interval cancer &#40;IC&#41; is considered to be any invasive colorectal cancer that is diagnosed after a test or screening event &#8211; in which cancer was ruled out &#8211; and before the recommended date for the next test&#46; In addition&#44; differentiating between ICs according to the type of previous test to which the IC is attributed &#40;faecal occult blood test&#44; colonoscopy&#44; sigmoidoscopy&#44; etc&#46;&#41; is recommended&#46;<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The frequency of IC determines the sensitivity of the screening programmes&#44; one of the most relevant indicators of its effectiveness&#46; The sensitivity of the programmes based on faecal occult blood tests varies between 42&#37; and 65&#37; for those who use the guaiac test&#44; and between 59&#46;1&#37; and 87&#46;0&#37; for those who use the faecal immunological test &#40;FIT&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> According to this same systematic review&#44; most ICs appear after a negative screening test&#44; and only about 10&#37; are diagnosed after a normal colonoscopy&#46; With regard to cancers detected through screening&#44; ICs are diagnosed in more advanced stages and appear more frequently in females and in the proximal colon&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">To date&#44; there are few studies that analyse the characteristics of ICs in the context of CRC screening programmes based on FIT&#44; and even fewer that relate it to the quantitative value of prior FIT&#46; It is widely considered that&#44; within the range considered negative&#44; the higher the FIT value&#44; the higher the probability of developing an IC&#46; The objectives of this study are to describe the characteristics of the ICs detected in a population screening programme throughout the first six years of the programme&#44; compare them with those of the other cancers diagnosed in the programme &#40;screening cancer&#41; and analyse the relationship between the quantitative value of the previous FIT and the appearance of IC&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">In December 2009&#44; the Barcelona Early Detection for Colon and Rectum Cancer programme was launched &#40;PDPCCR-Bcn&#44; <a id="intr0010" class="elsevierStyleInterRef" href="http://www.prevenciocolonbcn.org/">www&#46;prevenciocolonbcn&#46;org</a> &#8211; for its acronym in Spanish&#41;&#46; This programme comprises inviting males and females aged 50&#8211;69 every two years to carry out a immunological faecal occult blood test &#40;OC-Sensor 1 &#91;Eiken Chemical Co&#46;&#44; Japan&#93;&#59; positivity &#8805;20<span class="elsevierStyleHsp" style=""></span>&#956;g&#47;g faeces&#41;&#46; The organisation&#44; operation and main results of the first round of the PDPCCR-Bcn have been previously published in detail&#46;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a> People with FIT negative results are invited to carry out the test again two years later&#44; in the next round&#44; while those with positive FIT are offered a colonoscopy&#44; and if it is normal&#44; they will not be invited again for 10 years&#46; During the first six years of its implementation&#44; the PDPCCR-Bcn covered approximately half of the territory of the city of Barcelona&#44; with a target population of close to 198&#44;000 people&#46; As of 2015&#44; the programme was extended to the rest of the city&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">The present analysis is limited to the ICs that appeared in people with a previous negative FIT result&#59; it does not include ICs detected after a normal colonoscopy&#44; since the observation period for such persons is 10 years&#46; The IC associated with the first two rounds of the programme were studied&#58; round 1 &#40;2010&#8211;2011&#41; and round 2 &#40;2012&#8211;2013&#41;&#46; The follow-up was extended until June 2017&#44; to minimise possible losses due to a delay in entering the data from the records used&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">In approximately half of the ICs&#44; the source of information was the notification from the PDPCCR-Bcn by professionals and&#47;or patients&#59; the rest were identified through crossing the Programme&#44; hospital and Catalonia Minimum Basic Data Set &#40;CMBD &#8211; for its acronym in Spanish&#41; databases&#46; Of all the possible identified ICs&#44; the clinical history was reviewed to confirm the case and complete the information&#46; The information on the screening cancers and the total number of participants in the first two rounds were from the PDPCCR-Bcn databases&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall">The entire study was developed based on current legislation&#44; conventions and declarations from the European Union and national organisations regarding ethics in research projects&#46; Data was processed based on the provisions of Organic Law 15&#47;1999 on data protection and Royal Decree 994&#47;1999 on the security of automated files containing personal data&#46; This