More than a third of them will die of some type of cancer.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">1</span></a> It is estimated that 40% of smokers will die prematurely from tobacco use if they do not stop smoking.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">2</span></a> Half of those deaths occur in people aged 35–70, being therefore premature, causing a lot of years of potential life lost.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">It is estimated that in 2020 smoking will cause 18% deaths in developed countries and 11% in developing countries.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a> Smoking is associated with more than 25 diseases and is mainly responsible for 85% cases of lung cancer, 75% chronic bronchitis and 25% of ischemic heart disease.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">5</span></a> Spain is in phase 3 and 4 of the smoking epidemic model developed by Lopez et al.,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">6</span></a> but the maximum level of negative health effects has not been reached yet.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The main national benchmark on smoking data in Spain is the Encuesta Nacional de Salud de España or ENSE (National Health Survey of Spain). ENSE is a research on health status and its determinant factors from the perspective of citizens and provides information including perceived morbidity, lifestyle, behaviors related to risk factors, the use of health care services and preventive practices. In the last decade, the prevalence of smoking in Spain has been decreasing. In ENSE in 2003,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">7</span></a> the overall prevalence of smoking in the Spanish population aged 16 or older was 28.1% for daily smokers. In 2006, ENSE<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">8</span></a> prevalence decreased to 26.4% for daily smokers. The most recent data, belonging to the ENSE of 2011–2012,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a> shows an overall smoking prevalence of 24% in the Spanish population aged 15 and over.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Tobacco addiction is a chronic medical condition, amenable to treatment. The expected results from interventions to promote smoking cessation are as follows: (a) increase the number of years lived; (b) improve the health status and quality of the years lived; (c) prevent premature deaths and reduce healthcare costs of treating conditions associated with smoking.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">10</span></a> The aim of this study was to describe the prevalence of smoking and estimate the smoking-attributable mortality in Spain and its impact on premature mortality of the population.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methodology</span><p id="par0025" class="elsevierStylePara elsevierViewall">This is an observational study, based on the analysis of the ENSE in 2011–2012. The scope of the study is population residing in Spain, 18 years old or over in 2012, obtained from the review of the local census conducted by the National Statistics Institute (INE)<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">11</span></a> to date of 1 January each year.</p><p id="par0030" class="elsevierStylePara elsevierViewall">To calculate the smoking-prevalence, anonymized microdata file of the ENSE 2011–2012 provided by the Ministry of Health, Social Services and Equity<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">12</span></a> (MSSSI) was used. For this purpose, we have taken into account the sampling design of the ENSE 2011–2012, which is based on a three-stage stratified sampling.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Due to the characteristics of this study we have required:<ul class="elsevierStyleList" id="lis0020"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0040" class="elsevierStylePara elsevierViewall">To gather the data into groups by age and sex, with a lower age limit of 18 and an upper age limit of 100 and older.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0045" class="elsevierStylePara elsevierViewall">To quantify the status as smoker, former smoker and never smoker. Occasional smoker status has been omitted, since according to the methodology followed by the ENSE 2011–2012, the answer “Yes, I smoke, but not daily” is the same as the answer “No, I do not smoke or have never smoked on a regular basis,” i.e. it does not quantify either type or amount of cigarettes smoked.</p></li></ul></p><p id="par0180" class="elsevierStylePara elsevierViewall">For the operation of this file we have used SPSS<span class="elsevierStyleSup">®</span> syntax files provided by the MSSSI, which were obtained from the website developed by the Consejo Superior de Investigaciones Científicas (Spanish Council for Scientific Research) and Instituto de Mayores y Servicios Sociales (Institute for the Elderly and Social Services).<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">13</span></a> For all-cause mortality rates we have used the files supplied by the INE<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">14</span></a> for the year 2012. For calculating smoking-attributable mortality, smoking prevalence and mortality have been estimated in the population aged 35 and older, as well as the relative risks (RR) of death for each related disease, according to sex, obtained by Thun et al.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">15</span></a> based on the Cancer Prevention study II (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Since the diseases included in the study by Thun et al. were encoded according to the Ninth Revision of the International Classification of Diseases (ICD-9), and the statistics provided by the INE were on the tenth revision of this classification, these statistics were converted into ICD-9 following the methodology indicated by the Statistical Institute of the Community of Madrid.