including the formation of multidisciplinary referral units&#44; neonatal screening and new therapies&#44; among others&#44; have raised median survival in Europe to over 50 years&#44; with prospects for further increases mainly due to highly effective modulatory therapy&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This progress is creating a new entity that is changing the needs and care of these patients&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">The lung is the main cause of morbidity and mortality in CF&#44; but other organs such as the kidney are also affected&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The gene <span class="elsevierStyleItalic">CFTR</span>&#44; whose mutation is responsible for this disease&#44; is expressed in the kidneys&#44; where it can lead to changes in electrolyte homeostasis or an increased risk of nephrolithiasis&#44; IgA nephropathy&#44; and exposure to nephrotoxic agents&#44; among others&#46;<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2&#44;3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">However&#44; CF-related renal impairment has so far been little studied&#46; In fact&#44; we have not identified any studies in our country analysing renal function in CF&#46; However&#44; linked to the increase in survival&#44; there will presumably be greater exposure to risk factors for chronic renal failure &#40;CRF&#41;&#44; such as metabolic complications linked to obesity&#44; caused by poor dietary habits and weight gain due to new modulatory therapies&#59; micro and macrovascular complications due to CF-related diabetes &#40;CFRD&#41;&#44; or arterial hypertension&#44; among others&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Therefore&#44; the objective of our work was to assess renal function in adults with CF&#46; A secondary objective was to identify factors associated with impaired renal function&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">Retrospective cross-sectional observational study in adults with CF evaluated in two CF units in Madrid in 2022&#46; <span class="elsevierStyleItalic">Inclusion criteria&#58;</span> patients over 18 years of age with a confirmed diagnosis of CF<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and at least one serum creatinine determination during a stable period &#40;at least 28 days without acute therapies&#41; in the previous year&#46; <span class="elsevierStyleItalic">Exclusion criteria&#58;</span> severe liver failure &#40;Child-Pugh class&#8239;C&#41;&#44; previous solid organ or bone marrow transplantation&#44; active malignancy&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">Sociodemographic variables&#44; CF-related manifestations&#44; sputum culture&#44; exacerbations in the previous year&#44; lung function&#44; blood tests including renal function&#44; urine and main chronic treatments were collected from the electronic medical record&#46; A descriptive analysis was performed and data were compared between patients with a glomerular filtration rate &#40;GFR&#41; above and below 90&#8239;ml&#47;min&#47;1&#46;73&#8239;m<span class="elsevierStyleSup">2</span>&#44; measured by the <span class="elsevierStyleItalic">Chronic Kidney Disease Epidemiology Collaboration</span> &#40;CRF-EPI&#41; equation&#46;<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">Quantitative variables are expressed as means and standard deviation&#44; and categorical variables as frequencies&#46; Normal distribution was tested with the Kolmogorov-Smirnov test&#46; For parametric categorical and continuous comparisons&#44; the chi-square test and Student&#39;s <span class="elsevierStyleItalic">t</span>-test were used&#44; respectively&#59; and for non-parametric comparisons&#44; the Mann&#8211;Whitney <span class="elsevierStyleItalic">U</span> test Finally&#44; to investigate variables related to GFR&#44; univariate and multiple logistic regression analyses were used &#40;p&#8239;&#60;&#8239;0&#46;05 was considered statistically significant&#41;&#46; The SPSS&#174; software&#44; version 24&#44; was applied&#46;</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Baseline characteristics</span><p id="par0040" class="elsevierStylePara elsevierViewall">Eighty-nine patients &#40;48 males&#41;&#44; mean age 35 &#40;&#177;12&#41; years and body mass index &#40;BMI&#41; 21&#46;9 &#40;&#177;3&#46;0&#41; were evaluated&#46; Serum creatinine levels were 0&#46;8 &#40;&#177;0&#46;2&#41;&#8239;mg&#47;dl&#46; Sixty-six patients &#40;74&#46;2&#37;&#41; had a GFR greater than 90&#8239;ml&#47;min&#47;1&#46;73&#8239;m<span class="elsevierStyleSup">2</span> and 9 &#40;10&#46;6&#37;&#41; lower&#46; Urine albumin levels were 10&#46;9 &#40;&#177;27&#46;1&#41;&#8239;mg&#47;dl and creatinine levels were 118&#46;8 &#40;&#177;65&#46;7&#41;&#8239;mg&#47;dl&#46; No patient showed albuminuria&#46; Other characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Logistic regression model</span><p id="par0045" class="elsevierStylePara elsevierViewall">Variables related to a GFR greater or less than 90&#8239;ml&#47;min&#47;1&#46;73&#8239;m<span class="elsevierStyleSup">2</span> were analysed&#46; In the simple logistic regression model&#44; age &#40;OR&#58; 1&#46;068&#59; CI&#8239;95&#37;&#58; 1&#46;016&#8211;1&#46;122&#59; p&#8239;&#61;&#8239;0&#46;009&#41; and chronic use of any inhaled antibiotic &#40;OR&#58; 5&#46;14&#59; 95&#37; CI&#8239;95&#37;&#58; 1&#46;403&#8211;18&#46;849&#59; p&#8239;&#61;&#8239;0&#46;013&#41; were retained as risk factors&#46; In the multivariate model increasing age was the parameter independently associated with a GFR value &#60;&#8239;90&#8239;ml&#47;min&#47;1&#46;73&#8239;m<span