Different studies have demonstrated an association between elevated glucose values in the emergency department with a worse short-term and long-term prognosis in patients admitted to acute care units, whether diabetic or not.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">1–5</span></a> In some observational studies, it has been demonstrated that the correction of hyperglycaemia improves the prognosis of these patients.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">An association between elevated blood glucose and a worse outcome has also been described in patients with stroke.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">7–9</span></a> Some authors point out that hyperglycaemia is a stress response secondary to cerebral infarction, while others believe that hyperglycaemia itself increases the size of the infarction and the passage from the penumbra area to the infarcted area with the consequent increase in hemorrhagic transformation of an ischemic stroke treated with fibrinolysis. However, the studies are not homogeneous, since the definitions of hyperglycaemia are variable, with limits between 110 and 180<span class="elsevierStyleHsp" style=""></span>mg/dl.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In addition, it seems that the relationship between blood glucose and prognosis in diabetic patients is not linear, and that both high values and values below normal are related to a worse prognosis.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">9–11</span></a> For this reason, some authors argue that the cut-off point or target levels should be different for patients with or without diabetes.</p><p id="par0020" class="elsevierStylePara elsevierViewall">On the other hand, few studies have analyzed the relationship between glycosylated hemoglobin values (HbA1c) and the prognosis of these patients. HbA1c reflects the glycaemic control over the past 2–3 months and it does not vary significantly due to acute stress, drugs or previous ingestion.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">12</span></a> Levels<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>6.5% are diagnostic criteria for diabetes and levels<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>5.6% are considered normal. Intermediate values (5.7–6.4%) are classified as prediabetes and appear to increase the risk of cardiovascular disease with respect to patients without carbohydrate abnormalities.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">13,14</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Although levels of HbA1c have been linked to the incidence of cardiovascular disease, its association with mortality after an acute vascular episode is not clear.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">15</span></a> In some studies, an increased risk of mortality or new ischemic stroke has been demonstrated in patients with higher levels of HbA1c.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">16,17</span></a> Other studies have not found a relationship with prognosis in patients with acute coronary syndrome.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">18</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">The objective of this study is to know if a blood glucose ≥140<span class="elsevierStyleHsp" style=""></span>mg/dl in patients with stroke in the emergency department is associated with a higher overall mortality, and whether this relationship is influenced by the presence or absence of diabetes.</p><p id="par0035" class="elsevierStylePara elsevierViewall">On the other hand, the relation between HbA1c and the prognosis of these patients is also being evaluated; in diabetic patients by assessing whether better control is associated with lower mortality and in patients without diabetes by assessing if prediabetes range levels are associated with a worse prognosis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0040" class="elsevierStylePara elsevierViewall">All patients admitted to the stroke unit of our hospital for an acute (ischemic or hemorrhagic) cerebrovascular accident between August and December 2012<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>255 were retrospectively evaluated. Patients with transient ischemic attacks who required admission to the unit were also included. Patients admitted for another reason or patients with infarctions of more than 24<span class="elsevierStyleHsp" style=""></span>h of progression were excluded.</p><p id="par0045" class="elsevierStylePara elsevierViewall">All information regarding the demographic and laboratory characteristics of the patients, risk factors, neurological examination and complications were obtained from data collected in the medical records. Patients were considered to have hypertension and dyslipidaemia if they were under pharmacological treatment.</p><p id="par0050" class="elsevierStylePara elsevierViewall">During their stay in the emergency department, a cranial CT was performed on all patients in order to diagnose the stroke's cause, and the treatment was indicated by the Neurology department.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The stroke's severity was assessed according to the NIHSS clinical scale performed on arrival at the emergency department.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">19</span></a> This scale is the most used for the evaluation of basic neurological functions in the acute phase of a stroke and has a good correlation with the prognosis of patients. A high score indicates greater stroke severity.</p><p id="par0060" class="elsevierStylePara elsevierViewall">A blood test was performed on all patients on arrival at the emergency department including blood glucose levels. Hyperglycaemia was defined if levels were ≥140<span class="elsevierStyleHsp" style=""></span>mg/dl.