Objetivos

Evaluar la precisión diagnóstica de la secuencia TSE-T2 axial en el diagnóstico de coledocolitiasis.

Material

Revisión retrospectiva de todos los estudios de colangiografía RM realizados en nuestro centro, entre enero de 1998 y junio de 1999, que posteriormente han sido sometidos a colangiografía convencional (CPRE o intraoperatoria), considerada como patrón de oro. El número total de pacientes analizados es de 39.

Métodos

La secuencia analizada es una TSE-T2, orientada en el plano axial: supresión de la grasa, TE: 100 ms, TR: 1.800 ms, factor turbo: 23, FOV: 375 mm, NSA: 4, matriz: 228 ??256, compensación respiratoria, cortes: 35, espesor: 3 mm, cortes contiguos. Duración: 5 min 40 s.

Dos radiólogos, sin conocer el resultado de la colangiografía, analizaron de manera independiente esta secuencia, determinando la presenciao no de coledocolitiasis. Posteriormente se compararon los resultados con la colangiografía convencional calculando la sensibilidad, especificidad y concordancia entre radiólogos.

Resultado

Veintiún pacientes tenían coledocolitiasis. La secuencia analizada presentó una sensibilidad del 81% y una especificidad del 89% con una concordancia entre radiólogos del 98%, índice de Kappa 0,949.

Conclusión

La secuencia TSE-T2 axial es fiable para el diagnósticode coledocolitiasis.

Objectives

To evaluate diagnostic precision of the axial TSE-T2 sequence in the diagnosis of choledocholithiasis.

Materials

Retrospective analysis of all those MR cholangiography studies performed in our center between January 1998 and June 1999 which were later subjected to conventional cholangiography (intraoperative) as a golden standard. A total of 39 patients was studied.

Methods

Imaging parameters of the sequence evaluated, fat-suppressed TSE-T2 in the axial plane, were as follows: TE 100 ms, TR 1,800 ms, turbo factor 23, FOV 375 mm, NSA 4, 228 ??256 matrix, respiratory compensation, number of slices 35, slice thickness 3 mm, contiguous slices, scan duration 5’ 40”. Without having been informed as to the choliangiography result, two radiologists independently analyzed this sequence in order to determine the presence of choledocholithiasis. Their results were later compared with those of the conventional cholangiography. The sensitivity, specificity and agreement between results were all calculated.

Results

21 patients had choledocholithiasis. The analyzed sequence presented sensitivity 81%, specificity 89%, and agreement between radiologists 98%. Kappa index 0.949.

Conclusion

The axial sequence TSE-T2 is reliable for choledocholithiasis diagnosis.
