ECG: electrocardiogram; PCI: percutaneous coronary intervention; HD: hemodynamics; IRA: infarct-related artery.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Although there has been a decline in cardiovascular disease-related mortality in Europe, it still remains the primary cause of death, impacting 39% of males and 47% of females Notably, women experience higher morbidity and mortality rates compared to men.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">1,2</span></a> Percutaneous coronary intervention (PCI), a procedure aimed at early coronary reperfusion, has established itself as the preferred treatment for ST-segment elevation acute myocardial infarction (STEMI) within the first 12 hours after symptom onset.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In the case of STEMI, any delay in accessing healthcare negatively impacts the prognosis, making timely intervention crucial.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">3</span></a> This delay primarily stems from two factors: the time it takes for patients to reach out to the healthcare system and the time it takes for the healthcare system to respond.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">3</span></a> Fortunately, the establishment of care networks focused on early STEMI treatment has led to improved response times and a notable increase in the utilization of PCI as a therapeutic approach, resulting in reduced mortality rates.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,6</span></a> However, it has been reported that women benefit comparatively less from such treatment,<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,7</span></a> and there are substantial discrepancies among studies regarding mortality outcomes in STEMI between males and females.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">8–12</span></a> These variations may be attributed to differences in clinical manifestations, delays in receiving appropriate care, underutilization of PCI, and gender-specific risk factors.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">13–16</span></a> Therefore, conducting research specifically focused on the female gender is imperative to draw valid conclusions regarding these aspects.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The primary objective of this study is to examine whether there is a prolonged delay in women compared to men, starting from the onset of symptoms to the opening of the affected artery in cases of STEMI. Furthermore, the study aims to explore whether this delay is associated with a higher incidence of severe adverse events during the follow-up period.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0020" class="elsevierStylePara elsevierViewall">A retrospective, single-center cohort study was carried out in patients with STEMI at a tertiary hospital, treated between January 1, 2015, and December 31, 2020. The study included and analyzed all patients ≥18 years of age with an initial diagnosis of STEMI and who are treated within the REgistro GALlego del Infarto Agudo de Miocardio (REGALIAM) [Galician Registry for Acute Myocardial Infarction], with a one-year follow-up period. For the purpose of analysis, the patients were divided into two groups according to gender at birth (females versus male). Spain has a public and free healthcare system, where patients have different ways of accessing it, such as through their primary care physicians, going to the emergency department of their reference hospital, or via the emergency medical services (EMS).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The data for this project was part of REAGALIAM, comprising patients treated at Álvaro Cunqueiro Hospital in Vigo. Each event recorded in the database underwent a meticulous review by accessing the electronic medical records of the Galician Health Service (IANUS). The verification and supervision of data collection were carried out by an assigned cardiologist. Information on NACE outcomes was captured through the electronic medical records system of Galicia, which encompasses all hospitals and primary care centers (over 400) in the region.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study variables</span><p id="par0030" class="elsevierStylePara elsevierViewall">The analysis included data referred to cardiovascular risk factors, renal insufficiency was considered below 30<span class="elsevierStyleHsp" style=""></span>ml/min, and the date and time of pain onset, first medical contact, diagnostic electrocardiogram (ECG), alert to the Hemodynamics Unit, reperfusion, and completion of the procedure. The infarct location was recorded, along with the initial therapeutic decision (PCI or fibrinolysis), the point of first medical contact (FMC) with the healthcare system (primary care center, EMS, hospital with or without catheterization laboratory), and the means used for transfer. The current recommendations of the European guides on the diagnosis and treatment of patients with STEMI were followed.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">3</span></a> A delay in ECG diagnosis was considered when the latter was made more than 10<span class="elsevierStyleHsp" style=""></span>min after FMC. A delay in opening of the infarct-related artery (IRA) was considered when more than 120<span class="elsevierStyleHsp" style=""></span>min had elapsed from the ECG diagnosis of STEMI to passing of the PCI guide through the IRA. A delay in care was considered when more than 130<span class="elsevierStyleHsp" style=""></span>min had elapsed from FMC to passing of the PCI guide through the IRA (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The definition of net adverse clinical events (NACE) included: reinfarction, revascularizations (not scheduled following admission), stroke, death, hospital admission due to heart failure or major hemorrhage. Only the first event per patient was considered for coding purposes.