Efectos sobre la madre y el recién nacido" ] ] "resumenGrafico" => array:2 [ "original" => 0 "multimedia" => array:8 [ "identificador" => "fig0005" "etiqueta" => "Figure 1" "tipo" => "MULTIMEDIAFIGURA" "mostrarFloat" => true "mostrarDisplay" => false "figura" => array:1 [ 0 => array:4 [ "imagen" => "gr1.jpeg" "Alto" => 1973 "Ancho" => 2167 "Tamanyo" => 189773 ] ] "detalles" => array:1 [ 0 => array:3 [ "identificador" => "at0005" "detalle" => "Figure " "rol" => "short" ] ] "descripcion" => array:1 [ "en" => "<p id="spar0005" class="elsevierStyleSimplePara elsevierViewall">Flowchart.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Diabetes mellitus is one of the most prevalent and fastest growing diseases in the world in recent decades, and pregnancy is not freed from this situation.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Gestational diabetes is a carbohydrate metabolism disorder that is produced and diagnosed during pregnancy.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a> The main risk factors for developing it are advanced gestational age, family history of diabetes, overweight, obesity, excessive weight gain during pregnancy and belonging to a high-risk ethnic group (non-Caucasian).<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Due to the increase in the average age of pregnant women the incidence is increasing, affecting about 10% of those in our setting.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Gestational diabetes is due to insulin resistance generated as a result of the secretion by the placenta of hormones such as the growth hormone (GH), placental lactogen, progesterone and cortisol. Other influencing factors are higher caloric intake, increased adipose tissue and reduced exercise.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Due to this inability of the pancreas to cope with insulin resistance, the blood glucose concentration increases, thus favouring foetal utilization.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Gestational diabetes is the disease that most often causes medical disorders in pregnancy, and it is associated with an increase in morbidity and mortality in the mother and the foetus, in both the short and long term.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Women with this disease have a higher incidence of preeclampsia, infections during pregnancy, premature delivery, caesarean delivery, instrumented deliveries, etc. And the foetus suffers a higher incidence of loss of foetal well-being, macrosomia, polyhydramnios, neonatal hypohypoglycemia and intrauterine death. In addition, it is associated with an increased risk of obesity, type 2 diabetes mellitus and metabolic syndrome when the foetus reaches adulthood.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In certain situations, the diagnostic criteria for gestational diabetes is not reached, but the mother suffers a non-physiological alteration of the hydrocarbon metabolism defined as carbohydrate intolerance.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a> The percentage of affected pregnant women is not well defined since there are variations between different case series, but it is generally lower than that of gestational diabetes. Its direct relationship with specific adverse obstetric outcomes is not clearly defined due to the absence of studies in this regard.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">On the one hand, when the diagnosis of gestational diabetes is reached an action protocol is put in place, so as to have greater control of the pregnancy and to establish optimal treatment. Mothers undergo more check-ups and changes in lifestyle are prescribed as the first treatment measure: diet and exercise. The objective is to maintain a diet plan that allows the proper weight to be maintained, preventing hyperglycemia, hypoglycemia and ketoacidosis. This contributes to foetal well-being, which will be further improved if accompanied by at least 20–30<span class="elsevierStyleHsp" style=""></span>min exercise per day, which contributes to increased insulin sensitivity in tissues.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> The correct evolution of gestational diabetes will be controlled by outpatient monitoring of capillary hypoglycemia. If adequate basal and postprandial blood glucose levels are not achieved after 2 weeks, insulin treatment will be started.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> When adequate control of the disease is achieved, mothers show adequate weight gain, and obstetric morbidity and mortality linked to the disease is reduced.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">On the other hand, when the diagnosis of carbohydrate intolerance is reached, no treatment protocol is initiated, nor is there a more check-ups of this gestation, since there are no established guidelines for action. In general, the follow-up of pregnancy continues the same as for a pregnant woman without any disorder.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The objective of this study is to assess the relationship between the presence of a carbohydrate metabolism disorder during pregnancy and the risk of presenting adverse obstetric results in the mother and the foetus. Specifically, the study focuses on determining whether the carbohydrate intolerance group is associated with a higher risk of obstetric morbidity than the control group and/or the gestational diabetes group. This would give room for the need of tighter control and treatment in this group during pregnancy.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and method</span><p id="par0040" class="elsevierStylePara elsevierViewall">An observational study was conducted, which collected data from a population of women who had just given birth, and who were attended to in Valencia’s La Fe University and Polytechnic Hospital from January to October 2018. The data was collected by reviewing the medical records of the mother and the newborn, reviewing the maternity and newborn records, and directly interviewing the mothers. The maternity and medical records prospectively record the information on the different check-ups performed throughout the pregnancy and the peripartum period, thus avoiding possible memory bias. This information was completed by an interview with the mother after the birth. The presence of changes in the carbohydrate metabolism in mothers during pregnancy and its possible effects on the newborn, the mother and the birth process were analysed.