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">16</span></a> With this, we have calculated the population attributable fraction (PAF) of mortality from smoking for each cause, sex and age group, taking into account the relative risk of death and prevalences for each category of smoking, using the following formula<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">17</span></a>:<elsevierMultimedia ident="eq0005"></elsevierMultimedia>where <span class="elsevierStyleItalic">p</span>1 is the prevalence of smokers, <span class="elsevierStyleItalic">p</span>2 is the prevalence of former smokers, <span class="elsevierStyleItalic">RR</span><span class="elsevierStyleInf">1</span> is the relative risk of smokers compared to non-smokers and <span class="elsevierStyleItalic">RR</span><span class="elsevierStyleInf">2</span> is the relative risk of former smokers compared to non-smokers.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Multiplying PAF by the number of deaths from each cause allows us to obtain the number of smoking-attributable deaths.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Years of potential life lost (YPLL) were calculated using the following formula:<elsevierMultimedia ident="eq0010"></elsevierMultimedia>where <span class="elsevierStyleItalic">Di</span> is the number of deaths from smoking in age group <span class="elsevierStyleItalic">i</span> and <span class="elsevierStyleItalic">Ai</span> is the number of years of life lost (life expectancy [LE]<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>age of death [<span class="elsevierStyleItalic">i</span>]).</p><p id="par0070" class="elsevierStylePara elsevierViewall">We have considered a life expectancy of 70 years for both sexes, as proposed by Romeder and McWhinnie,<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a> since it is the limit used by the INE for calculating YPLL. The mean years of potential life lost (MYPLL) was obtained from YPLL divided by the overall number of smoking-attributable deaths for the disease.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">The ENSE 2011–2012 was performed on a sample of 21,007 adults, 9649 men and 11,358 women, aged 15–100 and older. For this study the sample selected included men and women of legal age (18 or older). The sample consisted of 9412 men and 11,175 women (total: 20,587). Data of cases regarding “occasional smokers” were ruled out. Therefore, the valid study population is 9096 men (94.3% of total) and 10,924 women (96.18% of total), the total valid population being 20,020 individuals (95.3% of total).</p><p id="par0080" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the results of the smoking prevalence by sex and age group. The overall smoking prevalence is higher in the population aged 18–35, up to 36.47% in men and 30.21% in women. Regarding the prevalence of former smokers, the group of men aged 65 or older is the most significant, with a 49.22%, which represents a substantial difference compared to women, which is 5.01%. The average age of starting smoking stands at 20.5 years for the smoking population and 20.4 for the former smoker population. In turn, the average age of smoking cessation stands at 42.1 years, which means an average age of 21.7 years of smoking by former smoker.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The PAF calculated for 2012 are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The highest rates are trachea, bronchi and lung cancer (0.89 in men and 0.71 in women) and chronic obstructive pulmonary disease (COPD) (0.84 in men and 0.74 in women). The estimated PAF were always higher in males, except for cerebrovascular diseases in the age group 35–64. The number of smoking-attributable deaths in 2012 was 60,456 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>), which accounted for 15.2% of all deaths produced in the population aged 35 or older (overall mortality in that year for that age group was 397,055 deaths, 202,157 men and 194,898 women). By sex, 45,669 men and 14,787 women died from smoking, representing 22.6% of all deaths in men and 7.6% in women, respectively. Depending on the various selected diseases, the highest smoking-attributable mortality was due to tracheal–bronchial–lung cancer, with 30.5% of all deaths attributable, followed by COPD, with 22.1%. By sex, the trachea–bronchi–lung cancer is most significant, since it is the disease with most mortality in men, with 34.46%, while in women it is heart disease, with 29.41%.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The number of smoking-attributable YPLL was 184,426 years, of which 148,599 (80.6%) were males and 35,827 (19.4%) females (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). 46.7% of these YPLL are due to tracheal–bronchial–lung cancer and 14.74% ischemic heart disease. By sex, trachea–bronchi–lung cancer is responsible for 46.12% in men and 49.93% in women. The mean smoking-attributable YPLL was 3.25 years in men and 2.42 in women (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). The highest value was in ischemic heart disease and cerebrovascular disease in the age group of 35–64 years, with values of 14.48 and 14.32 in men and 14.04 in women and 15 respectively.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">In Spain, as in other developed countries, there is a change in smoking habits by sex: smoking is decreasing among men and increasing among women, particularly younger women. According to the data obtained in this study, the smoking prevalence in 2012 compared to 2006<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> in the age group 35 or younger, has increased slightly in men (36.47% vs 32.69%) and it has largely increased in women (30.21% vs 19.59%). In the population aged 35–64 it has decreased in males (33.33% vs 38.69%) and has remained constant in women (26.25% vs 27.27%). In the population aged over 65 it has decreased in males (12.81% vs 15.39%) and has remained constant in women (2.76% vs 3.07%).