</p><p id="par0065" class="elsevierStylePara elsevierViewall">On the other hand, the HbA1c value was obtained in 216 patients (84.7%) within the first 48<span class="elsevierStyleHsp" style=""></span>h after admission. Patients were classified as diabetic if they had a previous history confirmed by the medical record or HbA1c<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>6.5% at admission, subsequently repeated, or that initiated post-admission antidiabetic treatment.</p><p id="par0070" class="elsevierStylePara elsevierViewall">To assess the impact of HbA1c, 2 groups were created, one with patients with diabetes and one without diabetes. In diabetic patients, depending on whether HbA1c was higher or lower than 7% and in patients without diabetes, higher or lower than 5.7% (prediabetes or without carbohydrate abnormalities).</p><p id="par0075" class="elsevierStylePara elsevierViewall">Later it was observed if the patient had died for any cause, up to 30 months after the stroke. Only 10 patients could not complete follow-up because they were transferred to another health area.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0080" class="elsevierStylePara elsevierViewall">Mean and standard deviation were used to describe the quantitative variables while number and percentage were used for the qualitative variables.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The chi-square test was used to compare 2 categorical variables and Student's <span class="elsevierStyleItalic">t</span>-test or ANOVA for quantitative variables.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The total survival was evaluated using Kaplan–Meier curves and the different curves were compared with the log-rank test. To assess the relationship between the different variables and mortality, univariate and multivariate analyses were performed using the proportional hazards model with the corresponding <span class="elsevierStyleItalic">hazard ratio</span> (HR). In the multivariate study, the significant factors (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) in the univariate analysis were used and other factors that were considered clinically relevant such as sex or blood pressure in the emergency department.</p><p id="par0095" class="elsevierStylePara elsevierViewall">All the analysis was carried out with the statistical program SPSS version 20.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Baseline characteristics</span><p id="par0100" class="elsevierStylePara elsevierViewall">255 patients with a mean age of 73.9 years (range 34–97) who were admitted in the stroke unit of our hospital between August and December 2012 were included in the study.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The characteristics of these patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. 28.2% (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>72) of the patients had a history of type 2 diabetes, of which 9 (12.5%) were diagnosed at admission.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Of the 183 non-diabetic patients, 72 (39.3%) had HbA1c values within the prediabetes range, while 111 (60.7%) had normal values.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Blood glucose ≥140<span class="elsevierStyleHsp" style=""></span>mg/dl was observed in the emergency department in 79 patients (31%, 49 diabetics and 30 non-diabetics).</p><p id="par0120" class="elsevierStylePara elsevierViewall">The overall patient survival in the study was 77.1% at 30 months.</p><p id="par0125" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the differences between patients with and without hyperglycaemia depending on whether they were diabetic or not. There is a higher prevalence of dyslipidaemia and worse glycaemic control in diabetic patients if they presented hyperglycaemia in the emergency department. In contrast, patients without diabetes and hyperglycaemia are older, have a higher prevalence of prediabetes, more severe episodes and higher mortality than patients with normal blood glucose.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Relationship between blood glucose in the emergency department and mortality</span><p id="par0130" class="elsevierStylePara elsevierViewall">In the total population, there is a higher mortality among patients with blood glucose in the emergency department ≥<span class="elsevierStyleHsp" style=""></span>140<span class="elsevierStyleHsp" style=""></span>mg/dl compared to those with values lower than this figure (HR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.22; 95% CI: 1.18–4; 16, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.013). This relationship is significant both in the univariate analysis and after adjustment for age, sex, reason for admission, NIHSS, and blood pressure in the emergency department. In the multivariate analysis, only age, NIHSS, and blood glucose in the emergency department were associated with a worse outcome (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">Among patients with diabetes there is no significant difference in mortality between values above or below 140<span class="elsevierStyleHsp" style=""></span>mg/dl in the emergency department (HR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.20; 95% CI: 0.66–7.40; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.201) as shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">However, in patients without diabetes, mortality with blood glucose levels ≥140<span class="elsevierStyleHsp" style=""></span>mg/dl in the emergency department was higher than those with levels below that figure (HR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.55; 95% CI: 1.11–5.85; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.027) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Relationship between glycosylated hemoglobin and mortality</span><p id="par0145" class="elsevierStylePara elsevierViewall">As seen