En 1988 la <span class="elsevierStyleItalic">US Food and Durg administration</span> (FDA) realizó un informe de advertencia sobre la potencial asociación de la prescripción de isotretinoína con la depresión, la ansiedad, la psicosis y la ideación suicida. Actualmente, a pesar del tiempo transcurrido, este tema continúa provocando una gran controversia.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Como objetivo primario de nuestro estudio piloto nos propusimos determinar la asociación entre el uso de isotretinoína y el riesgo de depresión en pacientes con acné en nuestro medio. Para ello realizamos un estudio prospectivo con muestreo consecutivo de pacientes con diagnóstico clínico de acné nódulo quístico o resistente a tratamiento antibiótico oral en estudio en consulta externa del Servicio de Dermatología del Hospital Universitario San Cecilio de Granada en el periodo comprendido entre octubre y diciembre de 2018.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Todos los pacientes o sus representantes legales firmaron el consentimiento informado para participar en el estudio. Este estudio obtuvo dictamen favorable del CEIC del Hospital Universitario San Cecilio para su realización.</p><p id="par0020" class="elsevierStylePara elsevierViewall">En el presente estudio se incluyeron 32 pacientes que aceptaron su participación. Se realizó la escala <span class="elsevierStyleItalic">Patient Health Questionnaire-2</span> (PHQ-2) antes de comenzar el tratamiento con isotretinoína a dosis de 0,5<span class="elsevierStyleHsp" style=""></span>mg/kg/día y tiempo estipulado en las últimas guías de práctica clínica<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a>. El PHQ-2 consta de las 2 primeras preguntas del <span class="elsevierStyleItalic">Patient Health Questionnaire-9</span> (PHQ-9). La puntuación varía de 0 a 6. El punto de corte recomendado es 3 o superior. Cuando ello sucede se recomienda administrar el PHQ-9 completo o bien llevar a cabo una entrevista clínica para evaluar el trastorno depresivo mayor. En nuestro caso se complementó el estudio con la escala de ansiedad y depresión de Goldberg. Es una prueba de detección con uso asistencial y epidemiológico que no solo orienta al diagnóstico, sino que discrimina entre ansiedad y depresión, y dimensiona su intensidad. Los puntos de corte se sitúan en 4 o más para la subescala de ansiedad y 2 o más para la de depresión. Ambas escalas se encuentran validadas. Este procedimiento se repitió una vez concluido el tratamiento. Se preguntó inicialmente por los antecedentes familiares y personales de depresión, y se valoró la gravedad del acné según el paciente con la escala visual analógica (EVA): 0-10, y según el personal médico mediante la escala <span class="elsevierStyleItalic">Investigator Global Assessment</span> (IGA): 0-4.</p><p id="par0025" class="elsevierStylePara elsevierViewall">La edad media de los pacientes en nuestra serie fue de 18,160 años con el mismo porcentaje de varones y mujeres (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>32; 50%). De los pacientes estudiados 8/32 presentaban antecedentes familiares de depresión, y solo uno presentaba antecedentes personales de depresión diagnosticado por un especialista en salud mental de nuestro hospital. El promedio de IGA valorado por el facultativo responsable fue de 2,48 y la media de la EVA de los pacientes 7,03.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Con relación a la escala PHQ-2, 10 pacientes de 32 (31,25%) mostraron una puntuación superior a 3, por lo que realizaron la Escala de ansiedad y depresión de Goldberg. En este caso solo la mitad de los pacientes (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>5/32) obtuvieron una puntuación superior a 4, por lo que solo el 15,62% de nuestra muestra mostró sintomatología depresiva pretratamiento en el despistaje realizado.</p><p id="par0035" class="elsevierStylePara elsevierViewall">En la realización de la escala PHQ-2, solo un paciente de los 32, que previamente había puntuado por encima de 3 previo a la realización del tratamiento continuó puntuando igual. El resto de los pacientes no puntuaron por encima de 3 y, por lo tanto, no realizaron la escala de Goldberg.</p><p id="par0040" class="elsevierStylePara elsevierViewall">En la <a class="elsevierStyleCrossRef" href="#fig0005">figura 1</a> se refleja la comparación del EVA de valoración de acné pre y postratamiento en nuestra serie.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Recientemente se han realizado diferentes metaanálisis para tratar de evaluar la relación entre el tratamiento con isotretinoína y la depresión<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1,3</span></a>. En ellos se establece que no hay relación causal entre la prescripción de isotretinoína y la depresión contraviniendo los resultados de 3 previos llevados a cabo por Strahan et al.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a>, Marqueling et al.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> y Bremner et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a>. Solo un estudio previo indica la existencia de relación entre la isotretinoína y la depresión, si bien su metodología es controvertida con relación a las depuradas técnicas bibliométricas que se realizan en la actualidad<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a>.</p><p id="par0050" class="elsevierStylePara elsevierViewall">A diferencia de otros test utilizados para evaluar la relación entre la depresión y el tratamiento con isotretinoina<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> (<span class="elsevierStyleItalic">Hospital Anxiety Depression Scale-Depression</span> [HAD-D], <span class="elsevierStyleItalic">Global Acne grading system</span> [GAGS] y <span class="elsevierStyleItalic">Cardiff Acne Disability Index</span> [CADI]), pensamos que se puede realizar un <span class="elsevierStyleItalic">screening</span> fidedigno usando la escala PHQ-2. En nuestro caso, se detectó una mejoría subjetiva de la percepción de la gravedad del acné por parte del propio paciente, medido mediante la escala EVA, y a pesar del escaso tamaño muestral no parece haber una relación entre la depresión y la isotretinoína, con disminución de las puntuaciones en la escala PHQ-2 en casi todos los pacientes. Esto coincide con las últimas publicaciones, que muestran una mejora significativa de los síntomas depresivos en los pacientes con acné tratados con isotretinoína<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">3,9,10</span></a>.</p><p id="par0055" class="elsevierStylePara elsevierViewall">En virtud a los resultados de nuestro estudio piloto no hemos podido comprobar relación alguna entre el tratamiento con isotretinoína y un mayor riesgo de depresión en los pacientes con acné. No obstante, existen múltiples factores de confusión clínicos y psicosociales difíciles de evaluar que pueden influir en esta asociación.
