It is a relatively common complication in haematological malignancies with high proliferative rates and less common in solid tumours (ST), although it has been described in a great variety of tumours in isolated cases.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> Spontaneous TLS (STLS), which occurs in the absence of chemotherapy or radiotherapy, is very rare in ST.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The clinical course of this disease is usually one of rapid progression, which, together with delayed diagnosis, determine a high mortality.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Few studies have examined the clinical characteristics and prognosis of tumour lysis in ST.</p><p id="par0020" class="elsevierStylePara elsevierViewall">This study includes a series of 19 patients with ST diagnosed with TLS and STLS in our hospital over a period of 16 years. Thus, the objectives were: to describe the clinical signs and symptoms and prognosis of these diseases and to analyse the differences as per the etiologic subtype.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0025" class="elsevierStylePara elsevierViewall">All patients with solid tumours diagnosed of TLS or STLS at the University Hospital Ramon y Cajal in Madrid between January 2000 and May 2016 were included.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The baseline characteristics of the medical records were collected, including demographics, Charlson comorbidity index, history of chronic kidney disease, or hyperuricemia, ECOG scale on admission,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> as well as the type of cancer and the extent of metastatic involvement.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The diagnosis was made following the Cairo-Bishop<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> criteria proposed in 2004. According to this classification, TLS laboratory abnormalities include the development of 2 or more of the following abnormalities within 3 days prior or 7 days after initiation of treatment: hyperuricemia, hyperkalaemia, hyperphosphatemia, hypocalcaemia, or a 25% increase over baseline values. TLS is clinically defined by increased serum creatinine, development of cardiac arrhythmia or sudden death, or seizures.</p><p id="par0040" class="elsevierStylePara elsevierViewall">STLS was considered when the previously described abnormalities developed in the absence of chemotherapy or radiotherapy.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Kidney function was estimated by calculating the glomerular filtration rate (eGFR) by the MDRD-4 formula. To stratify acute renal failure, the creatinine change criteria proposed by the <span class="elsevierStyleItalic">Acute Kidney Injury Network</span> (AKIN)<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> was used.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Given the retrospective nature of the study, it was not necessary to obtain an informed consent from patients, although the study was done according to the principles of the Declaration of Helsinki.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study design and statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">This study is retrospective and observational. The Student's t-test for unpaired samples was used for the comparison of 2 independent continuous variables, or the nonparametric Mann–Whitney test, according to the characteristics of the variables. The chi-square test with continuity correction was used to compare discrete variables. Multivariate analysis was not performed given the small size of the sample.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Data were presented as mean and standard deviation or as median and interquartile ranges. A <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 value was considered statistically significant.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Statistical analysis was performed using IBM's SPSS version 21.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">19 patients were included (mean age 63<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>16 years): 10 patients (53%) had TLS, and 9 (47%) STLS. The different types of neoplasms are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and the baseline clinical characteristics of the patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The prevalence of lung tumours (42%) is noteworthy, with advanced stages and poorly differentiated histological forms. In 18 patients (95%), the condition occurred close to the initial diagnosis of the disease. A history of chronic kidney disease was uncommon (16%) with stage G3A in all these patients (average estimated glomerular filtration rate 53.7<span class="elsevierStyleHsp" style=""></span>ml/min/1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">In general, tumour burden was high, with multiple liver metastasis in 11 patients (58%), peritoneal in 6 (32%) and bone in 4 (21%). Liver function was impaired in those patients with multiple liver metastasis.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The median time from the start of chemotherapy until the development of TLS was 3 days (interquartile range 2–7). Seven patients (70%) were treated with platinum.</p><p id="par0085" class="elsevierStylePara elsevierViewall">All patients started with ARF. A third of cases suffered severe renal impairment (AKIN 3) and the rest had mild-moderate impairment (AKIN 1–2). Nephrotoxicity by commonly used drugs (antibiotics, anti-inflammatory …) was ruled out in all cases. Obstructive aetiology was ruled out by urological ultrasound. It is noteworthy that digestive losses occurred in 4 cases in the form of vomiting and diarrhoea with the consequent decrease in circulating volume.</p><p id="par0090" class="elsevierStylePara elsevierViewall">STLS patients presented seizures and arrhythmias more often (atrioventricular block, QRS widening or ventricular fibrillation), although no statistically significant differences were observed.