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Sample size</span><p id="par0040" class="elsevierStylePara elsevierViewall">A sample size was calculated to detect differences in the means of two independent samples using the Student's <span class="elsevierStyleItalic">t</span>-test. A statistical power of 80% was defined for detecting differences in contrasting the null hypothesis, with a significance level of 5%. A standard deviation (SD) in one of the groups between 135 and 315,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a> 239<span class="elsevierStyleHsp" style=""></span>min was considered. No losses were assumed, as this was a retrospective study. Based on these data, a total of 2659 patients were seen to be needed (1994 males and 665 females).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">A descriptive analysis of the data was performed, reporting qualitative variables as absolute frequencies and percentages, and quantitative variables as the mean and SD or interquartile range (IQR) in the absence of a normal distribution. A bivariate analysis was carried out to determine which variables could exert an independent effect upon the delay in care. The Chi-squared test was used to compare two qualitative variables. The Student's <span class="elsevierStyleItalic">t</span>-test or Mann–Whitney <span class="elsevierStyleItalic">U</span>-test in turn was used to compare quantitative variables. A logistic regression analysis was performed to explore associations between selected variables and NACE. Variables included were chosen based on statistical significance in univariate analysis, clinical significance or <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.1, reflecting their importance in NACE research. The Log-rank test with Kaplan–Meier analysis was used to compare the time of occurrence of the combinations of events between males and females. In all cases of hypothesis contrasting, the <span class="elsevierStyleItalic">α</span> significance level accepted was 0.05. The SPSS version 19 statistical package was used to analyze the data.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">Data were collected corresponding to 2668 patients with an initial diagnosis of STEMI between January 1, 2015 and December 31, 2020. The study sample consisted of 2002 males (75%) and 666 females (25%). The women were significantly older, and atherothrombotic disease, and exhibited less active smoking, acute coronary syndrome (ACS) and previous coronary interventions, versus the men (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The main route of access to the healthcare system was through primary care (46.7% of the men and 44.1% of the women), while 22.9% and 20.3% of the men and women respectively reported directly to a hospital without catheterization laboratory, and 21.6% of the men and 20.3% of the women accessed through the EMS. Lastly, 8.8% of the men and 11.8% of the women reported to a hospital with a catheterization laboratory. The main symptom upon admission was chest pain (90.1% of the men and 89.7% of the women; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001), followed by cardiac arrest in men (4.4%) and syncope in women (5.0%). Patients presenting with dyspnea (3.2% women and 2.0% men) experienced delays both in seeking medical attention and in initiating appropriate treatment (<a class="elsevierStyleCrossRef" href="#sec0090">Supplementary data 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Times of care in STEMI</span><p id="par0055" class="elsevierStylePara elsevierViewall">The time elapsed from symptoms onset to opening of the IRA showed significant differences between the two groups. The median total time in men was 197<span class="elsevierStyleHsp" style=""></span>min (IQR: 140–300) versus 220<span class="elsevierStyleHsp" style=""></span>min (IQR: 152–340) in women (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004).</p><p id="par0060" class="elsevierStylePara elsevierViewall">The median time elapsed from symptoms onset to contact with the healthcare system was 60<span class="elsevierStyleHsp" style=""></span>min in men (IQR: 30–140) versus 71<span class="elsevierStyleHsp" style=""></span>min (IQR: 30–163) in women (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.022).