The inversion of this ratio may represent a maker for immune-senescence. In a recent study conducted in Spain, patients with clinical events not associated with AIDS had a lower CD4/CD8 ratio and this was independently associated with age-related markers.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In our study, conducted in Spanish population, we analyzed the influence of age on the epidemiology of clinical events not associated with AIDS and the impact that the CD4/CD8 inversion may have.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">The cohort of the Severo Ochoa Hospital is prospective; collects new diagnoses of HIV infection in over 16 year olds and consists of 450 patients. Participants include those with at least one visit from January 1985 to December 2014. Participants were categorized into 3 age groups: <50, 50–64 and ≥65.</p><p id="par0025" class="elsevierStylePara elsevierViewall">To ensure the reliability of the information we have only selected patients under follow-up by one of the authors. The cohort data collection protocol was approved by the local CREC.</p><p id="par0030" class="elsevierStylePara elsevierViewall">At each participant visit, in addition to collecting data related to HIV infection (CD4+ count, CD4/CD8 ratio and HIV viral load), we collected the information on diagnosis of diseases unrelated to HIV infection and new clinical events suggestive of AIDS and related mortality. In addition, we collected demographic data such as age, race, sex and anthropometric measures.</p><p id="par0035" class="elsevierStylePara elsevierViewall">We analyze the differences in the incidence rates of age-adjusted co-morbidities and evaluated CD4/CD8 ratio inversion as a risk factor for mortality and for the development of co-morbidities.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The analysis was performed using ANOVA for quantitative variables and chi<span class="elsevierStyleSup">2</span> for qualitative variables or Fisher's exact test. The OR is presented with 95% CI using the SPSS<span class="elsevierStyleSup">®</span> 20 statistical package for Mac.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">One hundred forty-two participants contributed during the study period between January 1985 and December 2014, to 2251 person-years of follow-up.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Baseline characteristics and participants</span><p id="par0050" class="elsevierStylePara elsevierViewall">Clinical and demographic characteristics of participants, stratified by age group, are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The median age of our cohort was 48 years (interquartile range [IQR] 44–52 years). Of the 142 patients, 88 (62%) were <50<span class="elsevierStyleHsp" style=""></span>years of age, 47 (33.1%) were between 50 and 64<span class="elsevierStyleHsp" style=""></span>years of age, and 7 (4.9%) ≥65<span class="elsevierStyleHsp" style=""></span>years of age. Overall, 45 (31.7%) were female and 53 (37.3%) had clinical events of AIDS. The nadir CD4 cell count was 140<span class="elsevierStyleHsp" style=""></span>cel/μl (IQR 70–266<span class="elsevierStyleHsp" style=""></span>cel/μl), the last count was 540 CD4<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleHsp" style=""></span>cel/μl (IQR 355–732<span class="elsevierStyleHsp" style=""></span>cel/μl) and the CD4/CD8 ratio of 0.57 (IQR 0.37–0.96).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">In people ≥65<span class="elsevierStyleHsp" style=""></span>years and 50–64<span class="elsevierStyleHsp" style=""></span>years, the median duration of HIV infection was 17 (IQR 5–17) and 20 years (IQR 15–23 years), respectively. The over 65s were less likely to belong to the group of intravenous drug addiction as HIV transmission group (0% in ≥65s vs 51% between 50 and 65 years), to be infected with HCV (0 vs 55.3%) and to be an active smoker (28.6 vs 55.3% age is associated linearly with an increase in the likelihood of developing high blood pressure (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0001), diabetes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.039), bone fractures (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.029) and COPD (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.016).</p><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Antiretroviral treatment status and co-medication not associated with human immunodeficiency virus infection</span><p id="par0060" class="elsevierStylePara elsevierViewall">A total of 133 (93.7%) of the 142 participants were taking ART, and 130 (97.9%) of the 133 participants had an undetectable viral load (<20<span class="elsevierStyleHsp" style=""></span>copies/μl) at the last visit of the cohort. The median age at the start of ART was 34 years (IQR 29–40 years), and median duration of treatment was 15 years (IQR 5–17 years). 38% were receiving a regimen based on a nonnucleoside inhibitor of reverse transcriptase, 28.9% based on a protease inhibitor regimen, and 10.6% in an integrase inhibitor regimen. Older patients were more frequently taking co-medication which was not associated with HIV. Of the 142 participants, 125 (88%) received at least one co-medication not associated with HIV. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the percentage of participants with their respective number of medications not associated with HIV, stratified by age. Twenty-three (50%) of 46 participants aged between 50 and 64 years and 6 (85.7%) of 7 participants aged >65 years received ≥4 co-medications.