Estudio ESPARTA" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The prevalence of atrial fibrillation (AF) increases significantly with age, exceeding 10% after the age of 70, rising to 18% from the age of 80.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Elderly patients with AF is a complex patient due to the high number of associated comorbidities, cognitive deterioration, the risk of falling or frequent polypharmacy, among others, which greatly interfere with treatment.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">2–5</span></a> All this means that the risk of death and complications is increased in this population.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Although AF increases the global risk of stroke, it increases with age, especially after the age of 75.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">7</span></a> Stroke associated with AF has a high morbidity and mortality, which is more striking in the elderly population.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> Even though, unless contraindicated, all patients ≥75 years of age with AF should receive oral anticoagulation,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> the reality is that the use of anticoagulant treatment decreases with increasing age and this has been associated with an increased risk of stroke and death.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">10,11</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Globally, compared to warfarin, direct oral anticoagulants (DOAC) have been associated with significant reductions in the risk of stroke, particularly intracranial bleeding and mortality, without increasing the risk of major bleeding.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> It seems that these results are also consistent in the elderly population.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">13,14</span></a> In addition, DOAC overcome many of the limitations of vitamin K antagonists (VKA), which could facilitate the increased use of anticoagulation in this population.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">However, information from clinical trials is not always transferable to the “real life” patient, and although clinical practice studies have been published in recent years, information is still scarce in the elderly population.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> On the other hand, the Spanish Agency for Medicines and Medical Devices published a Therapeutic Positioning Report (TPR) on the use of oral anticoagulants in patients with non-valvular AF (NVAF), making a series of recommendations about which situations would call for an anticoagulation indication, also, which cases would be compatible with maintaining the treatment with VKA and which cases would be compatible with the use of DOAC.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Unfortunately, information about the treatment of anticoagulation in elderly patients with NVAF in Spain is very scarce, and it is not known if the recommendations made about anticoagulation in this population are followed correctly under routine clinical practice. ESPARTA was an observational, cross-sectional and multicenter study, whose main objective was to evaluate, in routine clinical practice, compliance with TPR recommendations on oral anticoagulant treatment in patients with AF ≥75 years of age in Internal Medicine units in Spain.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0035" class="elsevierStylePara elsevierViewall">Patients ≥75 years of age of both sexes, with a diagnosis of NVAF, who were being treated by Internal Medicine units (outpatient or hospitalized) in Spain, under stable treatment with oral anticoagulants during the 3 months prior to inclusion in the study (excluding the period of adjustment of the initial dose in the case of VKA) and who had granted written informed consent were included in the ESPARTA study. Patients with mitral stenosis or other significant valve disease requiring specific or scheduled treatment (prosthesis, valvuloplasty), patients who had participated in clinical trials with DOAC in the 6 months prior to the inclusion in the study, as well as patients with any type of disorder that alters the ability to understand or complete the questionnaires were excluded. The study was approved by the reference Clinical Research Ethics Committee (La Princesa University Hospital of Madrid) and by the local clinical research ethics committees of the different participating centres that required it.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The study was carried out in Internal Medicine units distributed nationwide, according to routine clinical practice. A total of 63 centres participated in the study. The inclusion period lasted 5 months, starting in October 2015. The study consisted of a single visit, which coincided with the time of patient inclusion in the study, which was carried out consecutively and according to scheduled routine clinical practice visits. Sociodemographic and clinical data, including the presence of cardiovascular risk factors and the history of cardiovascular diseases, were collected by each investigator from the patient's medical records and were collected through an electronic Case Report Form by means of a web page created for that purpose.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The thromboembolic risk was stratified using the CHADS<span class="elsevierStyleInf">2</span> and CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc scores and the risk of bleeding using the HAS-BLED score.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">18,19</span></a> The anticoagulant treatment was analyzed (type, duration, previous antithrombotic treatment, time in therapeutic range [TTR] in case of VKA and dose used in case of DOAC). In case there was a change in the anticoagulant treatment, the reasons for this were analyzed. Likewise, the degree of compliance with the different recommendations made by the TPR regarding treatment with oral anticoagulants was analyzed.