It is a variant of thrombotic microangiopathy, caused by excessive microvascular complement activation.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Our aim in presenting this case is to alert about the under-diagnosis of this disease and its consequences, as well as to provide tools for a correct differential diagnosis.</p><p id="par0015" class="elsevierStylePara elsevierViewall">We report the case of a 34-year-old primigravida of 34.2 weeks gestation (sg) with mild pre-eclampsia; the foetus showed intrauterine growth restriction type 1. No personal, surgical or obstetric history of interest was recorded.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Admitted for induction of labour due to angiogenic factor levels of 300. The following day, at 34.3 sg, an unscheduled caesarean section was performed due to failed induction.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Two hours after the operation, the patient developed postpartum haemorrhage which resolved with pharmacological measures, and she was haemodynamically stable throughout. A control CBC was performed at 24 h, showing anaemia with increased LDH values, thrombocytopenia, moderate leukocytosis, abnormal renal profile and normal liver profile. The blood test at 24 h showed: haemoglobin 6.6 g/dl (12–16 g/dl), haematocrit 19% (36–46%), platelets 25.000/mm<span class="elsevierStyleSup">3</span> (150–500.000/mm<span class="elsevierStyleSup">3</span>), WBCs 18.000/mm<span class="elsevierStyleSup">3</span> (4-5-13.000/mm<span class="elsevierStyleSup">3</span>), creatinine 1.52 mg/dl (0.6–1.1 mg/dl). The next follow-up analysis showed a worsening of the parameters, with decreased C3 and C4. The case was classified as haemolytic uraemic syndrome (HUS) (thrombocytopenia + haemolytic anaemia + renal failure).</p><p id="par0030" class="elsevierStylePara elsevierViewall">The study was expanded by adding ADAMS 13, Shiga toxin and Von Willebrand factor multimers with results in the normal range, ruling out typical HUS.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Treatment was initiated with a monoclonal antibody that inhibits complement C5 activation (eculizumab). Vaccines against encapsulated bacteria and antibiotic prophylaxis with amoxicillin 500 mg/12 were administered prior to drug initiation to prevent infection due to drug-induced decreased immunity.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The patient showed a substantial clinical and laboratory improvement and given the stability of her vital signs on the ward, it was decided to discharge her home with follow-up by the unit and the Nephrology department. Genetic tests were performed on an outpatient basis, highlighting positive anti-FH antibodies with a low titre and having a plasminogen mutation and risk polymorphisms. It was decided to treat every 2 weeks with eculizumab and to re-evaluate at 6 months.</p><p id="par0045" class="elsevierStylePara elsevierViewall">HUS has been classified as typical and atypical, the former being common in children and caused by enteric infections. Atypical HUS is associated with mutations in the complement system and usually requires a trigger<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> such as malignant hypertension, sepsis and infections, autoimmune diseases, gestation, HELLP syndrome or puerperium.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The differential diagnosis of microangiopathy during pregnancy includes: thrombotic thrombocytopenic purpura (TTP), HUS, disseminated intravascular coagulation, pre-eclampsia, eclampsia and HELLP syndrome (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). In our case, the clinical suspicion was leaning towards HUS or incomplete HELLP.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">According to the literature, the treatment of this disease is based on two lines: plasmapheresis and eculizumab. The systematic review by Gupta et al. concludes that women treated with eculizumab had higher remission rates with fewer cases of renal failure and persistent dialysis.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">A history of obstetric complications, including pre-eclampsia, postpartum haemorrhage and stillbirth has been found to be common in the development of HUS or any microangiopathy. Therefore, differential diagnosis of this disease is very difficult, as it can coexist with other obstetric complications and early treatment is vital.</p><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0005">Ethical considerations</span><p id="par0065" class="elsevierStylePara elsevierViewall">The patient has been informed and the informed consent has been signed.
