Admittedly, the recommendations of clinical practice guidelines for the use of statins in primary prevention are based on the extrapolation of results from randomised clinical trials conducted in primary prevention, which usually include patients younger than 75 years, with no severe comorbidities and receiving no treatment with medications that interact with statins.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">2</span></a> However, we think there are uncertainties about the potential medium-long term effect in terms of benefits, risks and economic impact of the progressive “statinisation” of the population,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">3</span></a> especially now that the recommendations of the new American consensus guidelines suggest the intensification of statin treatment by using higher doses, or more powerful statins,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">4</span></a> including diabetic patients in primary prevention of cardiovascular diseases.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">We recently published an observational study which described the clinical and epidemiological profile of 69,737 adolescent and adult patients initiating treatment with statins for primary prevention of vascular diseases in Spain (2007–2011 period).<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">5</span></a> In that study, initiators of standard treatment and initiators of intensive treatment were distinguished, considering the latter as those patients with an initially prescribed dose superior to 40<span class="elsevierStyleHsp" style=""></span>mg of simvastatin (or equivalent dose if it was a different statin). In addition, multivariate logistic regression models were constructed to evaluate the strength of the association between diverse clinical characteristics and the use of intensive treatment. For this, the Database for Pharmaco-epidemiological Research in Primary Care (<span class="elsevierStyleItalic">Base de datos de Investigación Farmacoepidemiológica en Atención Primaria, BIFAP</span>) was used, which is a database that includes information from medical records of patients treated in the primary care setting of the Spanish National Health System (<span class="elsevierStyleItalic">Sistema Nacional de Salud, SNS</span>). The use of computerised health databases such as BIFAP is an efficient alternative to traditional sources of information, with the additional advantage of being able to monitor the use and effects of medicines in routine clinical practice in large patient samples.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Aware of the lack of epidemiological data on the profile of new statin users in the diabetic population of the SNS, in this letter we provide the characteristics of the subgroup of diabetic patients included in that same study (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). For this purpose, the same methods previously described have been followed.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">5</span></a> Briefly, we identified 17,288 diabetic patients initiating statins (5.9% of them were initiators of intensive treatment) in the period 2007–2011, with valid BIFAP records. The factors associated with the initiation of intensive statin treatment (compared to standard treatment) in the general population for primary prevention of cardiovascular diseases<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">5</span></a> were similar in diabetic patients, such as the year of the statin pre,scr,ipt,ion, male sex (adjusted odds ratio [OR]<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.49; 95% confidence interval [95% CI] 1.11–1.92), body mass index greater than or equal to 30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.32; 95% CI 1.00–1.75), low density lipoprotein cholesterol levels<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>190<span class="elsevierStyleHsp" style=""></span>mg/dl (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.18; 95% CI 1.49–3.19), smoker (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.26; 95% CI 0.96–1.67), high triglyceride levels (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.92; 95% CI 1.47–2.51), elevated total cholesterol levels (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.45; 95% CI 1.06–1.98), high blood pressure treated with angiotensin antagonists (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.35; 95% CI 1.04–1.75), and pre-treatment with oral antidiabetics (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.67; 95% CI 1.20–2.33), insulin (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.48; 95% CI 1.26–4.89), and insulin and oral antidiabetics (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.45; 95% CI 2.15–5.54). Associations were also observed with the simultaneous use of fibrates (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.19; 95% CI 1.41–3.41) or history of hypothyroidism –contraindicated for the use of high doses of rosuvastatin (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.67; 95% CI 1.08–2.59), which are risk factors predisposing to adverse reactions associated with statins at high or strong doses. Admittedly, some of these adverse reactions can be serious, although not very frequent, such as rhabdomyolysis.