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In the descriptive phase of the study, the clinical characteristics and obstetric history of the patients were analysed, as well as the data related to the pregnancy and the birth, including the state of the newborn (gestational age, height, weight, head circumference, Apgar score, foetal blood gasometry, etc.).</p><p id="par0050" class="elsevierStylePara elsevierViewall">In the analytical phase, the aforementioned collected data was evaluated in relation to a carbohydrate metabolism disorder present in mothers who were classified into 3 groups: normal, carbohydrate intolerance and gestational diabetes. This was decided objectively according to the results of the screening test (O’Sullivan) with 50<span class="elsevierStyleHsp" style=""></span>g of oral glucose, between week 24–28: if the plasma glucose after one hour of the test was equal to or greater than 140<span class="elsevierStyleHsp" style=""></span>mg/dl (7.8<span class="elsevierStyleHsp" style=""></span>mmol/l) the test was considered positive and the oral glucose tolerance test (OGTT) was performed, administering 100<span class="elsevierStyleHsp" style=""></span>g of fasting oral glucose and taking 4 measurements of plasma blood glucose: baseline, at 1, 2 and 3<span class="elsevierStyleHsp" style=""></span>h. According to the criteria of the National Diabetes Data Group (NNDG): if no parameter changes the woman does not suffer any disorder, if one parameter changes the woman suffers from carbohydrate intolerance, and if 2 or more change the woman suffers from gestational diabetes.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">All mother-child pairs admitted after delivery in the gynaecology and obstetrics department of the hospital were considered as study population. In total, 827 mothers were invited to participate, of which 799 finally participated, after completing the consent form and the details required for the study (96% participation rate). Those mothers whose data corresponding to the characteristics of the newborn were not available for consultation, as well as those whose responses were inconsistent or incomplete, were excluded (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">All subjects gave their informed consent, which included an agreement of confidentiality of the data collected in accordance with Organic Law 15/1999 of December 13 on data protection, prior to their participation in the study. The study was carried out in accordance with the Declaration of Helsinki, and its protocol was approved by the Ethics Committee of the La Fe Hospital (CEIC 2014/0116).</p><p id="par0065" class="elsevierStylePara elsevierViewall">The technique used to collect the information which was included in the study was: a review of the medical records of both the mother and the newborn, a review of the maternity and newborn records, and a personal and face-to-face interview with the mothers after the birth. Information about the mother’s age, country of origin, physical activity during pregnancy, parity, tobacco use, vitamin intake, weight, height, etc., as well as obstetric data and data of the newborn were obtained from the medical and maternity records and the interview. The BMI was calculated.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The variables collected in the study were: sociodemographic, lifestyle and anthropometric data of the mother. Regarding anthropometric data, the BMI was calculated as weight (kilograms)/height (m) squared. Pre-pregnancy weight was obtained through health records, all from within the year prior to the current pregnancy. Maternal weight gained was obtained from the medical records from the pregnancy check-ups. The physical activity contemplated 4 possible levels depending on the total time of physical exercise, combining work and free time: 0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no physical activity; 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>light activity (less than 30<span class="elsevierStyleHsp" style=""></span>min per day); 2<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>moderate activity (more than 30<span class="elsevierStyleHsp" style=""></span>min per day, but less than 1<span class="elsevierStyleHsp" style=""></span>h); 3<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>intense activity (more than one hour a day).</p><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding the clinical and obstetric history of the mother, the data was collected from previous pregnancies differentiating between primiparous and multiparous women. Another factor taken into account was the number of previous abortions.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The ‘carbohydrate metabolism disorder’ variable was defined according to the diagnostic criteria of the National Diabetes Data Group (NNDG).<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Regarding the newborn, data was obtained from the reviews of the medical history and the maternity record as well as from the interviews with the mothers. The collected data include anthropometric values: weight (g), height (cm) and head circumference (cm). The sex of the newborn was taken into account as it is a determining factor when these values are analysed. Newborns were classified as SGA (small for gestational age) when their weight was less than the 10th percentile expected for their gestational age and sex, according to Spanish standards.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Data were collected on problems at birth and if the newborn was admitted into the neonatal intensive care unit. The Apgar score was also collected.