Objetivo

Evaluar la eficacia de la resonancia magnética (RM) en el diagnóstico de lesiones de rodilla y la influencia del tiempo transcu-rrido entre la RM y la artroscopia sobre los resultados.

Material y métodos

Se analizaron de forma retrospectiva 248 ro-dillas estudiadas por RM y a las que después se había practicado ar-troscopia. Se consideró la artroscopia como patrón de oro. Se evaluó la capacidad diagnóstica de la RM para lesiones meniscales y de liga-mentos de cruzados. Se calculó la sensibilidad, la especificidad y el ín-dice Kappa para el conjunto de todas las rodillas incluidas en el estu-dio (248), para aquellas en las que el tiempo entre la RM y la artrosco-pia fue igual o menor a tres meses (134) y para aquellas en las que el tiempo transcurrido entre ambas técnicas fue igual o menor a un mes (26).

Resultados

La sensibilidad, la especificidad y el índice Kappa de la RM globalmente fueron 96,5%, 70% y 71% respectivamente. Cuan-do el intervalo entre la RM y la artroscopia fue tres meses o menor la sensibilidad, especificidad y el índice Kappa fueron 95,5%, 75% y 72% respectivamente, mientras que cuando fue un mes o menor, la sensibilidad fue del 100%, la especificidad del 87,5% y el índice Kappa del 91%.

Conclusión

La RM es una herramienta excelente para el diagnósti-co de lesiones de rodilla. Se aprecia una mayor sensibilidad, especifici-dad e índice Kappa de la RM cuanto menor es el intervalo de tiempo transcurrido entre ambas técnicas.

To evaluate the efficiency of magnetic resonance (MR) in the diagnosis of knee lesions and how the results are influenced by the time interval between MR and arthroscopy.

Materials and methods

248 knees studied by MR were retrospectively analyzed, as well as those which also underwent arthroscopy. Arthroscopy was considered to be the gold standard. MR diagnostic capacity was evaluated for both meniscal and cruciate ligament lesions. Sensitivity, specificity and Kappa index were calculated for the set of all knees included in the study (248), for those in which the time between MR and arthroscopy was less than or equal to three months (134) and for those in which the time between both procedures was less than or equal to one month (26).

Results

Sensitivity, specificity and Kappa index of the MR had global values of 96.5%, 70% and 71%, respectively. When the interval between MR and arthroscopy was less than or equal to three months, sensitivity, specificity and Kappa index were 95.5%, 75% and 72%, respectively. When it was less than or equal to one month, sensitivity was 100%, specificity was 87.5% and Kappa index was 91%.

Conclusion

MR is an excellent tool for the diagnosis of knee lesions. Higher MR values of sensitivity, specificity and Kappa index are obtained when the time interval between both procedures is kept to a minimum.
