In patients with endobronchial lesions, bronchial biopsy (BB) using FB is the technique of choice for pathological diagnosis. BB sensitivity figures are approximately 74%.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> In addition to BB, bronchial aspirate (BAS) samples can be obtained when carrying out a FB. The diagnostic value of BAS is lower than that of the BB. However, the combination of both techniques increases diagnostic performance. Therefore, it is recommended that in addition to a BB, BAS samples are also collected.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The optimal sequence to collect BAS before (preBAS) or after (postBAS) BB is controversial.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> In our centre, following previously published recommendations,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> BAS is usually collected and processed separately before and after each BB. The purpose of this study is to compare the diagnostic performance of preBAS with that of postBAS in a retrospective cohort of patients with LC and with endobronchial lesion.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study</span><p id="par0010" class="elsevierStylePara elsevierViewall">This is a retrospective study that included patients with a final diagnosis of LC and clear endobronchial lesion who underwent a BB, in which the BAS samples were collected before and after the BB, and processed separately.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Flexible bronchoscopy</span><p id="par0015" class="elsevierStylePara elsevierViewall">FB were carried out using the flexible video bronchoscope (Olympus BF-H190®, Olympus, Tokyo, Japan). Endobronchial lesions were divided into<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>: mass or infiltration and classified into three grades: I, II and III, according to the increasing probabilities of malignancy. Only patients with grade II and III lesions were included in the study. Patients with synchronous endobronchial tumours were excluded. The BAS was collected in specially designed 20<span class="elsevierStyleHsp" style=""></span>ml containers. Prior to the BB, the container was removed and replaced with another. Both samples were processed separately. The BBs were performed with single-use oval-cup biopsy ribbed forceps.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0020" class="elsevierStylePara elsevierViewall">The following were calculated: (1) The overall diagnostic performance of bronchoscopic techniques; (2) The joint diagnostic performance of BAS (preBAS and postBAS), and (3) The diagnostic performance of preBAS and postBAS separately. The diagnostic performance of preBAS and postBAS was compared separately using the Chi-square test.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">A total of 144 patients were included from January 2010 to December 2017. Their characteristics are described in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>: the majority were male (78.5%), with a median age of 65; there were no major complications in regard to the technique.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">The overall performance of the endoscopic procedures was 80.5%. BB was the most diagnostic test (112 [77.8%]). BAS as a whole (preBAS and postBAS) was diagnostic in 45 cases (31.25%) (Chi-square test: <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.009). BB was the only diagnostic method for 71 (68.7%) patients. BB was more diagnostic in grade III lesions than in grade II lesions (53.7 vs. 87.4%; Chi-square test: <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). No relationship was found between the diagnostic performance of BB and the laterality of the tumour (left or right).</p><p id="par0035" class="elsevierStylePara elsevierViewall">The results of preBAS and postBAS were concordant in 100 (69.4%) cases and not concordant in 44 (30.6%). PreBAS was diagnostic in 24 (16.7%) cases and postBAS in 33 (22.9%) cases (Chi-square test: <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.009) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). PreBAS material was not found to be valuable for cytological examination in 1.4% and postBAS in 8.3% (Fisher's exact test: <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.005). BAS as a whole was the only diagnostic method in four (2.7%) cases in which BB was non-diagnostic. In two cases (1.3%), preBAS was the only diagnostic procedure and in another two cases, the only diagnostic procedure was postBAS.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0040" class="elsevierStylePara elsevierViewall">Our study confirms that postBAS is compatible with malignancy in a higher percentage of cases compared to preBAS in patients with pulmonary neoplasia and endobronchial lesion. However, in our series this difference had no impact on the overall diagnostic performance of bronchoscopy given that when preBAS and postBAS were the only procedure, the diagnostic was identical.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In our series, BB showed a higher diagnostic performance than BAS. Traditionally histological samples have been considered of greater clinical value than cytological ones. Despite this, cytological techniques are not destined to become obsolete. First, as our study demonstrates, cytological techniques increase the overall diagnostic performance of bronchoscopy. Firstly, because, as our study demonstrates, a non-diagnostic bronchoscopy is frustrating for the clinician and traumatic for the patient, since it usually involves a delay in the diagnostic process and entails performing other procedures. Secondly because – although initially predictive biomarkers of drug response in LC were determined exclusively in histological samples – several studies have demonstrated comparable performance between cytology samples and surgical samples in terms of detection of epidermal growth factor receptor (EGFR) muations<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> and rearrangement of the anaplastic lymphoma kinase (ALK) gene.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Furthermore, two circumstances favour the determination of biomarkers in cytological samples: (1) the presence of stromal, inflammatory, fibrosis or necrosis elements, usually present in histological samples, makes molecular studies difficult, and (2) it has been shown that fresh cytological samples preserve DNA better than paraffinized samples for molecular tests,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> and that the time required for its extraction is significantly shorter in the former. With the development of the new generation sequencing techniques and ultrasensitive polymerase chain reaction, mutations of EGFR, KRAS and TP53 have been detected in bronchoalveolar lavage and bronchial aspirate samples.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">FB was described decades ago, however, the optimal sequence of BAS collection in relation to BB is controversial. Four studies<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,8–10</span></a> have compared the diagnostic performance of preBAS with postBAS, our series being the second largest in number of patients with endobronchial lesion, and the largest in the number of patients in whom preBAS and postBAS was collected simultaneously in the same patient (other series compared the different strategies in groups of patients separately). The results of the studies are very similar to ours: in most of the studies, postBAS was more diagnostic. However, in the two studies that analysed the percentage of cases in which either of the two cytologies was the only diagnostic method, no superiority of one over the other was found. The explanation that has usually been given for postBAS being more diagnostic than preBAS is that BB increases the cellular content of BAS, although it can cause bleeding that leads to contamination of the aspirate by red blood cells, making cytological analysis difficult. In our series, the sample proved to be invalid for diagnosis in postBAS on a greater number of occasions. This information had not been collected in previous similar studies. The retrospective and unicentric nature of our study limits the level of evidence and external validity of the results. However, considering that the results were statistically significant, we believe that the conclusions can be validated. According to the results of our study, the most plausible sequence of BAS collection would be to collect a first aspirate before performing the biopsy, and another after it, to leverage the desquamation produced by the biopsy.
