Resumen
Objetivo

Analizar la metodología y la eficacia diagnóstica de la RM-colangiografía funcional tras la administración de Mn-DPDP (RMCF-Mn) frente a la RM-colangiografía convencional T2 (RMCC-T2) en el análisis de la vía biliar.

Material y métodos

Se estudiaron mediante RMCF-Mn 16 pacien-tes (cuatro mujeres y 12 hombres, media de edad de 61,6 años) con una secuencia 3D-EG-T1 tras la administración de Teslascan®(ocho estudiados antes de los cinco minutos, cuatro entre los 6 y los 20 minu-tos, y cuatro después de 20 minutos) realizando posteriormente recons-trucciones MIP. Se estudiaron además a ocho de estos pacientes mediante RMCC-T2, seis de ellos antes de la administración del Mn-DPDP y dos tras la inyección.

Resultados

La RMCF-Mn demostró los radicales intrahepáticos en menor proporción y extensión que las secuencias RMCC-T2: sólo en el 12,5% visualizó las ramificaciones secundarias, visibles en el 66,6% de las RMCC-T2. La RMCF-Mn no objetivó el nivel ni la causa de la obstrucción en el caso de obstrucción completa, presentando mayor concordancia en las estenosis sin obstrucción. El relleno duodenal por el Mn-DPDP estaba ausente en caso de obstrucción biliar y en las imá-genes obtenidas antes de los 20 minutos tras la infusión, estando presente si la vía biliar era permeable y la secuencia se adquirió más tar-díamente. La RMCC-T2 obtuvo imágenes de alta calidad en todos los casos. Ningún paciente presentó efectos adversos asociados a la admi-nistración del Mn-DPDP.

Conclusiones

La RM-colangiografía tras la administración de Mn-DPDP permite un análisis funcional de la vía biliar, aunque presenta una escasa rentabilidad global respecto a la RM-colangiografía con-vencional por T2 en casos de obstrucción.

Abstract
Objective

To analyze the methodology and diagnostic efficiency of functional MR cholangiography after Mn-DPDP administration as opposed to conventional T2-weighted MR cholangiography in biliary tract exploration.

Materials and methods

16 patients were studied by means of functional MR cholangiography (4 women and 12 men, mean age 61.6 years) using a T1-3D-gradient echo-sequence after Teslascan®administration (8 patients studied before 5 min examination time, four between 6-20 min, and 4 after 20 min) with MIP reconstructions being performed afterwards. Eight of these patients were also studied with conventional T2 weighted MR cholangiography, six of them before Mn-DPDP administration and two afterwards.

Results

Functional MR cholangiography after Mn-DPDP administration revealed intrahepatic radicals in lesser proportion and extension than did the conventional T2-weighted MR cholangiography sequences: secondary branches were visible in only 12.5%, whereas 66.6% were visible with conventional T2-weighted MR cholangiography. Functional MR cholangiography after Mn-DPDP administration could not shed any light on either degree or cause of a total obstruction. It was more effective in the case of stenosis without obstruction. Duodenal content was enhanced by Mn-DPDP neither in the case of biliary obstruction nor in images obtained before 20 min into the infusion. It was enhanced, however, if the biliary tract was permeable and the sequence was acquired later. Conventional T2-weighted MR cholangio-graphy produced high-quality images in all cases. No patients presented adverse reactions associated with Mn-DPDP administration.

Conclusions

MR cholangiography after Mn-DPDP administration permits a functional analysis of the biliary tract, although it is of rather poor overall utility when compared to conventional T2-weighted MR cholangiography in cases of obstruction.