class="elsevierStyleSup">2</span> &#40;OR&#58; 0&#46;344&#59; CI&#8239;95&#37;&#58; 0&#46;004&#8722;0&#46;017&#59; p&#8239;&#61;&#8239;0&#46;002&#41; &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">10&#46;6&#37; of adults with CF had impaired kidney function&#44; independently associated with older age&#46; The cut-off point used to assess renal function impairment in CF was 90&#8239;ml&#47;min&#47;1&#46;73&#8239;m<span class="elsevierStyleSup">2</span>&#44; following other authors&#44; such as Lai et al&#46;<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> They described a prevalence of GFR below 90 in CF patients of 28&#46;8&#37;&#44; although they included 12&#46;4&#37; of transplant recipients&#44; a risk factor for CRF&#44; probably linked to immunosuppressive treatments&#46; When analysing those with GFR &#60;&#8239;60&#8239;ml&#47;min&#47;1&#46;73&#8239;m<span class="elsevierStyleSup">2</span>&#44; the prevalence obtained is 2&#46;7&#37;&#44; which resembles the 2&#37; described by Quon et al&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> in CF&#44; although this author showed that this figure doubles with each decade of increasing age&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">CRF is a serious health problem affecting more than 10&#37; of the general adult population&#44; with incidence increasing with age&#44; affecting 20&#37; and 35&#37; in those aged 60 and 70 years&#44; respectively&#46; Despite the limited number of cases in our cohort&#44; age was a risk factor independently associated with impaired renal function&#44; in line with other series in CF<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2&#44;5</span></a> and in the general population&#46;<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6&#44;7</span></a> Thus&#44; with increased survival in CF&#44; it is expected that we will see more adults with CRF&#44; and with it not only an increased risk of cardiovascular events&#44; hospitalisations and death&#44; but also a significant economic impact&#46;<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6&#44;7</span></a> It therefore seems logical to call for greater emphasis on controlling CRF-related risk factors such as obesity&#44; hypertension and CFRD&#44; and reduce the use of nephrotoxic drugs&#46; However&#44; the possible long-term beneficial effect of highly effective modulatory therapy in the kidney&#44; as in other organs where CFTR protein is expressed&#44; is unknown&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">From the data collected&#44; we have not obtained any other risk factors for renal function impairment in this cohort&#46; We could consider the role of CFRD&#44; given that it is present in almost 34&#37; of participants and diabetes is globally the leading cause of CRF and renal replacement therapy&#46;<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> However&#44; the absence of albuminuria&#44; the low percentage of patients with GFR &#60;&#8239;90 as well as a mean age of 35 years make the role of diabetic kidney disease in our population unlikely&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">While even a higher rate of albuminuria has been observed when comparing CRF secondary to CFRD with type 1 diabetes&#44;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> the most important complication described with CFRD is the worsening of the natural progression of CF&#44; mainly nutritional and respiratory parameters&#46; Considering that non-CF diabetes mellitus requires 10&#8211;15 years to develop microvascular complications&#44; the same is likely to be true for CFRD&#46;<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> It is foreseeable&#44; therefore&#44; that in the coming years we will face micro- and macrovascular complications&#44; at least in similar proportions to those with non-CF diabetes&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">Another possible cause of renal impairment to consider is iatrogenic&#46; 42&#37; of patients showed chronic bronchial <span class="elsevierStyleItalic">Pseudomonas aeruginosa</span> infection&#44; which would imply treatment with intravenous aminoglycosides in exacerbations&#44; a known risk factor for organ failure&#46;<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> However&#44; we have not shown an association with the number of previous exacerbations&#44; as other authors have&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Similarly&#44; the high use of inhaled antibiotics in CF does not seem to be associated with persistent kidney damage&#46;<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The main limitations of our study are its retrospective cross-sectional nature and the limited sample size&#46; Another limitation is that only one renal function determination per patient was considered&#46; However&#44; all analyses were performed when the patient was clinically stable&#44; and the aim was not to diagnose chronic renal failure&#44; but to analyse the renal function of these patients as a whole&#46; Nevertheless&#44; a prospective study with a larger sample size needs to be designed to further evaluate this understudied entity in CF for its potential future implications&#46;</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0080" class="elsevierStylePara elsevierViewall">11&#37; of adults with CF have reduced GFRs and age is the only associated independent risk factor&#46; Given the expected increase in survival of these patients in the coming years&#44; and in view of the known morbidity and mortality and decrease in quality of life associated with CRF in the general population&#44;