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> The analysis of compliance to the TPR recommendations was made based on the treatment the patient received before the visit, and the change in treatment was assessed during the study visit.</p><p id="par0050" class="elsevierStylePara elsevierViewall">In order to assess the patients’ <span class="elsevierStyleItalic">international normalized ratio</span> (INR) control, the TTR was calculated both by the direct method (percentage of INR values in range) and by the Rosendaal method.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> In order to calculate the HAS-BLED score, the abnormal renal function was defined as the presence of chronic dialysis, kidney transplantation or creatinine ≥2.26<span class="elsevierStyleHsp" style=""></span>mg/dl<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a>; excessive consumption of alcohol such as the intake of ≥8 alcoholic drinks/week; labile INR such as a TTR <60% (direct method) or <65% (Rosendaal method)<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a>; and liver failure such as chronic liver disease or biochemical data indicative of hepatic impairment.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> Polypharmacy was defined as the taking of ≥5 active ingredients on a regular basis at the time of the visit. The degree of adherence to oral anticoagulant treatment was determined by the Morisky-Green therapeutic compliance questionnaire.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistic analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">The categorical variables were described by absolute and relative frequencies. For the description of the continuous variables, the mean and standard deviation were used. For comparisons of subgroups of patients, mainly depending on the type of anticoagulant treatment (VKA or DOAC), nonparametric tests (Mann–Whitney <span class="elsevierStyleItalic">U</span> test) were used for the quantitative variables and the Chi-square test or the Fisher's exact test for the qualitative variables. A bilateral statistical significance level of 0.05 was applied in all the statistical tests. The statistical treatment of data was carried out with the support of the statistical package SAS version 9.4 (SAS Institute Inc., Cary, NC, USA).</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">Total of 63 centres from all the autonomous regions participated in the ESPARTA study, except Castilla-La Mancha (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 1</a>). 92.9% belonged to a public centre and the remaining 7.1% to a private one. The average time of professional experience was 23.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.8 years.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Of the 854 patients included in the study, 837 (98.0%) were found to be assessable for the final analysis of the study. The baseline characteristics of the patients are summarized in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 2</a>. The patients were of advanced age (11% were ≥90 years of age). Hypertension was the most prevalent medical history (84.3%), followed by heart failure (62.7%). 19.2% had a history of ischaemic stroke and 17.6% of bleeding. Polypharmacy was observed in most patients (average active ingredients 9.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.5; average number of daily tablets 11.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.8). With respect to thromboembolic risk and bleeding, CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASC was 5.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.4 (100% CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc ≥2) and the average HAS-BLED, of 2.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.9 (28.9% HAS-BLED ≥3).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Regarding the anticoagulant treatment, 70.8% of the patients were in treatment with VKA at the time of the visit (95.1% acenocoumarol, 4.9% warfarin), while the remaining 29.2% were in treatment with DOAC (rivaroxaban 45.1%, apixaban 35.2% and dabigatran 19.7%). The mean time of treatment with VKA was 73.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>59.3 months, while with the DOAC was 16.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.3 months TTR could be determined in 88.6% of the patients taking VKA. The mean TTR was 54.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>23.8% by direct method and 57.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>24.9% by Rosendaal method. 57.2% of the patients under treatment with VKA had poor INR control. The health centre was the usual place of INR control for 65.9% of the cases, followed by haematology (30.4%). <a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a> shows the regimen used in patients under treatment with DOAC.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">With respect to the change from the previous treatment to the current one, it was observed that 85.5% had not made any change, 13.3% had changed from VKA to DOAC, 1.0% had changed among DOAC and 0.2% had changed from DOAC to VKA. Virtually all the patients with VKA (99.7%) had only received that anticoagulant treatment, while, of the patients with DOAC, 51.2% started anticoagulant therapy with that treatment, 45.5% changed from VKA to DOAC and the remaining 3.3% made a change among DOAC. The main reasons for the change from VKA to DOAC are shown in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 4</a>, the most important being a poor INR control (62.2%).