in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>, patients with HbA1c levels<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>7% do not show differences in mortality compared to patients with HbA1c<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>7% (HR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.68; 95% CI: 0.23–1.98; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.475). However, patients without diabetes and with HbA1c levels of 5.7–6.4% (prediabetes) have a higher mortality compared to patients with HbA1c<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>5.7% regardless of the presence of other factors (HR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.62; 95% CI: 1.01–6.79; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.048) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0150" class="elsevierStylePara elsevierViewall">In our study, the relationship between blood glucose in the emergency department and mortality after a stroke is different between patients with and without diabetes.</p><p id="par0155" class="elsevierStylePara elsevierViewall">In patients without diabetes, elevated blood glucose is associated with a worse outcome regardless of the severity of the stroke (as measured by the NIHSS scale). There is much debate in this regard, as it is not entirely clear whether high blood glucose is a factor that leads to greater morbidity and mortality or whether it is only a marker of stroke severity. In fact, in our study, patients with a higher blood glucose level also had a higher score on the NIHSS scale, so blood glucose levels could reflect a more severe episode. However, after adjusting for the severity of the stroke, blood glucose remains an important predictor of mortality, as observed in other studies.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">7,8</span></a> Hyperglycaemia would be an independent factor, and therefore its correction through intensive treatment with insulin could increase the survival of these patients.</p><p id="par0160" class="elsevierStylePara elsevierViewall">On the other hand, it is universally known that patients with prediabetes have an increased risk of developing cardiovascular events compared to the population without abnormalities in carbohydrate metabolism. However, few studies have studied the mortality of this group of patients after a stay in a critical care unit. A recent meta-analysis in patients with acute coronary disease points to an increased risk of mortality of 84% in patients without diabetes and with elevated HbA1c, whereas this does not occur in diabetic patients.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">20</span></a> In our study, we observed a statistically significant relationship between levels of HbA1c compatible with prediabetes (5.7–6.4%) and a higher long-term mortality among patients without diabetes. This relationship persists despite adjusting for other factors such as the severity of stroke or blood glucose in the emergency department. Therefore, we believe that this group of patients should be considered as high risk, and a close follow-up should be carried out in order to reduce their excess risk of mortality. Although these results may be important in the prognosis of these patients, a clear cost-effectiveness ratio has not yet been demonstrated regarding the measurement of HbA1c in all patients admitted.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In agreement with other works, our cohort does not show any differences in the mortality of diabetic patients whether they have HbA1c values higher or lower than 7%. These results can be understood by taking into account that HbA1c<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>6.5% may indicate recent episodes of hypoglycaemia. These episodes increase mortality because they induce a procoagulant state and increase the inflammatory response.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">21,22</span></a> Thus, we believe it is necessary to individualize HbA1c targets in each patient, according to age, disease duration and risk of hypoglycaemia, among other factors.</p><p id="par0170" class="elsevierStylePara elsevierViewall">The main limitation of the study is that it is a retrospective study, so we do not have all the data of the patients, including HbA1c, although we consider that obtaining this information in 84.7% is a good percentage to be able to draw conclusions. It is also a single center study, so the results may not be extrapolated to another type of population.</p><p id="par0175" class="elsevierStylePara elsevierViewall">On the other hand, although a multivariate analysis has been performed, it is not possible to exclude other causes that could act as confounders, such as the patient's nutritional status, which could modify HbA1c levels and be related to a worse outcome. In the case of subjects who died, whether this was caused by the stroke itself or by another cardiovascular event or by another cause was not analyzed. The degree of dependence after stroke may also be an interesting variable to assess the severity of the stroke.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Another possible bias is the likely higher frequency of lacunar infarctions in diabetic subjects, who have a better outcome than other subtypes of stroke.</p><p id="par0185" class="elsevierStylePara elsevierViewall">We believe that further studies are needed to assess whether a strict glycaemic control improves survival and whether it is necessary to individualize glycaemic targets according to the history of diabetes and HbA1c levels on admission.</p><p id="par0190" class="elsevierStylePara elsevierViewall">Hyperglycaemia in the emergency department (≥140<span class="elsevierStyleHsp" style=""></span>mg/dl) is a factor associated with higher mortality in patients admitted to a stroke unit for acute stroke, regardless of other factors.
