Objetivo

Describir los hallazgos del mucocele apendicular en ecografía y tomografía computarizada (TC), valorando los signos que ayu-den a predecir el origen maligno del mismo.

Material y métodos

Se revisaron los hallazgos clínicos y radioló-gicos de 17 pacientes diagnosticados de mucocele apendicular en nuestro hospital en los últimos 10 años (seis cistoadenocarcinomas, ocho mucoceles de retención, dos cistoadenomas y un adenoma vello-so).

Resultados

La ecografía mostró masas quísticas en la teórica loca-lización del apéndice, con estructura de capas en la pared, de ecogeni-cidad interna variable, mostrando en siete casos calcificación parietal.

La TC mostró tumoraciones quísticas de baja atenuación en el área pericecal, con realce parietal, siendo nodular en cuatro casos (tres cis-toadenocarcinomas y un adenoma velloso). Ninguno de los cistoade-nocarcinomas presentó calcio en la pared. En cuatro pacientes se ob-servó la presencia de seudomixoma peritoneal (tres cistoadenocarcino-mas y un mucocele de retención).

Conclusiones

El mucocele apendicular tiene unos hallazgos eco-gráficos y en TC característicos. La presencia en la TC de un realce nodular en la pared del mucocele es un hallazgo que sugiere un origen maligno. La ausencia de calcio parietal y la existencia de seudomixo-ma peritoneal asociado también sugieren malignidad, aunque ambos signos pueden aparecer en mucoceles benignos.

Aim: to describe appendicular mucocele ultrasound scan and computerized tomography (CT) findings, evaluating signs that help in predicting whether or not they are malignant in nature.

Materials and Methods

Clinical and radiological findings of 17 patients in our hospital diagnosed with appendicular mucocele during the last 10 years (six cystadenocarcinomas, eight mucus retention cysts, two cystadenomas and 1 villus adenoma).

Results

The ultrasound scan showed cystic masses in the theoretical location of the appendix, with layered structure in the wall, and with variable internal echogenicity. Seven cases exhibited parietal calcification.

The CT showed cystic tumors of low attenuation in the pericecal area with parietal enhancement bands, being nodular in four cases (three cystadenocarcinomas and a villus adenoma). None of the cysta-denocarcinomas presented calcified walls. In four patients, peritoneal pseudomyxoma was observed (three cystadenocarcinomas and one mucus retention cyst).

Conclusions

Appendicular mucoceles present some characteristic echographic and CT findings. The CT image of a nodular enhancement in the mucocele wall is one finding that suggests a malignant nature. The absence of parietal calcium and existence of associated peritoneal pseudomyxoma also suggest malignancy, though both signs can also appear in benign mucoceles.