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Also included were the umbilical arterial and venous blood gas values were collected at the time of delivery (including pH, HCO<span class="elsevierStyleInf">3</span>, PO<span class="elsevierStyleInf">2</span> and PCO<span class="elsevierStyleInf">2</span>), as a method that objectively evaluated the condition of the newborn.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The 'loss of foetal well-being’ variable was defined as positive in cases that presented alterations in the cardiotocographic patterns suggestive of loss of foetal well-being during intrapartum monitoring that forced the urgent extraction of the foetus, in the presence of foetal acidosis defined as a pH lower than 7.20 in foetal capillary scalp blood and/or umbilical arterial blood,<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–18</span></a> and in cases that required respiratory resuscitation after delivery due to meconium aspiration.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0100" class="elsevierStylePara elsevierViewall">The primary variable of interest was a carbohydrate metabolism disorder in mothers during the pregnancy, and on which a descriptive study of the population for each level (normal, carbohydrate intolerance and gestational diabetes) was performed. The quantitative variables were expressed as mean and standard deviation and their differences were analysed using ANOVA. The qualitative variables were expressed as percentages and their differences were analysed using the test χ².</p><p id="par0105" class="elsevierStylePara elsevierViewall">The variables with significant results within the baseline maternal characteristics were: age, previous and final BMI, weeks of gestation and weight gained. The variables with significant results of the clinical characteristics of pregnancy, birth and the newborn were: caesarean delivery, loss of foetal well-being, preeclampsia, threat of premature delivery and weight gain.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Considering the carbohydrate metabolism disorder as an independent variable, a statistical analysis was performed to determine the presence of significant differences between the groups. Multivariable logistic regression was used to examine the association between the groups of the independent variable and the variables of interest in the study, creating 7 groups: instrumented delivery, caesarean delivery, episiotomy, loss of foetal well-being, threat of preterm delivery, preeclampsia and hospitalization. We calculated the crude <span class="elsevierStyleItalic">Odds ratio</span> (ORc), the adjusted <span class="elsevierStyleItalic">Odds ratio</span> (ORa) and the 95% confidence interval (95% CI) for each one, to estimate the degree of association. Regarding the ORa, to avoid confounding, a multiple conditional logistic regression was performed using different models in relation to the maternal characteristics identified as significantly different between the groups. All the analyses were performed using the IBM SPSS® Statistics 22 <span class="elsevierStyleItalic">software</span>. All the hypotheses were tested assuming a level of significance of p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0115" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the clinical, lifestyle and obstetric baseline characteristics of the mothers and the newborns, separated into 3 groups according to a carbohydrate metabolism disorder: 1) normal; 2) carbohydrate intolerance, and 3) gestational diabetes. Significant differences (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) were found among the groups in the variables: age, weeks of gestation, previous BMI, final BMI and weight gained during pregnancy. Data are shown as the mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">The mean age of the mothers in the normal metabolism group was 32.18<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.78, with 39.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.62 mean weeks of gestation and weight gained during pregnancy of 12.31<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.04<span class="elsevierStyleHsp" style=""></span>kg, with a prior BMI of 23.64<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.75<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> and a final BMI of 28.28<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.14<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>. In the carbohydrate intolerance group the mean age was 35.51<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.61, gestation weeks 38.66<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.47 and the mean weight gained was 7.99<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.43<span class="elsevierStyleHsp" style=""></span>kg with a prior BMI of 26.82<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.94<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> and a final BMI of 29.91<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.37<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>. And in the gestational diabetes group the mean age was 34.31<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.97 with 38.61<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.37 weeks of gestation and weight gained during pregnancy of 11.28<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.97<span class="elsevierStyleHsp" style=""></span>kg, with a prior BMI of 26.74<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.96<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> and a final BMI of 31.06<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.48<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>. It is important to highlight that the mothers of the groups with glucidic metabolism disorder had a higher mean age and a higher prior BMI.