They circulate in the blood for 7–10 days and play a critical role in haemostasis.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">1</span></a> Thrombocytopenia is defined as a platelet count below the normal range for the population (e.g. a platelet count of less than 100<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span><span class="elsevierStyleHsp" style=""></span>L<span class="elsevierStyleSup">−1</span>) and results from disturbances in platelet production, distribution, or destruction. Thrombocytopenia is frequently found in a variety of disease conditions such as immune thrombocytopenic purpura (ITP), chronic liver cirrhosis, chemotherapy-induced myelosuppression and myelodysplastic syndromes.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Romiplostim and eltrombopag are thrombopoietin receptor (TPOr) agonists that promote megakaryocyte differentiation, proliferation and platelet production. In the European Union, romiplostim and eltrombopag are approved for the treatment of chronic adult ITP splenectomised patients who are refractory to other treatments.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">4,5</span></a> More recently, eltrombopag has been authorized for the treatment of thrombocytopenia in adult patients with chronic hepatitis C virus infection, where the degree of thrombocytopenia is the main factor preventing the initiation or limiting the ability to maintain optimal interferon-based therapy.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">5</span></a> In healthy volunteers, romiplostim and eltrombopag increased platelet counts in a dose-dependent manner with platelet counts rising within the first 2 weeks after therapy has started.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">4–6</span></a> In patients with thrombocytopenia, high levels of platelet counts above the normal ranges have been associated with thromboembolisms, although this relationship may be affected by a number of other factors.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In 2011, we conducted a systematic review of the risk of thromboembolism with TPOr agonists in adult patients with thrombocytopenia.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> The main findings were published in 2012 suggesting a non-statistically significant increased risk of thromboembolic events for these agents, but analyses were limited by lack of statistical power and incomplete outcome reporting. Since then, new therapeutic indications have been approved for some of these therapies,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">5</span></a> and large randomized controlled trials (RCTs) have been recently published.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12</span></a> Therefore, we considered timely to update our original systematic review and meta-analysis examining whether TPOr agonists affect thromboembolism occurrence in adult thrombocytopenic patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">For this updated systematic review, we largely followed the methods as our 2012 review.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Search strategy and selection criteria</span><p id="par0025" class="elsevierStylePara elsevierViewall">We searched PubMed/MEDLINE, and the Cochrane Central Register of Controlled Trials with the specific terms: “romiplostim” and “eltrombopag”, for articles published from January 2011 to December 2014 and without language restriction. The Cochrane highly sensitive search strategy for identifying RCTs<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">13</span></a> was used (see <a class="elsevierStyleCrossRef" href="#sec0065">Appendix A</a>Appendix for specific search strategy). We also searched websites of the European Medicines Agency (<a id="intr0050" class="elsevierStyleInterRef" href="http://www.ema.europa.eu/">www.ema.europa.eu</a>) and of the U.S. Food and Drug Administration (<a id="intr0055" class="elsevierStyleInterRef" href="http://www.fda.gov/">www.fda.gov</a>). On-line clinical trials registries, including government (<a id="intr0060" class="elsevierStyleInterRef" href="http://www.clinicaltrials.gov/">www.clinicaltrials.gov</a>) and manufacturers (<a id="intr0065" class="elsevierStyleInterRef" href="http://www.amgentrials.com/">www.amgentrials.com</a> and, <a id="intr0070" class="elsevierStyleInterRef" href="http://www.gsk-clinicalstudyregister.com/">www.gsk-clinicalstudyregister.com</a>) were also searched, to identify eligible unpublished reports.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The selection criteria followed those of the original review.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> Briefly, selected studies had to be RCTs in adult thrombocytopenic patients exposed to romiplostim or eltrombopag in at least one treatment arm, and containing the outcome of interest (thrombotic and thromboembolic events). Only direct comparisons with control interventions (e.g., placebo and/or standard of care) were considered.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data collection and quality analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">Data extraction from source documents was done independently by two reviewers. Disagreements were resolved by consensus. Should more than one report of the same clinical trial exist, the report with the most updated or complete data was included. The quality of studies included was independently assessed by two reviewers without blinding to authorship or journal using the Jadad's scale.