Age-adjusted by the direct method, using the age group distribution of the 2003 SNHS participants: 16–39 years, 40–59 years and ≥60 years.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Excess body weight is a key risk factor for the development of type 2 diabetes mellitus (DM).<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,2</span></a> In addition, both overweight and obesity as well as diabetes are associated with increased morbidity and mortality.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">2–4</span></a> For these reasons and the worldwide increase in obesity<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a> and DM,<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a> both conditions have become high-priority public health problems.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Two cross-sectional studies have recently been conducted in Spain with measured data to assess overweight and obesity. The ENRICA study (≥18) showed a prevalence of obesity (body mass index [BMI]<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) 22.9%.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> Subsequently, the ENPE study (25–64 years of age) found an obesity rate of 21.6%.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> In addition, there has been an increasing trend in the prevalence of overweight, obesity and morbid obesity in Spain using data from the Spanish National Health Survey (SNHS).<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">9–11</span></a> However, another work with data obtained between 2000 and 2010 found a decrease in obesity in women over 60 years of age.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">On the other hand, in the Di@bet.es cross-sectional study, a DM frequency of 13.8% was observed in the Spanish population over 18 years of age.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> Additionally, in recent years, some studies have shown an increased prevalence of DM in Spain.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">10,14</span></a> But, as it occurs with obesity, these studies are limited in terms of not assessing long periods of time and do not provide data about the most recent years.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Since the SNHS contain data representative of the Spanish population and are conducted with a similar methodology over time, they are a good source of information to evaluate the over-time trend of excess body weight and DM, despite using reported (or declared) data. The aim of this study was to determine, through the information contained in the SNHS, the over-time trend of obesity and DM prevalence between 1987 and 2012 in Spain, both in the general population as well as stratified by sex and age.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">The SNHS are cross-sectional studies representative of the Spanish population conducted on a regular basis. In this study, the SNHS of the years 1987, 1993, 1995, 1997, 2001, 2003, 2006–2007 (2006/2007) and 2011–2012 (2011/2012) were used. The SNHS until 2001 were conducted by the Center for Sociological Research; from 2013 onwards the SNHS were carried out by the National Statistics Institute.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> Samples of adult SNHS are composed of non-institutionalized adults 16 years of age or over, except in the SNHS of 2011/2012, which also had 15 year olds, who were not included in this work. Participants in the SNHS from 1987 to 2001 were selected through a multistage sampling, stratified by clusters, with selection of primary sampling units (municipalities) and secondary units (sections) in a proportional randomized way, and the last units (individuals) by random routes and sex and age quotas. From the 2003 SNHS, participants have been selected through a multistage stratified sampling. The first stage units were the census tracts. The second stage units were the main family dwellings. One adult was selected within each household. The total number of adults participating in all the surveys about whom, at least, their BMI or presence of DM was known were 156,440. The subjects participating in the 1995 and 1997 SNHS were not weighted. In the rest of the SNHS, participants have a weighting coefficient to ensure the population representativeness.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> The average age of participants in all the SNHS was 44.8 years (SD 18.6), 51.5% of whom were women. The characteristics of the participants of different SNHS are shown in Appendix Bel annex (supplementary table available <span class="elsevierStyleItalic">on-line</span>).</p><p id="par0030" class="elsevierStylePara elsevierViewall">For the study of overweight, obesity and morbid obesity, reported (or declared) weight and height data were used, obtained through the following questions: “Could you tell me, approximately, how much you weigh without shoes or clothes?” and “could you tell me, approximately, how tall are you without shoes?”. Reported weight and height data in the SNHS were previously validated.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> BMI was calculated by dividing weight (kilos) by the height square (meters). Participants were grouped according to BMI in overweight (25.0–29.9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>), obesity (≥30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) and morbid obesity (BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>40<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>).