</p><p id="par0125" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the characteristics and clinical disorders of pregnancy, birth and newborn. Each variable is expressed as a percentage within each group (normal, carbohydrate intolerance and gestational diabetes). No significant differences were found for the variables of instrumented delivery, episiotomy, threat of abortion, macrosomic newborn and in vitro fertilisation. In contrast, significant differences were found for the variables of loss of foetal well-being, caesarean delivery, threat of premature delivery, preeclampsia, hospitalisation during pregnancy and gained weight (>12<span class="elsevierStyleHsp" style=""></span>kg; 8–12<span class="elsevierStyleHsp" style=""></span>kg; <8<span class="elsevierStyleHsp" style=""></span>kg). To be highlighted is the percentage of caesarean deliveries and loss of foetal well-being, being 22 and 10.3% respectively in the normal metabolism group; 40.5% and 45.9% in the carbohydrate intolerance group; and 33.8 and 22.1% in the gestational diabetes group.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the risk of presenting the main obstetric problems related to a carbohydrate metabolism disorder, especially those that have shown significant differences in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The results of the carbohydrate intolerance group and the gestational diabetes group are analysed against the group with normal metabolism (control group). This risk is expressed in the form of ORc and ORa by the variables that showed significant differences in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>: age, weeks of gestation and weight gained during pregnancy and BMI to eliminate possible confounding factors. The risk of having presenting more C-sections, more threats of premature delivery and greater loss of foetal well-being was significant in the carbohydrate intolerance group. But once adjusted, they only showed an increased risk of episiotomy; (ORa<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.61; 95% CI: 1.23–5.51) and loss of foetal well-being; (ORa<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>7.54; 95% CI: 3.65–15.56). In the gestational diabetes group, the risk of having more C-sections, more preeclampsia, greater loss of foetal well-being and more hospitalisations was significant. But once adjusted, they only showed an increased risk of loss of foetal well-being; (ORa<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.64; 95% CI: 1.39–5.08) and hospitalisations; (ORa<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.14; 95% CI: 1.15–8.61).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0135" class="elsevierStylePara elsevierViewall">Carbohydrate metabolism disorders during pregnancy are a problem on the rise, because it is developing in women at increasingly older ages, and together with the increase in obesity in the population. These are two of the main risk factors for presenting this disease.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,20</span></a> In this study, data of 799 women from the coverage area of La Fe Hospital in Valencia were analysed. The objective was to describe the baseline characteristics of a current sample of pregnant women, classified into 3 groups according to the metabolism of carbohydrates and to analyse the obstetric alterations and the associated risk according to the group. According to <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, pregnant women presented an average age in the 3 groups above 30 years, which corresponds to the average age of pregnant women in our country (between 30 and 35 years).<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> In addition, significant differences were found between the groups, with this average age being higher in the groups with glucidic metabolism disorders. In turn, the pre-pregnancy BMI was also statistically significantly higher in these groups, coinciding with the main risk factors stated in literature. The means of the gestation weeks were between 37 and 42 weeks, which is considered as full term. It was slightly lower in the 2 groups with metabolism changes, coinciding with the pathophysiology of the said disorder. The percentage of women with gestational diabetes is 8.5% in this study, which is between the approximate prevalence in pregnant women in our country which is 7–14%. The prevalence of carbohydrate intolerance in our study is 4.6%. Information about the prevalence of this disorder is scarce, but it is lower than that of gestational diabetes.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,22</span></a> Therefore, we have a sample that conforms to the average baseline characteristics described in pregnant women in Spanish territory, so it can be considered as a representative sample.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Gestational diabetes is a disease associated with an increased risk of presenting a series of medical disorders in pregnancy.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> These disorders together with other obstetric characteristics are analysed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. This verifies and defines which problems are associated with gestational diabetes in our sample, and if these problems are present equally in the pregnant women with carbohydrate intolerance, or alternatively if these behave like pregnant women with normal glucidic metabolism.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Of the variables analysed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, they presented a higher percentage in the groups of glucidic metabolism disorder compared to normal: instrumented delivery, termination of pregnancy by caesarean section, loss of foetal well-being, threat of premature delivery and hospitalisations during pregnancy. This coincides with that described in the bibliography,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> but when the p-value was obtained, the variables that showed a significant difference were: termination by caesarean section, loss of foetal well-being, threat of premature delivery, preeclampsia, hospitalisation and weight gain. The carbohydrate intolerance group behaved similarly to the gestational diabetes group, showing a higher percentage of obstetric problems than the normal group.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Regarding the significant variables, it should be noted that the percentage of caesarean sections in the gestational diabetes group was 33.8%, which is consistent with the results of other case series.