During 2016, 17.9 million deaths were attributed to these diseases, and the figure is estimated to increase to 23.6 million in 2030.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> In Spain alone, 122,426 people died from this cause in 2017, which accounted for 29% of total deaths.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Cardiovascular diseases constitute a major public health burden and have a high socioeconomic impact. They are the fourth cause of sick leave, accounting for 17% in Spain.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Seasonal influenza is an infectious disease that also has a major impact on health systems each year. It is an unpredictable infection that affects more than 1 billion people in the world annually. In at-risk population groups, such as people with heart and vascular disease, it can manifest itself in a severe form, requiring hospital admission and even leading to death. The highest mortality and serious complications occur in people over 65 years of age. In Spain, more than 90% of deaths from influenza occur in people over 60.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> In addition to all this, it has a large economic impact, both in direct and indirect costs.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The evidence that influenza acts as one of multiple factors that trigger or aggravate cardiovascular events is becoming increasingly strong. A considerable proportion of the estimated morbidity and cost of treating heart disease can be attributed to an increase in hospitalizations for cardiovascular causes during the influenza season.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> In addition, recent studies have shown that influenza virus infection increases the risk of suffering acute myocardial infarction (AMI) by 6–10 times within the first 7 days of laboratory-confirmed influenza, and the risk of stroke by 3–8 times.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–11</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The objective of this article is to review the available evidence regarding the effect of influenza vaccination in cardiovascular events, in the population without and with underlying diagnosed cardiovascular disease.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A literature search of MedLine/PubMed, Cochrane Library, WOS (Web of Science) and Embase electronic databases was conducted between April 2009 and April 2019 and restricted to articles published in English or Spanish. The search strategy consisted of a combination of terms related to cardiovascular diseases (“Myocardial Infarction”, “ST Elevation Myocardial Infarction”, “Non-ST Elevated Myocardial Infarction” “Heart Failure”, “Cardiovascular Diseases” “Cardiovascular Disease”, “Myocarditis”) and influenza vaccination (“influenza vaccination”, “influenza vaccine”, “flu vaccine”) using the Boolean operators “AND” or “OR”.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Original publications of observational studies whose objective was to evaluate the effect of influenza vaccination on cardiovascular events and their progression, published within the 10 years of reference. In order to get a broader picture of the impact of influenza vaccination, it was decided to include both, studies conducted in populations without previously diagnosed cardiovascular disease and studies conducted in populations where the disease had already been identified. In addition, systematic reviews and meta-analyses were included, as they are an essential tool for synthesising available scientific information, increasing the validity of the conclusions of individual studies and identifying areas of uncertainty where research is needed. Publications in the form of article commentaries, editorials and opinion pieces were excluded. Studies that included very specific populations (e.g., patients with HIV and multiple sclerosis), which evaluated preclinical data, were also excluded. The screening and review process was carried out by three researchers. Reviewers independently selected potentially eligible studies from the search. Studies were excluded when they did not meet the inclusion criteria. If eligibility was not clear in the title and abstract, the full text of the work was consulted.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Study selection</span><p id="par0035" class="elsevierStylePara elsevierViewall">The initial search detected a total of 1147 references among the four databases used. 197 duplicate references were excluded, and the remaining 950 publications were reviewed, selecting 135 publications in a first review, excluding 105 after reading the full text for not including in their methodology a direct assessment of the impact and progression of cardiovascular events after influenza vaccination. A total of 30 papers were included in the final analysis, as represented in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Influenza vaccination in patients without established cardiovascular disease</span><p id="par0040" class="elsevierStylePara elsevierViewall">Ten observational studies evaluated the effect of the influenza vaccine in patients without previous cardiovascular disease (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). A retrospective study carried out by Siriwardena et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> in the UK included 26,784 stroke patients and the same number of control patients, of whom about half received the influenza vaccine. Vaccination was associated with a 24% reduction (95% CI: 20−28%) in the incidence of stroke.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">In another study based on a self-controlled case series method, carried out between 2001 and 2009, a reduction of 30 and 17% was determined after vaccination at 8−14 days and 29−59 days, respectively.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Similarly, a study in a population aged 65 years and older found that seasonal influenza vaccination reduced the risk of admission for stroke by 24%, and revaccination during the previous 5 years was associated with a strong decrease in the risk of hospitalisation for stroke (odds ratio [OR]: 0.56; 95% CI: 0.38−0.83).