analysis is part of a research study approved by the Clinical Research Ethics Committee in 2016&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">Ninety-two ICs were identified in individuals who had had a previous negative FIT result &#40;47 in round 1 and 45 in round 2&#41; and 415 cancers were diagnosed by the PDPCCR-Bcn &#40;251 and 164 in round 1 and 2&#44; respectively&#41;&#46; The total sensitivity of the programme for this period was 82&#37;&#44; and approximately two thirds of the ICs were diagnosed during the second year of the two-year interval between invitations to screening &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#41;&#46; Compared with screening cancers&#44; ICs were diagnosed more frequently in advanced stages and in the proximal colon and rectum&#44; while there were no relevant differences according to sex and age &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">FIT quantitative values differed significantly &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace">&#60;</span><span class="elsevierStyleHsp" style=""></span>0&#46;001&#41; between the group of patients with negative FIT result and no further diagnosis of IC and those diagnosed with an IC &#40;<a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#41;&#46; While about 60&#37; of people with negative FIT without IC obtained a FIT score of &#61;0<span class="elsevierStyleHsp" style=""></span>&#956;g&#47;g faeces &#40;undetectable&#41;&#44; in males and females diagnosed with IC it accounted for 25&#46;9&#37; and 31&#46;6&#37;&#44; respectively&#46; 11&#46;1&#37; of males and 26&#46;3&#37; of females with IC had a FIT in the previous round of between 10&#46;0 and 19&#46;9<span class="elsevierStyleHsp" style=""></span>&#956;g&#47;g faeces&#44; respectively&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">The sensitivity of the PDPCCR-Bcn in the first two rounds was 82&#37;&#44; a figure that lies in the upper range compared to other published studies for immunological test population programmes&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> One of the factors related to sensitivity is the type of FIT test used&#58; for the same cut-off point&#44; the test used in the PDPCCR-Bcn &#40;OC-Sensor&#41; obtains higher detection and lower IC rates than others&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">4</span></a> However&#44; the Basque Country programme&#44; with characteristics very similar to the Barcelona programme&#44; recently reported a sensitivity of 92&#46;5&#37;&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">5</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">In this study&#44; ICs were located more frequently in the proximal and rectum colon and were diagnosed at more advanced stages than the screening cancers&#44; as has been found in previous studies&#46;<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">2&#44;5&#44;6</span></a> This is attributed to the lower bleeding tendency of cancers in the proximal colon and the greater rate of growth and aggressiveness of the ICs&#46; The reasons for a greater proportion of IC in rectum in a programme with FIT have not yet been described&#46; However&#44; while these and other studies describe an increased risk of IC in females&#44; no differences were observed according to sex in the PDPCCR-Bcn&#46; Patients in the PDPCCR-Bcn with IC obtained significantly higher FIT values than the group of people with negative FIT and without diagnosis of IC&#44; both in males and females&#46; The association between IC and the quantitative value of the previous FIT has not been studied much&#44; but the relation between the FIT cut-off point with sensitivity and with the IC rate is well-known&#46; Thus&#44; a study carried out in Scotland estimated that moving from its current threshold level of 80<span class="elsevierStyleHsp" style=""></span>&#956;g&#47;g faeces to 20<span class="elsevierStyleHsp" style=""></span>&#956;g&#47;g faeces would reduce the percentage of IC from the total of cancers from 51 to 44&#37;&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">7</span></a> One of this study&#39;s limitations is the lack of other relevant information&#44; such as IC symptoms and possible risk factors&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Our study obtained an acceptable IC rate for a early detection population programme<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> and shows that the appearance of IC is related to the quantitative value of FIT in the previous round&#46; Increasingly more authors recommend using different FIT thresholds depending on the variables related to the probability of cancer as a CRC screening personalisation strategy&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">8</span></a> Considering the survival and quality of life of people diagnosed with IC after a negative FIT&#44;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">9</span></a> these strategies should include not only the probability of cancer screening immediately after the FIT&#44;
