Objetivo

Valorar los patrones ecográficos de las microlitiasis testiculares (MT) en la edad pediátrica, sus asociaciones, implicaciones clínicas y determinar el manejo de las mismas.

Material y métodos

Estudiamos cuatro niños entre 11 y 13 años de edad con MT. El estudio ecográfico se realiza con sonda lineal de 7,5 MHz.

Resultados

Dos de los casos presentan microlitiasis bilaterales. En cinco de los testículos la presentación corresponde al patrón de micro-litiasis testicular clásica (MTC) (más o igual a cinco por campo de vi-sión) y un teste presenta microlitiasis testicular limitada (menos de cin-co por campo). La distribución en el caso de la MTC es difusa en dos testes, periférica en otros dos testículos y central en uno. En uno de los casos de presentación bilateral, se realiza biopsia de ambos testes ob-servando calcificaciones intratubulares en los dos. Ninguno de ellos ha desarrollado tumor en el tiempo de seguimiento que va desde nueve meses a cuatro años.

Conclusiones

Las MT tienen una apariencia ecográfica caracterís-tica. Ante ausencia de lesión focal, deben ser controladas ecográfica y clínicamente, sin ser necesario realizar controles de los marcadores tu-morales ni practicar biopsia.

Aim

To evaluate the ecographic patterns of testicular microlithiasis (TM) in paediatric age, its associations, clinical implications and how to manage them.

Material and methods

Material and methods: We study four children between 11 and 13 years old with testicular microlithiasis. The ecographic study is realized with a 7.5 Mhz linear probe.

Results

Two of the cases present bilateral microlithiasis. In five of the testicles, the presentation fits the pattern of classic testicular micro-lithiasis (CTM) (≥?5 echogenic foci per transducer field) and one testicle presents limited testicular microlithiasis (< 5 echogenic foci per transducer field). Distribution in the case of CTM is diffuse in two testes, peripheral in another two testicles and central in one of them. In one of the bilateral presentations, a biopsy of both testes is performed, observing intratubular calcification in both. None of them has developed a tumor during the follow-up period, which ranges from nine months to four years.

Conclusions

TM have a characteristic echographic appearance. In the absence of focal lesions, they must undergo echographic and clinical control. Neither tumor marker assessment nor biopsy is necessary.
