On examination, there was a soft-elastic, painless, non-adherent mass measuring 3 cm, with protrusion with Valsalva manoeuvres. A soft tissue ultrasound was requested, showing a 14 mm left upper dorsolumbar posterolateral wall defect through which mesenteric fat was protruding with a hernial sac measuring 50 × 20 mm, which looked like Grynfeltt hernia and was confirmed with an abdominopelvic CT scan (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>, arrow). Surgery was recommended, which the patient declined as he was asymptomatic.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Lumbar hernias are generally acquired and rare defects of the posterior abdominal wall. They can occur in the upper lumbar triangle (Grynfeltt hernia), lower lumbar triangle (Petit's hernia) or be diffuse, with Grynfeltt hernia being the most common. Clinically they are highly variable and their main complication is strangulation. Diagnosis is clinical, with the mass enlarging with Valsalva manoeuvres, although it is usually confirmed with an imaging test.
