Objetivos

. Conocer la prevalencia de linfedema postmastectomía en nuestro medio, así como la percepción que del mismo tienen las pacientes y el diagnóstico y tratamiento que de él se realiza.

Métodos

. Cincuenta y tres pacientes operadas de cáncer de mama entre 1995-1999, que acudieron a revisión a la consulta de cirugía en el primer trimestre de 2000.

Se recogieron datos demográficos, factores de riesgo y tratamientos realizados. El diagnóstico de linfedema se realizó con medida de perímetros circunferenciales de ambos miembros superiores cada 5 cm, cuando existían al menos dos perímetros consecutivos con una diferencia mayor de 2 cm respecto al miembro contralateral. Se valoró la sensación subjetiva de linfedema y el tratamiento realizado.

Resultados

. Dieciséis pacientes (30,2%) creían haber tenido edema tras la cirugía y de ellas sólo 6 consideraban padecerlo en el momento de la entrevista. La valoración mediante circunmetría reveló que 18 pacientes presentaban linfedema (34%). Doce de las 18 pacientes no eran conscientes de padecerlo. Las 6 pacientes que creían tener linfedema en ese momento, efectivamente lo presentaban. Sólo en 2 casos se había realizado algún tipo de tratamiento.

Conclusión

. Es necesaria una mayor educación y concienciación sobre este problema tanto del personal sanitario como de las pacientes.

Objectives

. To know the prevalence of postmastectomy lymphedema in our setting, as well as the perception that the patients have of it and the diagnosis and treatment performed on it.

Methods

. Fifty three patients operated on for breast cancer between 1995-1999, who came to check-up in the surgery out-patient clinic in the first quarter of 2000.

Demographic data, risk factors and treatments performed were gathered. The lymphedema diagnosis was performed with measurement of circumferential perimeters of both upper limbs every 5 cm, when there were at least two consecutive perimeters with a difference greater than 2 cm regarding the opposite limb. The subjective sensation of lymphedema and treatment performed was assessed.

Results

. Sixteen patients (30.2%) believed they had had edema after the surgery and only 6 of them considered that they had it at the time of the interview. Assessment by circumferential measurement revealed that 18 patients presented lymphedema (34%). Twelve of the 18 patients were not aware of suffering it. The 6 patients who believed they had lymphedema at this time really had it. Some type of treatment was only performed in 2 cases.

Conclusion

. Greater education and awareness on this problem, both by health care staff as well as the patients, are necessary.