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding compliance with the recommendations of the Spanish Agency for Medicines and Medical Devices mentioned in the TPR, it was considered adequate in 41.4% of patients (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 5</a>), with significant differences between the different autonomous regions (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 1</a>). It was considered adequate in 34.4% of patients with VKA, according to the TPR, a percentage that increased to 57.0% for patients with DOAC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001). In the case of patients with VKA, the main reason for compliance to be considered inadequate was to have a TTR lower than that established in the TPR, a situation which the TPR considers an indication of change to DOAC even though this change is not applied, while the main reason why compliance was considered inadequate in patients with DOAC was to receive an inadequate dose according to the TPR, in many cases due to prescribing the lowest dose when the recommendation indicated the usual drug dose, a fact which was more significant with apixaban, followed by dabigatran and, less frequently, with rivaroxaban (<a class="elsevierStyleCrossRef" href="#tbl0035">Table 6</a>).</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia><elsevierMultimedia ident="tbl0035"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Regarding adherence to anticoagulant treatment, 72.1% of patients were compliant according to the Morisky-Green test (73.0% with VKA versus 70.1% with DOAC; =0.39). There were no significant differences according to the type of VKA (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.61) or between DOAC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.81).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">The Spanish Agency for Medicines and Medical Devices has developed a series of recommendations for the use of oral anticoagulants in patients with NVAF. However, no previous study had evaluated the percentage of patients who adhere to these recommendations and the reasons why DOAC or VKA are used. The results of our study show that TPR follow-up in elderly patients anticoagulated due to NVAF leaves much scope for improvement, particularly due to the poor anticoagulation quality in patients with VKA, an aspect of maximum relevance considering that this population has the highest thrombotic and haemorrhagic risk. Unfortunately, not only is there a significant percentage of elderly patients with NVAF who are currently on antiplatelet agents,<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">4,5</span></a> but among those elderly patients on anticoagulant treatment, more than half of cases taking VKA are out of therapeutic range and in those treated with DOAC a non-negligible proportion is under-dosed.</p><p id="par0095" class="elsevierStylePara elsevierViewall">A large sample of elderly patients with NVAF was included in the ESPARTA study. Thromboembolic risk was high in this population, and less than 30% of patients had a high risk of bleeding. Consequently, the indication for anticoagulation seems evident, considering, in addition, that many of the factors that affect the risk of bleeding are modifiable (labile INR, uncontrolled hypertension, concomitant medication, excessive alcohol intake, etc.).<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">9,19</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">With respect to the anticoagulant treatment in the ESPARTA study, approximately 71% of the patients were taking VKA and the rest, DOAC. Considering that around 57% of patients treated with VKA usually had a bad INR control, frequent polypharmacy and the conditions previously mentioned, it seems that VKAs would not be the best option as an anticoagulant treatment in this population. In fact, taking into account the TPR recommendations, only 34.4% of VKA treatments would be considered adequate.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> The main reason for considering compliance as inadequate was to present a TTR lower than that established in the TPR and not changing to a DOAC, something that should have taken place in more than half of the patients treated with VKA. It is important to note that all considerations on the adequacy of treatment were made based on the treatment the patient was receiving when included, not implying that the doctor responsible for his/her inclusion was the prescriber of the same. However, only 13% of patients changed from VKA to DOAC, despite an inadequate TTR. In Spain several studies have determined that, overall, approximately 40% of patients with NVAF treated with VKA have poor anticoagulation control.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">21,22</span></a> The data from our study show how achieving adequate INR control in the elderly population is more difficult, and, consequently, it seems likely that the use of DOAC in these subjects could provide greater benefits.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> On the other hand, TTR should be measured in all patients anticoagulated with VKA to assess the correct anticoagulation, so this information should be easily accessible in daily practice. Although INR home monitoring could improve anticoagulation control with VKA<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">23</span></a> and, consequently, the safety of these, and it is possible that in these cases the relative benefit of the DOAC was somewhat lower than that reported in clinical trials, the reality is that this has not been demonstrated in specific studies and would only be applicable to a particular profile of patients, which does not seem to be the most frequent among the elderly population.