<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a> Morbidly obese subjects are also included in the obesity category. Given that in the 1987 SNHS weight was collected with only 2 digits, that SNHS was excluded for the study of the prevalence of morbid obesity. Participants weighing less than 30<span class="elsevierStyleHsp" style=""></span>kg (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>54) or with a height of less than 100<span class="elsevierStyleHsp" style=""></span>cm (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>31) were ruled out for the study of overweight, obesity or morbid obesity.</p><p id="par0035" class="elsevierStylePara elsevierViewall">For the study of DM, it was considered that a participant had the disease if a doctor had confirmed this diagnosis to the patient. DM data reported (or declared) in the SNHS had been previously validated.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Prevalence estimates are shown in percentages. To present data by age strata, this was categorized into 3 groups: 16–39 years, 40–59 years and 60 years. The age-adjusted prevalence of the different years was standardized by the direct method using the 2003 SNHS population as reference and using the 3 age categories described above. Logistic regression models were performed to assess the over-time trend of overweight, obesity and DM. In these models, the event that wanted to be studied was the dependent variable, such as obesity. Independent variables included age (3 categories), sex and survey year (continuous). The <span class="elsevierStyleItalic">odds ratios</span> (OR) express the increase by one calendar year. To assess the over-time trend of morbid obesity (dependent variable) a Poisson regression model was adjusted, since the episode studied was very rare. The independent variables of this model were the same as those of logistic regression models. Regressions were also conducted in both sexes and in the 3 age strata to evaluate over-time trends in these subgroups. On the other hand, in order to assess whether the over-time trend was different in men and women an interaction term was created between the year of the survey and sex adding this new independent variable in the adjusted model. Finally, in order to study whether the over-time trend was linear or non-linear, the year of the survey squared was introduced in the models as a new independent variable. Statistical analyses were performed using STATA version 12.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">According to data from the SNHS, the crude prevalence of overweight (BMI 25.0–29.9), obesity and morbid obesity in Spanish adults in 2011/2012 was 36.1% (95% CI 35.3–37.0), 16.7% (95% CI 16.1–17.3) and 0.89% (95% CI 0.71–1.06), respectively. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows how from 1987 to 2012 a slight increase in the adjusted overweight prevalence was observed, going from 34.0% (95% CI 33.2–34.8) to 35.8% (95% CI 35.0–36.6). When this adjusted over-time trend was assessed in both sexes, a statistically significant increase was observed in men but not in women (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). A significant increase in the adjusted prevalence of obesity was observed in the same period, from 8.0% (95% CI 7.5–8.5) to 16.5% (95% CI 15.8–17.1) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>; <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). The OR adjusted for each calendar year of having obesity was 1.04 (95% CI 1.03–1.04) for the whole of the Spanish adult population (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). This OR equals an annual (adjusted) absolute increase of about 0.3 percentage points in the prevalence of obesity. The adjusted prevalence of obesity increased in both sexes and in all age strata (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). An increase was observed when calculating the combined adjusted prevalence of overweight and obesity (BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>25), rising from 42.0% (95% CI 41.2–42.8) to 52.2% (95% CI 51.4–53.1) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). A growing trend was also observed in the adjusted prevalence of morbid obesity between 1993 and 2012, rising from 0.20% (95% CI 0.13–0.27) to 0.88% (95% CI 0.70–1.05) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>; <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). While referring to the 1993 SNHS, the prevalence ratio of morbid obesity adjusted for each calendar year was 1.06 (95% CI 1.04–1.08) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). This means, roughly, an absolute increase (adjusted) in the prevalence of morbid obesity of 0.03 percentage points annually. The increase was significant in both sexes. A significant growth was observed in all strata while assessing the over-time trend by age and sex strata, except in women aged 40–59 years (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">In the adjusted models, a worse outcome was observed in males versus females in the prevalence of overweight (interaction <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), obesity (interaction <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and morbid obesity (interaction <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005). On the other hand, a slowing down in the growth rate was observed regarding overweight (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and obesity (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001), but not of morbid obesity (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.14) in the adjusted models during the study period. In assessing the rate of increase by sex, it was observed that the overweight growth rate in men lessen (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) in the adjusted model during the study period. In the case of obesity, growth was attenuated in women (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001), but not in men (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.20) in the adjusted models.