elsevierViewall">Osteoporosis is characterized by decreased bone mass and altered bone microstructure. This causes decreased bone strength and increased fragility, which involves an increased risk of fracture. This is the major consequence of osteoporosis and has significant negative effects on individuals, families and society as they are associated with disability, dependency and even an increased mortality in cases of hip or vertebral fracture.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> The hip fracture has become a major public health problem in developed countries, with significant impact on life quality and expectancy in those affected by it.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> Its etiology is multifactorial, although osteoporotic or fragility fracture is the most prevalent.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> It has been observed that, in Spain, they tend to occur in the elderly, with female predominance, who are mostly admitted to hospitals and require surgical intervention.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a> Mortality from this type of fracture is higher in men, being around 5% during the first hospitalization,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">3</span></a> increasing 15–20% within the first year. The estimated impairment is high, between 20 and 60%.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">It is known that the incidence of hip fracture is increasing in absolute terms due to the progressive aging of the population, although a stabilization tendency has been observed recently, even a decline when adjusted for population in women under 80 years of age in some industrialized countries analyzed.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Both genetic and environmental factors have been raised as proof of these trend changes. Some of these changes could be explained by the so-called “cohort effect” which would be associated with the time of birth and development of some particular age groups. This could have contributed some etiologic factors favoring fracture in groups that are older now, which could be different in younger groups.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,6</span></a> Among the environmental factors, we find the nutritional and metabolic ones, (vitamin D deficiency), comorbidity and insufficient physical activity (muscle atrophy), drug abuse and polypharmacy, which have been associated with a loss of bone health and an increased risk of falls.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The incidence of hip fracture shows a significant geographical variation in the world, with higher rates in Nordic countries, the United States and northern Europe and lower in countries of South America and the south of Europe.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> The incidence of hip fracture within the Spanish territory also shows variability among different geographical areas,<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">10,11</span></a> although studies have been limited to the analysis of short periods of time or very specific geographical areas, something which has not allowed a global view or a real incidence map by geographic area.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12,13</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">We have a nationwide systematic recording of hospital discharges in which hip fracture appears as the primary diagnosis. This allows us to observe the incidence of this type of fracture in recent years.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The aim of this work is to analyze the differences in the incidence of hip fracture in people over 65 years in the different autonomous regions of Spain during 14 years (1997–2010).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0035" class="elsevierStylePara elsevierViewall">It is an ecological, observational and retrospective study that includes people ≥65 years of age, who have had a fracture in the proximal third of the femur in Spain during the study period, ranging from 1997 to 2010, both inclusive.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Population data were obtained from the records of the National Statistics Institute (INE, <a id="intr0010" class="elsevierStyleInterRef" href="http://www.ine.es/">http://www.ine.es/</a>). Fracture records were obtained from the databases of the Ministerio de Sanidad y Política Social (<a id="intr0015" class="elsevierStyleInterRef" href="http://pestadistico.inteligenciadegestion.msssi.es/">http://pestadistico.inteligenciadegestion.msssi.es/</a>). These records come from the minimum basic data set (MBDS) of each patient treated in all hospitals in the country.</p><p id="par0045" class="elsevierStylePara elsevierViewall">We counted all hip fracture cases that correspond to codes 820 (820.0–820.9) of the International Code of Diseases (ICD-9) for all years. The population analyzed is ≥65 years of age, differentiated by sex and 5-yr age groups (last group ≥85 years of age). According to INE, Spain had a population of 39,583,381 inhabitants in 1997 and 47,021,031 inhabitants in 2010.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The variables analyzed were:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0055" class="elsevierStylePara elsevierViewall">Incidence: number of registered hip fractures per year and sex according to the selected codes and regions</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0060" class="elsevierStylePara elsevierViewall">Crude incidence rate: number of registered hip fractures per 100,000 inhabitants/year and sex.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0065" class="elsevierStylePara elsevierViewall">Adjusted incidence rate per 100,000 inhabitants/year and sex according to the population of the period.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">-</span><p id="par0070" class="elsevierStylePara elsevierViewall">Aging indicator: percentage of population over 75 years of age by sex.</p></li></ul></p><p id="par0075" class="elsevierStylePara elsevierViewall">No statistical parameters are calculated because no samples are analyzed, but global incidents recorded in Spain during the study period are analyzed.</p><p id="par0080" class="elsevierStylePara elsevierViewall">This document follows the guidelines of the STROBE Initiative<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">14</span></a> for epidemiological studies.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0085" class="elsevierStylePara elsevierViewall">During the analyzed period there were 119,528 hip fractures in men and 414,518 in women. The average ratio across the country between women and men for the incidence of hip fracture was 3.5.