</p><p id="par0065" class="elsevierStylePara elsevierViewall">Diagnostic ECG was performed a median of 9<span class="elsevierStyleHsp" style=""></span>min after FMC (IQR: 5–22) in men versus 10<span class="elsevierStyleHsp" style=""></span>min (IQR: 5–27) in women (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006). A greater delay was observed in women versus men in terms of both the time of diagnostic ECG and the time from diagnostic ECG to passing of the PCI guide through the IRA (104<span class="elsevierStyleHsp" style=""></span>min in men [IQR 79–135]) versus 109<span class="elsevierStyleHsp" style=""></span>min in women ([IQR 81–143]; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.022) (<a class="elsevierStyleCrossRef" href="#sec0090">Supplementary data 2</a>). A gender difference was also observed in the total time dependent on the healthcare system (median 131<span class="elsevierStyleHsp" style=""></span>min in women [IQR: 98–176] versus 123<span class="elsevierStyleHsp" style=""></span>min in men [IQR: 95–161]; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Follow-up</span><p id="par0070" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> reports all the adverse events during follow-up after STEMI. Women had a significantly greater incidence of NACE, congestive heart failure and mortality versus men over the first year of follow-up. In the patients that suffered NACE, the median time from symptoms onset to opening of the IRA was 231<span class="elsevierStyleHsp" style=""></span>min (IQR: 148–342) versus 198<span class="elsevierStyleHsp" style=""></span>min in the patients without NACE (IQR: 142–300) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.018). In the multivariate analysis, the female gender, age, the presence of renal failure, peripheral vascular disease, and the healthcare system delay were significantly associated to the incidence of NACE over follow-up (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> displays the event progression during the first year in both groups.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">The main findings of our study can be summarized as follows: (a) women showed a longer delay from symptoms onset to opening of the IRA, and this delay was related to both delay in accessing the healthcare system and to delay derived from the healthcare system itself; and (b) such delays in treatment were associated to an increased risk of suffering NACE during the first year.</p><p id="par0080" class="elsevierStylePara elsevierViewall">In the studied sample, women were comparatively older, and multivessel coronary disease than men. In contrast, men more often had a history of previous cardiovascular disease, in coincidence with the observations of other authors.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">17,9</span></a> The symptoms for which medical care was sought differed, though chest pain was the main manifestation in both groups. Chest pain is the most frequent symptom related to heart disease in the general population.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">18,19</span></a> However, many women present with non-cardiac symptoms,<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">19,20</span></a> similar to those seen in myocardial infarction with non-obstructive coronary arteries (MINOCA) and other age groups.<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">21–23</span></a> The relative absence of chest pain and the distracting effect of many other symptoms may have obscured the diagnosis of STEMI in women, leading to delays in timely care and diagnosis.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">24</span></a> However, it is important to note that chest pain was still a prominent symptom in women. Multiple previous studies have demonstrated prolonged time to diagnosis in women,<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">25,26</span></a> where the relative lack of chest pain may work to make STEMI diagnosis seem less likely. Coupled with the fact that women significantly misinterpret their symptoms more often, it is crucial that when informing society or high-risk individuals about MI, other symptoms in addition to chest pain are also emphasized. Moreover, our findings revealed that patients presenting with dyspnea experienced delays both in seeking medical attention and in initiating appropriate treatment. This further underscores the importance of recognizing and addressing dyspnea promptly in cardiovascular care, as it may serve as an equivalent anginal symptom, indicating underlying ischemic heart disease and necessitating timely intervention.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The type of FMC with the healthcare system also influences the delays in care. Access through the EMS improves time of care even more than personally reporting to a hospital with the capacity to perform PCI.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">9,27</span></a> In our population, 20.3% of the women reported to the healthcare system through the EMS – the figure being similar in men. In both groups, most patients used primary care as FMC. It is particularly important to enhance awareness among the population of the need to resort to the medical emergencies system in life-threatening situations such as STEMI.</p><p id="par0090" class="elsevierStylePara elsevierViewall">In our study a correlation was observed between the female gender and the delay in FMC from STEMI symptoms onset. Different studies have pointed to the delay among women in contacting the healthcare system as one of the most important causes of prolonged myocardial ischemia times.<a class="elsevierStyleCrossRefs" href="#bib0310"><span class="elsevierStyleSup">27,28</span></a> This association between the female gender and the delay in FMC can be due to different reasons. One explanation is the development of more diffuse symptoms that mask the disease.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">29</span></a> Likewise, it has been reported that women are less often likely to associate their symptoms to coronary disease.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">18,27</span></a> Another possible factor is caregiver function, since some women may act as caregivers for dependent people, thus causing them to postpone seeking personal medical help.