En la actualidad, está en la segunda línea de tratamiento con atezolizumab (anti-PD-L1), en el segundo ciclo tras la progresión de la enfermedad con la primera línea de tratamiento con carboplatino y taxol. El estudio inmunohistoquímico para PD-L1 (test IHC-22C3) es negativo, así como para EGFR y el estudio por FISH para la translocación del locus del gen ALK (2p23.2).</p><p id="p0010" class="elsevierStylePara elsevierViewall">El Servicio de Oncología nos consultó por el desarrollo en este segundo ciclo de pápulas violáceas poligonales pruriginosas en el área lumbar, que coalescen formando placas (<a class="elsevierStyleCrossRef" href="#f0005">fig. 1</a>). No existía compromiso de la mucosa oral ni ungueal. Con diagnóstico clínico de liquen plano simple, se realizó una biopsia cutánea que confirma los resultados 3 semanas más tarde. En el transcurso de 2 semanas se pautó tratamiento con clobetasol propionato en espuma, 2 veces al día, con reevaluación a las 3 semanas donde solo se apreciaban máculas hiperpigmentadas residuales. En este periodo las pruebas complementarias (hemograma, bioquímica y serologías víricas) no habían sufrido alteraciones, sin introducción de nuevos fármacos ni que el paciente hubiera referido una variación importante en sus niveles de estrés.</p><elsevierMultimedia ident="f0005"></elsevierMultimedia><p id="p0015" class="elsevierStylePara elsevierViewall">Los agentes antineoplásicos que bloquean puntos específicos del sistema inmunitario han revolucionado el tratamiento de diferentes neoplasias. Específicamente, la implementación de inhibidores de puntos de control inmunes (<span class="elsevierStyleItalic">checkpoint inhibitors</span>), anticuerpos monoclonales del antígeno 4 asociados a diferentes neoplasias, específicamente linfocitos T citotóxico, la proteína 1 de muerte celular programada (PD-1) o ligando 1 de muerte celular programada (PDL-1)<a class="elsevierStyleCrossRef" href="#bb0005"><span class="elsevierStyleSup">1</span></a>. La inhibición del punto de control puede provocar numerosos efectos adversos, a menudo relacionados con el sistema inmunitario que se manifiestan en el tracto gastrointestinal, el hígado y la piel, aunque cualquier órgano puede verse afectado. Las características del paciente, que incluirían el perfil de citoquinas y el antígeno leucocitario humano podrían predecirlos, incluyendo el prurito, si bien su influencia específica en los efectos adversos dermatológicos sigue siendo, en gran parte, desconocida.</p><p id="p0020" class="elsevierStylePara elsevierViewall">El atezolizumab es un anticuerpo monoclonal que inhibe el punto de control inmune en la unión al ligando de muerte programada 1, para prevenir selectivamente su interacción con los receptores de muerte celular programada-1 (PD-1) y B7.1 (CD80)<a class="elsevierStyleCrossRef" href="#bb0010"><span class="elsevierStyleSup">2</span></a>. PD-L1 es una proteína transmembrana expresada en las células tumorales, al unirse a PD-1 y CD80 que reduce la actividad de los linfocitos T citotóxicos.</p><p id="p0025" class="elsevierStylePara elsevierViewall">El atezolizumab presenta indicación para el tratamiento del cáncer de mama triple negativo, metastásico o localmente avanzado, cáncer de pulmón de células pequeñas en estadios avanzados, cáncer de pulmón de células no pequeñas y carcinoma urotelial que ha progresado durante o después de una quimioterapia previa con cisplatino. Particularmente en los pacientes como el caso que nos ocupa, está indicado en combinación con bevacizumab, paclitaxel y carboplatino, para la primera línea tratamiento de los pacientes adultos sin tumor genómico EGFR o aberraciones en ALK; o bien, si hay progresión de la enfermedad tras terapias que contengan cisplatino.</p><p id="p0030" class="elsevierStylePara elsevierViewall">Los efectos secundarios de atezolizumab incluyen fatiga, fiebre, dolor abdominal y diarrea como sucede en el caso de otros agentes inmunomoduladores. Dentro de la afectación cutánea, si bien el espectro dermatológico de los fármacos anti-PD-1 y anti-PDL-1 es amplio, los más prevalentes son el liquen plano simple, el liquen plano penfigoide<a class="elsevierStyleCrossRef" href="#bb0015"><span class="elsevierStyleSup">3</span></a> y la psoriasis<a class="elsevierStyleCrossRef" href="#bb0020"><span class="elsevierStyleSup">4</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bb0025"><span class="elsevierStyleSup">5</span></a> (en placas e inverso). No obstante, tal y como apuntan Santos Juanes et al.<a class="elsevierStyleCrossRef" href="#bb0020"><span class="elsevierStyleSup">4</span></a>, el bajo número de casos de reacciones liquenoides y psoriasiformes asociadas a atezolizumab, puede estar relacionado con su mecanismo de acción que respeta la unión PD-1 y PDL-2. Por otro lado, en lo que respecta a las reacciones ampollosas, una reciente revisión de la literatura reveló 9 casos de trastornos ampollosos inducidos por PD-L1<a class="elsevierStyleCrossRefs" href="#bb0005"><span class="elsevierStyleSup">1–3</span></a>. La incidencia de dermatosis ampollares en tratamientos que combinan anti-PD-1/anti-PD-L1 es del 1%, mientras que si se consideran anti-PD-L1 exclusivamente varía de 1,3 a 5%, lo que genera mayor preocupación por un riesgo, en general, más alto<a class="elsevierStyleCrossRef" href="#bb0030"><span class="elsevierStyleSup">6</span></a>.</p><p id="p0035" class="elsevierStylePara elsevierViewall">Las dermatitis liquenoides asociadas a anti-PD-1 y anti-PD-L1, son menos frecuentes asociadas a ipilimumab, atezolizumab y darvalumab. Su inicio suele acontecer 12 semanas después del inicio de la medicación. El prurito es un síntoma habitual y el cuadro clínico no difiere del liquen plano común. El tronco y las extremidades son áreas típicamente afectadas. El antígeno epitelial que desencadena este tipo de respuesta liquenoide sigue siendo desconocido, pero es posible que los inhibidores PD-1 y, en menor frecuencia PD-L1, desenmascaren los linfocitos T autorreactivos<a class="elsevierStyleCrossRef" href="#bb0005"><span class="elsevierStyleSup">1</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bb0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="p0040" class="elsevierStylePara elsevierViewall">La mayoría de los efectos secundarios dermatológicos son leves y no precisan suspensión del tratamiento.