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">14</span></a> As in the original report,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> the number of patients with a thromboembolic event and total number of patients in each trial group were extracted and tabulated, along with other information including disease conditions studied, trial design and duration, age, sex, race, and platelet counts at baseline, among others (<a class="elsevierStyleCrossRefs" href="#tbl1">Tables 1 and 2</a>).</p><elsevierMultimedia ident="tbl1"></elsevierMultimedia><elsevierMultimedia ident="tbl2"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analyses</span><p id="par0040" class="elsevierStylePara elsevierViewall">We reported this systematic review and meta-analysis in line with recommendations from the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">15,16</span></a> In the primary analyses, relative risks (RR) were provided to compare the likelihood of thromboembolisms between two groups. Absolute risk ratios (ARR) and number needed to harm (NNH) were also calculated to provide the population health impact of the treatment exposures with TPOr agonists. Data were pooled by using both random-effects and fixed-effects models.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">17,18</span></a> Heterogeneity was assessed using Cochran's <span class="elsevierStyleItalic">Q</span> test and <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> statistic.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">19</span></a> Begg's rank-correlation method was used to assess publication bias and a funnel plot was also generated.<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">20,21</span></a> In addition, sensitivity analyses were conducted to measure the robustness of the main results by using a Jackknife approach (assessment on heterogeneity and/or effect size when removing one trial or a group of trials). A sensitivity analysis was also conducted to examine the possible influence of single studies by excluding possible outlier (extreme) observations. The identification of a study as an outlier was not based on an a priori statistical criterion, but rather on visual evaluation of forest plot with all selected studies.</p><p id="par0045" class="elsevierStylePara elsevierViewall">All the statistical analyses were conduced using Stata 12 (StataCorp, LP, College Station, TX, USA).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Study identification and selection</span><p id="par0050" class="elsevierStylePara elsevierViewall">Our previous review<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> included 8 studies in 1180 patients. Since then, 7 new studies have been identified. A total of 15 studies<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12,22–33</span></a> were included in this updated systematic review (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Study and patient characteristics</span><p id="par0055" class="elsevierStylePara elsevierViewall">Characteristics of the trials and patients are summarized in <a class="elsevierStyleCrossRefs" href="#tbl1">Tables 1 and 2</a>. Overall, the 15 studies<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12,22–33</span></a> enrolled a total of 3026 patients, with 2003 patients randomized to TPOr agonists, and 1023 assigned to controls (in a 2:1 ratio). Eight studies included eltrombopag as study drug.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10,11,26–29,33</span></a> Six trials included patients with ITP,<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">22–25,27–29</span></a> 3 trials were conducted in advanced cancer patients with chemotherapy-induced thrombocytopenia,<a class="elsevierStyleCrossRefs" href="#bib0305"><span class="elsevierStyleSup">26,32,33</span></a> 3 trials included thrombocytopenic patients with chronic liver disease<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10,11</span></a> and 3 trials in thrombocytopenic patients with myelodysplastic syndromes.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">12,30,31</span></a> The mean age of patients was 56.9 years-old and the percentage of women ranged from 21% to 87%. Nine trials had two arms,<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12,23–25,28,29,31</span></a> one had three arms<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">30</span></a> and five had four arms.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">22,26,32,33</span></a> Most of the trials with more than two arms included TPOr agonist arms of different doses and a placebo arm. Trial duration ranged from 1 to 58 weeks. All studies were reported as double-blind controlled studies except one open-label phase III study.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">25</span></a> None of the studies included thromboembolisms as study outcome, but most of them (14 out of 15 trials) defined thrombotic risk factors or previous thromboembolic events as exclusion criteria. We found that the quality of reporting among studies was variable. The average quality of the trials scored 2.5 in the Jadad's scale, and ranged from 0 (in two trials)<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">22,25</span></a> to 4 (in three trials).