<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">The protective effect of the vaccine on cardiovascular disease was also observed in three of these studies analysed, with reductions of 19–45% in the rate of acute myocardial infarction (AMI) in vaccinated patients.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15–17</span></a> The incidence of AMI was significantly lower at 60 days after influenza vaccination (compared to the baseline period), showing a range of 32% (95% CI: 22−40%) reduction in AMI during 1−14 days after vaccination, at 18% (95% CI: 10−25%) in the period of 29−59 days post-vaccination.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Another similar study collected data from 78,706 patients between 2001 and 2007, showing a 19% reduction in the rate of AMI in those vaccinated (OR: 0.81; 95% CI: 0.77−0.85).<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">Three of the selected publications analysed the effect of the vaccine in populations with other concomitant diseases, such as chronic kidney disease, diabetes type 2 or chronic obstructive pulmonary disease.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18–20</span></a> In the case of the latter, vaccination was associated with a lower risk of hospitalization for acute coronary syndrome (ACS) in all age groups over 55 that were included, and regardless of the influenza season analyzed.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In the cohort study of more than 120,000 patients with type 2 diabetes, after seven influenza seasons analysed, vaccination was associated with a 19% reduction in the rate of hospital admissions for AMI, 30% for stroke and 22% for heart failure.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> On the other hand, in patients with chronic kidney disease >55 years, it was found that vaccinated patients showed a lower risk of hospitalization for heart failure (relative risk [RR]: 0.31; 95% CI: 0.26−0.39; p < 0.001).<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Influenza vaccination in patients with established cardiovascular disease</span><p id="par0060" class="elsevierStylePara elsevierViewall">The effects of influenza may be especially important in patients with previously detected cardiovascular disease. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the 11 observational studies selected to analyse the impact of the influenza vaccine in this population. One was developed from data extracted from a randomized, double-blind study conducted in 47 countries: the so-called <span class="elsevierStyleItalic">Prospective Comparison of ARNI with ACEI to Determine Impact on Global Mortality and Morbidity in Heart Failure</span> (PARADIGM-HF), which included 8399 participants with heart failure; of them, 21% had received the influenza vaccine up to 12 months prior to recruitment.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Vaccination was associated with a reduced risk of death from all causes (RR: 0.81; 95% CI: 0.67−0.97), although it could not be determined whether this association obeys a causality criterion.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Research conducted on 11,374 patients with diagnosed atrial fibrillation and a 1:4 control reference group showed that the risk of atrial fibrillation was lower in the vaccinated group (OR: 0.881; 95% CI: 0.836−0.928).<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> In three other studies reviewed, a statistically significant decrease in hospitalizations due to coronary heart disease had been reported in vaccinated patients.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23–25</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">In the largest cohort study examining the effects of influenza vaccination, 134,048 adults with heart failure were recruited in Denmark between 2003 and 2015; patients who received one or more than one influenza vaccine during the follow-up period saw an 18% reduction in the risk of death from all causes and from cardiovascular causes.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Annual vaccination at the beginning of the influenza season was associated with a greater reduction in the risk of mortality in this population.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">In relation to the studies identified that analyse the effect of the antiviral vaccine specifically in geriatric patients with heart disease, a study of 9 seasons (2000−2009) of analysis was identified, which included 50,496 patients hospitalized for stroke age 66 or older.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> This study determined that the subjects who received the vaccine showed a lower risk of complications and mortality after the stroke, compared to the control population. Furthermore, the length of hospital stays, and healthcare costs were lower in the vaccinated group compared to the control group.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> In another cohort study in which the mean age was 73, the influenza vaccine showed synergistic protective effects against haemorrhagic stroke in patients with atrial fibrillation.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> Another study including 5048 patients aged 65 or older, with ischemic heart disease, concluded that the influenza vaccination was associated with a lower risk of mortality from all causes throughout the year (RR: 0.42; 95% CI: 0.35−0.49), as well as with a lower risk of hospitalization for cardiovascular disease during the influenza season.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> Finally, a multicentre case-control study, with a lower incidence of hospitalization for cardiovascular or pulmonary disease among those vaccinated compared to those not vaccinated, corroborates the protective effect of the vaccine in reducing the rate of hospitalization among patients with chronic heart disease, especially in people over the age of 65.