BMI: body mass index; PASI: Psoriasis Area Severity Index; VAS: Visual Analogue Scale of pain; BDI: Beck Depression Inventory; DLQI: Dermatological Related Quality of Life Index.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Psoriasis is a chronic inflammatory dermatological condition that affects approximately 2%–3% of adult population in Western countries.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> The prevalence of metabolic syndrome among subjects with psoriasis ranges from 20 to 50%.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> Besides, not only patients with psoriasis present greater prevalence and incidence of obesity, and these odds increase with the severity of the skin lesions, but also there is an increased risk of psoriasis with higher BMI, waist circumference and weight gain.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The common ethiopathogenic mechanism shared with both diseases seems to be a chronic inflammatory state. This situation would be aggravated by worse lifestyle habits.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1,3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Furthermore, non-pharmacological weight loss interventions have shown an improvement in the severity of psoriasis among subjects with a concomitant obesity.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Besides, obesity is a predictor of poorer response to anti-TNF-α agents among patients with psoriasis, and this metabolic condition could increase the risk of systemic treatment discontinuation.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg is a GLP-1 analogue that has been approved for the treatment of patients with obesity as an adjunctive therapy to lifestyle intervention. Currently, it is the only GLP-1 analogue approved in Spain as an anti-obesity drug.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> In this sense, liraglutide has proven its safety among subjects with psoriasis and both, type 2 diabetes, and obesity. Moreover, liraglutide could exert a beneficial effect on skin lesions among subjects with psoriasis.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> To date, there are no studies investigating the effect of liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg on weight loss and severity of psoriasis.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We aimed to study the effects of liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg among subjects with psoriasis and obesity on weight loss, skin lesions, metabolic and psychological outcomes.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Subjects</span><p id="par0035" class="elsevierStylePara elsevierViewall">A prospective cohort study was designed where 20 subjects with psoriasis and obesity were recruited consecutively. Exclusion criteria were as follows: (a) any type of diabetes; (b) contraindications to liraglutide; (c) subjects who had modified psoriasis treatment in the preceding three months; (d) subjects with a former use of liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg or with any active anti-obesity drug.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Written informed consent was obtained for both groups prior to study participation. The study was approved by the Ethics Committee of the hospital.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Interventions</span><p id="par0045" class="elsevierStylePara elsevierViewall">Diet counselling included a tailored quantitative diet with an average of 500<span class="elsevierStyleHsp" style=""></span>kcal/day reduction from calculated baseline metabolic rate adjusted by physical activity. Besides, a minimum of 150<span class="elsevierStyleHsp" style=""></span>min of aerobic exercise per week was also encouraged.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Regarding liraglutide titration, patients started with a dose of 0.6<span class="elsevierStyleHsp" style=""></span>mg per day for the first week increasing weekly up to 3<span class="elsevierStyleHsp" style=""></span>mg. When the dose was not tolerated due to gastrointestinal side effects, this dose was reduced again to the previous regimen.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Patients were evaluated baseline, and after three months since the beginning with liraglutide. We included anthropometric, dermatological, psychological, and biochemical parameters.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The Psoriasis Area and Severity Index (PASI) was used to assess the extension and severity of plaques. The Dermatological Related Quality of Life Index (DLQI) measured the health-related quality of life related to a skin disease, where the higher the score, the greater deterioration in quality of life. The Spanish version of the Beck Depression Inventory (BDI) was administered, where a cutoff equal or greater than 16 instead of 13 for the entire 21-item measure exhibits the best balance between sensitivity and positive predictive value among people with obesity.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0065" class="elsevierStylePara elsevierViewall">The distribution of the sample was analyzed by the Kolmogorov–Smirnoff test. BMI reductions, PASI evolution, BDI, VAS (visual analogue scale of pain), DLQI and inflammatory markers were compared by using <span class="elsevierStyleItalic">λ</span><span class="elsevierStyleSup">2</span> tests for categorical variables and <span class="elsevierStyleItalic">t</span> tests and univariate analysis of variance (ANOVA) for continuous variables. Potential independent predictors of skin lesions (dependent variable) were evaluated using univariate and multivariate logistic regression analysis of the variance, with baseline characteristics as independent variables.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">A total of 20 patients with psoriasis and obesity were included. 55% of subjects were women with a mean age of 45.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.7 years.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Mean psoriasis duration was 18.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10 years. All subjects included in the study were actively treated either with biological therapy (16/20) or phototherapy (4/20) for their psoriasis.</p><p id="par0080" class="elsevierStylePara elsevierViewall">When considering comorbidities, 30% of patients had psoriatic arthritis and another 30% had been diagnosed with depressive disorder. When considering metabolic and mechanical complications related to obesity, 35% had hypertension, 4 out of 20 had dyslipidemia and only 10% of patients had an obstructive sleep apnea.</p><p id="par0085" class="elsevierStylePara elsevierViewall">BMI was significantly reduced after three months of liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg. The mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD of absolute weight lost after treatment was −7.