Resumen

La lesión linfoma-like es una hiperplasia linfoide reactiva benigna que simula un linfoma maligno. Se han descrito menos de 20 casos en el cérvix uterino. Para su diagnóstico es necesario realizar biopsias cervicales y amplificación de ADN mediante técnicas de reacción en cadena de la polimerasa para descartar bandas clonales características de los linfomas malignos. El estudio inmunohistoquímico no suele ser concluyente. No son necesarios tratamientos especiales para esta entidad. Presentamos el caso de una paciente de 40 años que consultó de urgencias por metrorragia profusa. Durante la exploración ginecológica y ecográfica se observó un cuello uterino hipertrófico, aumentado de tamaño. Se realizó, de urgencias, citología triple toma y biopsia cervical, y se programó legrado-biopsia. El informe histológico reveló la existencia en cérvix de una infiltración por células linfoides muy sugestivas de linfoma, mientras que el citológico resultó negativo. Se realizó estudio inmunohistoquímico que reveló polimorfismo celular, coexistiendo elementos linfoides de inmunofenotipo B y T y negatividad para proteína bcl-2, lo que unido a la ausencia de reordenamiento clonal IgH, sugirió que se trataba de un proceso reactivo. El estudio de reacción en cadena de la polimerasa no detectó una banda correspondiente a un reordenamiento clonal IgH, informando de un proceso linfoide de tipo reactivo tipo seudolinfoma-hiperplasia linfoide. La tomografía axial computarizada abdominal informó de un útero ligeramente aumentado de tamaño, probablemente miohipertrófico, aumento de tamaño de la porción cervical del cuerpo uterino y discreto aumento del ovario derecho. Se realizó histerectomía total con doble anexectomía y revisión de las cadenas linfáticas pelvianas, que fueron negativas.

Summary

A lymphoma-like lesion is a benign reactive lymphoid hyperplasia that is similar to a malignant lymphoma. Less than 20 cases have been described in the uterine cervix. To establish the diagnosis, cervical biopsies must be performed as well as DNA amplification using polymerase chain reaction techniques to rule out clonal bands, which are a characteristic of malignant lymphomas. Immunohistochemical study usually fails to be conclusive. Special treatments are not required for this entity. We present the case of a 40-year-old woman who was admitted to the emergency service with profuse metrorrhagia. During the gynecological and ultrasound examination, a hypertrophic, slightly enlarged, uterine cervix was found. A triple sample smear was taken on admission, as well as a cervical biopsy. Biopsy curettage was scheduled. The histological report showed an infiltration in the uterine cervix of lymphoid cells suggesting a lymphoma, whereas the cytology report was negative. Immunohistochemical study revealed cellular polymorphism, together with lymphoid elements of immunophenotype B and T and was negative for the bcl-2 protein, which, together the absence of clonal IgH reordering, suggested a reactive process. Polymerase chain reaction did not detect a band corresponding to clonal IgH rearrangement. This information, together with the above-mentioned immunohistochemical study, pointed to a reactive lymphoid process of a pseudolymphoma/ hyperplasia lymphoid nature. Computerized axial tomography showed a slightly enlarged, possibly myohypertrophic, uterus and a slightly enlarged right ovary. Complete hysterectomy was performed with double annexectomy. The pelvic lymphatic nodes were examined but the results were negative.