</p><p id="par0105" class="elsevierStylePara elsevierViewall">In the ESPARTA study, patients treated with DOAC followed the recommendations made by the TPR more adequately than those treated with VKA (57.0 versus 34.4%). The most frequent cause of adherence failure to TPR recommendations with DOAC was not receiving the recommended dose, in many cases due to having prescribed the lowest dose when the recommendation indicated the usual drug dose, which was more significant with apixaban and less frequent with dabigatran and rivaroxaban, and which could be explained by the concern of bleeding in the elderly population, a situation that, mistakenly, also leads to antiplatelet therapy. However, it is surprising that this concern is not reflected in the case of poor anticoagulation control with VKA, which INR often contributes over the range considered safe (higher than 3). On the other hand, this concern regarding bleeding should be evaluated taking into account that the thrombotic risk of the population studied is greater than the risk of bleeding. Recent studies have also observed that there is a significant percentage of patients in whom low doses of DOAC are inadequately prescribed compared to usual doses, and that this is less frequent with rivaroxaban compared to other DOAC.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> This is relevant, since the use of inadequate doses of DOAC could be associated with a decrease in its efficacy, demonstrated in clinical trials.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> Consequently, it is essential to train the doctor about the proper use of DOAC, not only on its indication, but also on the dose to be used.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">Although less frequent, situations favouring intracranial bleeding, such as a history of intracerebral/subarachnoid/subdural/epidural bleeding, were also described as causes of non-follow-up of TPR in patients undergoing VKA who had not changed to DOAC, as well as a history of ischaemic stroke with HAS-BLED<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>3 and grade <span class="elsevierStyleSmallCaps">iii</span>-<span class="elsevierStyleSmallCaps">iv</span> leukoaraiosis lesions or cortical microbleeds in imaging tests. These situations are those that can benefit most from treatment with DOAC, given that the maximum benefit of these drugs is the reduction of intracranial bleeding, which is why they are clearly indicated in this context.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">On the other hand, the monitoring of the TPR per autonomous region was also analyzed. Although limited due to the fact that some regions only included a small number of patients, it seems that the TPR adherence differs from one autonomous region to another. Although there are several possible reasons, the differences in DOAC availability between different autonomous regions, as well as a different INR control according to region, could explain, at least in part, this situation.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">25</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Our study provides information on therapeutic compliance regarding anticoagulant treatment in the elderly population. One of the concerns identified in the case of DOAC is the possibility of a worse adherence due to not having to perform frequent laboratory controls. We found that, with respect to adherence to anticoagulant treatment, more than 70% of patients were considered compliant, without significant differences between the different types of anticoagulants, a figure similar to that reported in other studies.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">The main limitation of the study is its cross-sectional design, which does not allow an adequate control of all the confounding factors due to the lack of randomization. However, it is the design that best allows us to know what the treatment of a certain disease is in routine clinical practice. In addition, patients were included consecutively, there was practically no loss of data and data collection was performed in a very meticulous manner, which undoubtedly reduces the potential limitation of the study design.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0130" class="elsevierStylePara elsevierViewall">The elderly population with NVAF included in the ESPARTA study had a very high thromboembolic risk. Less than 30% of patients had a high risk of bleeding. Approximately 71% of the patients were taking VKA and the rest, DOAC. Treatment was adequate in only one third of patients taking VKA according to the recommendations of the TPR. The main reason to be considered as non-adequate was to present a TTR lower than that established in the TPR, not changing to a DOAC, a fact that should have taken place in more than half of the patients treated with VKA. 57.0% of patients treated with DOAC followed the recommendations made by the TPR. The most frequent cause of failure to follow TPR recommendations with DOAC was not receiving the recommended dose, usually due to under-dosing, which was more significant with apixaban.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conflict of interests</span><p id="par0135" class="elsevierStyleSectionTitle" id="sect0015">Patients and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Observational, cross-sectional and multicenter study in which 837 patients aged ≥75 years old with NVAF, with stable treatment with oral anticoagulants at least 3 months before inclusion, and that had started treatment with oral anticoagulants before the inclusion period were included.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" El 70,8% de los pacientes estaba en tratamiento con antagonistas de la vitamina K (AVK) y el resto con anticoagulantes orales de acción directa (ACOD). El 65,6% de los pacientes con AVK no siguieron las recomendaciones del IPT frente al 43,0% de los pacientes con ACOD (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,0001).
