(D) Actinomycosiscolony in the site of wide spread inflammatory cell infiltration (HE ×40).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Acute appendicitis ranks among the most prevalent reasons for patients to be admitted to emergency facilities with abdominal pain, and appendectomy is among the most frequently performed surgical procedures on a global scale. The incidence rate of acute appendicitis over a life span is approximately 8%.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">1,2</span></a> Actinomyces is one of the rare pathogens in the etiology of acute appendicitis. They are only responsible for 0.02–0.06% of appendicitis causes.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">3</span></a> Actinomyces is a constituent of the end ogenous flora found in various anatomical locations. In the gastrointestinal tract, the ileocecal region, especially the appendix, is the most common site of actinomycosis. Actinomycosis can look like other ileocecal diseases, like diverticulitis, abscesses, inflammatory bowel disease, obstruction, perforation, and malign and tumors, which makes it hard to diagnose.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">1–4</span></a> There are no specific clinical, laboratory, radiological, or endoscopic parameters for diagnosis, and the final diagnosis of actinomycosis is usually made with pathological confirmation after surgery.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">4–6</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0010" class="elsevierStylePara elsevierViewall">The main objective of this study is to share the demographic and histopathological, clinical, and laboratory characteristics of patients with actinomycosis in appendicitis samples. To achieve this goal, the pathology reports of patients operated on with a pre-diagnosis of acute appendicitis between January 2013 and November 2023 were reviewed retrospectively at the General Surgery Clinic of Denizli Servergazi State Hospital. We analyzed patients’ medical records, including clinical, radiological, pathological, and therapeutic data. Ethical approval for the study was received from the Pamukkale University Non-Interventional Clinical Research Ethics Committee (E-60116787-020-448882).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0015" class="elsevierStylePara elsevierViewall">In a nearly 10-year period, between January 1, 2013 and November 30, 2023, at the Servergazi State Hospital Department of General Surgery, a total of 5834 patients were operated on with a diagnosis of acute appendicites. Actinomyces appendicites were reported in 14 (8 male (57%) and 6 (43%) female) specimens (0.23%). It was determined that only appendectomy was performed in 10 patients (71.4%), ileocecal resection was performed in two patients (14.2%), and right hemicolectomy was performed in two patients (14.2%). The operations of five patients were performed laparoscopically, and the operations of nine patients were performed with open surgery. In three patients, the surgery began laparoscopically but had to be switched to open surgery because of concerns about an ileocecal mass and a perforated ceasure (one patient had a right hemicolectomy and two patients had an ileocecal resection). While the Mc Burney incision was preferred in seven patients, a paramedian incision was used in two patients (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). When the patients’ lab results were looked at when they were admitted, it was found that three of them had a white blood cell count (WBC) that was within the normal range (8–9.7<span class="elsevierStyleHsp" style=""></span>mg/dL), while other patients had leukocytosis (10.2–18.7<span class="elsevierStyleHsp" style=""></span>mg/dL). C-reactive protein (CRP) was normal in one patient and high in the other patients. While the first-hour erythrocyte sedimentation rate (ESR) was normal in five patients, it was found to be high in the other patients (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). It was determined that ultrasonography (US), abdominal computed tomography (CT), or a combination of the two methods were used as imaging methods. Findings on radiological imaging were reported as acute appendicitis, plastrone appendicitis, and ileocecal mass. During the perioperative period, the patients exhibited an ileocecal mass and symptoms consistent with acute appendicitis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The average hospital stay was found to be 3.2 days (2–7 days). Patients were prescribed cefuroximeaxetil or amoclavine clavulanic acid peroral twice a day as a discharge prescription. Actinomycosis was diagnosed as a result of the histopathological examination of the specimens. In light microscopy, typically the microorganism forms actinomycotic granules (sulfur granules). Microscopic examination of cases revealed acute phlegmonous appendicitis, perforated appendicitis, periappendicitis, acute suppurative appendicitis, localized peritonitis, abscess formation and reactive lymph nodes, and actinomyces sulfur granules scattered in the purulent exudate in the appendiceal lumen (hematoxylin and eosin (H&E), ×2 magnification) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). As a result of the pathology results, the patients were given oral penicillin or semi-synthetic penicillin derivatives for one month.