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.4<span class="elsevierStyleHsp" style=""></span>kg, with a range of −21.6 to 12.0<span class="elsevierStyleHsp" style=""></span>kg. Furthermore, the number (percentage) of patients that experimented a weight loss lower than 5%, between 5% and 10% and higher than 10% were 3 (15%), 11 (55%) and 6 (30%), respectively. Liraglutide treatment effected a significant decrease in psoriasis severity in terms of PASI, VAS and DLQI. These data are shown more detailed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, metabolic and inflammatory parameters improved significantly after three months of liraglutide.</p><p id="par0095" class="elsevierStylePara elsevierViewall">A significant correlation was found between change in BMI and DLQI improvement (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04). This finding is represented in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">In the multiple regression analysis, changes in BMI did not correlate with any inflammatory parameter, BDI or PASI.</p><p id="par0105" class="elsevierStylePara elsevierViewall">There were no treatment discontinuations due to adverse effects.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">We found that, after three months of treatment with liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg among patients with psoriasis and obesity not only improved weight loss but also psoriatic lesions and quality of life. Moreover, the amelioration of skin lesions was independent of weight loss. Besides, there was a significant reduction of inflammatory markers as well as metabolic parameters. The reduction of inflammatory parameters together with the improvement of dermatological lesions not attributable to weight loss would suggest that liraglutide could have anti-inflammatory properties.</p><p id="par0115" class="elsevierStylePara elsevierViewall">There is an increased risk of metabolic disorders among subjects with psoriasis. In fact, a strong association has been found between psoriasis and metabolic syndrome.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> The increased risk of cardiovascular disease (CVD) among these patients is secondary to a greater prevalence of metabolic comorbidities including obesity.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> The prevalence of obesity among people with psoriasis is significantly greater compared to controls, with prevalence rates of 10.8%–24%.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> However, not only is psoriasis a risk factor for developing obesity, but this relationship is bidirectional.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Both entities share a chronic low-grade inflammatory state with an increase in circulating pro-inflammatory cytokines. When these two diseases coexist, there could be a crosstalk between the adipose tissue and the skin mediated by these cytokines. In fact, adiposity can induce the activation of T-helper-17 cells, responsible for secreting IL-17, which is key in the etiopathogenesis of autoimmune diseases, including psoriasis. Furthermore, patients with psoriasis have an increased risk of social isolation, unhealthy dietary patterns, stigmatization, more sedentary habits and increased alcohol consumption, all of which are predisposing factors for obesity.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1,9</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Psoriasis per se negatively impairs both health-related quality of life and mood.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1,10</span></a> In fact, there is a higher prevalence of depressive disorder among subjects with psoriasis, with prevalence rates up to 62%.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> Depressive symptomatology worsens as the severity of psoriatic lesions does. We found a significant amelioration of depressive symptomatology, but this improvement was irrespective of weight loss. As mentioned before, the reduction of this inflammatory state shared by not only for psoriasis and obesity but also with depressive disorder could have mediated these results.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">Up to date, studies assessing the effects of liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg on weight and skin lesions among patients with psoriasis and obesity are lacking. There is little evidence about the effect of lower doses of liraglutide among subjects with psoriasis, type 2 diabetes, and obesity. In this sense, few previously published studies have shown that patients with type 2 diabetes treated with liraglutide improve the severity of psoriatic plaques, being this effect independent of weight loss. However, the studies published to date include a small number of patients, the doses of liraglutide are variable and their duration is short.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">It is well known that psoriasis and obesity have a negative impact on health-related quality of life, which is very important to consider when referring to chronic diseases. Treatment with liraglutide could have a positive impact on quality of life, either directly by reducing inflammation on skin, or indirectly, by weight loss.</p><p id="par0135" class="elsevierStylePara elsevierViewall">We acknowledge that our study has several limitations. In Spain, antiobesity treatments are not reimbursed by the national health system and, therefore, it may result in a selection bias. We did not have a control group. Also, patients were on different biologic therapies for their psoriasis. We also acknowledge the possibility of a type II error, which has an inverse relationship with the power of a statistical test. This means that the higher power of a statistical test, the lower the probability of committing a type II error. One of the simplest methods to increase the power of the test is to increase the sample size used in a test and, therefore, further studies with a larger sample size should be guaranteed to confirm our results. However, to our knowledge, this is the first study assessing the effects of liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg on patients with psoriasis and obesity. Despite the sample size seems relatively small, in meta-analysis where patients with psoriasis had type 2 diabetes in addition to obesity, the total sample comprised only 32 patients from 4 different studies.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> Also, the follow-up of the study, despite relatively short, was enough to achieve significant results.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Psoriasis approach should be multidisciplinary and holistic, considering not only skin lesions but also cardiovascular risk factors, including obesity, as well as the psychological aspects and quality of life.</p><p id="par0145" class="elsevierStylePara elsevierViewall">In conclusion, liraglutide 3<span class="elsevierStyleHsp" style=""></span>mg for three months is effective and safe in reducing weight and improving psoriatic lesions among patients with psoriasis and obesity.
