On arrival at accident and emergency, the patient shows sings of paraplegia of the lower limbs with no bilateral patellar reflex. A simple thoracolumbar spine X-ray is performed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>a) followed by a CT (<a class="elsevierStyleCrossRef" href="#fig0005">Figs. 1b and 2</a>) finding a fracture dislocation at D10⿿D11 with a horizontal fracture line that runs through the intervertebral space D10⿿D11 and the pedicles of D11, involving retrolisthesis at D10 on D11, grade iv/iv, with severe affection of the vertebral canal and probable serious spinal injury.
