The expected lifetime of SLE patients is lower than that of the general population but has improved thanks to diagnostic and treatment advances. According to the management of SLE patients, the Systemic Lupus International Collaborating Clinics (SLICC) group recommended 3 domains to be included in the description of SLE patients: disease activity, cumulative damage, and quality of life.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">1</span></a> Health-Related Quality of Life (HRQoL) is a multi-dimensional concept that includes domains related to physical, mental, emotional, and social functioning. In fact, patients with the same health status could present a different HRQoL.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">2</span></a> In recent years, HRQoL has garnered increased interest in the management of SLE, where the focus had previously been on controlling disease activity and organ damage. In fact, HRQoL assessments are important in SLE given the chronicity of the disease, unpredictable flares, medication side effects and organ damage, and their ability to systematically evaluate the patient’ perspective on the effects of the disease: information that is not provided using standardized disease activity tools.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a> Moreover, the assessment of patient-reported outcomes has been included in pharmaceutical clinical trials on SLE,<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">4</span></a> but it has not attained generalized use status in standard clinical practice.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">5</span></a> For HRQoL studies, the focus has been on whether disease activity impacts Quality of Life, but the results have been controversial. Some studies have demonstrated a lack of correlation between these outcomes, while others have shown that disease variables, such as disease activity, are strongly associated to HRQoL.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">3,6–8</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">We investigated, in patients with SLE, several different HRQoL parameters, including disability, fatigue, perceived quality of life and perceived global health status. The aim of this research was to analyze the association between the HRQoL and SLE disease activity during a 12-month follow-up period in patients with SLE, as well as to determine clinical factors that could impact HRQoL.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0015" class="elsevierStylePara elsevierViewall">This research was approved by the Biomedical Research Ethics Committee of La Fe Hospital in Valencia, Spain (Ref. 2017/0264) and complies with the ethical guidelines of the Declaration of Helsinki. All participants provided written informed consent. We consecutively enrolled 70 patients who satisfied the 2012 SLICC criteria.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">9</span></a> 42 patients from the 70 SLE patients enrolled were selected randomly and monitored for 12 months, with follow-up visits every 3 months. In all cases, a complete medical and analytical examination was performed. SLE disease activity using the SLEDAI and SLE cumulative damage using the Systemic Lupus International Collaborating Clinics/American College of Rheumatology (SLICC-ACR) score were assessed.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">1,10</span></a> The analytical laboratory variables were concurrently measured at the time of the medical visit.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data collection</span><p id="par0020" class="elsevierStylePara elsevierViewall">Data were collected at baseline in all patients and during quarterly follow-up visits. For this study, the following data were used: age at diagnosis and at inclusion, sex, disease duration, SLE disease activity index (SELENA-SLEDAI), SLE damage index (SLICC), SLE severity index (Katz), medication use (vitamin D supplementation, prednisone, antimalarials, immunosuppressive agents, i.e. azathioprine, mycophenolate mofetil, methotrexate, cyclophosphamide, biological therapy), current smoking and analytical laboratory variables including erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), anti-double stranded DNA (dsDNA) antibodies, cytopenia (haemoglobin, erythrocytes, lymphocytes, leucocytes, neutrophils, platelets) and complement levels (C3 and C4).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">HRQoL measures</span><p id="par0025" class="elsevierStylePara elsevierViewall">HRQoL was measured considering fatigue, quality of life, disability and the patient's self-perception of global health status. Fatigue was measured using the validated Functional Assessment of Chronic Illness Therapy (FACIT- FATIGUE) score.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">11</span></a> This 13-domain questionnaire generates a numerical score (0–52), where a lower score represents more fatigue. Global health was measured using the EuroQol 5-dimension (EQ-5D-5L) health questionnaire with 5 level.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">12,13</span></a> This questionnaire classifies the patient's symptoms, or dimensions (mobility, self-care, usual activities, pain/discomfort and anxiety/depression) into one of five levels representing: no problems [1], mild problems [2], moderate problems [3], severe problems [4] and extreme problems/impossibility [5].</p><p id="par0030" class="elsevierStylePara elsevierViewall">The Global Health Status Scale (GHS) is a 0–100 scale where patients are asked to indicate their overall health on the day they are completing the questionnaire, where a lower score represents declining health status. Functional disability was assessed using the Health Assessment Questionnaire (HAQ).<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">14</span></a> Patients completed questionnaires on fatigue (FACIT-FATIGUE), quality of life (EQ-5D-5L), disability (HAQ) and Global Health Status (GHS) (0–100) at the baseline visit and at each quarterly follow-up visit.