</p><p id="par0090" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the standardized crude rates per 100,000 inhabitants of the entire period, separated into 4 groups by age and sex. The crude incidence rate of hip fracture increases progressively with age in both sexes. 4% of hip fractures occur in the group aged 65–69 years. This rate has continuously increased up to becoming a 41.4% in those ≥85 years. In over 80 years of age, 68.7% of fractures occur in women and 61.1% in males. By sex, 77.6% occur in women.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows standardized crude rates per 100,000 inhabitants, the population-adjusted rates, the percentage of population ≥75 years and the ratio of the adjusted rate compared to the average of the whole country. The autonomous regions are sorted from highest to lowest adjusted incidence rate, combined with those occurring in women and men. Catalonia appears first, with adjusted fracture rate of 839.9 hip fractures/100,000 women and 359.1 in men. Galicia appears last, with 486.4 hip fractures/100,000 women and 179 in men. In women, there are 7 autonomous regions with adjusted fracture rates above average and 3 with a ratio of 1.2 (Catalonia, Valencian Community and Castilla-La Mancha). With a ratio of 0.7 are the Canary Islands and Galicia. In men, there are 4 autonomous regions with adjusted fracture rates above average and 2 with a ratio higher than 1.1 (Catalonia 1.3 and Valencian Community 1.2). The autonomous region with lowest adjusted incidence rate in women (Canary Islands) has a rate 44% lower than the one with the highest rate (Castilla-La Mancha). The region with the lowest adjusted incidence rate in males (Galicia) has a rate 50% lower than that which has the highest adjusted rate (Catalonia).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the graphical representation of the standardized rates in 3 categories for the 2 sexes: above average (ratio<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>1), equal to the average (ratio<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1) and below the average (ratio <span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>1).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> tries to illustrate the possible “cohort effect”. This figure represents, in 5-yr groups, the linear path of 65–90-year-old people over their first 30 years of life in relation to the years when bone maturation potentially happened.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">In this paper an extended period of time (1997–2010) is analyzed and this allows to know the trend of hip fracture in Spain in a global way, unlike other studies that examine shorter periods<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a>; besides, it allows a more detailed analysis based on geographical distribution by autonomous regions. The proportion of over 75 year-olds in each region has been calculated so that its possible association in the crude and adjusted incidence rates can be known. The data source used (MBDS) is well established, rooted and validated in Spain, and its communication to the central services in Spain is mandatory since 1992.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The prevalence of hip fracture in women is evident in the data obtained, with 3.5 being the average female/male ratio in the period studied. In previous studies this average ratio was close to 4, something that could indicate a change in trend 16. Among the regions of Spain this ratio varies from 3.9 in Navarra, Asturias and Galicia to 3.1 in Castilla-La Mancha.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Age is an important factor, since 85.4% of hip fractures occur in those over 75 years (86.7% in women and 80.7% in men). The progressive aging of the population would explain these figures, and given that forecasts indicate that this age group will amount to 35% by 2025, a worrying horizon awaits us 1. The age factor could explain the Canary Islands case, showing the lowest population rate ≥75 years in both men and women, being one of the lowest fracture rates among all the regions</p><p id="par0125" class="elsevierStylePara elsevierViewall">The so-called “cohort effect” of the population has been demonstrated in several studies.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,6</span></a> This effect would have affected the entire population born in a certain period in a similar manner. As an example, the population ≥85 years was born around 1925 or earlier and bone development must have occurred between the years 1935–1950. This period is associated with scarce food resources for people, as Europe was at war or had just suffered the war, especially in Spain (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><p id="par0130" class="elsevierStylePara elsevierViewall">Studies show a change in trend in the incidence of hip fracture in Spain in recent years.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a> It has been observed that although the annual crude rates increase 2.1% globally in Spain,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a> the age-adjusted rates have slowed their progress in recent years, especially in women under 75 years of age.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,15</span></a> This could be due to the aforementioned cohort effects, and has also raised some other influence, such as the widespread use of antiresorptive drugs, especially in women under 75 years of age.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">When we analyzed the hip fracture rates by region, a significant variability in incidence is observed among them. These differences between regions within the same country have also been observed in other countries, and have been independent of ethnicity and aging rates.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">9,17–19</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">According to the results of this study, the Spain could be divided in autonomous regions with high hip fracture rates compared to the average of the country (Catalonia, Valencia, Andalusia, Castilla-La Mancha, Baleares, Aragon and La Rioja), regions with the average rates for the country (Madrid, Extremadura, Murcia and Navarre) and regions with rates below the average (Galicia, the Canaries, Asturias, Cantabria, Castilla y León and the Basque Country) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). This phenomenon has been observed in other studies, but usually occurs when the north-south gradient is analyzed, as it happens in Europe between Scandinavia and the Mediterranean countries, and in America, between North and the South countries.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">18,20</span></a> Differences were also found between geographical areas within the same country in Europe.