id="par0005" class="elsevierStylePara elsevierViewall">Immunoglobulin IgG4-related disease (IgG4-RD) is an immune-mediated condition characterized by fibroinflammatory lesions that may affect virtually any anatomical site.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">1</span></a> Often presenting as a multiorgan disease, it poses significant diagnostic challenges.</p><p id="par0010" class="elsevierStylePara elsevierViewall">First recognized as a distinct disease entity in 2003,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">4</span></a> IgG4-RD encompasses conditions such as Riedel's thyroiditis (fibrosing thyroiditis), Küttner's tumor (enlargement and fibrosis of submandibular glands), Ormond's disease (retroperitoneal fibrosis), and Mikulicz syndrome (parotid gland enlargement and increased submandibular glands). It is hallmarked by tumefactive lesions, dense lymphoplasmacytic infiltrates rich in IgG4-positive cells, storiform fibrosis, and often, though not universally, elevated serum IgG4 levels.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Epidemiologically, IgG4-RD predominantly affects males, particularly those in their sixties and seventies, with a reported male-to-female ratio of approximately 2.8:1.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The disease most frequently targets organs such as the major salivary glands (submandibular, parotid, and sublingual), lacrimal glands, orbits, pancreas, biliary tree, lungs, kidneys, aorta, retroperitoneum, meninges, and thyroid gland.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">1,6,7</span></a> Although the disease is associated with IgG4, serum levels are not elevated in all patients; and they can also be found in other respiratory system diseases (bronchiectasis, asthma, sarcoidosis), pancreatic diseases (chronic pancreatitis), liver diseases (cirrhosis), or in relation to other autoimmune diseases such as SLE, rheumatoid arthritis (RA), or inflammatory myopathies.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">8,9</span></a> Therefore, it is important to make a suitable differential diagnosis including lymphoproliferative disorders (lymphomas), cancer, infections like tuberculosis, and other autoimmune-mediated diseases such as Sjögren's syndrome, vasculitis like granulomatosis with polyangiitis (GPA), systemic lupus erythematosus (SLE), or sarcoidosis.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Since its initial description, various classification criteria for IgG4-RD have been proposed, including the Umehara–Okazaki criteria of 2011<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">10–12</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), which include clinical features, laboratory tests (IgG4 levels, eosinophilia), imaging techniques (CT, MRI, PET-CT) and histopathology (storiform fibrosis, obliterative phlebitis). However, the diagnosis of IgG4-RD is currently based on the consensus of specific consensus criteria established in 2019 by ACR/EULAR<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Patients must meet an inclusion criterion, not to present any exclusion criteria, and obtain a score higher than 20 from a list of clinical, serological, radiological, and histopathological manifestations.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">Treatment strategies for IgG4-RD typically involve high-dose systemic corticosteroids, averaging 40<span class="elsevierStyleHsp" style=""></span>mg/day, followed by a gradual tapering.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">13</span></a> Second-line therapies, including steroid-sparing agents<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">7</span></a> such as azathioprine, mycophenolate, and biological treatments like rituximab are also considered for induction and maintenance therapy.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">14</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Understanding IgG4-RD's clinical manifestations makes us aware of the need to analyze the classification criteria. Due to the disease's intricate presentation and the presence of potential mimicking conditions, a complete evaluation of both historical and contemporary classification criteria, becomes essential. Especially because an accurate diagnostic is crucial to facilitate effective treatment strategies. This study aims to investigate and compare the Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> and ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> consensus criteria, highlighting their advantages, limitations, and clinical implications.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design and population</span><p id="par0040" class="elsevierStylePara elsevierViewall">We conducted a two-center retrospective cross-sectional study including diagnosed cases with an IgG4-RD by an experienced clinician, between January 2010 and July 2023. The data were collected after reviewing the patients’ electronic medical charts, from the databases of various medical departments, as well as the clinical analysis database of the patients who had elevated IgG4 levels.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Participants eligible for this study were adults over 18 years, diagnosed with IgG4-RD, as indicated by physicians through clinical diagnosis without following criteria established by a guideline. They were included regardless of serum IgG4 levels, according to the ELISA kit used (Optilite LK009.OPT), where the normal range is from 3.9 to 86, considering levels above 135 as elevated.</p><p id="par0050" class="elsevierStylePara elsevierViewall">For the remaining cohort with a diagnosis of IgG4-RD, both Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> and ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria were applied.</p><p id="par0055" class="elsevierStylePara elsevierViewall">We collected demographic data (age, sex, race), follow-up information, mortality, corticosteroid treatment history, and other treatments. We also gathered variables from the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria (constitutional, glandular, ocular, cutaneous, joint, renal, pulmonary, hematological, and immunological abnormalities) and the Umehara–Okazaki criteria<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> (elevated serum IgG4 levels, IgG4-positive plasma cells infiltration, histopathological findings such as lymphoplasmacytic infiltrate, fibrosis, obliterative phlebitis). IgG4 clinical phenotypes (pancreatohepatobiliary, retroperitoneum and aorta, head and neck limited, Mikulicz and systemic, and undefined) were also noted.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Laboratory investigations included hemograms, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and serum IgG4 levels using the Optilite kit-IgG4 (normal range interval 3.9–86<span class="elsevierStyleHsp" style=""></span>mg/dL).