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">Data are presented as the mean (standard deviation) and median (1st, 3rd quartile) in the case of continuous variables and as relative and absolute frequencies in the case of categorical variables. Bayesian linear regression analyses with monotonic effects were conducted with disease activity as the dependent ordinal variable, and the above four variables (FATIGUE, EQ, HAQ and GHS) as independent variables. All statistical analyses and graphs were performed using R statistical software.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">A total of 70 consecutive patients with SLE (90% female) participated in the study, with a mean age at diagnosis of 31.81 (15.44) years and a mean time of disease evolution of 16.41 (11.04) years. A total of 62.86% of patients were receiving glucocorticoid treatment, 47.14% immunosuppressants (methotrexate, azathioprine, biological therapy or mycophenolate) and 64.29% antimalarials. A total of 21.43% were active smokers. A total of 17.14% of patients had vitamin D deficiency (<20<span class="elsevierStyleHsp" style=""></span>ng/ml) and 15.71% vitamin D insufficiency (20–30<span class="elsevierStyleHsp" style=""></span>ng/ml). Cytopenia was presented by 57.14% and anaemia by 22.86%. Complement consumption was observed in 42.86% of patients, and 42.86% tested positive for anti-dsDNA-antibodies.</p><p id="par0045" class="elsevierStylePara elsevierViewall">At baseline, the median SLEDAI score was 4 (3, 8), median SLICC score was 1 (0, 2), median Katz score was 2 (1, 2) and 28.57% of cases showed SLEDAI<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>6. Mean FACIT-FATIGUE score was 31.96 (13.89), EQ-5D-5L 0.69 (0.28), HAQ 0.79 (0.72) and GHS scale 62.14 (26.73). Using the Pearson correlation coefficient, we observed that all four HRQoL measures correlate (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Considering the SLEDAI index as a dependent ordinal variable, we performed Bayesian linear regression analyses, using age, smoking habit, haematological alterations and immunosuppressant therapy as corrective variables, as well as the four HRQoL parameters. Statistical analysis showed significant association between the SLEDAI score and HRQoL variables. In particular, in those cases with high disease activity, we observed low scores on EQ-5D-5L (Est −0.04; CI95% −0.06–0.02), FACIT-FATIGUE (Est −1.55; CI95% −2.35–0.82) and GHS (Est −2.75; CI95% −4.33–1.25), and an increase in HAQ (Est 0.10; CI95% 0.06 0.15).</p><p id="par0055" class="elsevierStylePara elsevierViewall">Then, 42 patients with SLE were selected randomly and monitored for 12 months, with follow-up visits every 3 months, to evaluate if the association found would differ during the course of follow-up. A Wilcoxon rank sum test to ensure the homogeneity of the total group SLE patients and the follow-up group was performed. No statistically significant differences between both groups were observed. Clinical and HRQoL data are presented in (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Bayesian linear regression analyses with monotonic effects were performed for these 42 patients, and we observed that the significant association between SLEDAI score and HRQoL variables is maintained during follow-up (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Having included the influence of the quarterly follow-up in the model, we did not observe significant deterioration of HRQoL during the 12 months of follow-up, so no time effect is observed in the statistical model (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Moreover, in the statistical models, different clinical factors that may impact HRQoL parameters, such as age at inclusion, years of disease evolution, smoking, glucocorticoid use, immunosuppressant therapy, antimalarial treatment, haemoglobin (g/dL), leucocytes (10<span class="elsevierStyleSup">3</span>/μL), platelets (10<span class="elsevierStyleSup">3</span>/μL) and 25-OH-vitamin D (ng/ml), were included. Regarding perceived GHS in SLE patients, a significant negative association of worst perception of GHS was observed among those with a smoking habit, immunosuppressant therapy and older age, in addition to the significant association with disease activity. Regarding global health measured using the EQ-5D-5L health questionnaire, no significant association apart from disease activity was found. A high level of disability measured by HAQ was associated to glucocorticoid use, apart from the disease activity. On the other hand, antimalarial treatment presented a negative association with disability in SLE patients. High fatigue perception was significantly associated with smoking, immunosuppressant therapy and low levels of leucocytes and 25-OH-vitamin D.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">The HRQoL assessment in SLE is still important, due to the emerging understanding that physician and laboratory measures do not give a full picture of the patient's condition.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">15</span></a> In this prospective longitudinal study, we used four HRQoL questionnaires (HAQ, FACIT-FATIGUE, EQ-5D-5L and GHS), all of which are considered generic HRQoL instruments and not SLE-specific. Previous studies suggest that HRQoL assessed using a generic instrument yields similar information to that captured using an SLE-specific HRQoL instrument.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">16–18</span></a> In our study, we observed significant correlations between the four HRQoL parameters used and comparable associations in terms of their change during the follow-up. In fact, we did not report a significant deterioration in any of the HRQoL parameters during the 12 months of follow-up.