</p><p id="par0100" class="elsevierStylePara elsevierViewall">Smoking caused more than 60,000 deaths in 2012, with a mortality rate in men 3.12 times higher than in women. This shows that in 2012, 125 men and 40 women died daily due to smoking. Compared to previous studies<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> in former years, the overall number of deaths compared to 2006 has increased slightly (60,456 vs 58,523) in men (45,669 vs 45,028) and in women (14,787 vs 13,545). Compared to a study of 1998,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> this number is higher, since in 2012 there were 9056 deaths more than in 1998. This is because in women the number of deaths was increased by 10,187, varying from 8.14% smoking-attributable mortality in 1998 to 24.45% in 2012. As for the percentage of smoking-attributable deaths, it has varied from 14.25%<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> in 1998 to 15.76%<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> in 2006 and 15.2% in 2012.</p><p id="par0105" class="elsevierStylePara elsevierViewall">One third of the deaths reported in males is due to trachea–bronchi–lung neoplasm, whereas in the case of women almost one third of all deaths are associated with other heart diseases. This means that smoking causes more deaths than AIDS, alcohol, illegal drugs and traffic accidents together.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The number of YPLL in 2012 in males has been 4.14 times higher than in women. Compared to 2006<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> it has decreased globally (184,426 vs 211,252), (148,599 vs 176,765) in men and in women it has remained constant (35,827 vs 34,486). In both men and women, about half YPLL are due to trachea–bronchi–lung neoplasm (68,532 and 17,888 respectively).</p><p id="par0115" class="elsevierStylePara elsevierViewall">The mean YPLL in 2012 is about a year older in men than in women. This has been decreased in males (3.3 vs 3.9) and has remained constant in women (2.4 vs 2.5) compared to 2006.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> However, unlike YPLL, ischemic heart disease and cerebrovascular disease are responsible for a higher loss of mean YPLL (14.5 in men and 14 in women and 14.3 in men and 15 in women, respectively). This fact might be because in certain types of diseases, population growth and aging have a role to be taken into account.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">1</span></a> Although there has been a slight decrease in smoking prevalence in Spain, our results reveal that the smoking-attributable mortality remains too high, being currently one of the major preventable public health problems.</p><p id="par0120" class="elsevierStylePara elsevierViewall">As for the reliability of the sources used, although based on data of the respondents, since 2003 the sampling for the development of the ENSE has been performed randomly and not by age and sex, thus providing more valid results. Regarding mortality rates provided by the INE, in the case of smoking, the most difficult issue is that we are dealing with a risk factor contributing to all-cause mortality, and therefore, it has not been included as a direct cause of mortality. Another limitation of mortality records is death attributable to a single cause, which may limit the study of involvement of other diseases leading to death.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The methodology used for calculating the smoking-attributable mortality is similar to that used in former studies.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">1,19,20</span></a> Therefore, the results can be compared to each other.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Assuming life expectancy is 70 years for both sexes, as proposed by Romeder and McWhinnie,<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">18</span></a> this is consistent with the methodology used in similar studies.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">19</span></a> No timing preferences or age weighting have been used since this is a cross-sectional study, seeking an estimate of the effects of smoking in Spain in 2012.</p><p id="par0135" class="elsevierStylePara elsevierViewall">When calculating the PAF for each one of the selected diseases, the RR used were estimated from American and, not Spanish, cohorts, which may present some limitation because they are not entirely transferable to our environment. However, this limitation is common to those of other studies on this issue.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">17,21</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">The current development of public policies on tobacco control recommended by the WHO Framework Convention on Tobacco Control have succeeded in reducing smoking prevalence by 0.5–1% in the developed countries.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">22</span></a> In Spain, Law 42/2010 of 30 December has been in force since 2011, to modify Law 28/2005 of 26 December on health measures against smoking and regulating sales, supply, consumption and advertising of tobacco products. Despite these legislative measures and that the overall prevalence tends to decrease, these measures are considered insufficient as attributable mortality has increased compared to the data obtained in 2006.</p><p id="par0145" class="elsevierStylePara elsevierViewall">In a recent study<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">23</span></a> the list of smoking-related diseases is more extensive and the mortality rates are higher compared to those of nonsmokers.