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a> Possible genetic and environmental causes have been reports, such as diet, sun exposure or vitamin D plasma concentrations. Other environmental factors, such as smoking, obesity and alcohol consumption were not considered relevant in explaining the differences in hip fracture incidence.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Other studies have assessed the possible differences between the rural population and the urban environment, and which could represent a more physically active lifestyle.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a> It has been suggested that increased physical activity in agricultural occupations would contribute to those differences. This increased muscle activity would promote a better muscle mass maintenance, and this has proven to be influential in bone health and the prevention of falls in the elderly.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">23,24</span></a> This could explain why communities such as Galicia, Asturias and Castilla y Leon, which are among the oldest populations in the country, have, however, a lower hip fracture adjusted rate.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">Other environmental factors may be affecting the hip fracture rate progression in recent years. Among them we should highlight the changes in the management of osteoporosis that health services have as part of their clinical practice. Risk factors for fragility fractures and systems for their detection have been identified for over 25 years.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,6,8</span></a> In the same vein as in other diseases, such as cardiovascular,<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> scales that integrate various risk factors have been developed. These allow, in a weighted way, to establish the potential fragility fracture risk in advance.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> One of the most widespread and most used tools in Spain is FRAX<span class="elsevierStyleSup">®</span> devised by WHO.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> Its use is spreading worldwide and is based on large cohort studies, including several Spanish.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">20,26</span></a> There are countries where the results of fracture acceptably conform to the provisions of FRAX<span class="elsevierStyleSup">®</span>.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> However, adjustments to the tool have been necessary in other countries.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> In our country, the FRAX<span class="elsevierStyleSup">®</span> is shown useful for screening women who are at increased risk of fracture. They would also benefit from a DXA.<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">28,29</span></a> Its predictive ability for hip fracture has proved to be moderate, while with the other major fractures it seems to be far from ideal.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> Threshold values for FRAX<span class="elsevierStyleSup">®</span> have been recently proposed to stratify the absolute risk of fracture (low–medium–high) in Spanish women, which are based on the follow-up conducted on the FRIDEX cohort for a decade.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">This study has limitations and strengths. Among the first ones, we would point out that this is an ecological study and thus, any analyses and conclusions made should be treated with caution because these studies do not analyze the cause-and-effect, although they can present evidence. Other studies, both at national and international level, include in the analysis other less common ICD-9 codes (821-1: other fractures of the femur) as hip fractures, which would hinder the comparison with our results. However, this does not affect the present study, which compares incidence between the different autonomous regions using the same types of hip fracture and their codification. Another limitation is that fractures not considered osteoporotic (major trauma, fractures due to falls from a height, etc.) are not clearly excluded, as contralateral fractures or other previous fractures in the same person, although, again, it would not affect the comparative objective. As strengths, it should be noted that the data collection system is based on hospital records across the country, allowing us to analyze all autonomous regions and expand the results of other studies analysing isolated region or shorter periods of time, which often perform extrapolations.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> We must be careful interpreting the epidemiological results when incidence rates between areas, time periods or countries are compared. The standardized incidence rate of hip fracture varies greatly depending on the type of population included in the analysis. Thus, the rate will be much higher if fractures are analyzed in a population aged >65 years instead of, for example, in those aged >50 years. In our case we have analyzed fractures in those aged ≥65 years, since it is known that most hip fractures at these ages are due to low impact trauma and only 11% of hip fractures in Spain,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> or 8% in Italy<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> take place under 65 years of age. The collection of all hip fractures in a 14-year period prevents against the effects of a registration deficit or specific variations that could occur in some isolated years.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Again, the advanced age factor stands out in this study. Thus, in the over 65s, in which 90% of hip fractures occurs, 85% occurs in over 75s, and two thirds, in over 80s.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Finally, the considerable variability in the incidence of hip fracture between autonomous regions becomes evident and therefore, between geographical areas of Spain. This variability is difficult to explain by structural factors such as age, except in the Canary Islands, where a smaller proportion of people over 75 years of age is associated with a different sun exposure and a location much further south than the rest of the autonomous regions.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Future studies might be designed to analyze whether the differences between autonomous regions are due to their proportion of rural population compared to urban and whether this could influence the level of physical activity in the last stages of life. AA. y, por tanto, entre zonas geográficas. Excepto en Canarias, esta variabilidad es difícil de explicar solo por factores como la edad de la población, la exposición solar o el gradiente norte-sur.
