, did not find a lower incidence of mortality and of a new cardiovascular event in compliant patients.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,20</span></a> However, the data are difficult to extrapolate because the rate of low and high risk patients in the included populations is not known.</p><p id="par0185" class="elsevierStylePara elsevierViewall">In this study, together with advanced age, adherence to beta-blockers during the first year after discharge independently determines mortality, but not in a longer term. In this sense, the recommendations of the National Institute for Health and Care Excellence<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> and the American College of Cardiology Foundation<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> establish the need for an individual re-evaluation after a minimum duration of one to three years, taking into account the risk-benefit balance in relation to the extent of the disease and the presence of other comorbidities or adverse reactions. However, in the absence of more conclusive studies, the European Society of Cardiology indicates the impossibility of establishing a specific recommendation.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0190" class="elsevierStylePara elsevierViewall">Not surprisingly, among the factors that condition prescription and secondary adherence are some related to the occurrence of adverse reactions to beta-blockers. In our experience, the presence of bradycardia, atrio-ventricular block, smoking, or lung diseases limit treatment to beta-blockers, as is the case in some of the literature reviewed.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,17,18</span></a> Therefore, the correct selection of patients who can benefit from the treatment is very important, since they experience side effects and this limits proper compliance.</p><p id="par0195" class="elsevierStylePara elsevierViewall">This study has some limitations. The absence of left ventricular dysfunction is determined at discharge and its subsequent occurrence is not recorded, as well as its impact on the results. In addition, the calculation of adherence has been made from the dispensing files of the pharmacy units and not based on the actual taking of medications. However, a good correlation has been previously observed with other types of measures such as tablet count or electronic records.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a></p><p id="par0200" class="elsevierStylePara elsevierViewall">In conclusion, our results confirm that the prescription of beta-blockers during the first year after STEMI does not modify the progression of patients with preserved left ventricular function. However, secondary adherence to them does improve the prognosis. For this reason, we believe it is of interest to point out the need to carry out extensive studies to determine the impact of beta-blockers on the progression of this group of patients, but from the perspective of secondary pharmacological adherence. El objetivo del presente trabajo ha sido determinar el impacto de la prescripción y la adherencia secundaria a betabloqueantes en la morbimortalidad cardiovascular a medio y largo plazo, tras un primer episodio de infarto de miocardio con elevación del segmento ST (IAMCEST) tipo 1 sin insuficiencia cardiaca o con fracción de eyección del ventrículo izquierdo ≥ 40%.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Métodos</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">Estudio observacional y retrospectivo en una cohorte de pacientes ingresados entre 2008 y 2013 en el Hospital Clínico Universitario de Valencia. El análisis de incidencia acumulativa estableció la relación entre la mortalidad cardiovascular o la aparición de un nuevo evento vascular con la prescripción de betabloqueantes y la adherencia secundaria, definida como proporción de días cubiertos.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">Durante el primer año tras el alta, la prescripción de betabloqueantes no influyó significativamente sobre los resultados en salud de los 460 pacientes incluidos. Sin embargo, la mortalidad cardiovascular fue menor en los pacientes adherentes respecto a los no adherentes, 0,6% frente a 6,6%, (HR = 0083; IC 95%, 0,015-0,448; p = 0,003), y en los pacientes adherentes respecto a los que no recibieron el tratamiento por ausencia de prescripción o por falta de adherencia, 0,6% frente a 4,8% (HR = 0,115; IC 95%, 0,022-0,587; p = 0,009).