<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">11,28</span></a> Four studies<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">22,25–27</span></a> with score ≤1 were considered of unacceptable reporting quality. Regarding potential sources of sponsorship bias, the 7 trials on romiplostim<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">12,22,25,30–32</span></a> were funded by Amgen, the manufacturer of romiplostim (Nplate<span class="elsevierStyleSup">®</span>), and the 8 trials on eltrombopag<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10,11,26–29,33</span></a> were funded by GlaxoSmithKline, the manufacturer of eltrombopag (Revolade<span class="elsevierStyleSup">®</span>).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Main results</span><p id="par0060" class="elsevierStylePara elsevierViewall">The analysis of the outcome of interest was based on 89 patients with thromboembolisms (83.14% of them were TPOr agonists-treated patients). Estimated frequency of thromboembolisms was 3.69% (95% CI: 2.95–4.61%) for TPOr agonists and 1.46% (95% CI: 0.89–2.40%) for controls (e.g., placebo and/or standard of care).</p><p id="par0065" class="elsevierStylePara elsevierViewall">In the overall meta-analysis, for patients receiving TPOr agonists versus controls, we showed an absolute risk increase of 2.10% (95% CI: 0.03–3.90%) corresponding to a meta-analytic NNH of 48 and an 81% risk increase of thromboembolims (RR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.81; 95% CI: 1.04–3.14; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.041). Overall, we did not find evidence of statistical heterogeneity (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.43; <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.60%) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Subgroup analysis based on only trials conducted in non-ITP patients (9 trials, 2314 patients)<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12,26,30–33</span></a> yielded an absolute risk increase of 2.80% (95% CI: 0.00–5.50%) corresponding to a NNH of 36 and a RR of 2.31 (95% CI: 1.10–4.86; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.028) with no evidence of statistical heterogeneity (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.210; <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>16.40%). This was largely driven by risk of harm in the cohort of chronic liver disease: RR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.74 (95% CI: 0.97–7.75; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.058), absolute risk increase of 2.40% (95% CI: 0.04–4.3%) and NNH<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>42 (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><p id="par0075" class="elsevierStylePara elsevierViewall">The test statistic for Begg's rank-correlation method was non-statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.255) for thromboembolism occurrence, denoting no evidence of publication bias (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>; <a class="elsevierStyleCrossRef" href="#tbl3">Table 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="tbl3"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Sensitivity analysis are presented in <a class="elsevierStyleCrossRef" href="#tbl4">Table 4</a>. Overall, the results showed were consistent and similar to those of the base case meta-analysis. For example, when studies of poor reporting quality were excluded (four trials with Jadad's score ≤1),<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">22,25–27</span></a> the analysis yielded a RR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.18 (95% CI: 1.16–4.10) and a NNH<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>40. However, when the most influential trials were excluded,<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10,11,25</span></a> the sensitivity analysis yielded non-statistically significant results (RR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.42; 95% CI: 0.62–3.25).</p><elsevierMultimedia ident="tbl4"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">In this systematic review and updated meta-analysis of data from 15 RCTs, clinical evidence showed a higher risk of thromboembolic events associated to TPOr agonists compared with controls. Statistically significant results were largely driven by trials conducted in non-ITP populations, particularly in the cohort of chronic liver disease.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The TPOr agonists analyzed in this study, romiplostim and eltrombopag, represent a therapeutic option for the treatment of adult chronic ITP splenectomised patients who are refractory to other treatments. Treatment of chronic ITP has been traditionally based on strategies aimed to decrease platelet destruction either by surgical (e.g., splenectomy) or pharmacological (e.g., corticosteroids, immunoglobulins, immunosuppressants) approaches. In 2013, the European Commission granted an additional therapeutic indication for TPOr agonist eltrombopag as a treatment for thrombocytopenia in adult patients with chronic hepatitis C virus infection, where the degree of thrombocytopenia is an important factor preventing the initiation or limiting the ability to maintain optimal interferon-based therapy.