<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">20,30</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Diagnostic ECG was carried out within a time window in accordance with the recommendations in most patients of either gender.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">4</span></a> Nevertheless, almost half of the women took more than 10<span class="elsevierStyleHsp" style=""></span>min in being diagnosed. There are still differences in the diagnostic ECG times between men and women, as also evidenced in the literature.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">18,27</span></a> These differences could be explained in part by the presence of a less prominent ST segment in STEMI among women, along with more diffuse symptoms,<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">31</span></a> which sometimes may complicate and delay the diagnosis in females. Ongoing training of all healthcare professionals involved in these diagnostic tests is therefore essential.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Once STEMI has been diagnosed, the clinical practice guides (CPGs) establish 120<span class="elsevierStyleHsp" style=""></span>min as the maximum delay in passing the PCI guide through the IRA. The healthcare system took longer in performing coronary reperfusion after the diagnosis of STEMI in women – this representing a delay in opening of the IRA compared with men. Although most of the cases (both men and women) complied with the recommendations of the CPGs, 41.2% of the women exceeded the abovementioned 120-min period in this stage of care.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The total time elapsed from STEMI symptoms onset to PCI coronary reperfusion was significantly longer in women than in men. The results obtained suggest that the acute myocardial infarction care network in Galicia (REGALIAM) offers good quality care, with most patients being managed within the time limits recommended by the CPGs. Nevertheless, there is room for improvement in reducing the gender gap that still exists.</p><p id="par0110" class="elsevierStylePara elsevierViewall">In the present study, the risk of suffering a serious adverse event during follow-up was significantly greater in women than in men, in coincidence with the findings of previous studies.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">11,32</span></a> Women had a 5% greater incidence of NACE over follow-up after STEMI than men, although the difference was borderline statistically significant. Therefore, the results should be interpreted with caution, and the reasons that may be associated with a higher risk for the female sex are as follows. In turn, almost twice as many women suffered congestive heart failure, and 5.1% more women died in the first year compared with the men in our study sample. Several factors contribute to this gender difference, including older age at the time of presentation, increased cardiovascular risk, and differences in reperfusion time, with a greater delay in opening the IRA, affecting left ventricle contractility. In seeking to explain this more unfavorable evolution, consideration is also required of the impact of risk factors inherent to the female gender and of pathophysiological differences in STEMI among women.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">16,33,34</span></a> The gender differences in the pathophysiology of STEMI can also lead to differences in the administration of and response to STEMI treatment based on the CPGs. Compared with men, women are less likely to be identified with a culprit lesion at the time of angiography and have more probabilities of suffering MINOCA, which is associated to a 4.7% incidence of all-cause mortality at one year.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">35</span></a> Moreover, our results evidence a correlation between healthcare system delay and the variable NACE, independently of patient gender – this underscoring the importance of ensuring early care in patients with this disorder.</p><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Limitations</span><p id="par0115" class="elsevierStylePara elsevierViewall">This is a single-center study, though with a sample size of adequate size for addressing the study hypothesis. Its retrospective design implies a risk of confounding factors when interpreting the results; however, the data were entered on a consecutive basis by a health professional at the time of STEMI, which we understand minimizes such bias.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conclusions</span><p id="par0120" class="elsevierStylePara elsevierViewall">Women take longer from symptoms onset in seeking medical attention and are characterized by a longer delay from FMC to PCI opening of the IRA. In turn, the incidence of NACE is greater over the course of follow-up among women. The delay in providing care in patients with STEMI is independently associated to the subsequent appearance of NACE. There is a need to adopt measures to inform both health professionals and patients about the symptoms and the routes for accessing the healthcare system, in order to reduce the times of care and improve the prognosis – particularly in women. Likewise, establishing standardized protocols and guidelines considering gender-related differences in symptom presentation and treatment response can further optimize care delivery.