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Analysis of the meta-analysis results</span><p id="par0080" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>, 11 meta-analysis were included in this review. According to the data collected, all-cause mortality is significantly reduced by 22−40% in vaccinated patients,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">29–31</span></a> while death from cardiovascular events is reduced by an average of 56%<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,30,32–34</span></a> in vaccinated patients. Regarding hospital admissions, vaccinated patients reduced their cardiovascular-associated hospitalizations by 16%.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">29,35</span></a> However, no significant relationship was observed with respect to all-cause hospitalisations.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">35</span></a></p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The risk of cardiovascular events such as AMI or stroke also experienced a statistically significant reduction in patients who received the influenza vaccine according to the results of the meta-analyzes.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">31,34,36,37</span></a> In a systematic review based on 8 case-control studies, a 29% reduction in AMI was determined, comparable to the efficacy of treatments for secondary prevention.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">36</span></a> Another study analysing 292,383 patients found a significant 27% reduction in AMI among those vaccinated.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> However, two other meta-analyses reviewed did not show a significant value for the reduction in AMI, mainly due to the heterogeneity of the studies included in them.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,34</span></a> In the case of stroke reduction in vaccinated patients, one review found an 18% reduction based on 11 observational studies,<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">37</span></a> and another study found a value of 13% after analysing 5 randomised clinical trials (RCTs) and 7 observational studies in patients with and without previous cardiovascular pathology.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">34</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">More specifically, two of the meta-analysis focused on RCTs, one of them focused on 5 RCTs, classified as high quality, of influenza vaccination compared with placebo or control in patients at high risk of heart disease. In this case, vaccination was associated with a lower risk of cardiovascular events (RR: 0.64; 95% CI: 0.48−0.86; p = 0.003), and specifically this risk reduction effect was more remarkable among patients with a recent history of acute coronary syndrome (ACS) (RR: 0.45; 95% CI: 0.32−0.63; p < 0.001).<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a> On the other hand, a Cochrane Library study included 8 clinical trials with 12,029 subjects who had received vaccine or control treatment: 4 of them reported cardiovascular events among their safety analyses and another 4 focused on cardiovascular prevention among participants with established coronary disease.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">38</span></a> Cardiovascular mortality reported by 4 trials showed a significant reduction with influenza vaccination (RR: 0.45, 95% CI: 0.26−0.76, p = 0.003), with no significant heterogeneity between studies.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">In line with the aim of this study, the vast majority of the papers reviewed showed a benefit of influenza vaccination in the prevention of cardiovascular disease. Influenza vaccination reduces all-cause mortality by a statistically significant 22−40% and death from cardiovascular events by an average of 56%. Patients who receive the vaccine also have a lower risk of suffering cardiovascular events such as AMI or stroke. In addition, hospital admissions for cardiovascular causes are reduced by 16% in vaccinated individuals. Observational studies have shown that influenza vaccination in patients with heart failure was associated with a lower risk of hospitalization, especially for cardiovascular diseases,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> and a reduction in the risk of death from all causes and from cardiovascular reasons after extensive adjustment for confounding factors.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> In addition, several studies have shown that repeated vaccination annually and during the beginning of the influenza season is associated with a greater reduction in the risk of death.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">In general, this review has shown that influenza vaccination is associated with a significant reduction of cardiovascular events, mainly in the population aged 65 or older, but a potential benefit has also been observed in the prevention of AMI in a population under the age of 65, without previously described cardiovascular disease,<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15–17</span></a> especially in smoking-high cholesterol patient profiles. Also noteworthy is the large retrospective study of 7 influenza seasons in a population with type 2 diabetes that shows the association of vaccination with reductions in hospital admission rates for cardiovascular and acute respiratory diseases, and in all-cause mortality through vaccination.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> These findings reinforce the official recommendations in risk groups and should also boost the general population’s awareness of influenza infection and its potential complications.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The meta-analyses and observational studies reviewed, both in populations with and without previous cardiovascular disease, show alignment in their results with those presented in specific clinical trials that have been developed to assess the effect of influenza vaccination. There is a need for randomized and controlled studies to establish a relationship between vaccination and clinical outcomes in a robust and reliable way.