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The overall prevalence of DM according to the SNHS of 2011/2 was 7.4%. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> and <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows how the adjusted prevalence in Spain increased between 1987 and 2012 from 4.2% (95% CI 3.9–4.5) to 7.1% (95% CI 6.7–7.4). Using as reference the 1987 SNHS, the adjusted OR for each calendar year of having DM was 1.02 (95% CI 1.02–1.03) for the whole adult population (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). This OR represents an approximate absolute prevalence increase (adjusted) for DM of 0.1 percentage points annually. The adjusted prevalence of DM increased in both sexes (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). This increase was significant in men and women from the age of 60 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) when data was stratified. A faster growth in the prevalence of DM was observed in males than in females (interaction <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) in the adjusted model. On the other hand, the rate of increase in the prevalence of DM did not change during the study period (quadratic model <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.60) in the adjusted models.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">In Spain, between 1987 and 2012, this study found an increase in the prevalence of overweight in men and obesity and morbid obesity in both sexes. According to data from the 2011/2 SNHS, the prevalence of overweight (BMI 25.0–29.9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) was 36.1% and the prevalence of obesity (BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30.0<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) was 16.7% in Spanish adults aged 16 or older. These prevalence rates are somewhat lower than those found in studies using measured data rather than reported (or declared), such as the ENRICA cross-sectional study, conducted between 2008 and 2010 on a representative noninstitutionalized population sample aged 18 or older, which found a 39.4% overweight prevalence (BMI 25.0–29.9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) and 22.9% of obesity prevalence (BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30.0<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>).<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> Subsequently, the ENPE cross-sectional study observed a prevalence for overweight (BMI 25.0–29.9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) of 39.3% and 21.6% for obesity (BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30.0<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>).<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> The ENPE study was conducted in 2014–2015 on a representative noninstitutionalized population sample between 25 and 64 years of age with anthropometric home measurements. The lowest prevalence found in the SNHS is probably due to the use of reported (or declared) data, which underestimate the BMI.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> In addition, the age range of the participants of the 3 studies differ, which may also partly explain the differences.</p><p id="par0065" class="elsevierStylePara elsevierViewall">On the other hand, the prevalence of overweight and obesity of the ENPE and ENRICA studies is higher than the DORICA study.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> The DORICA study was conducted between 1990 and 2000, with objective measurements on representative adult samples (25–60 years) of 9 autonomous regions and found a prevalence of 15.5% for obesity and 39.2% for overweight (BMI 25.0–29.9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>),<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> which supports the growing trend found with SNHS. In addition, this growing trend obtained in Spain coincides with that observed worldwide.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">In assessing prevalence trends by sex, an increase in overweight was only observed in men, while an increase in obesity and morbid obesity was found in both sexes, although faster in males. In addition, when considering prevalence by sex, overweight is higher in men in all SNHS. The opposite is true when assessing the prevalence of morbid obesity, which is higher in women in all SNHS. In the case of obesity, the largest increase in males led to the prevalence being higher in women in the first SNHS studied but lower than that of men in the last SNHS (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The recent ENRICA<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> and ENPE<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> studies found a higher prevalence of overweight and obesity in men, which coincides with the findings in the last SNHS. In contrast, the DORICA<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> (1990–2000) study observed a higher prevalence of overweight but less obesity in men, confirming what was found in the SNHS. Although men have a higher prevalence of obesity in the last SNHS, this does not occur in the 60 or older age stratum, which was also published in the ENRICA<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> study, where in the 65 or older stratum women still have a higher obesity prevalence than men. The improved trend in the prevalence of obesity in women compared to men in recent years has also been observed in Catalonia.