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> This percentage was higher in the carbohydrate intolerance group: 40.5%. In the variable ‘threat of preterm birth’ the percentage of cases was higher in the carbohydrate intolerance group (8.1%), than in the gestational diabetes group (5.9%). The variable ‘loss of foetal well-being’ was 45.9% in the intolerance group compared to 22.1% in gestational diabetes. And the weight gain was insufficient in 45.9% of women with carbohydrate intolerance. Based on the descriptive analysis of the sample we found that the group with carbohydrate intolerance presents greater problems than pregnant women with normal metabolism, and in some variables this group presents more disorders than in the gestational diabetes itself. A possible explanation is that pregnant women with intolerance do not receive any additional check-ups during their pregnancy, while the diabetics receive treatment and a greater number of revisions<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a> thereby managing to improve the adverse obstetric outcomes if their gestational diabetes is well-controlled.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">Once the obstetric disorders described in literature were analysed in our sample, we calculated the risk of suffering the most representative ones and especially those variables that were significant in the study (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) by calculating the ORa (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). To avoid bias, we made adjustments for the confounding factors that are associated with greater morbidity in pregnancy<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> and that showed significant differences in our study groups in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><p id="par0160" class="elsevierStylePara elsevierViewall">With the ORc, a greater number of variables were significant, but once this was adjusted to ORa the variables which showed a significant increase in risk with respect to the control group were the following: For women with gestational diabetes we found an ORa risk 2.64 times greater for ‘loss of foetal well-being’ and a risk 3.14 times greater of ‘requiring hospitalisation’ during pregnancy. As we have mentioned, gestational diabetes is the disease that most often causes medical disorders in the pregnancy,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> and the results of our study suggest the same. Therefore, close monitoring with metabolic check-ups, cardiotocographs and Doppler ultrasounds are important, especially in the last weeks, to control foetal well-being.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">For pregnant women with carbohydrate intolerance we found an ORa risk of 2.61 times greater for ‘requiring episiotomy in vaginal delivery’, a 7.54 times greater risk of finding ‘loss of foetal well-being’ and an increased risk of 2.06 to ‘require hospitalisation’. Our results indicate that carbohydrate intolerance produces disorders similar to the group with gestational diabetes, and this is in line with other studies that relate the gradual increase in baseline blood glucose levels in pregnant women with greater obstetric morbidity,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> approaching those found in gestational diabetes. Therefore, there is a greater risk of episiotomy since in this group there are more macrosomic newborns than in mothers with normal glucidic metabolism.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> In addition, we found a risk of ‘loss of foetal well-being’ which was greater than in the mothers with gestational diabetes, probably because with carbohydrate intolerance no additional foetal check-ups are performed to that of a normal pregnant woman, in contrast to the action protocol of gestational diabetes.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0170" class="elsevierStylePara elsevierViewall">In conclusion, carbohydrate metabolism changes in pregnant women are associated with greater morbidity in the mother and in the newborn, with the main contribution of the study being the data collected on the carbohydrate intolerance group. This is a lesser-studied group and one in which we have found significant obstetric disorders when compared to the control group, and similar to those of the gestational diabetes group, even finding a greater risk for some variables such as loss of foetal well-being and episiotomy.</p><p id="par0175" class="elsevierStylePara elsevierViewall">It would be necessary to carry out further intervention studies to verify whether the treatment of pregnant women with carbohydrate intolerance influences obstetric morbidity. The objective of this study is to assess the relationship between the presence of alterations in the metabolism of carbohydrates during pregnancy and the risk of presenting adverse obstetric outcomes in the mother and the fetus.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Patients and method</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">An observational study was conducted in 799 women who had just given birth. The clinical and obstetric characteristics of the patients, as well as the data related to pregnancy and delivery, including the state of the newborn were analyzed. The information was evaluated in relation to the alterations in the carbohydrate metabolism.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results and discussion</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">For women with gestational diabetes a 2.64 times greater risk of loss of fetal well-being and 3.14 times greater risk of requiring hospitalization during pregnancy were found. For pregnant women with carbohydrate intolerance, a 2.61 times higher risk of requiring episiotomy in vaginal delivery, 7.54 times greater risk of finding fetal well-being loss, and 2.06 times greater risk of requiring hospitalization were found. Para las gestantes con intolerancia a los hidratos de carbono encontramos un riesgo de 2,61 veces mayor de requerir episiotomía en el parto vaginal, 7,54 veces mayor de encontrar pérdida del bienestar fetal y 2,06 veces mayor de requerir hospitalización.