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">5</span></a> The authorization granted to eltrombopag was based on results from the <span class="elsevierStyleItalic">Eltrombopag to INitiate and Maintain Interferon Antiviral Treatment to Benefit Subjects with Hepatitis C related Liver DiseasE</span> (ENABLE) program,<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,11</span></a> two randomized, double-blind, placebo-controlled trials. Patients in ENABLE-1 utilized peginterferon alfa-2a plus ribavirin as antiviral treatment and patients in ENABLE-2 utilized peginterferon alfa-2b plus ribavirin.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">11</span></a> On the current update of our previous review,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> both ENABLE trials were included. In ENABLE program, thromboembolic events were more common in the eltrombopag group in comparison with placebo (34 thromboembolic events in 31 eltrombopag patients [3%] and 5 thromboembolic events in 5 placebo patients [1%]). Reported thromboembolic complications included both venous and arterial events, but portal vein thrombosis was the most common thromboembolic event in both treatment groups (1.2% in eltrombopag versus 0.4% in placebo).<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">11</span></a> These results are overall consistent with the results of prior TPOr agonist trials. Particularly, in the ELEVATE trial,<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">10</span></a> a double-blind, placebo-controlled trial, 292 patients with thrombocytopenia due to chronic liver disease were randomized to receive TPOr agonist eltrombopag or placebo before an invasive procedure. In that trial,<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">10</span></a> eltrombopag reduced the need for platelet transfusions, but it was associated with an increased risk of thromboembolism, as compared with placebo. This finding led to the early termination of the trial. Six (4%) patients receiving TPOr agonist and 2 (1%) patients in the placebo group experienced thromboembolisms, most of them affecting the portal venous system. In the ELEVATE trial,<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">4,10</span></a> 5 of the 6 treated patients experienced thromboembolisms at platelet counts above 200<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span><span class="elsevierStyleHsp" style=""></span>L<span class="elsevierStyleSup">−1</span>.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Potential factors such as the rapid increase in platelet counts due to the administration of a higher dose of TPOr agonist, an enhancement of the prothrombotic state (hypercoagulability) due to an imbalance in coagulation factors, or the presence of endothelial damage seen in liver disease may also have contributed to these findings.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,9</span></a> Particularly in ENABLE program, patients with low albumin levels (≤35<span class="elsevierStyleHsp" style=""></span>g<span class="elsevierStyleHsp" style=""></span>L<span class="elsevierStyleSup">−1</span>) or Model for End Stage Liver Disease (MELD) score ≥10 had a 2-fold greater risk of thromboembolic events than those with higher albumin levels; and those aged ≥60 years had a 2-fold greater risk of thromboembolism compared to younger patients.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,11</span></a> The dosing adjustment schedule of TPOs is another important factor that might influence the thrombotic risk associated with these drugs. Early results from the ELEVATE trial motivated the European Medicines Agency's Committee for Medicinal Products for Human Use to modify the dosing recommendations for both TPOs from 250 to 150<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span><span class="elsevierStyleHsp" style=""></span>L<span class="elsevierStyleSup">−1</span> and from 400 to 250<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span><span class="elsevierStyleHsp" style=""></span>L<span class="elsevierStyleSup">−1</span> as the cut off point for dose reduction and dosing interruption respectively.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">4,5,9</span></a> These changes were not implemented by the U.S. Food and Drug Administration and the ENABLE program was conducted according to the U.S. dosing adjustment schedule, therefore the relevance of these results for different regions setting might not be homogeneous. In any case, the treatment paradigm of hepatitis C virus is rapidly moving and the role of interferon-based therapy in this setting is likely to be downplayed dramatically in the next future, thus relativizing the role of TPOr agonists in this therapeutic indication.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Studies in a wide variety of other clinical conditions where thrombocytopenia represents a significant clinical challenge (e.g., advanced myelodysplastic syndromes, or solid tumors receiving chemotherapy) are underway,<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">34</span></a> but the effectiveness and safety of TPOr agonists have not been established for use in other thrombocytopenic conditions than chronic ITP (for romiplostim and eltrombopag) and/or thrombocytopenic chronic liver disease (in the case of eltrombopag). Thus, TPOr agonists should not be used outside of clinical trials for the treatment of thrombocytopenia due to myelodysplastic syndromes or any other cause of thrombocytopenia other than the approved indications.