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> Finally, it should be noted that a higher prevalence of overweight and obesity was found in older people, as reported in other studies.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">7,8,19</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The increasing trend observed in the prevalence of morbid obesity is in line with findings in other Western countries.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a> In addition, we observed a greater increase in relative terms of morbid obesity versus obesity. The more unfavorable evolution of the most severe forms of obesity coincides with other publications.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">According to data from the SNHS, the obesity trend has continued to grow, but its growth rate has slowed down slightly. However, we have no data to confirm that it has stabilized, unlike that found in the USA,<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a> or in a study conducted among 60-year-old participants in Spain.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">12</span></a> This increase in the prevalence of obesity is probably multifactorial in origin. First, the adoption of a “westernized” dietary pattern, low in fiber and high in energy density and the loss of the Mediterranean dietary pattern, rich in fruits, vegetables, fish and olive oil.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> Increased consumption of certain foods such as chips, sugary sodas or red or processed meats, can especially contribute to this increase in obesity.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> The rise of sedentary lifestyles,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> changes in microbiota or the presence of endocrine disruptors may also be involved. Although our goal is not to investigate the causes of this epidemic, the results of this work, which show a rapid increase in obesity in 25 years, support the fact that a change in risk factors has occurred in a short period of time.</p><p id="par0085" class="elsevierStylePara elsevierViewall">On the other hand, the 2011/2 SNHS estimated the prevalence of DM in 7.4%. This figure is lower than the 13.8% observed in the Di@bet.es cross-sectional study conducted in 2009–2010 in a representative sample of the Spanish population aged 18 or older, and in which an oral glucose tolerance test was performed among participants.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> However, it is quite close to the 7.8% of known DM reported. In a 2009 cross sectional study in Catalonia conducted with electronic clinical data records obtained retrospectively, a type 2 DM prevalence in adults aged 31–90 years was estimated at 7.6%.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> Given that the SNHS has found a 7.4% prevalence in adults aged 16 or older, SNHS estimates are fairly close to the percentages of known DM observed in these two studies.</p><p id="par0090" class="elsevierStylePara elsevierViewall">An increasing trend in the prevalence of DM has been observed, in line with other studies conducted in Spain<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">14</span></a> and other Western countries.<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">26,27</span></a> The 2011/2012 SNHS shows a higher prevalence of DM in men, a finding that was also published in the Di@bet.es study.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> Note that in the pre-2006 SNHS women had a higher prevalence of diabetes than men, but this prevalence was reversed in the 2006/2007 and 2011/2012 SNHS, similar to what we found when studying the prevalence of obesity. A greater increase in the prevalence of DM in men versus women was to be expected if we consider that overweight, obesity and morbid obesity have increased more in men and the association between obesity and DM is very strong.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,2</span></a> This association has also been confirmed in other countries where the prevalence of obesity and DM has experienced a parallel progression.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> It must be highlighted that the prevalence of DM remained stable in women aged 40–59 years (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), a finding that is probably related to the decrease in overweight prevalence in women of that age as well as to the fact that the increased prevalence of obesity in that subgroup has been small in magnitude (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). No significant change in the prevalence of DM has been observed in women below 40 years of age, although type 2 DM is less prevalent at that age, and that fact, therefore, is heavily influenced by the prevalence of other types of DM. A higher prevalence of diabetes was found in older people, as shown in previous publications.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,26</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The main limitation of this study is the use of referred data. Since participants underestimate weight and overestimate height, the BMI calculated with data reported (or declared) is lower than the actual one.