People with negative FIT and without IC: 65,987 males, 86,357 females; people with negative FIT and diagnosis of IC: 54 males, 38 females.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">In the context of population programmes for early detection of colorectal cancer (CRC), interval cancer (IC) is considered to be any invasive colorectal cancer that is diagnosed after a test or screening event – in which cancer was ruled out – and before the recommended date for the next test. In addition, differentiating between ICs according to the type of previous test to which the IC is attributed (faecal occult blood test, colonoscopy, sigmoidoscopy, etc.) is recommended.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The frequency of IC determines the sensitivity of the screening programmes, one of the most relevant indicators of its effectiveness. The sensitivity of the programmes based on faecal occult blood tests varies between 42% and 65% for those who use the guaiac test, and between 59.1% and 87.0% for those who use the faecal immunological test (FIT).<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> According to this same systematic review, most ICs appear after a negative screening test, and only about 10% are diagnosed after a normal colonoscopy. With regard to cancers detected through screening, ICs are diagnosed in more advanced stages and appear more frequently in females and in the proximal colon.</p><p id="par0015" class="elsevierStylePara elsevierViewall">To date, there are few studies that analyse the characteristics of ICs in the context of CRC screening programmes based on FIT, and even fewer that relate it to the quantitative value of prior FIT. It is widely considered that, within the range considered negative, the higher the FIT value, the higher the probability of developing an IC. The objectives of this study are to describe the characteristics of the ICs detected in a population screening programme throughout the first six years of the programme, compare them with those of the other cancers diagnosed in the programme (screening cancer) and analyse the relationship between the quantitative value of the previous FIT and the appearance of IC.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">In December 2009, the Barcelona Early Detection for Colon and Rectum Cancer programme was launched (PDPCCR-Bcn, <a id="intr0010" class="elsevierStyleInterRef" href="http://www.prevenciocolonbcn.org/">www.prevenciocolonbcn.org</a> – for its acronym in Spanish). This programme comprises inviting males and females aged 50–69 every two years to carry out a immunological faecal occult blood test (OC-Sensor 1 [Eiken Chemical Co., Japan]; positivity ≥20<span class="elsevierStyleHsp" style=""></span>μg/g faeces). The organisation, operation and main results of the first round of the PDPCCR-Bcn have been previously published in detail.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a> People with FIT negative results are invited to carry out the test again two years later, in the next round, while those with positive FIT are offered a colonoscopy, and if it is normal, they will not be invited again for 10 years. During the first six years of its implementation, the PDPCCR-Bcn covered approximately half of the territory of the city of Barcelona, with a target population of close to 198,000 people. As of 2015, the programme was extended to the rest of the city.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The present analysis is limited to the ICs that appeared in people with a previous negative FIT result; it does not include ICs detected after a normal colonoscopy, since the observation period for such persons is 10 years. The IC associated with the first two rounds of the programme were studied: round 1 (2010–2011) and round 2 (2012–2013). The follow-up was extended until June 2017, to minimise possible losses due to a delay in entering the data from the records used.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In approximately half of the ICs, the source of information was the notification from the PDPCCR-Bcn by professionals and/or patients; the rest were identified through crossing the Programme, hospital and Catalonia Minimum Basic Data Set (CMBD – for its acronym in Spanish) databases. Of all the possible identified ICs, the clinical history was reviewed to confirm the case and complete the information. The information on the screening cancers and the total number of participants in the first two rounds were from the PDPCCR-Bcn databases.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The entire study was developed based on current legislation, conventions and declarations from the European Union and national organisations regarding ethics in research projects. Data was processed based on the provisions of Organic Law 15/1999 on data protection and Royal Decree 994/1999 on the security of automated files containing personal data. This analysis is part of a research study approved by the Clinical Research Ethics Committee in 2016.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">Ninety-two ICs were identified in individuals who had had a previous negative FIT result (47 in round 1 and 45 in round 2) and 415 cancers were diagnosed by the PDPCCR-Bcn (251 and 164 in round 1 and 2, respectively). The total sensitivity of the programme for this period was 82%, and approximately two thirds of the ICs were diagnosed during the second year of the two-year interval between invitations to screening (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Compared with screening cancers, ICs were diagnosed more frequently in advanced stages and in the proximal colon and rectum, while there were no relevant differences according to sex and age (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">FIT quantitative values differed significantly (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace"><</span><span class="elsevierStyleHsp" style=""></span>0.001) between the group of patients with negative FIT result and no further diagnosis of IC and those diagnosed with an IC (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). While about 60% of people with negative FIT without IC obtained a FIT score of =0<span class="elsevierStyleHsp" style=""></span>μg/g faeces (undetectable), in males and females diagnosed with IC it accounted for 25.9% and 31.6%, respectively. 11.1% of males and 26.3% of females with IC had a FIT in the previous round of between 10.0 and 19.9<span class="elsevierStyleHsp" style=""></span>μg/g faeces, respectively.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">The sensitivity of the PDPCCR-Bcn in the first two rounds was 82%, a figure that lies in the upper range compared to other published studies for immunological test population programmes.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> One of the factors related to sensitivity is the type of FIT test used: for the same cut-off point, the test used in the PDPCCR-Bcn (OC-Sensor) obtains higher detection and lower IC rates than others.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">4</span></a> However, the Basque Country programme, with characteristics very similar to the Barcelona programme, recently reported a sensitivity of 92.5%.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">5</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">In this study, ICs were located more frequently in the proximal and rectum colon and were diagnosed at more advanced stages than the screening cancers, as has been found in previous studies.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">2,5,6</span></a> This is attributed to the lower bleeding tendency of cancers in the proximal colon and the greater rate of growth and aggressiveness of the ICs. The reasons for a greater proportion of IC in rectum in a programme with FIT have not yet been described. However, while these and other studies describe an increased risk of IC in females, no differences were observed according to sex in the PDPCCR-Bcn. Patients in the PDPCCR-Bcn with IC obtained significantly higher FIT values than the group of people with negative FIT and without diagnosis of IC, both in males and females. The association between IC and the quantitative value of the previous FIT has not been studied much, but the relation between the FIT cut-off point with sensitivity and with the IC rate is well-known. Thus, a study carried out in Scotland estimated that moving from its current threshold level of 80<span class="elsevierStyleHsp" style=""></span>μg/g faeces to 20<span class="elsevierStyleHsp" style=""></span>μg/g faeces would reduce the percentage of IC from the total of cancers from 51 to 44%.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">7</span></a> One of this study's limitations is the lack of other relevant information, such as IC symptoms and possible risk factors.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Our study obtained an acceptable IC rate for a early detection population programme<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> and shows that the appearance of IC is related to the quantitative value of FIT in the previous round.