</p><p id="par0075" class="elsevierStylePara elsevierViewall">One important finding in this study is the significant association between disease activity evaluated with the SLEDAI index and HRQoL, even after adjustment for other variables that were associated with HRQoL. The relationship between disease activity and HRQoL in SLE patients is still controversial, probably due to a combination of varying study designs, different measures of activity and heterogeneous disease states.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">19–22</span></a> Various factors have been reported to adversely affect HRQoL in SLE patients,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">22</span></a> and in this study we have included corrective variables that could influence HRQoL factors, such as “age”, which did not yield a significant association with HRQoL except for GHS, probably due to the small sample size. In contrast, smoking showed a significant association to HRQoL parameters like fatigue and GHS, as is described in the literature.<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">23,24</span></a> Fatigue was found to be a very common symptom in our SLE cohort, as is described in the literature.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">25,26</span></a> A significant association between fatigue and disease activity was observed in our study, but in the literature, this finding is controversial. Several studies, in consonance with our work, reported a direct, though generally weak, correlation between increasing disease activity and higher levels of fatigue,<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">25,27–29</span></a> while there is one study that did not report said association.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">30</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding the influence of immunosuppressant therapy on fatigue reported in our study, there is limited information on HRQoL in prospective studies relating to pharmacological therapies. In a previous study where the impact of two biological therapies in HRQoL was evaluated, fatigue improved within the first months of treatment in the case of rituximab, but the improvement declined at one year of treatment,<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">31</span></a> so it would be in consonance with our sample series where immunosuppressant therapies were over one year of treatment, so it may explain the association found in the present study.</p><p id="par0085" class="elsevierStylePara elsevierViewall">SLE has a considerable impact on the HRQoL of patients and their ability to carry out normal daily activities, resulting in a high prevalence of disability.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">32</span></a> Our SLE cohort showed mild disability, but in spite of this, the association between disability and disease activity was reported. Moreover, a significant association between disability and glucocorticoid use, and mild disability and antimalarial treatment was found. Little is known about the impact of antimalarials on disability, and our findings contrast with those reported in the study by Parodis I et al.,<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">31</span></a> where in a subanalysis of their treatment cohorts, the impact of antimalarial treatment on HRQoL was evaluated. They observed that hydroxychloroquine treatment had no impact on physical component summary scores of the SF-36 scale, but in the mental components, there was a distinction between those who were taking antimalarial agents from those who were not. Perhaps the difference in the two studies could be due to the fact that, in our SLE series, most of the patients were receiving antimalarial treatment but not immunosuppressant therapies, in contrast to Parodis’ study, where all SLE patients received biological therapies.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusion</span><p id="par0090" class="elsevierStylePara elsevierViewall">In conclusion, this study confirmed that the four generic HRQoL instruments used were correlated and performed similarly in the assessment of HRQoL in SLE patients. This study also indicates that disease activity is directly associated with HRQoL. In summary, our study of patients with SLE demonstrates that disease activity impacts the majority of quality of life domains, as measured by the EQ-5D-5L, HAQ, GHS and FACIT fatigue scales. Disease activity, organ damage and other clinical factors that could affect HRQoL were recorded. The association between disease activity and HRQoL was assessed using Bayesian linear regression models with monotonic effects.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Data from 70 patients at the baseline visit and 42 patients with 1 year of follow-up were analyzed. At baseline, 28.57% of patients presented Systemic Lupus Erythematosus Disease Activity Index (SLEDAI)<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>6. In the 70 baseline patients, disease activity was associated with HRQoL in all four parameters. In the 42 patients with 12 months of follow-up, the positive association of disease activity with GHS, FACIT-FATIGUE and EQ-5D-5L and the negative association with HAQ was maintained.</p><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Patients who are smokers and those receiving immunosuppressant therapy presented low GHS and FACIT-FATIGUE scores. En la visita basal el 28,57% de los pacientes presentaban un índice SLEDAI<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>6. La actividad clínica medida mediante el índice SLEDAI se asociaba de forma estadísticamente significativa a los 4 parámetros de CVRS. En los 42 pacientes con un año de seguimiento la relación directa entre la actividad clínica y el FACIT-FATIGUE, EVA y EQ-5D-5L, así como la relación indirecta con el HAQ, se mantuvieron.</p><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">Los pacientes fumadores y aquellos bajo tratamiento inmunosupresor presentaban valores disminuidos de EVA y FACIT-FATIGUE. Además, los pacientes con edades más avanzadas presentaban valores disminuidos de EVA, y aquellos con niveles bajos de vitamina D o leucopenia presentaban mayor percepción de fatiga.</p></span> <span
