During admission, she is diagnosed with diabetes with frequent blood glucose controls. She presented with a very severe palmoplantar hyperkeratosis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), suffered from birth, for which she never consulted any doctor as her mother and maternal grandfather, siblings and some nephews/nieces also had it. Given the clinical, family history and progression, she was diagnosed with Unna-Thost syndrome. It is a hereditary disease with an autosomal dominant inheritance. It is characterized by a well-defined and symmetrical diffuse keratoderma affecting the palmoplantar region which stops suddenly at wrist level. It has no other associated clinical manifestations. The most beneficial treatment is the use of keratolytic agents associated with oral retinoids.
