Resumen
Objetivos

Determinar el valor como método diagnóstico, en la estadificación local del carcinoma colorrectal, de la técnica de TC con enema de agua, tomando como método de referencia el estudio anatomopatológico de las piezas quirúrgicas.

Material y métodos

Se han revisado 58 casos diagnosticados de carcinoma colorrectal por biopsia y estadificados por TC con enema de agua y contraste endovenoso, clasificando todos los casos en uno de estos grupos, T1-T2, T3 y T4. El estudio anatomopatológico de estadificación local se ha realizado en todos los casos y se ha tomado como referencia («estándar de oro»), solamente referido al crecimiento local del tumor (T).

Resultados

Se presentan en dos grupos, T1-T2 frente a T3 y T4 que nos muestra la eficacia para diferenciar estadios precoces de avanzados, siendo los resultados de este grupo los siguientes: sensibilidad = 88,46%, especificidad = 75%, valor predictivo del resultado positivo = 95,83%, valor predictivo del resultado negativo = 60%. El segundo grupo lo constituyen los T1-T2 y T3 frente a T4, para diferenciar los estadios precoces y avanzados de los irresecables, los parámetros en este grupo son: sensibilidad = 100%, especificidad = 98,14%, valor predictivo del resultado positivo = 80%, valor predictivo del resultado negativo = 100%.

Conclusión

Esta variante técnica presenta unos valores de eficacia diagnóstica muy altos.

Objectives

Determine the value of the water enema CT technique as a diagnostic method in the local staging of colorectal carcinoma, using the anatomopathological study of the surgical pieces as reference method.

Material and methods

A total of 58 cases diagnosed of colorectal carcinoma by biopsy and staged by water enema CT and intravenous contrast have been reviewed. All the cases were classified into one of the following groups, T1-T2, T3 and T4. The anatomopathology study of the local staging was performed in all the cases and was considered the gold standard only referring to the local growth of the tumor (T).

Results

Two groups are presented: T1-T2 compared to T3 and T4 that show the efficacy to differentiate early stages from advanced ones, the results of this group being the following: sensitivity 88.46%, specificity = 75%, positive predictive value result = 95.83%, negative predictive value result = 60%. The second group was made up by the T1- T2 and T3 compared to T4, to differentiate the early and advanced stages from the non-resectables ones. The parameters in this group are: sensitivity = 100%, specificity = 98.14%, positive predictive value result = 80%, negative predictive value result = 100%.

Conclusion

The technical variant presents very high diagnostic efficacy values.