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0020" class="elsevierStylePara elsevierViewall">Actinomycosis is an uncommon infectious disease primarily caused by <span class="elsevierStyleItalic">Actinomyces</span> spp., with A. İsraeli being responsible for 90% of cases. Actinomycosis is uncommon, with an incidence ranging from 1/300,000 to 1/1,000,000.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">5,7,8</span></a> These bacteria are gram-positive, branching, slow-growing, and can survive with little oxygen or no oxygen at all. Previously mistaken for a fungal organism, <span class="elsevierStyleItalic">Actinomyces</span> spp. causes a type of granulomatous infection. Actinomycosis can be categorized as a facial type (50%), a thoracic type (15%), an abdominal and pelvic type (20%), and other forms (15%).<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">9</span></a> The disease is often seen in populations with low socio-economic levels and in individuals with bad oral hygiene aged between 20 and 50 years. Actinomycosis infection is more prevalent among men, with a male-to-female ratio of 3:1. Where as the pelvic form is more frequent among women, the cause is often the prolonged use of intrauterine contraceptive devices.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">2,10</span></a> Some of the predisposing factors can be classified as: diabetes, immunosuppression from steroid intake, HIV infection, transplants, local tissue damage caused by trauma, recent abdominal surgery, perforation, and intrauterine instruments used for more than 10 years, etc.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">11–13</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The most important step in the pathogenicity of actinomyces, which is found endogenously in the human flora, is the disruption of the mucosal barrier.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">2,11,14</span></a> Actinomyces-related findings may occur weeks to months after mucosal disruption.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">14,15</span></a> Therefore, patients should be questioned about their history of recent abdominal surgery, bowel perforation, cancer, poor oral hygiene, dental procedures, and intrauterine contraceptive devices. Direct spread into near by tissue is an other method of spread that Actinomyces frequently uses.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">16</span></a> Various abdominal organs, including the gastrointestinal tract, ovaries, liver, gallbladder, and pancreas, may be involved. The gastrointestinal tract that appears to be affected most frequently is the ileocecal area and the appendices.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">3,4</span></a> The clinical features of the infection depend on the organ involved. The most common symptoms are pain, fever, vomiting, diarrhea or constipation, and weight loss. In ileocecal and appendix involvement, clinical and examination findings include acute appendicitis, plastron appendicitis, and cecal mass. In less than 10% of instances, a preoperative diagnosis is made.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">8,17</span></a> Isolated actinomycosis of the appendix presents with clinical, laboratory, and radiographic symptoms that are not specific. As a result, it is challenging and often impossible to diagnose this condition as acute appendicitis before surgery.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The laboratory findings in actinomycosis are non-specific.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">7,14</span></a> Inflammatory parameters (WBC, ESR, and CRP) may be increased or normal.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">6,14</span></a> Ultrasonographic findings lack specificity, thus making the CT scan of the abdomen the most effective diagnostic imaging investigation prior to surgery.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">10,18,19</span></a> From a radiological point of view, appendiceal actinomycosis might look like a cancerous tumor because it can enhance contrast and spread into nearby tissue. While these formations are indeed granulation tissue and fibrotic reactions linked to actinomycosis, it is frequently challenging to completely rule out the presence of cancer. The periappendiceal area that is impacted is encompassed by compact fibrous tissue, and the condition can imitate malignant disorders because of the presence of this mass-like appearance.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">18,20,21</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">In the pathological examination of samples, it is important to see that the microorganism for ms actinomycotic granules (sulfur granules). These consist of irregularly rounded clusters of bacteria surrounded by eosinophilic material and are typical of the disease but are not pathognomonic. Diagnosis is mostly possible by observing typical sulfur granules detected during histopathological examination.