On physical examination, a large tumor was observed on the left leg, causing deformity and limited leg movement (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). An X-ray showed an osteolytic metastatic lesion causing a loss of bone continuity of the left tibia (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B). Staging studies revealed a right renal tumor associated with pulmonary and adrenal disease; bone involvement was also found at the level of the right ilium and sacrum. Histopathologic assessment of a biopsy performed on the leg lesion reported a clear cell renal cell carcinoma. The case was evaluated by a multidisciplinary team and it was decided to administer palliative radiotherapy, achieving an adequate symptomatic control. After surveillance, the patient was discharged to receive ambulatory systemic treatment. Metastatic bone disease due to renal cell carcinoma (RCC) is very destructive and leads mainly to osteolytic lesions that compromise bone integrity and negatively impact the outcome of these patients.
