Se observan múltiples nódulos (fig. 3) que infiltran de modo difuso el hígado, hipoatenuados en la TC sin contraste, mostrando, algunos, captación del medio de contraste en la fase portal (flechas blancas): el parénquima hepático adopta un realce heterogéneo, parcheado. Líquido libre de localización subfrénica, subhepática y periesplénica (estrellas negras).</p><p class="elsevierStylePara"><span class="elsevierStyleBold">Comentario</span></p><p class="elsevierStylePara">En los antecedentes del paciente (40 años atrás) constaba el uso de Thorotrast<span class="elsevierStyleSup">®</span> como agente de contraste vascular para la valoración de posibles alteraciones vasculares asociadas al Klippel-Trenaunay: hemangiomas, alteraciones del sistema venoso profundo, varicosidades superficiales, fístulas arteriovenosas, etc.<span class="elsevierStyleSup">1</span>.</p><p class="elsevierStylePara">El Thorotrast<span class="elsevierStyleSup">®</span> (dióxido de torio con torio-232), agente de contraste radiológico usado en los años 1930-1955, tiene una vida media física de 14 billones de años y, al ser difícilmente eliminado mediante el sistema retículo-endotelial (SRE), los tejidos donde se acumula reciben radiaciones alfa a lo largo del resto de la vida del portador. Las principales causas de muerte en estos pacientes son cirrosis y neoplasias hepáticas, leucemia y tumores de otras localizaciones<span class="elsevierStyleSup">2</span>. La anatomía patológica más frecuente de estas neoplasias hepáticas es el colangiocarcinoma, pero el angiosarcoma es el más característico secundario a radiación alfa<span class="elsevierStyleSup">2</span>.</p><p class="elsevierStylePara">El angiosarcoma, raro en la población general (2% del total de los tumores hepáticos primarios), es el más frecuente de los de origen mesenquimal<span class="elsevierStyleSup">3,4</span>, grupo que forma con el hemangioendotelioma y el sarcoma de Kaposi. Su importancia radica en la asociación que presenta con agentes ambientales carcinógenos (solución coloide de dióxido de torio, arsénico, radiación, cloruro de vinilo, etc.<span class="elsevierStyleSup">3</span>). Derivado de células endoteliales, puede formar vasos pobremente diferenciados, siendo conocida su histología pleomórfica que parece correlacionarse con el amplio espectro de patrones de captación de contraste que puede adoptar en la TC. Esta variabilidad no suele ocurrir entre las lesiones tumorales del mismo paciente<span class="elsevierStyleSup">5</span>. Generalmente multifocal, su comportamiento más constantemente descrito en los casos asociados al Thorotrast<span class="elsevierStyleSup">®</span> consiste en áreas hipoatenuadas en la TC sin contraste<span class="elsevierStyleSup">4</span>, manteniéndose mayoritariamente hipoatenuadas en las fases contrastadas, si bien en algunos casos pueden aparecer hiperatenuadas<span class="elsevierStyleSup">5</span>, como ocurre en nuestro caso. Pueden identificarse depósitos de Thorotrast<span class="elsevierStyleSup">®</span> en el SRE hepático, esplénico y ganglionar. Se describe la hemorragia intratumoral como niveles líquido-líquido intralesionales en la TC que, en caso de romperse, pueden ocasionar hemoperitoneo<span class="elsevierStyleSup">5</span>, como ocurrió en nuestro paciente (el líquido libre visualizado correspondía a sangre).</p><p class="elsevierStylePara">Aunque descrita como peligrosa la biopsia hepática percutánea<span class="elsevierStyleSup">4</span>, se realizó en nuestro caso con control ecográfico sin incidencias (el hemoperitoneo visualizado en la TC expuesta es previo a la misma), obteniéndose el diagnóstico histológico de angiosarcoma y un material granular refringente en los espacios porta, similar al Thorotrast<span class="elsevierStyleSup">®</span> (fig. 4).</p><p class="elsevierStylePara">La multifocalidad del angiosarcoma obliga al diagnóstico diferencial con enfermedad metastásica<span class="elsevierStyleSup">3</span>. Si bien los hallazgos radiológicos pueden resultar inespecíficos, conviene tener en cuenta esta entidad ya que, aunque en la actualidad no se emplee el Thorotrast<span class="elsevierStyleSup">®</span>, aún podemos ver en nuestros servicios las patologías asociadas a su uso, dado el largo período de latencia.