</p><p id="par0065" class="elsevierStylePara elsevierViewall">For those patients clinically diagnosed or with highly suspicion diagnosis of IgG4-RD the Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> and ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria were subsequently applied, followed by a comparison of these two patient populations. Among the fulfillment of the Umehara–Okazaki 2011 criteria, and the diagnosis (definite, probable, or possible); and the fulfillment of ACR/EULAR 2019,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> the punctuation of the weighted inclusion criteria domains; the affected tissues and the different clinical phenotypes.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">Descriptive statistics were expressed including measures of central tendency and dispersion for quantitative variables, and absolute and relative frequencies for categorical variables.</p><p id="par0075" class="elsevierStylePara elsevierViewall">To assess the agreement between these two diagnostic criteria, the Cohen's Kappa coefficient was calculated. A 2<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>2 table was constructed to evaluate the sensitivity, specificity as well as the Positive Predictive Value (PPV) and Negative Predictive Value (NPV) of the Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> Criteria, treating ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria as the ‘gold standard’.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Furthermore, to evaluate the discriminative capability of the Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria against the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria, a Receiver Operating Characteristic (ROC) curve was plotted, and the Area Under the Curve (AUC) was calculated. To assess the association between the classifications by the two criteria, Pearson's Chi-squared test with Yates’ continuity correction was applied to the contingency table. All statistical analyses were performed using R software version 4.3.1.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Ethics</span><p id="par0085" class="elsevierStylePara elsevierViewall">Our study received approval from the Hospital Ethics Committee N° 95/2022 and was conducted aligned with the Helsinki Declaration's principles and recommendations.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Patient characteristics</span><p id="par0090" class="elsevierStylePara elsevierViewall">A total of 34 patients with clinical diagnosis of IgG4-RD were collected. The average age was 66 years, with the majority (97%) being caucasian. The median follow-up was 3.5 years. Notably, 73.6% of these patients exhibited elevated IgG4 blood levels with a median value of 90<span class="elsevierStyleHsp" style=""></span>mg/dL; and a median CRP and ESR of 0.4<span class="elsevierStyleHsp" style=""></span>mg/dL and 19<span class="elsevierStyleHsp" style=""></span>mm/h, respectively. The main characteristics of these patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Diagnostic outcomes using different criteria</span><p id="par0095" class="elsevierStylePara elsevierViewall">Upon applying the Umehara and Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> diagnostic criteria 20 out of the 34 patients (mean age: 67 years; 50% women); revealing varied diagnostic categorizations with 5 being classified as definitive disease, 7 as probable disease, and 8 as possible disease. One death was recorded in this group, which was related to heart failure. IgG4 levels in blood were elevated in 85% of the patients, with a median of 131<span class="elsevierStyleHsp" style=""></span>mg/dL; and a median CRP and ESR of 0.4<span class="elsevierStyleHsp" style=""></span>mg/dL and 27<span class="elsevierStyleHsp" style=""></span>mm/h, respectively. In regard of the treatment, 90% of patients were initially treated with corticosteroids, and additionally 20% were treated with rituximab, 20% with azathioprine, 15% with methotrexate, 10% with mycophenolate mofetil, 10% with surgery and 5% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1) with hydroxychloroquine. Despite meeting or not the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria, they had an average score of 19. The diagnostic sequence used with IgG4-RD patients are shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">Simultaneously, when the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> classification criteria<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> were applied to the same patient cohort (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), 9 out of the 34 patients (mean age: 67 years; 55% women) with a mean follow-up of 4 years. IgG4 levels in blood was elevated in 77% (7/9) of patients, with a median of 130.2<span class="elsevierStyleHsp" style=""></span>mg/dL; and a median CRP and ESR of 3.2<span class="elsevierStyleHsp" style=""></span>mg/dL and 27<span class="elsevierStyleHsp" style=""></span>mm/h, respectively. Regarding treatment, 88.9% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>8) of the patients were initially treated with corticosteroids, and additionally 22.2% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2) were treated with rituximab, 33.3% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3) with azathioprine, 11.1% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1) with methotrexate, 11.1% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1) with mycophenolate mofetil, 11.1% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1) with surgery and 11.1% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1) with hydroxychloroquine. The average score of this group of patients reached 29 points, exceeding the threshold of 20.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Clinical phenotypes</span><p id="par0105" class="elsevierStylePara elsevierViewall">Despite the undefined phenotype, retroperitoneal fibrosis and aortitis were the most prevalent form of presentation for those who met the 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> and 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria accounting for 25% and 22.2% respectively. The clinical phenotypes of both groups are shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">15</span></a></p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">In our study, we conducted a comprehensive evaluation of two diagnostic criteria, where Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria yielded a sensitivity of 100%, specificity of 56%, PPV of 45% and NPV of 100%, while ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> demonstrated both sensitivity and specificity of 100% and PPV and NPV of 100% too. To assess the concordance between these two diagnostic criteria, we calculated the Cohen's Kappa coefficient, yielding a value of 1, with a confidence interval from −8.8098 to 10.8098.