On examination, she had painful and erythematous nodules in the lower (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A, arrows black) and upper limbs (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B, black arrows), suggestive of erythema nodosum. The chest radiograph showed an increase in the size of the pulmonary hilum (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A, white arrows). Chest computed tomography showed mediastinal lymphadenopathy (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>B, white arrows) and pulmonary nodules with perilymphatic distribution. Given the clinical and radiological suspicion of Löfgren's syndrome, symptomatic treatment with nonsteroidal anti-inflammatory drugs was indicated. After 3 months, the patient progression was favourable, afebrile with resolution of arthralgia and erythema nodosum.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Sarcoidosis is a systemic granulomatous disease that mainly affects the lung (90%) and can occur with different forms of presentation. Löfgren's syndrome is a common form of sarcoidosis (48%). It occurs predominantly in young or middle-aged adult women. It is characterized by bilateral hilar lymphadenopathy, erythema nodosum and arthritis or arthralgias. The presence of this triad is highly specific for the diagnosis of sarcoidosis (95%), so it is not necessary to perform a confirmation biopsy. The treatment is symptomatic and is based on non-steroidal anti-inflammatory drugs.