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">39</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">Although influenza can affect people of all ages, the risk of serious cardiovascular complications increases with age. However, the clinical effectiveness of the standard-dose vaccine in people aged 65 years and older is generally lower than in younger adults, with lower immunogenicity<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">40</span></a> also observed. The use of more immunogenic vaccines in this population, such as high-dose vaccines with a demonstrated improvement in effectiveness,<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">40,41</span></a> is presented as a consolidated alternative.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Multiple etiopathogenic mechanisms have been proposed to explain the relationship between influenza virus infection and the development or worsening of heart disease.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">42–44</span></a> On the one hand, there is speculation about a possible direct effect of the viral infection, leading to an increase in pro-inflammatory cytokines, resulting in pericarditis or myocarditis and ultimately heart failure.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">42</span></a> On the other hand, an indirect mechanism has been described by which influenza infection would cause the rupture of atherosclerotic plaques, triggering cardiovascular responses such as acute myocardial infarction or stroke.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">43</span></a> The viral infection would act as an inflammatory stimulus and activate the immune system causing the genesis, progression, and instability of atherosclerotic plaques.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">43</span></a> Another more recently proposed mechanism involves a cardiotoxic role of influenza virus,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">44</span></a> leading to myocarditis.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The cardioprotective role of the influenza vaccine can be explained by two processes. On the one hand, a nonspecific mechanism is proposed by which the effect of the vaccine would be due to the reduction of influenza-related symptoms (fever, tachycardia, dehydration, or hypoxemia) that could decompensate patients with cardiovascular diseases.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">45</span></a> On the other hand, since the effect of the influenza vaccine seems not to be restricted to the influenza season alone,<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23,32</span></a> a pleiotropic effect of the vaccine is also proposed via an “antigen mimicry” mechanism between influenza virus and atherosclerotic plaque antigens that would inhibit the immuno-inflammatory pattern of atherosclerosis.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">46</span></a> Specifically, the antibodies generated by influenza vaccines would act as agonists of the bradykinin-2 receptor, activating a signalling pathway that would contribute to the stabilization of atherosclerotic plaques.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">46</span></a> Precisely bradykinin-2 has been related to the mechanism of action of the SARS-CoV-2 virus that caused the recent COVID-19 pandemic.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">47</span></a> Recent studies suggest that SARS-CoV-2 infection would modify the renin-angiotensin system, causing the mechanisms for regulating bradykinin to be altered and bradykinin to no longer be broken down effectively. Elevated levels of bradykinin in multiple tissues and systems would cause increases in vascular dilation, vascular permeability, and hypotension, symptoms seen in COVID-19.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">47</span></a> Future research should focus on knowing in more detail the impact of viral infections such as influenza or COVID-19 on vascular damage.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Despite the fact that both health authorities and scientific societies and advisory bodies recommend the influenza vaccine in patients with cardiovascular disease,<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">48–50</span></a> the coverage percentages of seasonal vaccination remain suboptimal and highly variable depending on the country. In the PARADIGM-HF study, only 21% of patients with heart failure were vaccinated, with a great disparity among the 47 countries included.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> The clear protective role of vaccination observed in this review suggests an urgent need to promote influenza vaccination, especially in the middle-aged and cardiovascular susceptible population.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Cardiologists, together with other specialists such as family doctors and internists, play a key role in making the population at risk aware of the importance of influenza vaccination. The inclusion of influenza vaccination as a primary and secondary prevention measure in the follow-up of patients with coronary heart disease is essential. Recent data show that the cardioprotective effect of influenza vaccination is similar to other secondary prevention measures. While the influenza vaccine reduces the relative risk of myocardial infarction between 15% to 45%, actions such as quitting smoking, taking anti-hypertensive drugs, or statins for hypercholesterolemia produce similar decreases (32–43, 17−25 and 19–30%, respectively).<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">51</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">The body of evidence analysed in this study supports the fact that influenza vaccination collaborates in the prevention of cardiovascular events in patients with and without heart disease. This is of considerable clinical and health importance, given the underuse of vaccination in the general population and the impact that this preventive strategy could have on high-risk patients with heart disease. The near future is promising with new the new, great-value, preventive anti-influenza tools, as they have been shown to have a protective effect on cardiorespiratory events in people over 65 years of age in clinical trials and real-life data.
