But they also have to be considered in the differential diagnosis of fast-growing skin tumors.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> Some of these tumors are caused by Poxvirus. It is necessary to provide a differential diagnosis from other microorganisms capable of causing a similar clinical symptomatology and histopathology and which frequently cause this type of professional infections, such as <span class="elsevierStyleItalic">Bacillus anthracis</span>, <span class="elsevierStyleItalic">Erisipelothrix insidiosa</span> and <span class="elsevierStyleItalic">Francisella turalensis</span>.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Milker's nodule and injuries caused by Orf virus are localized skin infections, caused by viruses belonging to the <span class="elsevierStyleItalic">Poxviridiae</span> family (<span class="elsevierStyleItalic">genus Parapoxvirus</span>). It is not easy to differentiate the causal agent of the clinical entity since both viruses cause a pathological process with a similar clinical symptomatology and histopathology, and in most cases they infect people in contact with animals (professional dermatozoonosis).<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> However, sporadic cases have been reported in non-professionals.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> Transmission to humans is through abraded skin, by direct contact with infected livestock or through fomites (knives, fences, animal troughs, etc),<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> causing benign injuries usually located in hands,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> proliferative, nodular and vascularized of rapid growth, sometimes ulcerated. Incubation period is 4–7 days.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> They are frequently asymptomatic. However, some patients may have fever and lymphadenopathy during the first days of infection. Although rare, there may be complications associated with this type of disease, such as erythema multiforme.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Diagnosis is provided depending on clinical history (being anamnesis data concerning contact with sheep or cattle important) and biopsy showing viral particles by electron microscopy (cylindrical shape and large size viruses: 200–400<span class="elsevierStyleHsp" style=""></span>nm wide)<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> as well as cell cultures (human fibroblasts, monkey or bovine kidney),<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> in both cases indistinguishable. Diagnosis is faster and more sensitive with the novel genomic amplification techniques.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The main aim of our study was to develop a nested PCR protocol for detecting genome of <span class="elsevierStyleItalic">Parapoxvirus</span>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Five exudates of ulcerative lesion were sent to the Virology section of Microbiology Service of HUCA in viral transport medium (UTM, Copan) of other immunocompetent patients with suspected infection by <span class="elsevierStyleItalic">Parapoxvirus</span>: 4 women and 1 man (mean age: 48<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>12.8; 31–61 years old) all of them were from rural areas except for one of the women. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the characteristics and clinical data of patients.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">For the diagnosis of infection PCR was performed using primers designed in the region encoding the viral DNA polymerase, one of the most preserved regions.</p><p id="par0035" class="elsevierStylePara elsevierViewall">To identify the <span class="elsevierStyleItalic">Parapoxvirus</span> responsible for the clinical entity, sequencing protocols were performed on 189<span class="elsevierStyleHsp" style=""></span>pb fragments obtained after PCR. Genotypic characterization of the virus was performed by analyzing sequence homology with those in NCBI databases.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Suspected <span class="elsevierStyleItalic">Parapoxvirus</span> infection was verified in all patients, being <span class="elsevierStyleItalic">Pseudocowpoxvirus</span> the viral species responsible for skin lesion in all cases regardless of the livestock in contact. This was being traditionally performed after clinical suspicion, previous contact with a particular type of animal and/or microscopic visualization of structures consistent with poxvirus. It also allowed for differential diagnosis with other microorganisms that cause diseases associated with livestock-contact, in which antibiotic treatment is necessary.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Treatment is not necessary in this infection with involution injuries. The resolution of the symptomatology, in the absence of bacterial infections, occurs within 4–6 weeks,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> with the exception of some immunosuppressed cases<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a> where treatment is required due to the poor spontaneous regression and because the lesions are large. Topical cidofovir has been used successfully.
