It is caused by a defect in the development of the mantle and can extend to the subarachnoid space. There are several types of schizencephaly, which usually makes an appearance in the form of a convulsive crisis during childhood or youth. We present the case of a 75-year-old woman with no previous neurological history and with no limitations when performing the basic activities of daily life. She presented with a low level of consciousness and clonic movements in her left arm. The neuroimaging tests (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>) revealed an abnormality of the left ventricular system compatible with open-lip schizencephaly. The electroencephalogram (EEG) reported right temporal lobe focal status epilepticus and treatment was established with anticonvulsant drugs, however a surgical approach was ruled out by the neurosurgery department. Despite the measures adopted, the patient persisted with a low level of consciousness. Due to the irreversibility of the aetiology responsible for her condition and the persistent vegetative state, it was decided to transfer her to a social health centre. The peculiarity in our case lays in the age of the patient when diagnosed.