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">To the best of our knowledge, no comprehensive update of the original 2012 systematic review and meta-analysis<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> has been carried out specifically for examining TPOr agonist class effect on thromboembolism. The present review supersedes all previously published findings on this topic. Our previous 2012 systematic review included 8 RCTs with a total of 1180 adult patients with thrombocytopenia, most of them in ITP patients.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> That meta-analysis revealed that TPOr agonist treatment of adult thrombocytopenic patients was associated with a non-statistically significant trend to increase the risk of thromboembolisms compared to controls (RR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.49; 95% CI: 0.68–3.27), with a more marked effect in non-ITP studies. However, the low number of total events impeded to show statistical significance. For the current update of our review, we used the same systematic and statistical methods. This updated synthesis of the clinical evidence for the risk of thromboembolism and TPOr agonists identified 7 more RCTs not included in the previous review, all of them contributing new data. The total number of participants increased by 61.0% (from 1180 to 3026 adult patients with thrombocytopenia) and the number of events by 65.2% (from 31 to 89 patients with thromboembolisms).</p><p id="par0110" class="elsevierStylePara elsevierViewall">Our study has several limitations. More importantly, we primarily derived our conclusions from trials with highly selected and heterogeneous thrombocytopenic populations of adult patients. Although findings from trials are generally consistent, the evidence is limited to a small number of trials because the majority of trials enrolled small numbers of patients, and most lacked sufficient power to determine potentially subtle differences. For example, we found only statistically significant results in one study (namely, ENABLE-2 trial)<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">11</span></a> where treatment effects were considerable and are likely to have clinical relevance. Most trials defined thrombotic risk factors or previous thromboembolic events as exclusion criteria but none used the occurrence of thromboembolisms as the primary outcome. As in other systematic reviews and meta-analyses, we used trial-level data and did not adjust our analyses for dosage of drugs used, compliance to assigned treatment, duration or outcome nature because of the limited reporting data in each trial.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Publication bias is a concern for all systematic reviews and has been empirically proven to be problematic for evaluating safety outcomes.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">35</span></a> Although we followed current recommendations for searching complementary sources (including clinical trial registries), potential reporting bias may not have been effectively ruled out. The small number of studies for individual conditions limits the validity of statistical methods to explore publication bias, such as funnel plots. Thus, the potential underreporting of adverse events in published trials is a major concern.</p><p id="par0120" class="elsevierStylePara elsevierViewall">In summary, TPOr agonists are associated with potential long-term safety problems, being the statistically significant increased risk of thromboembolic events one of the most obvious. Our results are supportive of the current information provided in the product information in the European Union,<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">4,5</span></a> indicating caution when administering TPOr agonists to patients at risk of thromboembolism.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conflict of interests</span><p id="par0125" class="elsevierStylePara elsevierViewall">The authors state that they have no conflict of interest. In 2012, a systematic review and meta-analysis reported a non-statistically significant increased risk of thromboembolic events for these drugs, but analyses were limited by lack of statistical power. Our objective was to update the 2012 meta-analysis examining whether TPOr agonists affect thromboembolism occurrence in adult thrombocytopenic patients.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs). Updated searches were conduced on PubMed, Cochrane Central, and publicly available registries (up to December 2014). RCTs using romiplostim or eltrombopag in at least one group were included. Relative risks (RR), absolute risk ratios (ARR) and number needed to harm (NNH) were estimated. Heterogeneity was analyzed using Cochran's <span class="elsevierStyleItalic">Q</span> test and <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> statistic.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Fifteen studies with 3026 adult thrombocytopenic patients were included. Estimated frequency of thromboembolism was 3.69% (95% CI: 2.95–4.61%) for TPOr agonists and 1.46% (95% CI: 0.89–2.40%) for controls. TPOr agonists were associated with a RR of thromboembolism of 1.81 (95% CI: 1.04–3.14) and an ARR of 2.10% (95% CI: 0.03–3.90%) meaning a NNH of 48.