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Severe hyperuricemia together with ARF were the key findings in the diagnosis. Uric acid crystals in urine were observed in 14 patients (74%), and although the uric-creatinine ratio could be determined only in 5 cases, this was above 1<span class="elsevierStyleHsp" style=""></span>mg/g of creatinine (mean 1.2<span class="elsevierStyleHsp" style=""></span>mg/g creatinine) in all these cases.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The degree of hypocalcaemia was significantly more severe in the TLS group, while hyperphosphatemia, although higher in this group, did not reach statistical significance with respect to STLS.</p><p id="par0105" class="elsevierStylePara elsevierViewall">All patients were treated with intensive IV hydration therapy (physiological saline and bicarbonate) and diuretics (furosemide). A uricolytic agent (rasburicase) was used in 63% of patients, and xanthine oxidase inhibitors (allopurinol) in 26%.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Renal replacement therapy with haemodialysis was only performed in 3 patients (16%) with TLS who were on chemotherapy, 2 of them recovering renal function afterwards. The haemodialysis median was 8 days.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Seven patients (37%) recovered renal function after 9 days (median, interquartile range: 4–12 days), and this recovery was more frequent in the TLS group.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Mortality during hospitalisation was high, especially in the STLS group. Of the total, 9 patients (47%) died from tumour lysis and 3 (16%) for other reasons despite having recovered from tumour lysis (acute myocardial infarction, pneumonia, and gastrointestinal bleeding). <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the characteristics of patients according to the clinical course of tumour lysis. Patients with an unfavourable outcome were younger, with further deterioration of renal function and increased frequency of STLS, although the differences did not reach statistical significance.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0125" class="elsevierStylePara elsevierViewall">The results of this study show that TLS in ST is associated with high mortality, especially in spontaneous forms (STLS), and the degree of deterioration of renal function may be one of the main prognosis determinants.</p><p id="par0130" class="elsevierStylePara elsevierViewall">This study reports the largest series of cases published of TLS in ST in a single hospital. It has been described that the different incidence of TLS between hematologic and solid tumours could be due to differences in sensitivity to antitumor drugs, heterogeneity of solid tumours as well as increased difficulty in its identification since TLS in ST may appear some weeks after treatment initiation.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Patients with a high tumour burden, high levels of lactate dehydrogenase and more chemosensitive neoplasms would have a higher risk of developing TLS, and, therefore, prophylactic treatment before chemotherapy would be advisable.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">STLS is associated with increased mortality, although the underlying mechanisms are still unknown. It has been speculated that necrosis of bulky tumour masses as a result of insufficient blood supply or pressure of the surrounding tissue (especially at liver or bone level), could initiate the pathogenic cascade of TLS, also activating inflammatory and immunological mechanisms that ultimately resulting in systemic inflammatory response, ARF and arrhythmias.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">2,3,7</span></a> In addition, in our study, hepatic dysfunction may have led to an altered purine metabolism.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Another noteworthy finding in this series was the younger age of patients with unfavourable progression. Some speculations to justify this association could be a delay in diagnosis due to lower clinical suspicion, as well as a more aggressive clinical course in this group of patients.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The development of ARF within tumour lysis may be multifactorial, but is usually the result of uric acid and calcium phosphate precipitation at tubular level.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">3,9</span></a> In this sense, there is controversy regarding the usefulness of the uric-creatinine ratio for its differential diagnosis.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> However, the effectiveness of current treatment on the generation of uric acid appears to have decreased the incidence of acute urate nephropathy in haematological patients series.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">Furthermore, it has been proposed that STLS is associated with a lower hyperphosphatemia and hypokalaemia due to the quick absorption of phosphorus by the remainder of the active tumour,<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">3,10</span></a> consistent with the findings of our study.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Although tumour lysis is a potentially reversible condition, very few patients received dialysis treatment in this series. Some patients did not meet strict criteria for dialysis, and in other cases it was not considered suitable due to the poor prognosis of the underlying neoplasm.</p><p id="par0160" class="elsevierStylePara elsevierViewall">This study has several limitations; first, causal relationships were not established given the observational and retrospective character of the same. The small size of the sample made the statistical analysis of possible worse outcome determinants difficult. In addition, given that no patient underwent a renal biopsy, neither could an associated glomerular or interstitial disease component be ruled out, although this is unlikely, given the chronology and clinical findings.