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> This makes the prevalence of overweight, obesity and morbid obesity undervalued, but does not invalidate the study of over-time trends since the methodology used in the different SNHS is similar. Using reported (or declared) data also underestimates the true prevalence of DM<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a>; however, it does not prevent the study of the over-time trend. In addition, the 2011/2 SNHS data are quite similar to the prevalence of known DM found in the Di@bet.es<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> study and data of DM diagnosed in Catalonia.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> On the other hand, a study conducted in the USA,<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> using data from NHANES, found an increase in known DM between 1988–1994 and 2005–2006, not so of the undiagnosed DM in the study period, which would support the usefulness of the SNHS in order to know the changes in the prevalence of DM. Another limitation of this study is the change in diagnostic criteria for DM in 1997, when baseline glucose was lowered from ≥140<span class="elsevierStyleHsp" style=""></span>mg/dl to ≥126<span class="elsevierStyleHsp" style=""></span>mg/dl, as the basis for DM diagnosis.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a> Since the prevalence of DM had already increased before 1997 according to SNHS data and it has also increased in recent years, we do not believe that this change alone explains the increase found. The inclusion of glycated hemoglobin as a diagnostic criterion of DM in 2010 may have had an influence in the prevalence of DM in the 2011/2 SNHS.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a> A further limitation is the inability to distinguish between different types of DM. Finally, one cannot rule out that some bias may have changed over the years and could have influenced the results.</p><p id="par0100" class="elsevierStylePara elsevierViewall">However, this work has important strengths, such as the use of data representative of the Spanish population over 25 years and the large sample size. The use of adjusted measurements allows a better trend assessment of the various diseases, controlling confounders such as the population aging.</p><p id="par0105" class="elsevierStylePara elsevierViewall">In conclusion, between 1987 and 2012, a growing trend in the prevalence of overweight and obesity in men and morbid obesity in both sexes has been observed in Spain, especially in males. Also, an increase in the prevalence of DM has been observed, but this has not affected women under 60 years of age. Our objective was to examine trends in the prevalence of overweight, obesity, morbid obesity and DM in Spain from 1987 to 2012.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Participants and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Data were obtained from 8 waves of the national health surveys which are cross-sectional studies conducted in representative samples of the Spanish adult population. Self-reported data of 156,440 adults (≥16 years) from 1987 to 2012 were used. Body mass index (BMI) was calculated from self-reported weight and height. Overweight was defined as a BMI of 25.0 to 29.9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>, obesity as a BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> and morbid obesity as a BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>40<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>. DM was considered present if the participant reported having been diagnosed. Age-adjusted prevalence was adjusted by the direct standardization method to the 2003 survey population.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">From 1987 to 2012 age-adjusted prevalence of overweight increased from 34.0% (95% confidence interval [95% CI] 33.2–34.8) to 35.8% (95% CI 35.0–36.6), prevalence of obesity from 8.0% (95% CI 7.5–8.5) to 16.5% (95% CI 15.7–17.1%) and DM prevalence from 4.2% (95% CI 3.9–4.5) to 7.1% (95% CI 6.7–7.4%). Morbid obesity increased from 0.20% (95% CI 0.13–0.27) in 1993 to 0.88% (95% CI 0.70–1.05) in 2012. El objetivo de este trabajo es conocer la tendencia de la prevalencia de sobrepeso, obesidad, obesidad mórbida y DM en España entre 1987 y 2012.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Participantes y métodos</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">Se utilizaron 8 encuestas nacionales de salud, que son estudios transversales representativos de la población adulta española. Se incluyeron datos declarados de 156.440 adultos (≥16 años) desde 1987 a 2012. El índice de masa corporal (IMC) se calculó a partir de los datos autorreferidos de peso y talla. Se definió sobrepeso como un IMC entre 25,0-29,9<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>, obesidad como IMC<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> y obesidad mórbida como IMC<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>40<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>. Se consideró que un participante presentaba DM si refería haber sido diagnosticado. Se calculó la prevalencia ajustada por edad por el método directo de estandarización, usando como referencia la población de la encuesta de 2003.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">La prevalencia de sobrepeso ajustada por edad aumentó del 34,0% (intervalo de confianza del 95% [IC 95%] 33,2-34,8) en 1987 al 35,8% (IC 95% 35,0-36,6) en 2012, la de obesidad, del 8,0% (IC 95% 7,5-8,5) al 16,5% (IC 95% 15,7-17,1) y la de DM, del 4,2% (IC 95% 3,9-4,5%) al 7,1% (IC 95% 6,7-7,4%). La obesidad mórbida aumentó del 0,20% (IC 95% 0,13-0,27) en 1993 al 0,88% (IC 95% 0,70-1,05) en 2012.