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Additionally, Pearson's Chi-squared test with Yates’ continuity correction indicated a significant association between the classifications by the two criteria (<span class="elsevierStyleItalic">X</span>-squared<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>6.412, <span class="elsevierStyleItalic">p</span>-value<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01134). The ROC analysis for the Umehara–Okazaki 2011 criteria,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> when using ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> as the gold standard, resulted in an area under the curve (AUC) of 0.78, indicating good discriminative ability.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">IgG4-RD is a recently recognized entity known for its clinical heterogeneity and histopathological and pathogenesis complexity which poses challenges in treatment. This study aims to compare the two sets of classification criteria, the older Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria and the more recent ACR/EULAR 2019.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a></p><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Comparison of criteria</span><p id="par0125" class="elsevierStylePara elsevierViewall">Our evaluation revealed that Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria has high sensitivity, which means accurate identification of all patients with the disease. Its specificity, however, stood at 56%, suggesting a notable number of false positives in classifying patients without the disease. The importance of high specificity in the context of IgG4-RD cannot be understated; it ensures patients aren’t misclassified and subjected to unnecessary treatments for IgG4-RD. Pearson's Chi-squared test with Yates’ continuity correction indicated a notable difference in diagnostic outcomes between the two criteria (<span class="elsevierStyleItalic">p</span>-value<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.011), pointing toward a difference in diagnostic outcomes. Moreover, the ROC analysis showcased an AUC of 0.78, suggesting a good discriminative ability of the Umehara–Okazaki 2011 criteria.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">In contrast, the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> displayed sensitivity, specificity, PPV and NPV of 100%. This perfect agreement signifies that the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> might provide more accurate results in identifying patients with and without the disease.</p><p id="par0135" class="elsevierStylePara elsevierViewall">To assess the concordance between these two diagnostic criteria, we calculated the Cohen's Kappa coefficient, yielding a value of 1. However, it is noteworthy that the confidence interval for the Kappa coefficient ranged from −8.8098 to 10.8098, indicating substantial uncertainty in this estimation. These results may be influenced by the sample size (<span class="elsevierStyleItalic">n</span>).</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Disparity in diagnosis and clinical implications</span><p id="par0140" class="elsevierStylePara elsevierViewall">A striking observation was the diagnostic disparity between the two criteria. When using de Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria, a significant number of patients (20 out of 34) were classified under the IgG4-RD spectrum, whereas the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria limited this classification to 9 patients. This considerable gap stems from the intrinsic differences between the criteria, with the 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria placing substantial weight on histopathological findings, manifestations of presentation, and analytical data.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Such diagnostic variance has pronounced clinical implications, like higher number of patients classified under the older criteria could mean that many individuals may receive unnecessary treatments, or even misdiagnoses. The more stringent 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria seem to provide a more accurate classification by narrowing down true IgG4-RD patients.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Interestingly, irrespective of the diagnostic set, retroperitoneal fibrosis and aortitis remained the dominant presentation forms of IgG4-RD. This suggests that while there's variability in the total number of diagnoses, certain manifestations of IgG4-RD are universally recognized.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Alarmingly, 11 out of 20 patients were diagnosed as IgG4-RD based on the older criteria, but not with the ACR/EULAR 2019.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> This indicates the possibility of overdiagnosis with the older criteria. Furthermore, the treatment patterns among those patients diagnosed on the 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria were diverse, with most receiving corticosteroids and a significant portion also receiving rituximab, azathioprine, and other treatments. This variety in treatment approach suggests that even within the cohort diagnosed with the 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria, there was a lack of uniformity in therapeutic strategy.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Though the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> criteria appear more specific and potentially accurate, it is important to advocate for a multidisciplinary approach in diagnosing and managing IgG4-RD. Owing to the disease's varied presentation, requires a collaborative approach to diagnosis and management. Rheumatologists, pathologists, and radiologists play indispensable roles in ensuring reliable diagnosis and optimal patient care.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In conclusion, this study emphasizes the evolution of our understanding of IgG4-RD, and the necessity for adopting the most up-to-date criteria to provide the most accurate diagnoses possible. The move from the Umehara–Okazaki 2011<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> criteria to the ACR/EULAR 2019<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> seems to be a step in the right direction. However, more extensive, multicenter studies and a more extended follow-up would help in further validating the current findings and understanding the long-term outcomes of patients diagnosed based on different criteria.</p></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Ethical considerations</span><p id="par0170" class="elsevierStylePara elsevierViewall">Our study received approval from the Hospital Ethics Committee N° 95/2022. Informed consent was not requested as a retrospective observational study was conducted. Information from the patients’ medical electronic charts was used.
