In fact, we have not identified any studies in our country analysing renal function in CF. However, linked to the increase in survival, there will presumably be greater exposure to risk factors for chronic renal failure (CRF), such as metabolic complications linked to obesity, caused by poor dietary habits and weight gain due to new modulatory therapies; micro and macrovascular complications due to CF-related diabetes (CFRD), or arterial hypertension, among others.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Therefore, the objective of our work was to assess renal function in adults with CF. A secondary objective was to identify factors associated with impaired renal function.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">Retrospective cross-sectional observational study in adults with CF evaluated in two CF units in Madrid in 2022. <span class="elsevierStyleItalic">Inclusion criteria:</span> patients over 18 years of age with a confirmed diagnosis of CF<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and at least one serum creatinine determination during a stable period (at least 28 days without acute therapies) in the previous year. <span class="elsevierStyleItalic">Exclusion criteria:</span> severe liver failure (Child-Pugh class C), previous solid organ or bone marrow transplantation, active malignancy.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Sociodemographic variables, CF-related manifestations, sputum culture, exacerbations in the previous year, lung function, blood tests including renal function, urine and main chronic treatments were collected from the electronic medical record. A descriptive analysis was performed and data were compared between patients with a glomerular filtration rate (GFR) above and below 90 ml/min/1.73 m<span class="elsevierStyleSup">2</span>, measured by the <span class="elsevierStyleItalic">Chronic Kidney Disease Epidemiology Collaboration</span> (CRF-EPI) equation.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">Quantitative variables are expressed as means and standard deviation, and categorical variables as frequencies. Normal distribution was tested with the Kolmogorov-Smirnov test. For parametric categorical and continuous comparisons, the chi-square test and Student's <span class="elsevierStyleItalic">t</span>-test were used, respectively; and for non-parametric comparisons, the Mann–Whitney <span class="elsevierStyleItalic">U</span> test Finally, to investigate variables related to GFR, univariate and multiple logistic regression analyses were used (p < 0.05 was considered statistically significant). The SPSS® software, version 24, was applied.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Baseline characteristics</span><p id="par0040" class="elsevierStylePara elsevierViewall">Eighty-nine patients (48 males), mean age 35 (±12) years and body mass index (BMI) 21.9 (±3.0) were evaluated. Serum creatinine levels were 0.8 (±0.2) mg/dl. Sixty-six patients (74.2%) had a GFR greater than 90 ml/min/1.73 m<span class="elsevierStyleSup">2</span> and 9 (10.6%) lower. Urine albumin levels were 10.9 (±27.1) mg/dl and creatinine levels were 118.8 (±65.7) mg/dl. No patient showed albuminuria. Other characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Logistic regression model</span><p id="par0045" class="elsevierStylePara elsevierViewall">Variables related to a GFR greater or less than 90 ml/min/1.73 m<span class="elsevierStyleSup">2</span> were analysed. In the simple logistic regression model, age (OR: 1.068; CI 95%: 1.016–1.122; p = 0.009) and chronic use of any inhaled antibiotic (OR: 5.14; 95% CI 95%: 1.403–18.849; p = 0.013) were retained as risk factors. In the multivariate model increasing age was the parameter independently associated with a GFR value < 90 ml/min/1.73 m<span class="elsevierStyleSup">2</span> (OR: 0.344; CI 95%: 0.004−0.017; p = 0.002) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">10.6% of adults with CF had impaired kidney function, independently associated with older age. The cut-off point used to assess renal function impairment in CF was 90 ml/min/1.73 m<span class="elsevierStyleSup">2</span>, following other authors, such as Lai et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> They described a prevalence of GFR below 90 in CF patients of 28.8%, although they included 12.4% of transplant recipients, a risk factor for CRF, probably linked to immunosuppressive treatments. When analysing those with GFR < 60 ml/min/1.73 m<span class="elsevierStyleSup">2</span>, the prevalence obtained is 2.7%, which resembles the 2% described by Quon et al.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> in CF, although this author showed that this figure doubles with each decade of increasing age.</p><p id="par0055" class="elsevierStylePara elsevierViewall">CRF is a serious health problem affecting more than 10% of the general adult population, with incidence increasing with age, affecting 20% and 35% in those aged 60 and 70 years, respectively. Despite the limited number of cases in our cohort, age was a risk factor independently associated with impaired renal function, in line with other series in CF<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5</span></a> and in the general population.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> Thus, with increased survival in CF, it is expected that we will see more adults with CRF, and with it not only an increased risk of cardiovascular events, hospitalisations and death, but also a significant economic impact.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> It therefore seems logical to call for greater emphasis on controlling CRF-related risk factors such as obesity, hypertension and CFRD, and reduce the use of nephrotoxic drugs. However, the possible long-term beneficial effect of highly effective modulatory therapy in the kidney, as in other organs where CFTR protein is expressed, is unknown.</p><p id="par0060" class="elsevierStylePara elsevierViewall">From the data collected, we have not obtained any other risk factors for renal function impairment in this cohort. We could consider the role of CFRD, given that it is present in almost 34% of participants and diabetes is globally the leading cause of CRF and renal replacement therapy.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> However, the absence of albuminuria, the low percentage of patients with GFR < 90 as well as a mean age of 35 years make the role of diabetic kidney disease in our population unlikely.</p><p id="par0065" class="elsevierStylePara elsevierViewall">While even a higher rate of albuminuria has been observed when comparing CRF secondary to CFRD with type 1 diabetes,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> the most important complication described with CFRD is the worsening of the natural progression of CF, mainly nutritional and respiratory parameters. Considering that non-CF diabetes mellitus requires 10–15 years to develop microvascular complications, the same is likely to be true for CFRD.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> It is foreseeable, therefore, that in the coming years we will face micro- and macrovascular complications, at least in similar proportions to those with non-CF diabetes.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Another possible cause of renal impairment to consider is iatrogenic. 42% of patients showed chronic bronchial <span class="elsevierStyleItalic">Pseudomonas aeruginosa</span> infection, which would imply treatment with intravenous aminoglycosides in exacerbations, a known risk factor for organ failure.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> However, we have not shown an association with the number of previous exacerbations, as other authors have.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Similarly, the high use of inhaled antibiotics in CF does not seem to be associated with persistent kidney damage.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The main limitations of our study are its retrospective cross-sectional nature and the limited sample size. Another limitation is that only one renal function determination per patient was considered. However, all analyses were performed when the patient was clinically stable, and the aim was not to diagnose chronic renal failure, but to analyse the renal function of these patients as a whole. Nevertheless, a prospective study with a larger sample size needs to be designed to further evaluate this understudied entity in CF for its potential future implications.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0080" class="elsevierStylePara elsevierViewall">11% of adults with CF have reduced GFRs and age is the only associated independent risk factor. Given the expected increase in survival of these patients in the coming years, and in view of the known morbidity and mortality and decrease in quality of life associated with CRF in the general population, we must be vigilant about this understudied entity.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Funding</span><p id="par0085" class="elsevierStylePara elsevierViewall">No funding has been obtained for this study.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Ethical considerations</span><p id="par0090" class="elsevierStylePara elsevierViewall">CREC approval has been obtained for this work. Clinical, respiratory function, microbiological, blood and urine analysis, and major chronic treatments received were collected.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Eighty nine patients with a mean age of 35 ± 12 years were analyzed. Mean serum creatinine levels were 0.8 ± 0.2 mg/dL. 10.6% had a glomerular filtration rate less than 90 ml/min/1.73 m<span class="elsevierStyleSup">2</span>. No patient showed albuminuria.