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Incidence of clinical events</span><p id="par0065" class="elsevierStylePara elsevierViewall">The number of comorbidities, HIV-related complications and deaths are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The incidence rate of mortality, clinical events of AIDS and any clinical event were 4.5 deaths per 1000 person-years (95% CI 2.1–8.3), 23.92 cases per 1000 person-years (95% CI 17.92–31.13) and 48.3 cases per 1000 person-years (95% CI 39.58–58.37), respectively. The number of different clinical events not associated with AIDS, stratified by age, is shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>. Eight (17%) of the 47 participants aged between 50 and 64 years and 2 (28.6%) of 7 participants aged ≥65 years had ≥4 comorbidities. The age was associated with an increased incidence of diabetes mellitus (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.039), fractures (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.007), COPD (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.011) and malignancies not associated with AIDS (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.011). Ten (14.2%) of the 142 patients died. The main cause of death was neoplasm (6 [60%] of 10), infectious disease (10%), suicide (10%), cirrhosis (10%) and MPL (10%). Of the 142 participants, 53 developed a new AIDS diagnosis event. 107 clinical events not associated with AIDS were observed during the study period. Among the 36 neoplasms unrelated to AIDS, anal cancer was the most frequent (18 [12.7%]), followed by lung cancer (3.5%), liver cancer (2.1%), bladder cancer (1.4%), tonsillar cancer (0.75%) and skin cancer (0.7%). We found, after adjusting for age, an increased risk of clinical events not associated with AIDS (OR 2.25, 95% CI 1.025–4.94; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.041) and AIDS associated episodes (OR 3.48, 95% CI 1.58–7.64; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) in individuals with a CD4/CD8 ratio <0.7. Also, although it was not statistically significant, patients with a CD4/CD8 ratio <0.7 had a higher mortality risk (OR 5.96, 95% CI 0.73–48.40; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.061).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Although some studies suggest that HIV-infected patients have a higher prevalence of comorbidities,<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">8–12</span></a> such as fractures, osteoporosis, lung disease, neoplasms unrelated to AIDS or cardiovascular disease, these analyses are limited as they have not taken into account the effect of ART's long-term toxicity, metabolic changes associated with HIV infection and other important risk factors, such as alcohol consumption or smoking, drug abuse and the effect of HBV/HCV coinfection.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The effects of age and HIV infection in increased clinical events not associated with AIDS<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">13</span></a> has been recently documented, and comparative studies that have explored in populations with HIV infection and without it the epidemiology of the most frequent comorbidities found increased rates of fractures, acute myocardial infarction, diabetes mellitus and neoplasms non-diagnostic of AIDS in older HIV infected individuals.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">8,11,12,14</span></a> We know from the publication of the <span class="elsevierStyleItalic">Veterans Aging Cohort Study</span><a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">13</span></a> study that HIV-infected patients ≥50 years of age present with more hypertension, diabetes mellitus, vascular disease, lung disease and kidney disease, while substance abuse and psychiatric illnesses are lower in this group. The concept of premature aging has been raised by data from a cross-sectional study entitled <span class="elsevierStyleItalic">National Health and Nutrition Examination Survey</span>,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">15</span></a> in which a higher prevalence of hypertension, hypertriglyceridemia, low bone mineral density and lipodystrophy is detected in patients ≥50 years of age, compared to that found in HIV-uninfected population adjusted for age. In our study, the relative risk of hypertension, renal disease as measured by MDRD, diabetes, fractures and COPD was higher in groups of 50–64<span class="elsevierStyleHsp" style=""></span>years of age and ≥65<span class="elsevierStyleHsp" style=""></span>years of age, compared to the participants <50<span class="elsevierStyleHsp" style=""></span>years of age.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Residual immunodeficiency and a state of chronic inflammation may add a risk to developing non-AIDS related diseases associated with age, even in patients taking ART.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">5</span></a> One way to estimate the degree of immune deficiency is to measure the CD4/CD8 ratio. Like us, other authors have reported that the CD4/CD8 ratio inversion is associated with an increased risk of clinical events–both related to AIDS as well as non-related–and to a higher mortality.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">16</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">The main limitation of our study is the lack of comparison in the disease incidence rate in HIV-infected patients as there was no reference group with similar demographic characteristics and not HIV-infected. This complicates the interpretation that the increase of comorbidity in elderly patients should be attributed to the effect of age or to a reduction of immune recovery.</p><p id="par0090" class="elsevierStylePara elsevierViewall">In conclusion, comorbidities not associated with AIDS, particularly cardiovascular disease, osteoporosis, diabetes mellitus and neoplasms not associated with AIDS, are increasing in HIV-infected patients and increases with age.
