The pathophysiology of BEUM remains unknown, although some authors have linked it to a dysfunction of the sympathetic and parasympathetic nervous systems.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Migraine is a very prevalent disease characterized by a pulsating hemicranial headache that can be accompanied by other typical symptoms, such as visual auras, photophobia, sonophobia, nausea, or vomiting. Migraine refractory to treatment can occasionally last for days or even weeks, and atypical signs, such as BEUM, may appear.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The purpose of this study is to describe the clinical and epidemiological characteristics of migraine patients with BEUM who were diagnosed in the Ophthalmology Service of a tertiary-level hospital, and, in turn, to assess the potential association between BEUM and migraine.</p><p id="par0020" class="elsevierStylePara elsevierViewall">To this end, we retrospectively selected four patients who had visited the Ophthalmology Emergency Department of Clinical University Hospital Lozano Blesa of Zaragoza (Spain) between July and October 2021. All patients presented with unilateral mydriasis in relation to a migraine. After entering these cases into a coded database, we reviewed their medical records to collect the following variables (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>): sex, age, chief complaint, personal history, duration of the current episode, affected eye, and associated symptoms.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">The four study subjects were women, and they all visited the hospital complaining of migraine symptoms, such as hemicranial headache (2 cases), blurry vision (1 case), and pupil asymmetry (1 case). Three subjects experienced symptoms that had only begun a few hours earlier, whereas one of them had symptoms that had persisted for 7 days. None of them yielded relevant findings in an ophthalmologic and neurologic examination.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Anisocoria is a relatively frequent cause of medical consultations in both Ophthalmology and Neurology Emergency Departments.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> There are many causes that underly this alteration, most of which are benign in nature, although some can potentially be serious. Thus, it is important to carry out an adequate history-taking and a thorough physical examination to orient potential symptoms and signs of alarm that could allow for ruling out serious lesions.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Two pathophysiological mechanisms underly BEUM: hyperactivity of the sympathetic system or hypofunction of the parasympathetic system.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">In previous articles published on BEUM, most cases correspond to relatively young women with a personal and/or family history of migraine.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">As this is an episodic disorder that is occasionally hard to assess by patients themselves or their close contacts, BEUM is often underdiagnosed in many situations.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Therefore, it would be advisable to include BEUM as a potential differential diagnosis in a patient with a history of migraine presenting with a pulsating hemicranial headache and unilateral mydriasis, although not before ruling out other causes by means of an adequate history-taking, a thorough physical examination, and, if necessary, complementary tests.</p><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0005">Ethical considerations</span><p id="par0055" class="elsevierStylePara elsevierViewall">No personal data of the patients described in this work were disclosed. All four patients were informed of the possibility that their symptoms be included in a study, which they understood and accepted.