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">5,14,16</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Antibiotics are the mainstay of actinomycosis treatment. Although there are no guidelines, the duration of antibiotics depends on the initial extent of the disease and the clinical and radiological response to treatment.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">12,15,22</span></a> In the literature, studies report an over 90% success rate in the treatment of appendicial–ileocecal actinomycosis infections when treated with a combination of antibiotics and surgical resection. Preferred antibiotic treatment includes high-dose crystalline penicillin G (2–4 weeks) and consequent long-term (6–12 months) oral penicillin or semi-synthetic penicillin derivatives. Doxycycline, clindamycin, or erythromycin are acceptable alternatives for penicillin-allergic patients.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">14,19,22</span></a> Patients in the study were given oral penicillin or semi-synthetic penicillin derivatives for one month after their pathological diagnosis was made, regardless of the treatment prescribed at discharge. The personal health data systems of all patients were examined, and no recurrence was detected in the controls.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0045" class="elsevierStylePara elsevierViewall">Ileocecal and appendix actinomycosis are rare; preoperative diagnosis is difficult. Clinical, laboratory, and imaging findings are similar to appendicitis, inflammatory bowel disease, diverticulitis, and ileocecal mass. A definitive diagnosis is usually made after a histopathological examination. Peropative ileocecal mass and mesenteric lymph adenopathy may be detected. The current findings may be confused with malignancy, and unnecessary hemicolectomy and partial resections may be applied to the patients. It prolongs hospital stays and leads to an increase in health care costs. radiological, and histological characteristics of Actinomyces infection identified in appendectomy specimens.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Between January 2013 and November 2023, 5834 patients underwent appendectomy in our clinic, and their pathology reports were retrospectively reviewed.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Actinomyces appendicites were reported in 14 specimens (0.23%). It was determined that appendectomy was performed in only 10 patients (71.4%), ileocecal resection was performed in two patients (14.2%) and right hemicolectomy in two patients (14.2%). The operations on five patients were performed by laparoscopy, and the operations on the other nine patients were performed by open surgery. Laparoscopy was started in three patients and converted to open surgery due to suspicion of an ileocecal mass and cecal perforation. It was found that the white blood cell count of three patients was within the normal range of reference (8–9.77<span class="elsevierStyleHsp" style=""></span>mg/dL); leukocytosis was detected in other patients (10.2–18.7<span class="elsevierStyleHsp" style=""></span>mg/dL). C-reactive protein was normal in one patient and high in the rest of the patients. While the first-hour erythrocyte sedimentation rate was normal in five patients, it was found to be high in the other patients. Findings on radiological imaging were reported as acute appendicitis, appendicular plastron, and ileocecal mass. As a result of the pathology findings, the patients were given oral penicillin or semi-synthetic penicillin derivatives during one month.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Ileocecal and appendecular actinomycosis are rare, and preoperative diagnosis is difficult. A definitive diagnosis is usually made after a histopathological examination. Se determinó que solo se realizó apendicectomía en 10 pacientes (71,4%), resección ileocecal en 2 pacientes (14,2%) y hemicolectomía derecha en 2 pacientes (14,2%). Las operaciones de 5 pacientes se realizaron por vía laparoscópica y las operaciones de 9 pacientes se realizaron con cirugía abierta. La operación se inició por vía laparoscópica en 3 pacientes y se convirtió a cirugía abierta ante la sospecha de masa ileocecal y perforación cecal. Se encontró que el recuento de glóbulos blancos de 3 pacientes estaba dentro del rango normal de referencia (8-9,77<span class="elsevierStyleHsp" style=""></span>mg/dl); en otros pacientes se detectó leucocitosis (10,2-18,7<span class="elsevierStyleHsp" style=""></span>mg/dl). La proteína C reactiva fue normal en un paciente y alta en los demás. Si bien la velocidad de sedimentación globular en la primera hora fue normal en 5 pacientes, se encontró que fue alta en los demás pacientes. Los hallazgos en las imágenes radiológicas fueron informadas como apendicitis aguda, plastrones apendiculares y masa ileocecal. Como resultado de los hallazgos de anatomía patológica, los pacientes recibieron penicilina oral o derivados de penicilina semisintéticos durante un mes.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusión</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Las actinomicosis ileocecal y apendicular son raras y el diagnóstico preoperatorio es difícil. El diagnóstico definitivo suele realizarse tras un examen histopatológico.
